    Fly-By-Wireless for Next Generation Aircraft: Challenges and Potential solutions

    ”Fly-By-Wireless” paradigm based on wireless connectivity in aircraft has the potential to improve efficiency and flexibility, while reducing weight, fuel consumption and maintenance costs. In this paper, first, the opportunities and challenges for wireless technologies in safety-critical avionics context are discussed. Then, the assessment of such technologies versus avionics requirements is provided in order to select the most appropriate one for a wireless aircraft application.     Big Data and the Internet of Things

    Advances in sensing and computing capabilities are making it possible to embed increasing computing power in small devices. This has enabled the sensing devices not just to passively capture data at very high resolution but also to take sophisticated actions in response. Combined with advances in communication, this is resulting in an ecosystem of highly interconnected devices referred to as the Internet of Things - IoT. In conjunction, the advances in machine learning have allowed building models on this ever increasing amounts of data. Consequently, devices all the way from heavy assets such as aircraft engines to wearables such as health monitors can all now not only generate massive amounts of data but can draw back on aggregate analytics to "improve" their performance over time. Big data analytics has been identified as a key enabler for the IoT. In this chapter, we discuss various avenues of the IoT where big data analytics either is already making a significant impact or is on the cusp of doing so.     Treball desenvolupat dins el marc del programa 'European Project Semester'.The project describes how to apply Root Cause Analysis (RCA) in the form of a Failure Mode Effect and Criticality Analysis (FMECA) on hydraulically actuated Surface Safety Valves (SSVs) of Xmas trees in oil and gas applications, in order to be able to predict the occurrence of failures and implement preventive measures such as Condition and Performance Monitoring (CPM) to improve the life-span of a valve and decrease maintenance downtime. In the oil and gas industry, valves account for 52% of failures in the system. If these failures happen unexpectedly it can cause a lot of problems. Downtime of the oil well quickly becomes an expensive problem, unscheduled maintenance takes a lot of extra time and the lead-time for replacement parts can be up to 6 months. This is why being able to predict these failures beforehand is something that can bring a lot of benefits to a company. To determine the best course of action to take in order to be able to predict failures, a FMECA report is created. This is an analysis where all possible failures of all components are catalogued and given a Risk Priority Number (RPN), which has three variables: severity, detectability and occurrence. Each of these is given a rating between 0 and 10 and then the variables are multiplied with each other, resulting in the RPN. The components with an RPN above an acceptable risk level are then further investigated to see how to be able to detect them beforehand and how to mitigate the risk that they pose. Applying FMECA to the SSV mean breaking the system down into its components and determining the function, dependency and possible failures. To this end, the SSV is broken up into three sub-systems: the valve, the actuator and the hydraulic system. The hydraulic system is the sub-system of the SSV responsible for containing, transporting and pressurizing of the hydraulic fluid and in turn, the actuator. It also contains all the safety features, such as pressure pilots, and a trip system in case a problem is detected in the oil line. The actuator is, as the name implies, the sub-system which opens and closes the valve. It is made up of a number of parts such as a cylinder, a piston and a spring. These parts are interconnected in a number of ways to allow the actuator to successfully perform its function. The valve is the actual part of the system which interacts with the oil line by opening and closing. Like the actuator, this sub-system is broken down into a number of parts which work together to perform its function. After breaking down and defining each subsystem on a functional level, a model was created using a functional block diagram. Each component also allows for the defining of dependencies and interactions between the different components and a failure diagram for each component. This model integrates the three sub-systems back into one, creating a complete picture of the entire system which can then be used to determine the effects of different failures in components to the rest of the system. With this model completed we created a comprehensive FMECA report and test the different possible CPM solutions to mitigate the largest risks

    Developments in the Safety Science Domain and in Safety Management From the 1970s Till the 1979 Near Disaster at Three Mile Island

    Objective: What has been the influence of general management schools and safety research into causes of accidents and disasters on managing safety from 1970 till 1979? Method: The study was limited to original articles and documents, written in English or Dutch from the period under concern. For the Netherlands, the professional journal De Veiligheid (Safety) has been consulted. Results and conclusions: Dominant management approaches started with 1) the classical management starting from the 19th century, with scientific management from the start of the 20st century as a main component. During the interwar period 2) behavioural management started, based on behaviourism, followed by 3) quantitative management from the Second World War onwards. After the war 4) modern management became important. A company was seen as an open system, interacting with an external environment with external stakeholders. These schools management were not exclusive, but have existed in the period together. Early 20th century, the U.S. 'Safety First' movement was the starting point of this knowledge development on managing safety, with cost reduction and production efficiency as key drivers. Psychological models and metaphors explained accidents from ‘unsafe acts’. And safety was managed with training and selection of reckless workers, all in line with scientific management. Supported by behavioural management, this approach remained dominant for many years, even long after World War II. Influenced by quantitative management, potential and actual disasters after the war led to two approaches; loss prevention (up-scaling process industry) and reliability engineering (inherently dangerous processes in the aerospace and nuclear industries). The distinction between process safety and occupational safety became clear after the war, and the two developed into relatively independent domains. In occupational safety in the 1970s human errors thought to be symptoms of mismanagement. The term ‘safety management’ was introduced in scientific safety literature as well as concepts as loose, and tightly coupled processes, organizational culture, incubation of a disaster and mechanisms blinding organizations for portents of disaster scenarios. Loss prevention remained technically oriented. Till 1979 there was no clear relation with safety management. Reliability engineering, based on systems theory did have that relation with the MORT technique as a management audit. The Netherlands mainly followed Anglo-Saxon developments. Late 1970s, following international safety symposia in The Hague and Delft, independent research started in The Netherland

    Towards design of prognostics and health management solutions for maritime assets

    With increase in competition between OEMs of maritime assets and operators alike, the need to maximize the productivity of an equipment and increase operational efficiency and reliability is increasingly stringent and challenging. Also, with the adoption of availability contracts, maritime OEMs are becoming directly interested in understanding the health of their assets in order to maximize profits and to minimize the risk of a system's failure. The key to address these challenges and needs is performance optimization. For this to be possible it is important to understand that system failure can induce downtime which will increase the total cost of ownership, therefore it is important by all means to minimize unscheduled maintenance. If the state of health or condition of a system, subsystem or component is known, condition-based maintenance can be carried out and system design optimization can be achieved thereby reducing total cost of ownership. With the increasing competition with regards to the maritime industry, it is important that the state of health of a component/sub-system/system/asset is known before a vessel embarks on a mission. Any breakdown or malfunction in any part of any system or subsystem on board vessel during the operation offshore will lead to large economic losses and sometimes cause accidents. For example, damages to the fuel oil system of vessel's main engine can result in huge downtime as a result of the vessel not being in operation. This paper presents a prognostic and health management (PHM) development process applied on a fuel oil system powering diesel engines typically used in various cruise and fishing vessels, dredgers, pipe laying vessels and large oil tankers. This process will hopefully enable future PHM solutions for maritime assets to be designed in a more formal and systematic way

    Piggybacking on an Autonomous Hauler: Business Models Enabling a System-of-Systems Approach to Mapping an Underground Mine

    With ever-increasing productivity targets in mining operations, there is a growing interest in mining automation. In future mines, remote-controlled and autonomous haulers will operate underground guided by LiDAR sensors. We envision reusing LiDAR measurements to maintain accurate mine maps that would contribute to both safety and productivity. Extrapolating from a pilot project on reliable wireless communication in Boliden's Kankberg mine, we propose establishing a system-of-systems (SoS) with LIDAR-equipped haulers and existing mapping solutions as constituent systems. SoS requirements engineering inevitably adds a political layer, as independent actors are stakeholders both on the system and SoS levels. We present four SoS scenarios representing different business models, discussing how development and operations could be distributed among Boliden and external stakeholders, e.g., the vehicle suppliers, the hauling company, and the developers of the mapping software. Based on eight key variation points, we compare the four scenarios from both technical and business perspectives. Finally, we validate our findings in a seminar with participants from the relevant stakeholders. We conclude that to determine which scenario is the most promising for Boliden, trade-offs regarding control, costs, risks, and innovation must be carefully evaluated.Comment: Preprint of industry track paper accepted for the 25th IEEE International Conference on Requirements Engineering (RE'17
