東海市における災害に強いまちづくりに向けた支援－潜在看護職のマンパワーを活用した災害支援共助システムの構築に向けて－

　There is only a limited number of local doctors in Tokai city. In order to strengthen the mutual support function to save as many lives as possible between the time immediately after a large-scale disaster and the arrival of external assistance, the authors implemented a program to develop local nurses for a mutual support system in case of a disaster in Tokai city. The subjects of the program were locally residing potential nurses who live in Tokai city. In the previous year, a test program consisted of 3 courses was implemented. Based on the program evaluation, it was revised, and the following 3 courses were newly added: "Listening to the experiences of disaster sites", "Assisting childbirth at shelters", and "Creating a network among latent nurses". The overall understanding of each course among the participants was high. However, one of the newly added courses, "Assisting childbirth at shelters", presented lower results compared with the other courses, indicating a need to repeat the training. At the "Creating a network among locally residing potential nurses", the participants carried out a group work for each residential area. Knowing their peers\u27 existence in the area created a sense of security and willingness to act at the time of a disaster and contributed to forming a foundation that will result in building a mutual disaster support system. The study results indicate that the program should be continued and further developed.　地域に在住している開業医師が少ないという特徴をもつ東海市において, 大規模災害発生直後から外部の救助者が到着するまでの間, 一人でも多くの命を救うための共助機能を強化するために, 市内在住の潜在看護職を対象に地域在住災害支援ナース育成プログラムを実施した. このプログラムは, 前年度に3 回コースとして実施した試案を評価し, 新たに, ｢災害現場での実体験を聴く｣, ｢避難所での分娩に対応する｣, ｢潜在看護職間のネットワークづくり｣を内容に加えて, 全6 回コースに改編したものである. 研修の各回の理解度では, 全体的に高い評価が得られたが, その中でも, 新たに加えた｢避難所での分娩に対応する｣の回が他の回に比べて低い結果となり, 繰り返し研修を実施する必要性を認めた. ｢潜在看護職間のネットワークづくり｣では, 参加者が居住地域別にグループワークを行い, 仲間の存在を知ったことが安心感と災害時の行動に対する意欲に結び付き, 災害

病院前周産期症例への対応に係る救急隊員訓練生の経験と学習ニーズ

病院前周産期症例に係る救急隊員訓練生の教育プログラムを検討するために，北海道内の救急隊員訓練生92人を対象に，所属地域の状況，病院前周産期症例の経験状況，および学習ニーズの内容について質問紙調査を実施した。その結果，これまでに周産期症例の救護活動に携わった経験がある者は４人（4.3％） だった。救急車に救命士が搭乗しない場合がある者は17人（18.5％），分娩を取り扱う施設がない環境で救護活動をする者は24人（26.1％）だった。病院前周産期症例に係る学習ニーズの上位は，「救急車内での出産時の対処方法」「分娩が間近に迫った産婦の観察点と判断」「異常経過の妊産婦の状態と応急処置」であった。知りたいと思っている者の割合が80％以上だった項目は，調査11項目中9項目であった。救急隊員訓練生は周産期の救護体制が整わない環境下で救護活動をする状況があり，分娩場面や異常時の対応方法についての学習ニーズを持っていることが明らかになった

北海道苫小牧市消防本部における周産期症例への病院前救護活動についての視察報告

我々は，北海道消防学校において救急隊員訓練生を対象に周産期演習を実施している。これまで分娩が急速に進行し自宅出産となった正常経過の母子事例を設定し，シミュレーション演習を行ってきた。しかし，演習後のアンケートでは，救急車内での分娩対応，異常経過となった母子の初期対応，医療機関との連絡・連携の実際などを希望する声が多い。救急隊員訓練生のニーズに即した演習を検討するため，救急現場の実際を知る必要があると考えた。そこで今回，病院前周産期症例の搬送経験のある苫小牧市消防本部を訪れ，周産期救急に係わる救急隊員が実際に経験した救急搬送事例や救急車内分娩の具体的対応を知 り，救急現場に即した周産期演習への示唆を得ることを目的に視察を行った。消防本部の救急隊員および救急救命士との意見交換から，周産期演習の企画についての示唆を得たので報告する

高校クラブ活動におけるスポーツ外傷・障害の検討

高校クラブ活動時のスポーツ外傷・障害について計694名に本人直接のアンケート調査を行った.屋外スポーツで多い外傷・障害部位は野球で肘関節,ラグビー・サッカーで足関節,陸上競技で大腿および下腿であった.外傷・障害名で最も多いのは捻挫,次いで骨折,筋挫傷,シンスプリントの順であった.屋内スポーツで多い外傷・障害部位はバスケットで足関節および膝関節,バドミントンおよびバレーボールで足関節,柔道で膝関節が多かった.外傷・障害名では,捻挫が最も多く,次いで骨折,脱臼,腰痛であった.応急処置はアイシング81.6%,安静40.2%,圧迫・固定29.8%であった.熱中症が疑われたのは526名中141名(26.8%)で,重症例は認めなかった.外傷・障害の予防には筋力や柔軟性のトレーニングやコンディショニングを行うこと,またバスケットやバドミントンでは筋損傷の予防が大切である.4人に1人は熱中症の症状を経験し,予防対策の取り組みが重要であった.とくに選手および指導員(教職員)は対策および処置について学ぶ必要があると考えられた.(著者抄録

本学では、平成20年度に「質の高い大学教育の推進プログラム(教育GP)」として文部科学省から認定を受けた「国際人道法の理念を行動化する教育の推進」に取り組んでいる。その教育プログラムの取り組みの1つとして、災害現場でも活躍できる、特に劣悪な条件下でも的確に行動できる人材に必要な知識、技術、態度、精神力を養うことをねらいとする避難訓練および災害救護訓練を実施した。訓練実施に向けた企画運営の計画や内容、実際の訓練の概要を紹介し、訓練の成果と今後の課題について報告する。This university is working on "Promotion of Education for Acting‑out the Principle of International Humanitarian Law", which was designated by the Ministry of Education, Culture, Sports, Science and Technology, as a Program for Promoting High‑Quality University Education (Education GP) in 2008. As one of the activities for the program, evacuation drills and relief operation drills were conducted this year, aiming at fostering knowledge, skills, attitudes, and spiritual strength that are required of people who can work properly at disaster sites, especially under harsh conditions.
