    Supporting novice mathematics teachers: The impact of an e- mentoring and video-based professional development program on teachers? noticing skills

    Karadeniz Technical University Scientific Research Foundation [7178]The study was supported by Karadeniz Technical University Scientific Research Foundation with the project number 7178.This paper focuses on e-mentoring of novice mathematics teachers as professional development, and reports findings of the e-mentoring practices enriched with videos to improve noticing skills of novice teachers. A single group pre- and post-test study was conducted with the participation of 17 mentees, novice middle school mathematics teachers. Participants' noticing skills were assessed through a whole-class teaching video shown to them before and after the intervention. The responses of the teachers to the video assessment were analyzed considering attending, interpreting and decision-making dimensions of noticing.     Bannayan-Riley-Ruvalcaba Syndrome in a Case Evaluated Due to Multinodular Goiter

    Bannayan-Rıley-Ruvalcaba syndrome (BRRS) is characterized by macrocephaly, pigmented macules on the glans penis and benign mesodermal hamartomas. 9.6-year-old boy was referred to the pediatric surgeon following an observation of a subcutaneous lipomatous lesion and numerous nodules in the thyroid gland via ultrasonography performed due to swelling in the neck first noticed approximately 3 months previously. Thyroid ultrasonography revealed numerous nodules with distinct margins in both lobes of the thyroid gland, some exhibiting calcification and others hypoechoic areas, and a total thyroidectomy was performed due to a suspicion of malignity. After surgery, the patient was referred to the Pediatric Endocrinology Department. On physical examination, his weight was 30 kg [standard deviation score (SDS): -0.38], height 140 cm (SDS: 0.71) and head circumference 59.5 cm (SDS: +3.21). Pubic hair was Tanner stage 2, bilateral testes 3+3 mL palpable. There was multiple hyperpigmented lesions on the penile skin. His past medical history revealed that pubic hair development was reported at the age of 8 years. Laboratory examinations revealed a 17-OH progesterone level of 4.8 ng/mL, bone age compatible with 8 years. P. V281L heterozygous mutation was determined via CYP21A2 mutation screening performed for non-classic congenital adrenal hyperplasia. BRRS was primarily suspected in this case of macrocephaly, lipomatous lesions and pigmented macular lesions on the penis. Heterozygous p.C136R mutation was determined via PTEN mutation scanning

    Mean Platelet Volume, Neutrophil Lymphocyte Ratio and Platelet Lymphocyte Ratio to Predict Complications During Postoperative Recovery of High Risk Surgical Patients

    Objective:This study sought to define the correlation between the occurrence of the complications and certain blood parameters in high risk surgical patients who were monitored for 24 hours in the intensive care unit postoperatively.Method:Two hundred-fifty seven cases (126 females 131 males; mean age 42.18±8.54 years; range 19-73 years) who were operated on in our hospital and followed up in the intensive care unit for 24 hours were included in this study. The files of all cases included demographic characteristics, erythrocyte count (RBC), leukocyte count (WBC), platelet count (PLT), mean platelet volume (MPV), neutrophil, lymphocyte, neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), serum blood urea nitrogen (BUN) and creatinine values. Group A consisted of cases that did not develop complications in the first 24 hours, and group B consisted of cases that developed complications in the first 24 hours postoperatively. The blood hemogram parameters RBC, WBC, PLT, MPV, NLO, PLO, and the biochemistry parameters BUN and creatinine were compared between the groups.Results:Preoperative MPV value and postoperative MPV value were significantly higher in group B than group A (p-values 0.038 and 0.045, respectively). In groups A and B, the postoperative MPV value increased significantly compared to the preoperative period (p=0.032, p=0.000, respectively). Preoperative PLR and postoperative PLR values did not differ significantly in groups A and B (p=0.101 and p=0.458, respectively). The postoperative PLR value increased significantly in groups A and B compared to the preoperative period (p=0.000 and p=0.047, respectively) Preoperative and postoperative NLR values in group B were significantly higher than group A (p=0.006 and p=0.025 respectively). In groups A and B, the postoperative NLR value increased significantly compared to the preoperative period (p=0.000 and p=0.006, respectively).Conclusion:In this study, the cases that developed complications had more significant increases in their MPV, NLR, and PLR values during the postoperative period than their counterparts who experienced no complications

    The effects of Covid-19 on physical medicine and rehabilitation in Turkey in the first month of pandemic

    Objectives: The outbreak of novel coronavirus-2019 (COVID-19) has affected Turkey very seriously, as well as all around the world. Many urgent and radical measures were taken due to the high contagious risk and mortality rate of the outbreak. It is noteworthy that isolation recommendations and the provision of health services for pandemic have a negative impact on Physical Medicine and Rehabilitation (PMR) services. In this study, we aimed to evaluate the effects of COVID-19 on the PMR services and physiatrists immediately after the first month of pandemic in Turkey. Patients and methods: An online survey consisting of 45 items was sent to the members of the Turkish Society of Physical Medicine and Rehabilitation. The main goal of the survey was to evaluate the changes in the provided service of PMR and conditions of physiatrists one month after the first reported COVID-19 case in Turkey. Results: A total of 606 PMR specialists and residents responded to the survey. The mean number of the patients visited the outpatient clinics was 148.2 +/- 128.5 per week before the pandemic, it significantly decreased to 23.4 +/- 33.1 per week after the first month of the reported first COVID-19 case. Similarly, the mean number of the patients of inpatient service significantly decreased from 21.7 +/- 39.3 per week to 2.5 +/- 10.0 per week after the first month of the pandemic. Most of the residents (69%) reported that their training was seriously affected due to pandemic. From the economic aspect, 69.2% of the participants who were working at private hospitals reported a decrease in their monthly salary, and 21% of them were sent to an unpaid vacation. A total of 21.9% of private-practice institutions paused their services. During the first month, 46.9% of the participants were assigned to the different services such as COVID-19 inpatient service, emergency or COVID-19 outpatient clinics. According to the Republic of Turkey, Ministry of Health guideline and algorithm, 15.7% of the physicians were in the category of healthcare workers with suspected COVID-19. Conclusion: The COVID-19 pandemic affected seriously both the services and the PMR physicians as early as the first month. This effect is expected to become worse, when the duration of pandemic prolongs. Proper arrangements and measures should be planned to ameliorate the negative effects of the pandemic on the patients and PMR physicians

    Tez çalışmasında, enerji kullanımının azaltılması ve enerjinin korunumu amacıyla yeni nesil genleştirilmiş perlit esaslı ısı yalıtım paneli geliştirilmiştir. Granüler ve toz genleştirilmiş perlit 1:5, 2:4, 3:3, 4:2 oranında kullanılmıştır. Bağlayıcıların ısıl iletkenliğe etkisi ve en uygun bağlayıcı oranının belirlenmesi için sodyum silikat ve (6, 8, 10 ve 12M) sodyum hidroksit çözeltisi kullanılmıştır. Hazırlanan panellerin fiziksel, mekanik, ısıl iletkenlik özelliklerinin belirlenmesi için deneyler yapılmıştır. Deneysel çalışmalar sonucunda, en düşük ısıl iletkenlik, 1:5 granüler ve toz GP içeren, 6M sodyum hidroksit ve Na2SiO3/NaOH 1,5 oranı kullanılarak hazırlanan A10 numaralı panelde 49,19 mW/m.K olarak ölçülmüştür. En yüksek basınç dayanımı ise 2:4 granüler ve toz GP içeren, 12M sodyum hidroksit ve Na2SiO3/NaOH 2,5 oranı kullanılarak hazırlanan D8 numaralı panelde 147,739 kPa olarak ölçülmüştür. Elde edilen verilere göre, iki farklı tane yapısına sahip GP esaslı ısı yalıtım panellerinde granüler ve toz GP oranının, sodyum hidroksit derişiminin ve Na2SiO3/NaOH oranının artmasıyla birlikte görünür yoğunluk ve basınç dayanımı artarken, ısı iletkenlik olumsuz yönde etkilenmiştir. Ek olarak, panel yoğunluklarının ısı iletkenlik değerleri üzerine doğrudan etkisi olduğu tespit edilmiştir. Bu sonuçlara göre, GP esaslı ısı yalıtım panelleri ile ilgili daha fazla çalışma yapılmasının tavsiye edilmesinin yanı sıra mevcut durumda yapı sektöründe yalıtım paneli olarak kullanılabileceği tespit edilmiştir.In the thesis study, a new generation of expanded perlite based thermal insulation panel has been developed in order to reduce energy use and to preserve energy. Granular and fine expanded perlite was used in 1:5, 2:4, 3:3, 4:2 ratios. Sodium silicate and (6, 8, 10 and 12M) sodium hydroxide solution were used to determine the optimum binding ratio and the effect of the binders on the thermal conductivity. Physical, mechanical and thermal properties of the samples were determined. As a result of the experimental studies, the lowest thermal conductivity was measured on A10 panel as 49.19 mW/m.K in 1:5 granular and fine expanded perlite ratio with 6M sodium hydroxide and Na2SiO3/NaOH 1,5 ratio. The highest compressive strength was measured on D8 panel as 147,739 kPa in 2:4 granular and fine expanded perlite ratio with 12M sodium hydroxide and Na2SiO3/NaOH 2,5 ratio. According to these results, the increase in the ratio of coarse and fine expanded perlite, NaOH concentration and Na2SiO3/NaOH ratio in expanded perlite based thermal insulation panels with two different sizes, negatively affected the thermal conductivity while increasing the density and compressive strength. In addition, sample densities have a direct effect on thermal conductivity values. According to these results, it has been determined that expanded perlite based thermal insulation panels can be used as insulation panel in the construction sector, besides recommending further studies on the subject

    Post-antibiotic Effect of Various Antibiotics against <i>Achromobacter xylosoxidans</i> Strains Isolated from Patients with Cystic Fibrosis

    Objectives: Achromobacter xylosoxidans is an emerging pathogen in cystic fibrosis (CF) disease, although relatively limited is known about its clinical impact and optimal management. Pharmacodynamics such as post-antibiotic effect (PAE) are increasingly being applied to the design of antibiotic dosing regimens. In the present study, meropenem, doripenem, colistin, levofloxacin, tigecycline, tobramycin, and chloramphenicol were tested for their PAE. Materials and Methods: The in vitro activities of meropenem, doripenem, colistin, levofloxacin, tigecycline, tobramycin, and chloramphenicol were determined by the microbroth dilution technique. To determine the PAEs, A. xylosoxidans strains in the logarithmic phase of growth were exposed for 1 h to antibiotics. Recovery periods of test cultures were evaluated using viable counting after centrifugation. Results: The mean values of in vitro PAEs at 1xMIC and 4xMIC concentrations were 3.21 ± 0.66 and 3.58 ± 0.68 hours for colistin, 1.54 ± 0.46 and 2.27 ± 0.41 hours for levofloxacin, 2.34 ± 0.48 and 3.47 ± 0.64 hours for tobramycin, 1.72 ± 0.47 and 2.24 ± 0.41 hours for tigecycline 1.91 ± 0.14 and 2.13 ± 0.13 hours for chloramphenicol, respectively. The carbapenem antibiotics exhibited weak PAEs, as expected. Conclusions: This study’s findings could have important implications for the timing of doses during antimicrobial therapy with tested antibiotics
