    An Experimental Rat Model Study: Is There Any Effect of Syringic Acid on Ischemia-Reperfusion Injury in Priapism?

    Purpose The purpose of this research is to examine the impact of syringic acid on ischemia-reperfusion injury in cavernosal tissue, utilizing a rat model of induced priapism. Materials and methods A total of 24 rats were allocated into three groups. Group 1 was designated as the control group, while Group 2 underwent ischemia-reperfusion injury assessment using the priapism model. Group 3 underwent the same procedures as Group 2, with the addition of intraperitoneal administration of syringic acid (100 mg/kg) 60 min after priapism initiation. All rats underwent penectomy, and sufficient blood samples were collected. Histopathological assessment of penile cavernosal tissue involved grading tissue damage, inflammation, vasocongestion, desquamation, and edema on a scale of 0-3 (0: normal, 1: mild, 2: moderate, 3: severe). Result Significant differences were observed among the three groups in terms of IL-1 beta and TNF-alpha levels (p=0.001 and p<0.001, respectively). IL-1 beta and TNF-alpha levels in Group 2 were found to be significantly higher than Group 3 (p=0.003 and p=0.004). There was also a significant difference among the three groups in terms of median MDA levels (p<0.001). Furthermore, the median MDA level in Group 2 was found to be significantly higher than that in Group 3 (p<0.001). While significant differences were observed among the three groups in terms of median SOD and GSH-px levels, no significant difference was found among the groups in terms of median PC levels (p=0.004, p= 0.048, and p=0.159, respectively). In direct microscopic examination, a significant improvement in pathological scores was noted in Group 3 compared to Group 2 (p<0.001). Conclusion Syringic acid demonstrated protective properties against ischemia-reperfusion injury caused by priapism in cavernosal tissue

    A rare case of pancreatic fracture due to blunt trauma was presented. The patient was 70 year old male who had a motor vehicle collision and was suspected a pan-creatic trauma due his examinations with ultrasound and computerized tomogra-phy. The diagnosis of splenic injury and pancreas body total fracture in the point where the portal vein crosses the pancreatic body was made with the help of magnetic resonance cholangiopancreatography. He was taken to emergency sur-gery where a splenectomy and a distal pancreatectomy were performed. We rep-resented this infrequent case of pancreatic fracture and its complications after blunt abdominal trauma and discuss the diagnostic and management practices

    The effect of bosentan on plasma ischemia-modified albumin levels in acute mesenteric ischemia

    Objective: Our aim in this study was to examine the effects of bosentan, an orally active antagonist of endothelin A and B receptors, on plasma ischemia-modified albumin (IMA) levels in mesenteric ischemia induced rats. Material and Method: In this randomized, controlled trial 36 mature female wistor rats were divided into three groups. Group 1 (n=12) was the control group which only laparotomy was performed. Group 2 (n=12) was the ischemia group and group 3 (n=12) was the bosentan pretreated (100 mg/kg-1 day, 1 gavage/day for two days before surgery) ischemia group. In the ischemia groups (GR 2 and GR 3) following laparotomy the superior mesenteric artery was clamped using a bulldog clamp during laparotomy. Blood samples were taken at 30 minutes from all groups and IMA levels were studied. Results: Plasma IMA levels in the ischemia group (GR 2) was significantly higher compared to those of the control (p<0.001) and bosentan pretreated group (GR 3) (p=0.002). Serum IMA levels were higher in GR 3 than control group but there were no statistical significance (p=0.659). Conclusion: These preliminary results suggest that bosentan might have a protective effect in mesenteric ischemic conditions

    Thoracopagus Conjoined Twins: A Case Report

    Objective. Conjoined twin is a rarely seen congenital anomaly together with severe mortality and morbidity. The more common types of conjoined twins include the thoracopagus type, where the fusion is anterior, at the chest, and involves the heart. We are reporting one case of conjoined thoracopagus twins diagnosed by ultrasonography at 11 weeks. Case Report. In a multigravid pregnant woman who has been admitted to our clinic with a diagnosis of conjoined twins, thoracopagus, by ultrasonography at an 11-week gestation, termination of the pregnancy was performed. Conclusion. Making an early diagnosis with ultrasonographic examination gives the parents a chance to elect pregnancy termination

    The course of acute appendicitis disease i̇n the COVID-19 period

    The COVID 19 outbreak has changed the course of diseases requiring emergency surgery. Our aim in this study is to reveal whether the COVID 19 pandemic complicates acute appendicitis (AA) disease or not. We retrospectively investigated the patients who were diagnosed with AA in our city (Rize, Turkey). Our study includes two patient groups over 16 years of age who were operated on for AA. Group 2 includes patients operated between 11 March 2020 and 31 December 2020, while Group 1 includes patients who were operated on during the same period in 2019. There were 298 patients (57%) in Group 1 and 226 patients (43%) in Group 2. In Group 2, midline or paramedian incisions were used significantly more than the classical Mc Burney incision: (%10 vs %20, p<0.003). Postoperative complications were significantly higher in Group 2: (%5 vs %12, p<0.004). White blood cells and CRP were significantly different in Group 2 in the admission blood tests (p<0.005, p<0.001, respectively). Operation time and hospitalization time were significantly different in Group 2 (p<0.001, p<0.014, respectively). During the COVID-19 pandemic, there was a serious decrease in the number of patients admitted to the emergency services. Because people were afraid of being infected, they preferred to stay home and with the increase in complaints, they applied to the hospital. AA disease has also become more complicated, as people delayed their admission to the hospital. Serious changes happened in the course of the disease

    Length composition of anchovy (Engraulis encrasicolus, L.) and sprat (Sprattus sprattus L.), important small pelagic species of Black Sea, were determined in this study. Growth parameters and mortality rates for anchovy and sprat, using length composition data of fishes, were estimated. A total of 150 tons anchovy and 75 tons sprat (225 tons fish) were caught end of the 13 midwater trawl tows in the study. Length of 1516 numbers anchovy and 4214 numbers sprat were measured in the study. Maximum, minimum and average length of species were established 15.2 cm, 5.2 cm, 11.28±0.04 cm; 12.8 cm, 5.7 cm, 8.5±0.01 cm respectively.Growth and population parameters of anchovy and sprat were calculated; asymptotic length (L∞) 17.01 cm, 13.38 cm, Brody growth coefficient (K) 0.28, 0.23; instantaneous rate of mortality (Z) 2.73 year-1, 2.88 year-1; rate of natural mortality (M) 0.41 year-1, 0.52 year-1, rate of fishing mortality (F) 2.18 year-1, 1.51 year-1, exploitation rate (E) 0.80 year-1, 0.82 year-1 respectively. Growth and population parameters, calculated by the length composition for species, were compared with other studies.Bu çalışmada Karadeniz’in önemli küçük pelajik türleri olan hamsi (Engraulis encrasicolus, L.) ve çaça (Sprattus sprattus L.) balıklarının boy kompozisyonu verileri kullanılarak büyüme parametreleri ve ölüm oranları tahmin edilmiştir. Araştırmada 13 ortasu trol ağı çekimi yapılarak 150 tonu hamsi, 75 tonu çaça olmak üzere toplam 225 ton balık avlanmıştır. Yapılan örneklemede 1516 adet hamsi ve 4214 adet çaça balığının boyu ölçülmüştür. Türlerin maksimum, minimum ve ortalama boyları sırasıyla 15.2 cm, 5.2 cm, 11.28± 0.04 cm; 12.8 cm, 5.7 cm, 8.5± 0.01 cm olarak tespit edilmiştir. Hamsi ve çaçanın büyüme ve populasyon parametreleri sırasıyla; asimptotik boy (L∞) 17.01 cm, 13.38 cm; büyüme katsayısı (K) 0.28, 0.23; anlık ölüm katsayısı (Z) 2.73 yıl-1, 2.88 yıl-1; doğal ölüm katsayısı (M) 0.41 yıl-1, 0.52 yıl-1, balıkçılık ölüm oranı (F) 2.18 yıl-1, 1.51 yıl-1 ve sömürülme oranı (E) 0.80 yıl-1, 0.82 yıl-1 olarak hesaplanmıştır.     Effects of COVID-19 pandemic on colorectal cancer surgery

    BACKGROUND: The coronavirus disease-19 (COVID-19) pandemic has changed the course of diseases that require emergency surgery. OBJECTIVE: To evaluate the effect of the COVID-19 pandemic on colorectal cancer disease stage. DESIGN AND SETTING: Retrospective analysis in the city of Rize, Turkey. METHODS: This was a comparative analysis on two groups of patients with various symptoms who underwent surgical colorectal cancer treatment. Group 1 comprised patients operated between March 11, 2019, and December 31, 2019; while group 2 comprised patients at the same time of the year during the COVID-19 pandemic. RESULTS: Groups 1 and 2 included 56 and 48 patients, respectively. The rate of presentation to the emergency service was higher in Group 2 (P < 0.02). The stage of the pathological lymph nodes and the rate of liver metastasis was higher in Group 2 (P < 0.004 and P < 0.041, respectively). The disease stage was found to be more advanced in Group 2 (P < 0.005). The rate of postoperative complications was higher in Group 2 (P < 0.014). CONCLUSION: The presentation of patients with suspicious findings to the hospital was delayed, due both to the fear of catching COVID-19 and to the pandemic precautions that were proposed and implemented by healthcare authorities worldwide. Among the patients who presented to the hospital with emergency complaints and in whom colorectal cancer was detected, their disease was at a more advanced stage and thus a higher number of emergency oncological surgical procedures were performed on those patients

    Prenatal diagnosis of tetralogy of Fallot with an absent pulmonary valve: is this malformation still associated with a poor prognosis? A 5-year single-center experience

    Objective: This study sought to assess the prenatal features and clinical outcomes of cases with a fetal diagnosis of tetralogy of Fallot (TOF) with an absent pulmonary valve (APV) at our maternal-fetal medicine unit. Methods: Twelve cases of TOF and APV prenatally diagnosed at Kanuni Sultan Süleyman Training and Research Hospital between 2015 and 2020 were retrospectively reviewed. Prenatal characteristics, additional cardiac and extracardiac anomalies, and postnatal outcomes of the cases were examined. Results: The median gestational age at diagnosis was 22 weeks (range: 18–24 weeks). The absence of ductus arteriosus was found in all cases (100%). Karyotype analysis was performed in 5 cases. A chromosomal abnormality was detected in 3 of these cases (60%); 2 cases with 22q11 microdeletion, and 1 case with trisomy 21. Parents opted for termination of pregnancy in two of these cases; 1 case with 22q11 microdeletion, and 1 case with trisomy 21. Two patients experienced spontaneous intrauterine fetal demise. Finally, 8 live-born fetuses underwent total correction surgery during the postnatal period. Four (33.3%) out of 12 cases survived at the end of the 4-years follow-up period. Conclusion: While TOF with APV cases were predominantly associated with poor prognoses in the past, more promising results have been obtained in recent years in parallel with the developments in surgery and postnatal care. For this reason, we think that the prognosis will be even better in the coming years as the developments in surgical technique and care continue
