    Primary vs. Secondary Antibody Deficiency: Clinical Features and Infection Outcomes of Immunoglobulin Replacement

    <div><p>Secondary antibody deficiency can occur as a result of haematological malignancies or certain medications, but not much is known about the clinical and immunological features of this group of patients as a whole. Here we describe a cohort of 167 patients with primary or secondary antibody deficiencies on immunoglobulin (Ig)-replacement treatment. The demographics, causes of immunodeficiency, diagnostic delay, clinical and laboratory features, and infection frequency were analysed retrospectively. Chemotherapy for B cell lymphoma and the use of Rituximab, corticosteroids or immunosuppressive medications were the most common causes of secondary antibody deficiency in this cohort. There was no difference in diagnostic delay or bronchiectasis between primary and secondary antibody deficiency patients, and both groups experienced disorders associated with immune dysregulation. Secondary antibody deficiency patients had similar baseline levels of serum IgG, but higher IgM and IgA, and a higher frequency of switched memory B cells than primary antibody deficiency patients. Serious and non-serious infections before and after Ig-replacement were also compared in both groups. Although secondary antibody deficiency patients had more serious infections before initiation of Ig-replacement, treatment resulted in a significant reduction of serious and non-serious infections in both primary and secondary antibody deficiency patients. Patients with secondary antibody deficiency experience similar delays in diagnosis as primary antibody deficiency patients and can also benefit from immunoglobulin-replacement treatment.</p></div

    Association between consumption of black tea and iron status in adult Africans in the North West Province: The THUSA study

    The association between black tea consumption and iron status was investigated in a sample of African adults participating in the cross-sectional THUSA (Transition and Health during Urbanization of South Africans) study in the North West Province, South Africa. Data were analysed from 1605 apparently healthy adults aged 15-65 years by demographic and FFQ, anthropometric measurements and biochemical analyses. The main outcome measures were Hb and serum ferritin concentrations. No associations were seen between black tea consumption and concentrations of serum ferritin (men P=0.059; women P=0.49) or Hb (men P=0.33; women P=0.49). Logistic regression showed that tea consumption did not significantly increase risk for iron deficiency (men: OR 1.36; 95 % CI 0.99, 1.87; women: OR 0.98; 95 % CI 0.84, 1.13) nor for iron deficiency anaemia (men: OR 1.28: 95 % Cl 0.84, 1.96; women: OR 0.93; 95 % Cl 0.78, 1.11). Prevalence of iron deficiency and iron deficiency anaemia was especially high in women: 21.6 and 14.6 %, respectively. However, the likelihood of iron deficiency and iron deficiency anaemia was not significantly explained by tea consumption in sub-populations which were assumed to be at risk for iron deficiency.     Recently, Schlage-Puchta proved super multiplicity of pp-deficiency for normal subgroups of pp-power index. We extend this result to all normal subgroups of finite index. We then use the methods of the proof to show that some groups with non-positive pp-deficiency have virtually positive pp-deficiency.     Temporary nutrient deficiency - a difficult case for diagnosis and prognosis by plant analysis

    Plant analysis aims to either detect deficiency at the time of sampling (diagnosis) or predict its occurrence at a later stage of growth (prognosis). Its use is based on the presumption that the plant nutrient status will either be constant with plant age or follow a predictable pattern of change over time after sampling. However, a period of deficiency during plant growth followed by the recovery of nutrient uptake to satisfactory rates may cause an irreversible impairment of growth which plant analysis fails to diagnose or predict. Several cases are considered, each involving a temporary deficiency of, or interruption to nutrient supply. Such cases generally involve but are not restricted to micronutrient deficiency. For example, B deficiency impairs early seedling growth when seeds low in B are planted, even on B fertilised soils. Low B concentration in seeds diagnoses the subsequent impairment of seed germination or seedling establishment: however, leaf analysis after emergence does not. Similarly, Zn deficiency impairs early growth of transplanted oilseed rape (Brassica napus L.) seedlings and eventually depresses seed yield. However, leaf analysis during crop growth fails to diagnose a Zn deficiency. Finally, temporary B deficiency induced by low vapour pressure deficit or low soil water especially during reproductive development may depress yield markedly but remain difficult to diagnose by plant analysis. Strategies for diagnosing and predicting such temporary deficiencies are discussed including the measurement of environmental parameters such as pan evaporation or rainfall and their inclusion in multi‐variate regression models of plant response to nutrients

    Study of zinc deficiency in pregnant women

    Zinc is one of the elements necessary for growth and health in human. Some evidences indicate that the zinc deficiency is one of real difficulties for the public health in developed and developing countries. Since the pregnant women are more at risk by the zinc deficiency, and this can cause many problems, in this study we tried to find out the rate of zinc deficiency in pregnant women within the region. This research project was analytical-descriptive study which was done on the 400 pregnant women whom referred to Zahedan Ghods hospital. A questionnaire was set up for each case which contained the following items, mother age, pregnancy age, numbers of deliveries, education, and consumption of iron tablet during pregnancy. The serum zinc level in each mother in this project was determined by atomic absorption technique. Prevalence of zinc deficiency among the pregnant women was 49%. Statistical analysis indicated that zinc deficiency had correlation with mother age, term of pregnancy and iron consumption. But zinc deficiency showed no correlation with numbers of deliveries and education. © Iranian Journal of Public Health. All rights reserved
