Injeksi Toksin Botulinum pada Disfonia Spasmodik Tipe Adduktor
Disfonia spasmodik adalah gangguan fonasi akibat gerakan involunter, intermiten dan ireguler dari otot-otot intrinsik laring. Hingga saat ini patofisiologinya masih belum jelas. Gejalanya yang tidak familiar di kalangan klinisi sering menyebabkan keterlambatan diagnosis. Terapi injeksi botulinum masih menjadi pilihan pertama untuk mengobati gejala disfonia spasmodik. Laporan Kasus: Seorang wanita 50 tahun mengeluh suara seperti tercekik dan terputus-putus saat bicara sejak 1,5 tahun lalu. Pemeriksaan laring tidak menemukan adanya lesi patologis. Diagnosis disfonia spasmodik tipe adduktor ditegakkan berdasarkan persepsi suara. Dilakukan injeksi botulinum pada otot tiroaritenoid, dan memberikan hasil yang memuaskan selama 3 bulan. Kesimpulan: Penegakkan diagnosis disfonia spasmodik masih merupakan tantangan untuk klinisi. Injeks
Facial expressions and dysphonia: a pilot study in dysphonic women
"Foram estudadas cinco mulheres adultas com disfonia num estudo exploratório prospetivo, com cinco
voluntárias saudáveis, sem perturbação vocal, para fins de controlo. A metodologia utilizada foi descritiva,
comparativa e correlacional. Realizou-se um protocolo de avaliação complexo incluindo: análise da expressão facial
(sobrancelhas, olhos e boca) durante a leitura oral de textos que representam diferentes emoções (neutro, alegria,
tristeza) e narrativas sobre acontecimentos de vida. Foram delineados como objetivos: 1) identificar as expressões
faciais utilizadas nos diferentes contextos emocionais; 2) verificar a relação entre o grau de disfonia e as expressões
faciais para diferentes contextos emocionais; 3) verificar se as mulheres com e sem disfonia apresentam expressões
faciais significativamente diferentes nos vários contextos emocionais. Os resultados mostraram diferenças
estisticamente significativas (p<0,05) entre os dois grupos na manifestação de alegria através da face. Os sujeitos com
perturbação vocal são menos expressivos facialmente, em todos os contextos e significativamente menos expressivos
ao nível da emoção alegria. O grau de disfonia encontra-se associado a uma diminuição de manifestações faciais de
alegria e a uma maior manifestação de traços associáveis à tristeza, sendo a transmissão emocional é bimodal.""Five adult speakers with vocal perturbation were studied in a prospective exploratory study, with five
healthy volunteers without vocal perturbation for control purposes. The methodology used was descriptive, comparative
and correlational. A complex evaluation protocol including: facial expression analysis (eyebrows, eyes and
mouth during oral reading of passages representing different emotions (neutral, joy and sadness) and talking about a
personal event were carried out. The aims of this study were: 1) identification of the facial expressions used in different
emotional contexts; 2) verification of the relation between the rank of vocal perturbation and the facial expressions
for different emotional contexts; 3) comparison between subjects to investigate whether significant differences
exist between two groups of different voice conditions on the facial expressions used in different emotional contexts.
The results show statistically significant differences (p <0.05) between the two groups in the facial expression of joy.
Subjects with vocal disorder are less facially expressive in all contexts and significantly less expressive of joy. The
degree of dysphonia is associated with a reduction of facial expressions of joy and greater manifestation of assignable
sadness features. The emotional transmission is bimodal.
Voice loss in teachers and non-teachers
PURPOSE: To investigate teachers' and non-teachers' perception regarding the implications of an eventual loss of voice. METHODS: Participated in the study 205 individuals (106 women and 99 men), 105 teachers and 100 non-teachers, with ages varying from 23 to 65 years old. The participants were asked to answer a questionnaire with four questions regarding an eventual loss of vision, hearing, voice and deambulation, and the inferred impact degree (from 0 to 4). RESULTS: For the teachers group, not being able to see caused the most negative impact (mean of 3.8), followed by not being able to walk (mean of 3.7), loss of voice (mean of 3.7) and hearing loss (mean of 3.6). For the non-teachers group, loss of sight also caused the most negative impact (mean of 3.4), followed by not being able to walk (3.0), hearing loss (2.2), and loss of voice (2.0). Regarding the impact of an eventual voice loss, the teachers group indicated negative consequences at work, in their social relationships and in their daily routines. The non-teachers group indicated negative consequences to their daily routine, at work, in their social relationships and in their emotional manifestations. CONCLUSION: The teachers showed to value their voices differently from the non-teachers group. Both groups agreed that voice loss would not bring the most negative consequences. Although teachers noticed more the impact of an eventual loss of voice than non-teachers, the feelings towards this hypothetical situation were similar in both groups.OBJETIVO: Verificar a percepção de professores e não-professores sobre as implicações de uma eventual perda de voz. MÉTODOS: Participaram 205 indivíduos sendo 105 professores e 100 não professores entre 23 a 65 anos, 106 mulheres e 99 homens. Foi aplicado um questionário contendo quatro perguntas referentes a uma eventual perda de visão, audição, voz e deambulação e o grau de impacto inferido (de 0 a 4). RESULTADOS: Para o grupo de professores, não enxergar gerou o maior impacto negativo (média de 3,8), seguido por não andar (média de 3,7), não ter voz (média de 3,7) e não ouvir (média de 3,6). Para o grupo de não-professores, não enxergar também gerou o maior impacto negativo (média de 3,4), seguido por não andar (média de 3,0); não ouvir (média de 2,2) e não ter voz (média de 2,0). Em relação ao maior impacto de uma eventual perda da voz, professores indicaram prejuízos no trabalho, relacionamento social e atividades rotineiras e, no grupo de não professores, nas atividades rotineiras, trabalho, relacionamento social e manifestações das emoções. CONCLUSÕES: Os professores valorizam sua voz de modo diverso dos não-professores e ambos os grupos avaliam a perda da voz como algo que não acarreta consequências negativas. Craniocervical posture in dysphonic women
OBJETIVO: Analisar a postura e a função da região crânio-cervical em sujeitos disfônicos. MÉTODOS: Participaram do estudo 28 mulheres (31,25±8,14 anos), divididas em dois grupos: experimental (N=16 portadoras de disfonia) e controle (N=12 clinicamente normais). As voluntárias foram submetidas à avaliação do Índice de Disfunção Crânio-Cervical (IDCC) e fotogrametria, sendo determinado o ângulo anterior formado entre a sétima vértebra cervical e o tragus, o qual corresponde à posição da cabeça no plano sagital. A análise das fotos foi realizada por três examinadores, duas vezes cada um, com intervalo de uma semana entre elas. A análise dos dados constou do teste de Shapiro-Wilk, seguido do teste t de Student, (p<0,05). RESULTADOS: Em relação à fotogrametria, não houve diferença (p=0,2565) entre os valores médios do ângulo anterior do grupo controle (50,92±5,18 graus) e do grupo experimental (49,63±5,46 graus). O IDCC mostrou que o grupo experimental apresentou disfunção crânio-cervical, sendo 37,5 % leve, 37,5% moderada e 25% severa. Já no grupo controle 100% das voluntárias apresentaram disfunção crânio-cervical leve. CONCLUSÃO: Não houve diferença na posição da cabeça entre os grupos avaliados. Porém, as mulheres disfônicas apresentaram disfunção crânio-cervical mais acentuada que as do grupo controle.Purpose: To analyze the posture and the function of the craniocervical region in women with dysphonia. METHODS: Twenty eight women participated in the study (31.25±8.14 years), divided into two groups: experimental (N=16, patients with dyphonia) and control (N=12, clinically normal). The volunteers were submitted to evaluation of the Craniocervical Dysfunction Index (CDI) and photogrammetry, determining the anterior angle formed between the seventh cervical vertebra and the tragus, which corresponds to the head position in the sagittal plane. The analysis of the photos was carried out by three examiners, twice each, with an interval of one week between them. Data analysis used the Shapiro-Wilk test, followed by the Student's t-test, (p<0,05). RESULTS: No difference was found between the mean values of the anterior angle of the control (50.92±5.18 degrees) and the experimental (49.63±5.46 degrees) groups in the photogrammetry (p=0.2565). The CDI showed that the experimental group had craniocervical dysfunction, which was mild in 37.5% of the cases, moderate, in 37.5%, and severe in 25%. In the control group, 100% of the volunteers had presented mild craniocervical dysfunction. CONCLUSION: There was no difference between the evaluated groups regarding head position. However, dysphonic women presented more severe craniocervical dysfunction than the group control

“Barnabé and his adventure”: a vocal health education project in childhood dysphonia
"Os nódulos vocais, associados etiologicamente a um comportamento disfuncional de mau uso e
abuso vocal, são os principais responsáveis pela disfonia crônica em crianças de ambos os gêneros. A
disfonia infantil pode influenciar a sedimentação das relações sociais da criança disfônica e conduzir
a processos de estigmatização e constrangimento por parte de seus pares. O presente instrumento de
Educação para a Saúde tem como principal objetivo consciencializar a criança disfônica para o uso correto
da sua voz, diminuindo o impacto biopsicossocial inerente à patologia vocal. Este instrumento, destinado essencialmente a crianças de ambos os sexos entre os sete e os nove anos de idade, e simultaneamente
aos seus pais e professores/educadores de infância, consiste num livro de literatura infantil que aborda a
problemática da voz e da patologia nodular infantil, tendo sido concretizado em dois formatos: impresso
em formato A5 e em formato digital, com narração áudio incluída. O instrumento elaborado, ideal para
a população-alvo pré-definida, consiste num excelente veículo de consciencialização da importância da
voz e da promoção de hábitos vocais salutogênicos, podendo ser usado em diversos contextos de vida
da criança (terapêutico, familiar e escolar).""Vocal nodules, etiologically associated to vocal abuse and misuse, are one of the major factors for
chronic dysphonia in childhood, in both genders. Childhood dysphonia may influence the effectiveness
of dysphonic child’s social relationships, and to lead to stigmatization and embarrassment by their peers.
The developed health education instrument principally aims to aware dysphonic children to a correct
use of their voice, decreasing biopsychosocial impact of this vocal disturbance. This instrument firstly
aimed to children of both genders, in ages between seven and nine years old, and simultaneously to their
parents and educators, consists in a child’s literature book which addresses the problem of voice and
child’s nodular pathology, and it was made in two different formats: printed in A5 format and in digital
format with audio narration included. This instrument, ideal for the target population, is a great vehicle
to raise awareness of the importance of voice and promoting salutogenic vocal habits, so it can be used
in various contexts of the child’s life (therapeutical, familiar and educational).
