    Workflow Management for Multiple Sclerosis Patients: IT and Organization

    Patients with Multiple Sclerosis (MS) visit various healthcare providers during the course of their disease. It was suggested that IT might help to\ud orchestrate their care provision. We have applied the USE IT-tool to get insight in the relevant problems, solutions and constraints of the MS-care and the MS care providers both in the organizational and the information technological area. There is hardly a chain of healthcare, but rather, a network in which informal communication plays an important role. This informal network worked reasonably effective, but inefficient and slow. The patient himself plays a keyrole in information exchange between care-providers. Many providers were unaware of the services that other healthcare providers could give in general or did provide to a specific patient. MS patients-count is only small for most care providers. None of the interviewed patients mentioned a lack of contacts between careproviders as a problem. They thought that lack of\ud experience caused their major problems: insufficient and inadequate care. To improve care, we proposed a solution that combines a “short MS-protocol”, the\ud introduction of a central coordinator of care and a Patient Relation Management (PRM) System. This is a simple web-based application that is based on agreement by the caregivers that supports routing, tracking and tracing of a MS patient and supplies the caregivers with professional guidelines, as written down in the protocol. It is likely that we would have suggested a far more complicated ICT solution if we had only analyzed the MS-care process as such, without specific consideration of the USE IT dimensions

    International audienceThe planetary exospheres are poorly known in their outer parts, since the neutral densities are low compared with the instruments detection capabilities. The exospheric models are thus often the main source of information at such high altitudes. We present a new way to take into account analytically the additional effect of the radiation pressure on planetary exospheres. In a series of papers, we present with an Hamiltonian approach the effect of the radiation pressure on dynamical trajectories, density profiles and escaping thermal flux. Our work is a generalization of the study by Bishop and Chamberlain (1989). In this second part of our work, we present here the density profiles of atomic Hydrogen in planetary exospheres subject to the radiation pressure. We first provide the altitude profiles of ballistic particles (the dominant exospheric population in most cases), which exhibit strong asymmetries that explain the known geotail phenomenon at Earth. The radiation pressure strongly enhances the densities compared with the pure gravity case (i.e. the Chamberlain profiles), in particular at noon and midnight. We finally show the existence of an exopause that appears naturally as the external limit for bounded particles, above which all particles are escaping

    Current and forthcoming issues in the South African electricity sector

    One of the contentious issues in electricity reform is whether there are significant gains from restructuring systems that are moderately well run. South Africa's electricity system is a case in point. The sector's state-owned utility, Eskom, has been generating some of the lowest-priced electricity in the world, has largely achieved revenue adequacy, and has financed the bulk of the government's ambitious electrification program. Moreover, the key technical performance indicators of Eskom's generation plants have reached world-class levels. Yet the sector is confronted today with serious challenges. South Africa's electricity system is currently facing a tight demand/supply balance, and the distribution segment of the industry is in serious financial trouble. This paper provides a careful diagnostic assessment of the industry and identifies a range of policy and restructuring options to improve its performance. It suggests removing distribution from municipal control and privatizing it, calls for vertical and horizontal unbundling, and argues that the cost-benefit analysis of different structural options should focus on investment incentives and not just current operating efficiency.Energy Production and Transportation,Electric Power,Environment and Energy Efficiency,Energy and Environment,Infrastructure Economics

    Whereas open digital platforms drive innovation in industries, platforms in primary healthcare are mostly closed. Policy-makers have been looking for ways to open up primary healthcare platforms to stimulate collaboration and innovation and need to do so even more due to the ongoing COVID-19 crisis. Yet, there is not one way of opening up platforms in primary healthcare, just as it is unclear how different ways of openness can lead to more innovation. This paper analyzes the opportunities and challenges in realizing platform openness while examining alternative forms of openness. To answer this, we (1) conceptualize different forms of platform openness (sponsor-provider-platform-user openness), (2) examine how these forms of openness can resolve barriers to innovation, and (3) examine what challenges need to be overcome to realize that form of openness in practice, such as complexity in roles, regulations, and ICT infrastructure. The findings are relevant to structure further research on how platform openness leads to more innovations in healthcare
