    Augmented and virtual reality in spine surgery, current applications and future potentials

    BACKGROUND CONTEXT: The field of artificial intelligence (AI) is rapidly advancing, especially with recent improvements in deep learning (DL) techniques. Augmented (AR) and virtual reality (VR) are finding their place in healthcare, and spine surgery is no exception. The unique capabilities and advantages of AR and VR devices include their low cost, flexible integration with other technologies, user-friendly features and their application in navigation systems, which makes them beneficial across different aspects of spine surgery. Despite the use of AR for pedicle screw placement, targeted cervical foraminotomy, bone biopsy, osteotomy planning, and percutaneous intervention, the current applications of AR and VR in spine surgery remain limited. PURPOSE: The primary goal of this study was to provide the spine surgeons and clinical researchers with the general information about the current applications, future potentials, and accessibility of AR and VR systems in spine surgery. STUDY DESIGN/SETTING: We reviewed titles of more than 250 journal papers from google scholar and PubMed with search words: augmented reality, virtual reality, spine surgery, and orthopaedic, out of which 89 related papers were selected for abstract review. Finally, full text of 67 papers were analyzed and reviewed. METHODS: The papers were divided into four groups: technological papers, applications in surgery, applications in spine education and training, and general application in orthopaedic. A team of two reviewers performed paper reviews and a thorough web search to ensure the most updated state of the art in each of four group is captured in the review. RESULTS: In this review we discuss the current state of the art in AR and VR hardware, their preoperative applications and surgical applications in spine surgery. Finally, we discuss the future potentials of AR and VR and their integration with AI, robotic surgery, gaming, and wearables. CONCLUSIONS: AR and VR are promising technologies that will soon become part of standard of care in spine surgery. (C) 2021 Published by Elsevier Inc

    Spinal fixation procedures have the inherent risk of causing damage to vulnerable anatomical structures such as the spinal cord, nerve roots, and blood vessels. To prevent complications, several technological aids have been introduced. Surgical navigation is the most widely used, and guides the surgeon by providing the position of the surgical instruments and implants in relation to the patient anatomy based on radiographic images. Navigation can be extended by the addition of a robotic arm to replace the surgeon’s hand to increase accuracy. Another line of surgical aids is tissue sensing equipment, that recognizes different tissue types and provides a warning system built into surgical instruments. All these technologies are under continuous development and the optimal solution is yet to be found. The aim of this thesis was to study the use of Augmented Reality (AR), Virtual Reality (VR), Artificial Intelligence (AI), and tissue sensing technology in spinal navigation to improve precision and prevent surgical errors. The aim of Paper I was to develop and validate an algorithm for automatizing the intraoperative planning of pedicle screws. An AI algorithm for automatic segmentation of the spine, and screw path suggestion was developed and evaluated. In a clinical study of advanced deformity cases, the algorithm could provide correct suggestions for 86% of all pedicles—or 95%, when cases with extremely altered anatomy were excluded. Paper II evaluated the accuracy of pedicle screw placement using a novel augmented reality surgical navigation (ARSN) system, harboring the above-developed algorithm. Twenty consecutively enrolled patients, eligible for deformity correction surgery in the thoracolumbar region, were operated on using the ARSN system. In this cohort, we found a pedicle screw placement accuracy of 94%, as measured according to the Gertzbein grading scale. The primary goal of Paper III was to validate an extension of the ARSN system for placing pedicle screws using instrument tracking and VR. In a porcine cadaver model, it was demonstrated that VR instrument tracking could successfully be integrated with the ARSN system, resulting in pedicle devices placed within 1.7 ± 1.0 mm of the planed path. Paper IV examined the feasibility of a robot-guided system for semi-automated, minimally invasive, pedicle screw placement in a cadaveric model. Using the robotic arm, pedicle devices were placed within 0.94 ± 0.59 mm of the planned path. The use of a semi-automated surgical robot was feasible, providing a higher technical accuracy compared to non-robotic solutions. Paper V investigated the use of a tissue sensing technology, diffuse reflectance spectroscopy (DRS), for detecting the cortical bone boundary in vertebrae during pedicle screw insertions. The technology could accurately differentiate between cancellous and cortical bone and warn the surgeon before a cortical breach. Using machine learning models, the technology demonstrated a sensitivity of 98% [range: 94-100%] and a specificity of 98% [range: 91-100%]. In conclusion, several technological aids can be used to improve accuracy during spinal fixation procedures.     Augmented Reality: Mapping Methods and Tools for Enhancing the Human Role in Healthcare HMI

    Background: Augmented Reality (AR) represents an innovative technology to improve data visualization and strengthen the human perception. Among Human–Machine Interaction (HMI), medicine can benefit most from the adoption of these digital technologies. In this perspective, the literature on orthopedic surgery techniques based on AR was evaluated, focusing on identifying the limitations and challenges of AR-based healthcare applications, to support the research and the development of further studies. Methods: Studies published from January 2018 to December 2021 were analyzed after a comprehensive search on PubMed, Google Scholar, Scopus, IEEE Xplore, Science Direct, and Wiley Online Library databases. In order to improve the review reporting, the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines were used. Results: Authors selected sixty-two articles meeting the inclusion criteria, which were categorized according to the purpose of the study (intraoperative, training, rehabilitation) and according to the surgical procedure used. Conclusions: AR has the potential to improve orthopedic training and practice by providing an increasingly human-centered clinical approach. Further research can be addressed by this review to cover problems related to hardware limitations, lack of accurate registration and tracking systems, and absence of security protocols

    Robot and working tube-assisted invasion-controlled surgery for spinal metastases

    ObjectiveThis study aims to highlight the use of robots in surgery and that of tube-assisted minimally invasive surgery for spinal metastases, as well as elaborate on the concept of invasion-controlled surgery (ICS).Summary of backgroundMany patients with spinal metastasis cancer cannot afford serious complications when undergoing traditional open surgery because of their poor physical condition. Robots and minimally invasive technology have been introduced into the field of spine surgery and they have shown significant advantages.MethodsSix patients who underwent robot and working tube-assisted ICS for spinal metastases. Relevant demographic, medical, surgical, and postoperative data were collected from medical records and analyzed.ResultsMean operative time was 3.8 h and the mean length of the surgical incision was 4.9 cm. The mean estimated blood loss was 400 ml. The mean bedtime and hospital length of stay were 3.2 days and 6.5 days, respectively. No obvious complications were reported during treatment. The mean accuracy of screw placement was 98%. The mean time for further system treatment after surgery was 5.8 days. All patients experienced significant pain relief. The mean preoperative visual analog scale (VAS) was 7.83 points. The mean VAS at 1 day, 1 week, and 1 month after surgery were 2.83, 1.83, and 1.17 points, respectively. Frankel grade was improved in five of six patients. One patient preoperatively with Frankel grade D was the same postoperatively.ConclusionThe concept of ICS is suitable for patients with spinal metastases. Robot and working tube-assisted ICS for spinal metastases is one of the safest and most effective treatment methods

    Augmented reality for minimally invasive spinal surgery

    BackgroundAugmented reality (AR) is an emerging technology that can overlay computer graphics onto the real world and enhance visual feedback from information systems. Within the past several decades, innovations related to AR have been integrated into our daily lives; however, its application in medicine, specifically in minimally invasive spine surgery (MISS), may be most important to understand. AR navigation provides auditory and haptic feedback, which can further enhance surgeons’ capabilities and improve safety.PurposeThe purpose of this article is to address previous and current applications of AR, AR in MISS, limitations of today's technology, and future areas of innovation.MethodsA literature review related to applications of AR technology in previous and current generations was conducted.ResultsAR systems have been implemented for treatments related to spinal surgeries in recent years, and AR may be an alternative to current approaches such as traditional navigation, robotically assisted navigation, fluoroscopic guidance, and free hand. As AR is capable of projecting patient anatomy directly on the surgical field, it can eliminate concern for surgeon attention shift from the surgical field to navigated remote screens, line-of-sight interruption, and cumulative radiation exposure as the demand for MISS increases.ConclusionAR is a novel technology that can improve spinal surgery, and limitations will likely have a great impact on future technology

    Background: The application of virtual and augmented reality technologies to orthopaedic surgery training and practice aims to increase the safety and accuracy of procedures and reducing complications and costs. The purpose of this systematic review is to summarise the present literature on this topic while providing a detailed analysis of current flaws and benefits. Methods: A comprehensive search on the PubMed, Cochrane, CINAHL, and Embase database was conducted from inception to February 2021. The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines were used to improve the reporting of the review. The Cochrane Risk of Bias Tool and the Methodological Index for Non-Randomized Studies (MINORS) was used to assess the quality and potential bias of the included randomized and non-randomized control trials, respectively. Results: Virtual reality has been proven revolutionary for both resident training and preoperative planning. Thanks to augmented reality, orthopaedic surgeons could carry out procedures faster and more accurately, improving overall safety. Artificial intelligence (AI) is a promising technology with limitless potential, but, nowadays, its use in orthopaedic surgery is limited to preoperative diagnosis. Conclusions: Extended reality technologies have the potential to reform orthopaedic training and practice, providing an opportunity for unidirectional growth towards a patient-centred approach

    Development of a software system for surgical robots based on multimodal image fusion: study protocol

    BackgroundSurgical robots are gaining increasing popularity because of their capability to improve the precision of pedicle screw placement. However, current surgical robots rely on unimodal computed tomography (CT) images as baseline images, limiting their visualization to vertebral bone structures and excluding soft tissue structures such as intervertebral discs and nerves. This inherent limitation significantly restricts the applicability of surgical robots. To address this issue and further enhance the safety and accuracy of robot-assisted pedicle screw placement, this study will develop a software system for surgical robots based on multimodal image fusion. Such a system can extend the application range of surgical robots, such as surgical channel establishment, nerve decompression, and other related operations.MethodsInitially, imaging data of the patients included in the study are collected. Professional workstations are employed to establish, train, validate, and optimize algorithms for vertebral bone segmentation in CT and magnetic resonance (MR) images, intervertebral disc segmentation in MR images, nerve segmentation in MR images, and registration fusion of CT and MR images. Subsequently, a spine application model containing independent modules for vertebrae, intervertebral discs, and nerves is constructed, and a software system for surgical robots based on multimodal image fusion is designed. Finally, the software system is clinically validated.DiscussionWe will develop a software system based on multimodal image fusion for surgical robots, which can be applied to surgical access establishment, nerve decompression, and other operations not only for robot-assisted nail placement. The development of this software system is important. First, it can improve the accuracy of pedicle screw placement, percutaneous vertebroplasty, percutaneous kyphoplasty, and other surgeries. Second, it can reduce the number of fluoroscopies, shorten the operation time, and reduce surgical complications. In addition, it would be helpful to expand the application range of surgical robots by providing key imaging data for surgical robots to realize surgical channel establishment, nerve decompression, and other operations
