    Lessons from cholera response in Kathmandu Valley, Nepal

    The first recorded cholera epidemic in Nepal took place in 1823, followed by a series of epidemics occurring in the Kathmandu Valley in 1831, 1843, 1856, 1862 and 1887. Kathmandu Valley still witnesses cholera and other water borne disease cases almost every year. In 2015 and 2016, cholera cases for the valley was highest with 76 and 150 confirmed cases respectively along is with huge caseload on Acute Watery Diarrhoea (AWD). WASH Situation of the Valley especially of City Centres comes with lots of challenges owing to the complexities of urban set-up and thus the city centres are the potential hotspots in context to outbreak vulnerabilities. Based on lesson learned in 2016 cholera response, this paper presents a way forward for minimizing the occurrence of cholera and AWD which includes developing a system for cholera prevention and outbreak response

    Despite the results of the Crimean War and the Paris Peace Treaty (1856) British influence in the Principality of Serbia could not have been extended to the point of competing with that of Russia. British consuls soon perceived that the activity of Russian agents was aimed to gradually under-mine the 1856 Treaty and disturb Turkey. Neither Fonblanque, nor Lytton or Longworth were diplomatically weighty enough to impede Russian influence.Odredbama Pariskog mirovnog ugovora zaključenog 30. marta 1856. godine je između ostalog, ukinut ruski protektorat nad Kneževinom Srbijom. Srbija je ovim Ugovorom, stavljena pod kolektivni protektorat velikih sila. Međutim dugogodišnji uticaj i podrška Rusije Srbiji ostavili su duboke tragove u političkom mišljenju priličnog broja srpskih državnika i, možda još dublje urezali osećanje duga i nade u svest srpskog građanina i seljaka. Britanska politika očuvanja Turske Carevine je, pak, diktirala britanskim konzulima u Beogradu Fonblanku i Longvortu rezervisanost pa i protivljenje težnjama srpskog naroda i saradnju sa austrijskim konzulom i beogradskim pašom. Takva politika nikako nije mogla biti bliska ni srpskim političarima ni srpskom običnom čoveku pa su njihove želje, još uvek, tražile oslonac u Rusiji i dosta ređe, u Francuskoj. Zbog toga britanski konzuli u Beogradu ne samo da nisu mogli da ostvare svoj stalniji i stabilniji uticaj u srpskim političkim krugovima već su, često, bili na mukama prilikom prikupljanja osnovnih pouzdanijih informacija. Ni Fonblank ni Longvort nisu uspeli da organizuju mrežu sigurnih i sposobnih saradnika preko kojih bi suzbijali ruski uticaj u Srbiji. Nije zato neobično što su ova dvojica diplomata, ponekad, videli ruske prste i tamo gde ih nije bilo. Ipak, njihovi izveštaji nose dovoljno pouzdanih podataka za stvaranje slike srpske političke scene na kojoj se ruski uticaj, još uvek, veoma jasno vidi

    Analysis of postpartum intrauterine contraceptive devices

    Background: PPIUCDs are the only postpartum family planning  method for couples requesting a highly effective and reversible, yet long acting, family planning method that can be initiated during the immediate postpartum in lactating women. The public health benefits from PPIUCDs stemmed from the women’s increased accessibility to PPIUCDs following facility births, as PPIUCDs could be offered at health facilities after childbirth. It is an attempt to reduce unmet family planning need. The specific aims of this study were to assess the acceptability, feasibility expulsion rate and complications of PPIUCD insertion among women delivering at term in our institute who were eligible and counselled for PPIUCD.Methods: Prospective study undertaken at Dr. Vaishampayan memorial government medical college, Solapur, Maharashtra conducted from September 2015 to August 2017. All women undergoing delivery at term in this institute and not having any contraindications for postpartum IUCD insertion were counseled for PPIUCD.Results: A total 3032 were eligible for PPIUCD insertion and were counseled for the same. 1.97% women accepted PPIUCD insertion while 98.03% of them declined insertion. 1124 underwent caesarean section while 1908 had vaginal delivery. Among vaginal delivery group rate of acceptance was 1.20% while that in caesarean section group was 3.29%.Conclusions: The present study concluded that overall acceptance of PPIUCD in our institute is 1.97 %. Considering fear of complications related to IUCD and partner refusal as major causes of refusal for PPIUCD in our study, emphasis on this aspect during counseling can improve acceptance

    A COMPUTATIONAL STUDY OF PROPERTIES OF CORE-SHELL NANOWIRE HETEROSTRUCTURES USING DENSITY FUNCTIONAL THEORY

    Nanoscale systems, especially the one-dimensional semiconducting nanowires, have been the subject of immense research interests due to their potential applications in nanoelectronics and optoelectronics that demand cheaper, smaller, faster, and energy-efficient components. In particular, the core/shell nanostructures, in which the core materials are shielded by materials with larger bandgap called shell, have been shown to enhance the performance of field effect transistors (FETs), solar cells, light emitting diodes (LEDs), and thermoelectric devices due to their outstanding features like valence band offset between the core and shell, higher stability against oxidation, reduction in the surface trap states, diminished nonradiative recombination processes, and enhancement in the carrier multiplication and carrier transport processes. Incorporation of spin functionality via doping of a magnetic impurity into such core/shell (non-magnetic) nanostructures also offers additional advantages for next-generation spin-based electronic devices. Such devices are not only smaller, cost-effective, and non-volatile but also have increased data processing speed, consume less power, and assist reducing heat dissipation compared to the traditional electronic devices. In the first part of my thesis, I have studied the role of Mn and Cr dopants on the electronic structure, magnetic properties, and strain-induced magnetic phase transitions in Ge/Si core/shell nanowire heterostructures using the many-body density functional theory (DFT) approach. Subsequently, I have designed a spin filtering device using Mn-doped Ge/Si core/shell nanowire and a switching device using Cr-doped Ge/Si core/shell nanowire. To understand the spin-transport properties of these devices, I have used a real space orbital based DFT in conjunction with the single-particle non-equilibrium Green’s function approach. In the second part of my thesis, I have studied the effect of size and growth direction on the electronic structure, stability, mechanical, and optical properties for PbTe/PbS core/shell nanowires. To understand the thermodynamic stability of these complex structures, I have performed the ab-initio molecular dynamics simulations that demonstrate the possibilities of core-to-shell diffusion at room temperature in certain growth direction
