De leidende gedachte binnen het vakgebied Klinische epidemiologie is dat
medisch handelen gebaseerd dient te zijn op het best beschikbare bewijs van
zijn werkzaamheid en doeltreffendheid. Dat bewijs is niet gegrond op de persoonlijke
ervaring van patiënten, noch op de klinische expertise van individuele artsen, hoe
waardevol deze elementen ook zijn. Maar het bewijs voor de effectiviteit van het
klinisch handelen is bij voorkeur afkomstig van wetenschappelijk onderzoek onder
groepen patiënten, waarin theoretische concepten worden getoetst, en waarbij
gezocht wordt naar relaties tussen het optreden en het beloop van ziekten enerzijds,
en verklarende factoren of determinanten daarvan anderzijds.|1 Deze determinanten
kunnen zowel oorzakelijke als niet-oorzakelijke factoren zijn. Vanzelfsprekend dient
onderzoek naar determinant-uitkomst relaties te worden opgezet en uitgevoerd in
overeenstemming met kwalitatief hoogstaande standaarden
Vergelijkende erfelijke en pathogenische kenmerken van hypertrofische cardiomyopathie bij de kat en de mens
Hypertrophic cardiomyopathy (HCM) is characterized by a hypertrophic, non-dilated left ventricle. With a prevalence of approximately 0.2% in humans and 15% in cats, this is one of the most common heart diseases. The disorder is often inherited but may also be acquired during life. The clinical symptoms may differ. Genotypically affected individuals can exhibit (severe) symptoms of heart failure or die suddenly, but they can also stay asymptomatic during their entire life.
More than 1400 gene polymorphisms have already been detected in thirteen genes that code for sarcomere proteins in the heart in humans. Some of them are, together with modifying genes and environmental factors, responsible for the development of HCM. To date, only three mutations have been identified in cats. Commercial tests are already available to diagnose them. These mutations are responsible for only a small fraction of the cases of HCM in cats. It is important to identify as many mutations as possible in order to develop a control strategy. There are a lot of common grounds between the human and feline forms of HCM.
There are similarities in the mode of inheritance (autosomal), the clinical appearance and the histopathological changes. Still, there is not enough known about sarcomere proteins and their mutations to extrapolate information from humans to cats and vice versa
Thiol-based cardioprotection
Acute myocardial infarction (MI) constitutes a major health problem worldwide. Over the last decades, the prognosis after MI has improved. Nevertheless, rates of death and occurrence of heart failure after MI remain substantial. It is hypothesized that reperfusion of ischemic myocardium paradoxically induces myocardial injury and experimental data suggest that this injury can contribute up to 50% of the final myocardial infarct size. Effective treatments targeting this so called ‘ischemia-reperfusion (I/R) injury’ in humans which may potentially limit infarct size and improve prognosis are currently lacking. In this thesis several compounds that might provide a therapy to target I/R injury were explored. In Part I, associations of ketone bodies and free thiols with outcomes after MI were investigated. In case that associations with outcomes are observed, this might suggest that these compounds might provide a future therapy for I/R injury. In part II of this thesis, a first attempt in humans was performed to translate the wealth of preclinical data demonstrating that hydrogen sulfide (H2S)-donating compounds provide infarct-sparing effects. In the GIPS-IV randomized clinical trial it was investigated whether sodium thiosulfate, a strong antioxidant and H2S-donating compound, reduces infarct size in 380 patients with a first myocardial infarction
