ObjectiveThe soluble guanylate cyclase stimulator riociguat is approved for the treatment of adult patients with pulmonary arterial hypertension (PAH) and inoperable or persistent/recurrent chronic thromboembolic pulmonary hypertension (CTEPH) following Phase 3 randomized trials. The EXPosurE Registry RiociguaT in patients with pulmonary hypertension (EXPERT) study was designed to monitor the long-term safety of riociguat in clinical practice.MethodsEXPERT was an international, multicenter, prospective, uncontrolled, non-interventional cohort study of patients treated with riociguat. Patients were followed for at least 1 year and up to 4 years from enrollment or until 30 days after stopping riociguat treatment. Primary safety outcomes were adverse events (AEs) and serious adverse events (SAEs) coded using Medical Dictionary for Regulatory Activities preferred terms and System Organ Classes version 21.0, collected during routine clinic visits and collated via case report forms.ResultsIn total, 956 patients with CTEPH were included in the analysis. The most common AEs in these patients were peripheral edema/edema (11.7%), dizziness (7.5%), right ventricular (RV)/cardiac failure (7.7%), and pneumonia (5.0%). The most common SAEs were RV/cardiac failure (7.4%), pneumonia (4.1%), dyspnea (3.6%), and syncope (2.5%). Exposure-adjusted rates of hemoptysis/pulmonary hemorrhage and hypotension were low and comparable to those in the long-term extension study of riociguat (Chronic Thromboembolic Pulmonary Hypertension Soluble Guanylate Cyclase–Stimulator Trial [CHEST-2]).ConclusionData from EXPERT show that in patients with CTEPH, the safety of riociguat in routine practice was consistent with the known safety profile of the drug, and no new safety concerns were identified.</div

The most common AEs in these patients were dizziness (11.7%), right ventricular (RV)/cardiac failure (10.7%), edema/peripheral edema (10.7%), diarrhea (8.6%), dyspnea (8.0%), and cough (7.7%). The most common SAEs were RV/cardiac failure (10.1%), pneumonia (6.1%), dyspnea (4.0%), and syncope (3.4%). The exposure-adjusted rate of hemoptysis/pulmonary hemorrhage was 2.5 events per 100 patient-years.ConclusionFinal data from EXPERT show that in patients with PAH, the safety of riociguat in clinical practice was consistent with clinical trials, with no new safety concerns identified and a lower exposure-adjusted rate of hemoptysis/pulmonary hemorrhage than in the long-term extension of the Phase 3 trial in PAH.</p

Capital punishment and consideration about justice has been spread throughout our society for its enitre existence. The death penalty is the only punishment which deals with human life and. with the exception for the legal and criminological aspects, it involves moral and philosophical views, as well. This thesis attempts to examine the ethical attitudes towards capital punishment of the members of the Police of the Czech Republic, as well as that of the city police. The main topic of my thesis is a survey of ethical assessment of the capital punishment. The theoretical part of my thesis deals with the history of capital punishment, various ways in which executions take place, the documents which are connected with the death penalty and human rights. It deals with the ethical and philosophical views, as well as the statement of the Roman Catholic Church. There are many arguments (pros and cons) regarding capital punishment. The practical part of my theis will examine the ethical views of members of the Police of the Czech Republic and city police who are on duty in Třebíč. The method of quantitative analytic research has been applied. The questionnaire survey and its assessment have been used. The chosen respondents are members of the police

The effect of air temperature on yield of Holstein dairy cattle

The study was carried out in the agricultural company Petrovice during the summer and winter seasons. The experiment included Holstein dairy cattle. Air temperature was measured using a data logger with sensors (Datalogger COMET 3120) in the stable. Data on average yield were taken from farm records and then processed using Microsoft Excel. The aim of the study was to determine how the values of ambient temperature affect the welfare of the animals with regard to the average performance. The air temperature is very variable and its changes animals react immediately. Measured values of air temperature in the stable are important for optimal welfare. It affects the productivity of dairy cows, milk quality, reproduction and animal health

EVALUATION OF SELECTED EFFECTS ON MILK PRODUCTION AND FERTILITY IN HOLSTEIN DAIRY CATTLE

The breeding of dairy cattle, in terms of investment and operating costs, is the most demanding sector of livestock production. In terms of importance for agricultural enterprises and all of society, in most EU Member States and the Czech Republic it is one of the major agrarian sectors. Milk production in the Czech Republic has been an important source of income for farmers. For this reason, it is important to ensure an adequate level of milk production in dairy cattle herds and the resulting profit of the enterprise. At the present time in the Czech Republic, it is difficult to ensure profitability in dairy farming because input costs have a continuously rising trend, as compared to the price of milk, which is very unstable. The evaluation of selected indicators of milk yield and fertility in dairy herds is an important tool for evaluating the economics and the level of the breeding. The highest milk yield in kilograms during the first lactation was attained by dairy cows that calved at the age of 27 to 29 months (9,335 kg of milk). When utilizing the DOUBLE OVSYNCH synchronization protocol, the average calving interval was 412 days, and a higher conception rate after first insemination (43%) was found as compared to the nationwide average in the Czech Republic (34.2%)

Monitoring Lameness in Cattle Using the Vitalimeter

The most serious medical illness in breeding dairy cattle include diseases of the limbs, especially the hooves, which is a cause of premature decommissioning of the breeding. This is a crucial economic factor, because it can also significantly reduce the production of milk, and weight. It is therefore very important early detection of lameness. The aim of this study was to monitor the correct movement of dairy cows and subsequent detection of lameness through the automatic scoring system (vitalimeter), which facilitates the search for sick animals
