    Neural Signatures of Semantic and Phonemic Fluency in Young and Old Adults

    As we age, our ability to select and produce words changes, yet we know little about the underlying neural substrate of word-finding difficulties in old adults. The present study was designed to elucidate changes in specific frontally mediated retrieval processes involved in word-finding difficulties associated with advanced age. We implemented two overt verbal (semantic and phonemic) fluency tasks during functional magnetic resonance imaging and compared brain activity patterns of old and young adults. Performance during the phonemic task was comparable for both age-groups and mirrored by strongly left lateralized (frontal) activity patterns. On the other hand, a significant drop of performance during the semantic task in the older goup was accompanied by additional right (inferior and middle) frontal activity, which was negatively correlated with performance. Moreover, the younger group recruited different subportions of the left inferior frontal gyrus for both fluency tasks, while the older participants failed to show this distinction. Thus, functional integrity and efficient recruitment of left frontal language areas seems to be critical for successful word-retrieval in old age

    Can Semantic-Phonemic Discrepancy in Verbal Fluency Help to Detect Alzheimer’s Dementia?

    Word retrieval difficulty is one of the early signs of Alzheimer’s disease, although such difficulties can also occur in typically aging. Therefore, it is necessary to find a task that differentiates the early stages of Alzheimer’s dementia from typically aging. Verbal fluency is a widely used measure to assess subjects’ cognitive processes following neurological damage, and often includes two subtests: semantic fluency, in which participants are asked to produce words which meet a semantic criterion, such as food or animals; and letter fluency, which requires participants to produce words starting with a certain letter, such as F or S. People with Alzheimer’s disease have more difficulty with semantic than letter fluency, although this pattern has also been shown in typically aging.     May subjective language complaints predict future language decline in community-dwelling subjects?

    Subjective cognitive complaints are rather prevalent in the elderly population and are associated with an increased risk of cognitive impairment and dementia. However, the predictive role of specific types of cognitive complaints has been less systematically assessed. The aim of the present study is to examine the predictive value of language complaints for cognitive and language decline in a cohort of community-dwelling healthy older adults, followed longitudinally over a 5-year period. A total of 402 subjects were enrolled in a prospective longitudinal study on aging and cognition. Participants answered a cognitive complaints questionnaire including two questions directed to language and were classified at baseline as having “Language Complaints” (LC) or “No Language Complaints” (NLC). They also performed a neuropsychological assessment tackling attention/processing speed, memory, executive functioning, and language at baseline. From these, 275 (68.4%) participated in a follow-up evaluation 4.9 (±0.6) years later. At re-evaluation, subjects had a mean age of 70.4 (±8.3) years, 7.5 (±4.4) years of education, and 63.3% were female. Multivariate linear regression analysis was used to investigate whether language complaints at baseline predicted poorer language performance at follow-up or increased the risk of cognitive decline, with correction for sex, depressive symptoms, living status, baseline performance, and composite memory and executive performance. Results indicated that LC subjects had significantly worse performances than NLC subjects on semantic fluency 5 years later, but with a similar rate of decline overtime that was not associated with a follow-up outcome of cognitive decline/dementia. Language difficulties may represent a specific type of age-related cognitive complaints. Longer follow-ups are necessary to understand if they are associated with an increased risk of language or cognitive decline.info:eu-repo/semantics/publishedVersio

    Improved conceptual generation and selection with transcranial direct current stimulation in older adults

    Normal aging is associated with deficits in various aspects of spoken language production, including idea generation and selection, and involves activity in frontal brain areas including left inferior frontal cortex (LIFG). These conceptual preparation processes, largely involving executive control, precede formulation and articulation stages and are critical for language production. Noninvasive brain stimulation (e.g., transcranial direct current stimulation, tDCS) has proven beneficial for age-related fluency and naming deficits, but this has not been extended to conceptual preparation mechanisms.We investigated whether tDCS could facilitate idea generation and selection in 24 older adults aged 60-80\ua0years. In the first phase, participants completed an idea generation test and a selection test with no stimulation. In the second phase they completed an alternate version of the tests in conjunction with either active or sham stimulation. Active stimulation applied 1-mA anodal tDCS over LIFG for the test duration (10\ua0min).\ua0 Responses were faster following active stimulation than following sham. Furthermore, improvements were specific to test conditions involving novel generation (p\ua0=\ua0.030) and selection (p\ua0=\ua0.001) and were not observed in control conditions for which these mechanisms were minimally involved.\ua0 We concluded that tDCS benefits conceptual preparation mechanisms. This preliminary evidence is an important step for addressing age-related decline in propositional language generation, which is integral to conversational speech. This approach could also be extended toward rehabilitation in neurological patients with deficits in these processes

    Executive Function in Pediatric Bipolar Disorder and Attention-Deficit Hyperactivity Disorder: In Search of Distinct Phenotypic Profiles

    Often, there is diagnostic confusion between bipolar disorder (BD) and attention-deficit hyperactivity disorder (ADHD) in youth due to similar behavioral presentations. Both disorders have been implicated as having abnormal functioning in the prefrontal cortex; however, there may be subtle differences in the manner in which the prefrontal cortex functions in each disorder that could assist in their differentiation. Executive function is a construct thought to be a behavioral analogy to prefrontal cortex functioning. We provide a qualitative review of the literature on performance on executive function tasks for BD and ADHD in order to determine differences in task performance and neurocognitive profile. Our review found primary differences in executive function in the areas of interference control, working memory, planning, cognitive flexibility, and fluency. These differences may begin to establish a pediatric BD profile that provides a more objective means of differential diagnosis between BD and ADHD when they are not reliably distinguished by clinical diagnostic methods

    Examining qualitative and quantitative features of verbal fluency tasks to investigate the mental lexicon in postpartum women: A neuropsychological approach of executive functions applied to language

    During the postpartum period, women experience neurobiological and psychosocial variations that impact language functioning. Word production in verbal fluency tasks (VFTs) is a cognitive indicator of associative (semantic categorization and phonological analysis) and executive (inhibitory control and cognitive flexibility) processes. Also, a linguistic analysis allows for understanding production strategies (e.g., orthographic and use of rhymes), with multivariate statistics facilitating cluster identification of the most common words. Considering these approaches, this study aimed to optimize semantic and phonological VFT analysis for the identification of postpartum women's mental lexicon using quantitative and qualitative scores. These outcomes were evaluated together with sociodemographic and reproductive data of 100 postpartum women (from Argentina). Mental lexicon description was statistically improved and showed that multiparous women clustered words more concisely than primiparous women, with increased correct words and better organizational strategies. In sum, female reproductive history improved VFT outcomes. The current results also show that factor analysis can optimize the neuropsychological study of language structuring.Fil: Barral, Paula Eugenia.     Functional Lateralization of Speech Processing in Adults and Children Who Stutter

    Developmental stuttering is a speech disorder in fluency characterized by repetitions, prolongations, and silent blocks, especially in the initial parts of utterances. Although their symptoms are motor related, people who stutter show abnormal patterns of cerebral hemispheric dominance in both anterior and posterior language areas. It is unknown whether the abnormal functional lateralization in the posterior language area starts during childhood or emerges as a consequence of many years of stuttering. In order to address this issue, we measured the lateralization of hemodynamic responses in the auditory cortex during auditory speech processing in adults and children who stutter, including preschoolers, with near-infrared spectroscopy. We used the analysis–resynthesis technique to prepare two types of stimuli: (i) a phonemic contrast embedded in Japanese spoken words (/itta/ vs. /itte/) and (ii) a prosodic contrast (/itta/ vs. /itta?/). In the baseline blocks, only /itta/ tokens were presented. In phonemic contrast blocks, /itta/ and /itte/ tokens were presented pseudo-randomly, and /itta/ and /itta?/ tokens in prosodic contrast blocks. In adults and children who do not stutter, there was a clear left-hemispheric advantage for the phonemic contrast compared to the prosodic contrast. Adults and children who stutter, however, showed no significant difference between the two stimulus conditions. A subject-by-subject analysis revealed that not a single subject who stutters showed a left advantage in the phonemic contrast over the prosodic contrast condition. These results indicate that the functional lateralization for auditory speech processing is in disarray among those who stutter, even at preschool age.     Anodal transcranial direct current stimulation temporarily reverses age-associated cognitive decline and functional brain activity changes

    The rising proportion of elderly people worldwide will yield an increased incidence of age-associated cognitive impairments, imposing major burdens on societies. Consequently, growing interest emerged to evaluate new strategies to delay or counteract cognitive decline in aging. Here, we assessed immediate effects of anodal transcranial direct current stimulation (atDCS) on cognition and previously described detrimental changes in brain activity attributable to aging. Twenty healthy elderly adults were assessed in a crossover shamcontrolled design using functional magnetic resonance imaging (fMRI) and concurrent transcranial DCS administered to the left inferior frontal gyrus. Effects on performance and task-related brain activity were evaluated during overt semantic word generation, a task that is negatively affected by advanced age. Task-absent resting-state fMRI (RS-fMRI) assessed atDCS-induced changes at the network level independent of performance. Twenty matched younger adults served as controls. During sham stimulation, task-related fMRI demonstrated that enhanced bilateral prefrontal activity in older adults was associated with reduced performance. RS-fMRI revealed enhanced anterior and reduced posterior functional brain connectivity. atDCS significantly improved performance in older adults up to the level of younger controls; significantly reduced task-related hyperactivity in bilateral prefrontal cortices, the anterior cingulate gyrus, and the precuneus; and induced a more "youth-like" connectivity pattern during RS-fMRI. Our results provide converging evidence from behavioral analysis and two independent functional imaging paradigms that a single session of atDCS can temporarily reverse nonbeneficial effects of aging on cognition and brain activity and connectivity. These findings may translate into novel treatments to ameliorate cognitive decline in normal aging in the future
