The combined use of computer-guided, minimally invasive, flapless corticotomy and clear aligners as a novel approach to moderate crowding: a case report
The aim of this case report was to describe an innovative orthodontic treatment
method that combined surgical and orthodontic techniques. The novel method
was used to achieve a positive result in a case of moderate crowding by
employing a computer-guided piezocision procedure followed by the use of clear
aligners. A 23-year-old woman had a malocclusion with moderate crowding. Her
periodontal indices, oral health-related quality of life (OHRQoL), and treatment
time were evaluated. The treatment included interproximal corticotomy cuts
extending through the entire thickness of the cortical layer, without a fullthickness
flap reflection. This was achieved with a three-dimensionally printed
surgical guide using computer-aided design and computer-aided manufacturing.
Orthodontic force was applied to the teeth immediately after surgery by using
clear appliances for better control of tooth movement. The total treatment time
was 8 months. The periodontal indices improved after crowding correction, but
the oral health impact profile showed a slight deterioration of OHRQoL during
the 3 days following surgery. At the 2-year retention follow-up, the stability of
treatment was excellent. The reduction in surgical time and patient discomfort,
increased periodontal safety and patient acceptability, and accurate control of
orthodontic movement without the risk of losing anchorage may encourage the
use of this combined technique in appropriate cases
Port choice by intra-regional container service operators : an application of decision-making techniques to liner services between Malaysian and other Asian ports
Intra-regional container service operators are challenged to design regular and reliable liner services connecting regional ports at the lowest cost and shortest transit time while considering customer demand. This paper focuses on the selection of ports of call in regular intra-regional container services, an under-researched part of the container shipping market. A combination of decision-making techniques (i.e. Analytical Hierarchy Process, fuzzy link-based and Evidential Reasoning) are presented to assist intra-regional container service operators in selecting ports of call. The proposed methodology is empirically applied to container services between Malaysian and other nearby Asian ports. While Port Klang is the main gateway to Malaysia, the results show that other Malaysian ports should play a more prominent role in accommodating intra-Asian container services. This research can assist maritime stakeholders in evaluating intra-regional port-to-port liner service configurations. Furthermore, the novel mix of decision-making techniques complements and enriches existing academic literature on port choice and liner service configuration
Characterization and prognostic value of mutations in exons 5 and 6 of the p53 gene in patients with colorectal cancers in central Iran
Background/Aims: We aimed to investigate the relation-ships among various mutations of the p53 gene and their protein products, histological characteristics, and disease prognosis of primary colorectal cancer in Isfahan, central Iran.
Methods: Sixty-one patients with colorectal adenocarcinoma were enrolled in the study. Mutations of the p53 gene were detected by single-stranded conformation polymorphism and DNA sequencing. The protein stability was evaluated by immunohistochemistry. Patients were followed up to 48 months.
Results: Twenty-one point mutations in exons 5 and 6 were detected in the tumor specimens of 14 patients (23%). Of those, 81% and 9.5% were missense and nonsense mutations, respectively. There were also two novel mutations in the intronic region between exons 5 and 6. In 11 mutated specimens, protein stability and protein accumulation were identified. There was a relationship between the type of mutation and protein accumulation in exons 5 and 6 of the p53 gene. The presence of the mutation was associated with an advanced stage of cancer (trend, p<0.009). Patients with mutated p53 genes had significantly lower survival rates than those with wild type p53 genes (p<0.01).
Conclusions: Mutations in exons 5 and 6 of the p53 gene are common genetic alterations in colorectal adenocarcinoma in central Iran and are associated with a poor prognosis of the disease
Comparing Results of Five Glomerular Filtration Rate-Estimating Equations in the Korean General Population. MDRD Study, Revised Lund-Malmö, and Three CKD-EPI Equations
Estimated glomerular filtration rate (eGFR) is a widely used index of kidney function. Recently, new formulas such as the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equations or the Lund-Malmö equation were introduced for assessing eGFR. We compared them with the Modification of Diet in Renal Disease (MDRD) Study equation in the Korean adult population.
METHODS: The study population comprised 1,482 individuals (median age 51 [42-59] yr, 48.9% males) who received annual physical check-ups during the year 2014. Serum creatinine (Cr) and cystatin C (CysC) were measured. We conducted a retrospective analysis using five GFR estimating equations (MDRD Study, revised Lund-Malmö, and Cr and/or CysC-based CKD-EPI equations). Reduced GFR was defined as eGFR <60 mL/min/1.73 m².
RESULTS: For the GFR category distribution, large discrepancies were observed depending on the equation used; category G1 (≥90 mL/min/1.73 m²) ranged from 7.4-81.8%. Compared with the MDRD Study equation, the other four equations overestimated GFR, and CysC-based equations showed a greater difference (-31.3 for CKD-EPI(CysC) and -20.5 for CKD-EPI(Cr-CysC)). CysC-based equations decreased the prevalence of reduced GFR by one third (9.4% in the MDRD Study and 2.4% in CKD-EPI(CysC)).
CONCLUSIONS: Our data shows that there are remarkable differences in eGFR assessment in the Korean population depending on the equation used, especially in normal or mildly decreased categories. Further prospective studies are necessary in various clinical settings
Sacrococcygeal Teratoma: A Survey by the Korean Association of Pediatric Surgeons in 2018
Purpose: The Korean Association of Pediatric Surgeons (KAPS) performed a nationwide survey on sacrococcygeal teratoma in 2018.
Methods: The authors reviewed and analyzed the clinical data of patients who had been treated for sacrococcygeal teratoma by KAPS members from 2008 to 2017.
Results: A total of 189 patients from 18 institutes were registered for the study, which was the first national survey of this disease dealing with a large number of patients in Korea. The results were discussed at the 34th annual meeting of KAPS, which was held in Jeonju on June 21–22, 2018.
Conclusions: We believe that this study could be utilized as a guideline for the treatment of sacrococcygeal teratoma to diminish pediatric surgeons' difficulties in treating this disease and thus lead to better outcomes.ope
