    Application of Smartphone Technology in the Management and Treatment of Mental Illnesses

    Abstract: Background: Mental illness continues to be a significant Public Health problem and the innovative use of technology to improve the treatment of mental illnesses holds great public health relevance. Over the past decade telecommunications technology has been used to increase access to and improve the quality of mental health care. There is current evidence that the use of landline and cellular telephones, computer-assisted therapy, and videoconferencing can be effective in improving treatment outcomes. Smartphones, as the newest development in communications technology, offer a new opportunity to improve mental health care through their versatile nature to perform a variety of functions. Methods: A critical literature review was performed to examine the potential of smartphones to increase access to mental health care, reduce barriers to care, and improve patient treatment outcomes. The review was performed by searching several electronic databases using a combination of keywords related to smartphones and mental health interventions using mobile devices. Literature concerning the use of cell phones, handheld computers, and smartphones to improve access to mental health care and improve treatment outcomes was identified.Results: The majority of studies identified were feasibility and pilot studies on patients with a variety of diagnosed mental illnesses using cell phones and PDAs. Authors report that most study participants, with some exceptions, were capable of using a mobile device and found them acceptable to use. Few studies extensively measured treatment outcomes and instead reported preliminary results and presented case illustrations. Studies which used smartphones successfully used them collect data on patients and deliver multimedia interventions. Discussion: The current literature offers encouraging evidence for the use of smartphones to improve mental health care but also reflects the lack of research conducted using smartphones. Studies which examine care provider use of smartphones to improve care is encouraging but has limited generalizability to mental health care. The feasibility of patient use of smartphones is also encouraging, but questions remain about feasibility in some sub-populations, particularly schizophrenia patients. Pilot testing of mobile devices and applications can greatly increase the feasibility of using smartphones in mental health care. Patients who are unfamiliar with smartphones will likely need initial training and support in their use. Conclusion: The literature identified several ways in which smartphones can increase access to care, reduce barriers, and improve treatment outcomes. Study results were encouraging but scientifically weak. Future studies are needed replicating results of studies using cell phones and PDAs on smartphones. Larger and higher quality studies are needed to examine the feasibility, efficacy, and cost-effectiveness of smartphones to deliver multiple component interventions that improve access to mental health care and improve treatment outcomes

    Evaluation of Young Adults' Preferences, Needs, and the Understandability of the Personal Health Record Data Contents

    This research study examines Personal Health Records (PHRs), focusing on the issues of data contents from the end users' perspectives. The study evaluates the understandability of the Continuity of Care Record (CCR) standard terminology currently used in PHR system and explores users' preferences and needs for data contents.PHRs are becoming an increasingly important and popular means of enabling individuals to have more direct and stronger ownership and management of their health information. One of the potential barriers to the PHRs adoption is the usability of the system, particularly the fact that PHR data contents contain difficult terminology and does not meet the users' needs and preferences. A review of currently available PHR systems shows that vendors are trying to design a comprehensive PHRs primarily based on data contents from the health providers' perspectives, especially the CCR standard. However, this comprehensive data set may be neither suitable nor appealing to most individuals with a busy schedule. Therefore, this research aims at identifying the needs and preferences of the primary users of PHRs with the ultimate goal of designing a user-friendly PHR system that caters to the specific and individual needs of a healthy young adult population.  A mixed-method of qualitative and quantitative research in the form of an exploratory-descriptive study was conducted to examine the individual's needs in terms of PHR contents and terminology. Data was collected through an in-depth, semi-structured interview. Furthermore, a qualitative review study was conducted to identify each data element in the currently available free and for-purchase PHR systems and compare those with the CCR. The PHR included in this study were randomly chosen from the list of PHR tools and services available at www.myphr.com. The results of this research provide insight for PHR developers, enabling them to better design and tailor PHR technology in order to fulfill the needs and desires of each specific individual group and subgroup. A PHR system tailored to the user's individualized needs will serve to make the user feel more comfortable using and maintaining it, and then could lead to wider adoption of PHR within the population

    Mental Health Referral in Primary Care: Influence of a Screening Instrument and a Brief Educational Intervention

    Although less than half of all patients with mental disorders seek mental health treatment per se, approximately 80% of all people will visit their primary care physician (PCPs) within a year (Strosahl, 1998). However, it is not well understood how to best handle patients presenting with mental health issues in primary care practices. The purpose of this project was to implement an intervention involving a screening measure for anxiety and mood disorders in a primary care setting to increase the volume of anxiety and mood disorder screening, to increase the accuracy of disorder detection, and to also enhance PCPs patterns of referral to mental health professionals (MHPs). Though starting with a quantitative design, difficulties encountered throughout the project eventually led to a largely qualitative analysis, which did yield useful information. A pilot project demonstrated anxiety and mood disorders were commonly noted in patients’ medical charts (46%), but also found referrals were rarely made for mental health services (7%), despite colocation of a licensed psychologist and licensed clinical social worker within the practice. This indicated that services available to provide comprehensive integrated total health care may not be have been used to their full potential. In the main project, 59 participants from a family medicine clinic and 20 PCPs from that clinic participated. The My Mood Monitor (M3) was administered to the patients and became part of their Electronic Medical Records (EMR). The M3 screens for anxiety, depression, and bipolar disorders within primary care settings. In 2 separate noon conferences, PCPs were trained on diagnostic criteria for anxiety disorders and mood disorders, interpretation of M3 results, and the internal Mental Health Professional referral process. The project was hampered by a full-scale switch from paper-based medical records to an EMR and accompanying lack of user experience with EMR functions, lack of efficient transfer of M3 results into the EMR, and an unforeseen switch of psychologists mid-way through the study. However, results were obtained that showed relatively low levels of PCP review of M3 results, potentially high rates of anxiety disorders and mood disorders within the setting, relatively high levels of PCP knowledge of diagnostic criteria for anxiety and mood disorders, and that patients may not prefer a ‘warm handoff’ model of mental health referral. These findings are couched within a number of important caveats, but future directions for research were clearly implied

    EVALUATING PATIENT MEDICATION AND COMPLEMENTARY THERAPIES DOCUMENTATION: COMPARATIVE ANALYSIS OF SOURCES, DISCREPANCIES AND THE POTENTIAL IMPACT OF ERRORS ON PATIENT CARE

    Complete knowledge of a patient's medications, including over-the-counter and alternative medicines, is essential to the healthcare professional in providing quality care. In addition to the multiple steps from prescribing, dispensing to administering of a drug medication, there are several factors that increase an individual's risk for an adverse event and approaches to reduce medication errors. The movement of healthcare systems to an electronic medical record provides the potential of building a better health care system. This retrospective study compares five sources of medication, medical record chart, specialist, electronic medical record, pharmacy, insurance provider and patient, to determine what is the most accurate source of documentation, and what factors leading to better knowledge and documentation of all of a patient's medications. This study also identifies additional risk factors, specifically drug affordability and the influence it has on a patient's behavior, and discusses some considerations for reducing medication errors. The prevention and reduction of adverse events is of public health significance as there is both a health and financial cost to treating these adverse events
