    The clinical assessment of depth of burns misdiagnosed in up to 35% of cases, especially in the early stages of thermal trauma to the 5th postoperative day. The correct determination of the depth of burns is crucial for planning the adequate therapeutic approach, ie, conservative or surgical treatment of burn wounds. Four basic grades of burn depth are distinguished cliniccaly. Grade I, IIa, IIb can be treated conservatively. Grade III and IV, but also some deeper grade IIb should be operated. Burns IIb cause the most diagnostic difficulties and the indications for surgical treatment is determind by depth of corium affection, which correlates with blood circulation in the dermis and the length of healing. Clinically, it is very difficult to estimate the depth of the affected dermis. LDI, laserdoppler imaging is the one of the ways to objectively and non-invasively assess the depth of the burns. LDI is an imaging technique that uses a laser radiation and the Doppler effect to detect blood flow in the skin capillaries. When thermal trauma directly damages the walls of capillaries, the subsequent tissue necrosis occures. Specificity and sensitivity of LDI in determining of the burn depth states 95% [1]. In our work, we propose a diagnostic algorithm for evaluation of capillary perfusion measured by LDI. Testing can be done up to 9th posttraumatic day, taking into account the current day after the accident. LDI is a suitable tool for the indication of surgical treatment. The following procedures should facilitate this process

    Cíl: Roboticky asistovaný trénink chůze představuje moderní koncept neurorehabilitace u pacientů po CMP. Cílem naší randomizované intervenční studie je zhodnotit přídatný efekt robotické rehabilitace chůze u pacientů v subakutní fázi ischemické CMP a porovnání s kohortou pacientů absolvujících standardní (protokolem definovanou) ústavní rehabilitaci. Primárním sledovaným parametrem je úroveň funkční kategorizace chůze. Sekundárními sledovanými parametry jsou časové parametry chůze (10metrový test chůze, Timed Up and Go), změny tělesného složení, modifikovaná Rankinova škála, index Barthelové, balanční škála Bergové a dotazník Subjektivní hodnocení strachu z pádů. Radiologická substudie sleduje dynamiku vývoje strukturálních změn a atrofie mozkové tkáně pomocí MR. Metody: Prospektivní randomizovaná otevřená monocentrická studie zařazující pacienty do 6 týdnů od první ischemické CMP. Konvenční rehabilitací (fyzioterapie, ergoterapie a mechanoterapie) jsou léčeny obě skupiny po dobu 60 min 5× týdně, celkem 15× po dobu 3–4 týdnů (celkem 1 200 min). Intervenční skupina navíc absolvuje roboticky asistovaný trénink chůze pomocí přístroje Lokomat 20–50 min 5× týdně, celkem 15× po dobu 3–4 týdnů (celkem 1 800 min). Sběr dat probíhá ve čtyřech časových obdobích: před zahájením intervence (T0), v polovině intervence (T1; 8. den), hodnocení po ukončení rehabilitace (T2; 15. den) a 3 měsíce po ukončení (T3).Aim: Robot-assisted gait training represents a modern concept of neurorehabilitation in stroke patients. Our randomized interventional study aims to assess the additive effect of robot-assisted gait rehabilitation in subacute ischemic stroke patients and to compare its effect with patients undergoing standard institutional protocol-defined rehabilitation. The primary endpoint is the functional ambulation category. The secondary endpoints include gait time parameters (10 Meter Walk Test, Timed Up and Go), changes in body composition, modified Rankin scale, Barthel index, Berg balance scale, and a questionnaire Falls Efficacy Scale – International. Radiological sub-study evaluates the dynamics of brain structural changes and atrophy using MRI. Methods: This is a prospective randomized open monocentric study enrolling patients within 6 weeks from the onset of the firs ischemic stroke. Both groups are treated with conventional rehabilitation (physiotherapy, occupational therapy and mechanotherapy) for 60 min 5 times a week, a total of 15 times for 3 to 4 weeks (a total of 1,200 min). The Lokomat group undergoes robot-assisted gait training using the interventional exoskeleton for 20-50 minutes 5 times a week for a total of 15 times for 3 to 4 weeks (a total of 1,800 min). Data collection takes place over four time periods: pre-intervention (T0), mid-intervention (T1; day 8), post-rehabilitation assessment (T2; day 15), and 3 months post-intervention (T3).Web of Science84436636

    Addition of platelet concentrate to Dermo-Epidermal Skin Graft in deep burn trauma reduces scarring and need for revision surgeries

    Backround. Deep skin burn injuries, especially those on the face, hands, feet, genitalia and perineum represent significant therapeutic challenges. Autologous dermo-epidermal skin grafts (DESG) have become standard of care for treating deep burns. Additionally, human autologous thrombin activated autologous platelet concentrate (APC) has gained acceptance in the setting of wounds. While each of these interventions has been independently shown to accelerate healing, the combination of the two has never been evaluated. We hypothesized that the addition of platelets (source of growth factors and inhibitors necessary for tissue repair) to the DESG (source of progenitor cells and of tissue proteases necessary for spatial and temporal control of growth regulators released from platelets) would create the optimal environment for the reciprocal interaction of cells within the healing tissues. Methods: We used clinical examination (digital photography), standardised scales for evaluating pain and scarring, in combination with blood perfusion (laser Doppler imaging), as well as molecular and laboratory analyses. Results: We show for the first time that the combination of APC and DESG leads to earlier relief of pain, and decreased use of analgesics, antipruritics and orthotic devices. Most importantly, this treatment is associated with earlier discharges from hospital and significant cost savings. Conclusions: Our findings indicate that DESG engraftment is facilitated by the local addition of platelets and by systemic thrombocytosis. This local interaction leads to the physiological revascularization at 1-3 months. We observed significant elevation of circulating platelets in early stages of engraftment (1-7 days), which normalized over the subsequent 7 and 90 days.Web of Science158225824

    New semi-spherical radiofrequency energy device for liver resection: an experimental study in pigs

    The aim of this experimental study was to verify a new semi-spherical surgical tool for bipolar radiofrequency liver ablation, which can solve some of the disadvantages of the commonly used device, such as long duration of ablation. A total of 12 pigs which were randomly divided into two groups were used. Each pig underwent resection of the two liver lobes. In group 1, pigs were treated with the commonly used device; in group 2 the newly developed semispherical device was used. During surgery and the post-surgical period, many categories were observed and later analyzed. The blood count and biochemistry were monitored on days 0, 14 and 30 from the operation. On day 14 since the liver resection, pigs underwent diagnostic laparoscopy to evaluate their condition focusing on the site of the liver lobe resection. On day 30 after operation, all pigs were euthanized and subjected to histopathological examination. Histopathological evaluation of thermal changes at the resection margin showed strong thermal alteration in both groups. Data between both groups were compared using median test for continuous variables and Fisher’s exact test for categorical variables. Statistical analyses were performed with IBM SPSS software version 18.0. Statistical analysis of collected data did not prove any significant (P < 0.05) differences between the commonly used device and the newly designed surgical tool.Web of Science84439

    The logical foundations of processes handling uncertainty in information use some classes of algebras as algebraic semantics. The sets of provable formulas in corresponding inference systems from the point of view of uncertain information can be described by fuzzy filters of those algebraic semantics. Bounded residuated lattice ordered monoids (Rℓ-monoids) are a common generalization of pseudo BL-algebras (and consequently of pseudo MV-algebras) and Heyting algebras, i.e. algebras behind fuzzy and intuitionistic reasoning. In the paper, we introduce and investigate fuzzy filters of bounded Rℓ-monoids and fuzzy prime filters of pseudo BL-algebras
