    Volumetric analysis of arteriovenous malformation using computed tomographic angiography

    Thesis (M.A.)--Boston UniversityAn arteriovenous malformation (AVM) is an abnormal collection of blood vessels in which arterial blood flows directly into the draining vein without the normal interposed capillaries. It is an important and growing public healthcare problem affecting millions of Americans and many more people internationally. There are several potential treatment options for the AVM, and the best treatment depends on the maximum length of nidus based on the Spetzler- Martin grading system. However, this grading system is insensitive to volume, because it was designed on the basis of two dimensional digital subtraction angiography images. Here, we report a method using computed tomographic angiography to measure the volume of AVM nidus, as a means for noninvasively assessment. The initial results show statistically significant differences between healthy and AVM subject groups in the direct comparisons of the volume (cm3) through the method we suggested (2.456 ± 1.482, 12.478 ± 5.743 and 53.963 ± 9.338 (mean ± stdev.); Normal (No AVM), Small (< 3cm), Medium (3 ~ 6 cm) respectively; P < 0.005 for all), and they also show the exponential correlation between the AVM volume and the maximum length of a nidus (trend-line: y = 4.4183e0.536x with R2 = 0.945). These results provide more accurate volumetric information. Therefore, this noninvasive imaging-based method is a promising means to measure the volume of AVM using clinically available imaging tools

    Computed tomographic morphometry of tympanic bulla shape and position in brachycephalic and mesaticephalic dog breeds

    Anatomic variations in skull morphology have been previously described for brachycephalic dogs; however there is little published information on interbreed variations in tympanic bulla morphology. This retrospective observational study aimed to (1) provide detailed descriptions of the computed tomographic (CT) morphology of tympanic bullae in a sample of dogs representing four brachycephalic breeds (Pugs, French Bulldogs, English Bulldog, and Cavalier King Charles Spaniels) versus two mesaticephalic breeds (Labrador retrievers and Jack Russell Terriers); and (2) test associations between tympanic bulla morphology and presence of middle ear effusion. Archived head CT scans for the above dog breeds were retrieved and a single observer measured tympanic bulla shape (width:height ratio), wall thickness, position relative to the temporomandibular joint, and relative volume (volume:body weight ratio). A total of 127 dogs were sampled. Cavalier King Charles Spaniels had significantly flatter tympanic bullae (greater width:height ratios) versus Pugs, English Bulldogs, Labrador retrievers, and Jack Russell terriers. French Bulldogs and Pugs had significantly more overlap between tympanic bullae and temporomandibular joints versus other breeds. All brachycephalic breeds had significantly lower tympanic bulla volume:weight ratios versus Labrador retrievers. Soft tissue attenuating material (middle ear effusion) was present in the middle ear of 48/100 (48%) of brachycephalic breeds, but no significant association was found between tympanic bulla CT measurements and presence of this material. Findings indicated that there are significant interbreed variations in tympanic bulla morphology, however no significant relationship between tympanic bulla morphology and presence of middle ear effusion could be identified

Of particular clinical interest is its ability to non-invasively evaluate coronary arteries in patients presenting to the emergency room. In acute coronary syndromes, myocardial ischemia is most often caused by atherosclerosis. We present a case of a rare cause of acute coronary syndrome, spontaneous coronary artery dissection (SCAD), which was initially evaluated with MDCT and followed by intravascular ultrasound (IVUS) and invasive coronary angiography (ICA). We discuss the findings and role of each modality with particular attention to coronary computed tomographic angiography (CCTA) in the diagnosis and management of SCAD. As the use of CCTA in the emergency department continues to rise, radiologists must become familiar with CT appearance of SCAD. CASE REPORT: We report the multidetector computed tomography (MDCT), intravascular ultrasound (IVUS), and invasive coronary angiography (ICA) findings in a case of spontaneous coronary artery dissection of the left anterior descending artery in a previously healthy 23-year-old man. The role of coronary computed tomographic angiography (CCTA) in diagnosis and management of this potentially life-threatening condition is discussed. CONCLUSIONS: In the clinical setting of acute coronary syndrome, SCAD must be a consideration, particularly in young patients without clear risk factors for coronary artery disease and in women in the peripartum period. CCTA is a very helpful diagnostic tool to diagnose the condition in a non-invasive manner and to follow up after treatment

    Tomographic image quality of rotating slat versus parallel hole-collimated SPECT

    Parallel and converging hole collimators are most frequently used in nuclear medicine. Less common is the use of rotating slat collimators for single photon emission computed tomography (SPECT). The higher photon collection efficiency, inherent to the geometry of rotating slat collimators, results in much lower noise in the data. However, plane integrals contain spatial information in only one direction, whereas line integrals provide two-dimensional information. It is not a trivial question whether the initial gain in efficiency will compensate for the lower information content in the plane integrals. Therefore, a comparison of the performance of parallel hole and rotating slat collimation is needed. This study compares SPECT with rotating slat and parallel hole collimation in combination with MLEM reconstruction with accurate system modeling and correction for scatter and attenuation. A contrast-to-noise study revealed an improvement of a factor 2-3 for hot lesions and more than a factor of 4 for cold lesion. Furthermore, a clinically relevant case of heart lesion detection is simulated for rotating slat and parallel hole collimators. In this case, rotating slat collimators outperform the traditional parallel hole collimators. We conclude that rotating slat collimators are a valuable alternative for parallel hole collimators

    INFLUENCE OF CARDIOVASCULAR RISK FACTORS ON AORTIC WALL MOTION AFTER REPAIR OF TYPE A AORTIC DISSECTION: AN ECG-GATED CT STUDY

    OBJECTIVES: To evaluate aortic shape changes during cardiac cycle with dynamic computed tomographic angiography at important thoracic aorta anatomic landmarks in patients who previously underwent ascending aorta repair because of type A dissection, and correlate aortic wall motion with several cardiovascular risk factors. METHODS: From December 2009 to December 2011, 18 patients (14 men and 4 women, mean age 64 ± 12 y.o.) with previous aortic repair, underwent ECG-gated-CT follow-up. Aortic systolic and diastolic diameter and cross-sectional area were measured at 4 levels: 1 cm proximal (level A) and 1 (B), 3 (C) and 10 cm (D) distal to the origin of left subclavian artery. Results were assessed according to presence of diabetes, hypertension, smoking and age (2 groups: ≤ 55 and ≥56 years). RESULTS: This morpho-functional evaluation of aortic distensibility demonstrated a significant influence (p<0,05) on aortic wall-motion of hypertension at level A and diabetes at level D. Smoke has a borderline significance at level C and D. No significant correlation between aortic wall motion and age was evident, being results not significantly different in two age groups. CONCLUSIONS: Smoking, diabetes and hypertension play a role in impairing aortic distensibility and previous surgical repair does not interfere with vessel wall motion. Aortic distensibility might predict wall structural alteration due to cardiovascular risk factors before they become morphologically evident. This might influence timing of surveillance, making this specifically tailored for any single subject

    Inter-observer agreement of the Coronary Artery Disease Reporting and Data System (CAD-RADS^{TM}) in patients with stable chest pain

    Purpose: To assess inter-observer variability of the Coronary Artery Disease - Reporting and Data System (CAD-RADS) for classifying the degree of coronary artery stenosis in patients with stable chest pain. Material and methods: A prospective study was conducted upon 96 patients with coronary artery disease, who underwent coronary computed tomography angiography (CTA). The images were classified using the CAD-RAD system according to the degree of stenosis, the presence of a modifier: graft (G), stent (S), vulnerable plaque (V), or non-diagnostic (n) and the associated coronary anomalies, and non-coronary cardiac and extra-cardiac findings. Image analysis was performed by two reviewers. Inter-observer agreement was assessed. Results: There was excellent inter-observer agreement for CAD-RADS (k = 0.862), at 88.5%. There was excellent agreement for CAD-RADS 0 (k = 1.0), CAD-RADS 1 (k = 0.92), CAD-RADS 3 (k = 0.808), CAD-RADS 4 (k = 0.826), and CAD-RADS 5 (k = 0.833) and good agreement for CAD-RADS 2 (k = 0.76). There was excellent agreement for modifier G (k = 1.0) and modifier S (k = 1.0), good agreement for modifier N (k = 0.79), and moderate agreement for modifier V (k = 0.59). There was excellent agreement for associated coronary artery anomalies (k = 0.845), non-coronary cardiac findings (k = 0.857), and extra-cardiac findings (k = 0.81). Conclusions: There is inter-observer agreement of CAD-RADS in categorising the degree of coronary arteries stenosis, and the modifier of the system and associated cardiac and extra-cardiac findings

    Coronary CT Angiography and 5-Year Risk of Myocardial Infarction.

    BACKGROUND: Although coronary computed tomographic angiography (CTA) improves diagnostic certainty in the assessment of patients with stable chest pain, its effect on 5-year clinical outcomes is unknown. METHODS: In an open-label, multicenter, parallel-group trial, we randomly assigned 4146 patients with stable chest pain who had been referred to a cardiology clinic for evaluation to standard care plus CTA (2073 patients) or to standard care alone (2073 patients). Investigations, treatments, and clinical outcomes were assessed over 3 to 7 years of follow-up. The primary end point was death from coronary heart disease or nonfatal myocardial infarction at 5 years. RESULTS: The median duration of follow-up was 4.8 years, which yielded 20,254 patient-years of follow-up. The 5-year rate of the primary end point was lower in the CTA group than in the standard-care group (2.3% [48 patients] vs. 3.9% [81 patients]; hazard ratio, 0.59; 95% confidence interval [CI], 0.41 to 0.84; P=0.004). Although the rates of invasive coronary angiography and coronary revascularization were higher in the CTA group than in the standard-care group in the first few months of follow-up, overall rates were similar at 5 years: invasive coronary angiography was performed in 491 patients in the CTA group and in 502 patients in the standard-care group (hazard ratio, 1.00; 95% CI, 0.88 to 1.13), and coronary revascularization was performed in 279 patients in the CTA group and in 267 in the standard-care group (hazard ratio, 1.07; 95% CI, 0.91 to 1.27). However, more preventive therapies were initiated in patients in the CTA group (odds ratio, 1.40; 95% CI, 1.19 to 1.65), as were more antianginal therapies (odds ratio, 1.27; 95% CI, 1.05 to 1.54). There were no significant between-group differences in the rates of cardiovascular or noncardiovascular deaths or deaths from any cause. CONCLUSIONS: In this trial, the use of CTA in addition to standard care in patients with stable chest pain resulted in a significantly lower rate of death from coronary heart disease or nonfatal myocardial infarction at 5 years than standard care alone, without resulting in a significantly higher rate of coronary angiography or coronary revascularization.
