    Phase-locked inhibition, but not excitation, underlies hippocampal ripple oscillations in awake mice in vivo

    [[abstract]]Sharp wave-ripple (SWR) oscillations play a key role in memory consolidation during non-rapid eye movement sleep, immobility, and consummatory behavior. However, whether temporally modulated synaptic excitation or inhibition underlies the ripples is controversial. To address this question, we performed simultaneous recordings of excitatory and inhibitory postsynaptic currents (EPSCs and IPSCs) and local field potentials (LFPs) in the CA1 region of awake mice in vivo. During SWRs, inhibition dominated over excitation, with a peak conductance ratio of 4.1 ± 0.5. Furthermore, the amplitude of SWR-associated IPSCs was positively correlated with SWR magnitude, whereas that of EPSCs was not. Finally, phase analysis indicated that IPSCs were phase-locked to individual ripple cycles, whereas EPSCs were uniformly distributed in phase space. Optogenetic inhibition indicated that PV+ interneurons provided a major contribution to SWR-associated IPSCs. Thus, phasic inhibition, but not excitation, shapes SWR oscillations in the hippocampal CA1 region in vivo

    Interest-driven creator theory: towards a theory of learning design for Asia in the twenty-first century

    Asian education is known for its examination-driven orientation, with the downsides of distorting the processes of learning and teaching, diminishing students’ interest in learning, and failing to nurture twenty-first century competencies among students. As a group of Asian researchers, we have been developing Interest-Driven Creator (IDC) Theory, a design theory based on three anchored concepts, namely interest, creation, and habit. Each of these anchored concepts is represented by a loop composed of three components. In the interest loop, the three components are triggering, immersing, and extending. The components of the creation loop are imitating, combining, and staging. The habit loop consists of cuing environment, routine, and harmony. These three loops are interconnected in various ways, with their characteristics revealed by the design process. We hypothesize that technology-supported learning activities that are designed with reference to IDC Theory will enable students to develop interest in learning, be immersed in the creation process, and, by repeating this process in their daily routines, strengthen habits of creation. Furthermore, students will excel in learning performance, develop twenty-first century competencies, and become lifelong interest-driven creators. To sharpen our understanding and further the development of the theory, we need more discussion and collaborative efforts in the community. Hypotheses arising from this theory can be tested, revised, or refined by setting up and investigating IDC Theory-based experimental sites. By disseminating the framework, foundations, and practices to the various countries and regions of Asia, we hope that it will bring about compelling examples and hence a form of quality education for the twenty-first century, which is an alternative to the examination-driven education system. In this paper, we present an overall introduction to IDC Theory and its history, and discuss some of the steps for advancing it in the future

    A Novel IoT Based Positioning and Shadowing System for Dementia Training.

    A rapid increase in the number of patients with dementia, particularly memory decline or impairment, has led to the loss of self-care ability in more individuals and increases in medical and social costs. Numerous studies, and clinical service experience, have revealed that the intervention of nonpharmacological management for people with dementia is effective in delaying the degeneration caused by dementia. Due to recent rapid developments in information and communications technology, many innovative research and development and cross-domain applications have been effectively used in the dementia care environment. This study proposed a new short-term memory support and cognitive training application technology, a "positioning and shadowing system," to delay short-term memory degeneration in dementia. Training courses that integrate physical and digital technologies for the indoor location of patients with dementia were constructed using technologies such as Bluetooth Low Energy, fingerprint location algorithm, and short-range wireless communication. The Internet of Things was effectively applied to a clinical training environment for short-term memory. A pilot test verified that the results demonstrated learning effects in cognitive training and that the system can assist medical personnel in training and nursing work. Participants responded with favorable feedback regarding course satisfaction and system usability. This study can be used as a reference for future digital smart cognitive training that allows observation of the performance of patients with dementia in activities of daily living

    The Irrelevant Values Problem of Decision Tree in Medical Examination

    [[abstract]]Data mining technique is extensively used in medical application. One of key tools is the decision tree. When a decision tree is represented by a collection of rules, the antecedents of individual rules may contain irrelevant values problem. When we use this complete set of rules to medical examinations, the irrelevant values problem may cause unnecessary economic burden both to the patient and the society. We used a hypothyroid disease as an example for the study of irrelevant values problem of decision tree in medical examination. Hypothyroid disease is used to associate to the mechanism of thyroid-stimulating hormone (TSH). Physicians will combine lots of information; such as patient's clinical records, medical images, and symptoms, prior to the final diagnosis and treatment, especially surgical operation. Therefore, to avoid generating rules with irrelevant values problem, we propose a new algorithm to remove irrelevant values problem of rules in the process of converting the decision tree to rules utilizing information already present in the decision tree.     Performance Analysis of a Medical Record Exchanges Model(SCI)

    [[abstract]]Electronic medical record exchange among hospitals can provide more information for physician diagnosis and reduce costs from duplicate examinations. In this paper, we proposed and implemented a medical record exchange model. According to our study, exchange interface servers (EISs) are designed for hospitals to manage the information communication through the intra and interhospital networks linked with a medical records database. An index service center can be given responsibility for managing the EIS and publishing the addresses and public keys. The prototype system has been implemented to generate, parse, and transfer the health level seven query messages. Moreover, the system can encrypt and decrypt a message using the public-key encryption algorithm. The queuing theory is applied to evaluate the performance of our proposed model. We estimated the service time for each queue of the CPU, database, and network, and measured the response time and possible bottlenecks of the model. The capacity of the model is estimated to process the medical records of about 4000 patients/h in the 1-MB network backbone environments, which comprises about the 4% of the total outpatients in Taiwan.     The role of adherence in the relationship between self-efficacy and self-management in diabetic patients undergoing CABG in Taiwan

    [[abstract]]To determine the role of adherence and its significance in the relationship between self-efficacy and self-management of diabetic patients undergoing coronary artery bypass graft (CABG) in Taiwan. Descriptive and correlational survey design. Three outpatient clinics in Taiwan. Patients diagnosed with diabetes undergoing CABG at least 6 months before the study, 18 years of age or older, able to communicate verbally without any psychiatric problems, and with a life expectancy longer than 1 year. Self-management assessment (self-efficacy for managing disease and adherence to guidelines and medication measured on a scale of 0-8), the higher aspects of self-management (keeping appointments, taking medication properly and keeping follow-up appointments) and the lower aspects of self-management (inability to share decisions with primary physician, inability to take correct actions when symptoms worsen and inability to adapt habits to improve health). The mean score obtained for self-management among the 166 participants was 6.48, with 57 (34.3%) of them showing non-adherent behaviour. Self-efficacy accounts for 38% (R(2)=0.380, F(1,103)=63.124, p < 0.001), and 54% of good self-management was explained by self-efficacy and adherence in managing disease (R(2)=0.540, F(2,102)=56.937, p<0.001). Adherence accounts for 16% of better self-management, age and education combined account for 4.9% (R(2)=0.589, F(6.98)=23.399, p<0.001), and lifestyle items account for 5.2% (R(2)=0.641, F(14,90)=11.457, p<0.001). Disease-related variables contribute 3.4% (R(2)=0.674, F(17,87)=10.599, p<0.001). Thus self-efficacy, adherence, age, education, primary care provider and systolic pressure are considered significant predictors of self-management. With the exception of adherence, none of the variables had a statistically significant mediating effect. The results confirm strong relationships between self-efficacy, adherence and self-management, with adherence having a significant mediating effect in post-CABG patients with diabetes in Taiwan.     The Effects of Low Volume Versus High Volume Sled-Push Training on Muscular Adaptation

    Abstracts PURPOSE This study aimed to compare the effects of low-volume and high-volume sled-push resistance training on muscle strength, power, and body composition. METHODS Twenty-four college students were recruited and matched based on baseline one-repetition maximum (1-RM) into one of the three groups: 1) low volume (LV) resistance training, 2) high volume (HV) resistance training, or 3) control (CON) (n=8 per group). The LV training consisted of five single repetitions of pushing a weighted sled for 9.1 m. The HV training consisted of three sets of five repetitions of pushing a weighted sled for 9.1 m. Training consisted of three weekly workouts performed on nonconsecutive days for 6 weeks. This study utilized a pre-test and post-test design consisting of 1-RM, Wingate power test, standing long jump, vertical jump, and body composition. RESULTS After 6 weeks of training, there was a similar but significant increase in 1-RM in both training groups (pre-test: LV=226.8±14.8 kg vs. HV=217.7±19.5 kg; post-test: LV=298.5±15 kg vs. HV=286.9±16 kg, p.05). CONCLUSIONS The results suggested that low-volume resistance training was as effective as a high-volume protocol for improving muscle strength.     A MAP Overhead Aware Two-Dimensional OFDMA Burst Construction Algorithm

    Conventional orthogonal frequency division multiple access (OFDMA) burst construction methods can only support limited numbers of connections due to the map overhead and corresponding limitations in the numbers of orthogonal resources blocks, which limits the capacity of current 4G and the following 5th generation (5G) networks. This study therefore provides a novel OFDMA burst construction algorithm and enhanced burst indexing aware algorithm (EHA), which try to maximize the throughput while considering the subchannel diversity and optimizing burst indexing issues. The EHA not only allocates the subchannels with the best channel quality for each burst, but also groups the bursts to alleviate the MAP overhead. Simulation results showed that the EHA yields two times the throughput that has been achieved using previous algorithms under a heavy load. Two contributions of the EHA are: (1) the overhead of burst indexing decreases because massive numbers of connections can be accommodated by one burst; and (2) the overall throughput increases due to that one connection with large data transferring requirements can be split and distributed into several bursts and placed on the subchannels with good channel quality to adopt better modulation coding scheme (MCS), if the saved bandwidth in this burst construction is more than the increased overhead of burst indexing

    Psychometric Properties of the Health Literacy Scale Used in the Taiwan Longitudinal Study on Middle-Aged and Older People

    Health literacy, an important factor in public and personal health, is regarded as the core of patient-centered care. Older people with high health literacy are more likely to maintain a healthier lifestyle, with good control and management of chronic diseases, than those lacking or with poor health literacy. Purpose: The present study investigated the validity and reliability of the Taiwan Longitudinal Study on Aging (TLSA) Health Literacy Scale. We also evaluated the health literacy of middle-aged and older Taiwanese adults, and its probable association with health outcomes and life satisfaction. Method: We analyzed the internal consistency reliability of the nine items of the 2015 TLSA Health Literacy Scale, and their relationship with the demographic variables. Brody Instrumental Activities of Daily Living (IADL) and the Life Satisfaction Index were used for criterion validity. Moreover, exploratory factor analysis was used to examine the construct validity and to test the known-group validity. Results: The TLSA health literacy scale has good internal consistency reliability. Criterion-related validity was supported by the fact that the health literacy score was significantly correlated with the IADL and Life Satisfaction Index. Factor analysis indicated a three-factor structure. Known-group validity was supported by the results, showing that middle-aged and older people with good self-reported health status had better health literacy. Conclusions: The TLSA health literacy scale is a reliable and valid instrument for measuring health literacy in middle-aged and older people

    An Examination of COVID-19 Mitigation Efficiency among 23 Countries

    The purpose of this paper was to compare the relative efficiency of COVID-19 transmission mitigation among 23 selected countries, including 19 countries in the G20, two heavily infected countries (Iran and Spain), and two highly populous countries (Pakistan and Nigeria). The mitigation efficiency for each country was evaluated at each stage by using data envelopment analysis (DEA) tools and changes in mitigation efficiency were analyzed across stages. Pearson correlation tests were conducted between each change to examine the impact of efficiency ranks in the previous stage on subsequent stages. An indicator was developed to judge epidemic stability and was applied to practical cases involving lifting travel restrictions and restarting the economy in some countries. The results showed that Korea and Australia performed with the highest efficiency in preventing the diffusion of COVID-19 for the whole period covering 105 days since the first confirmed case, while the USA ranked at the bottom. China, Japan, Korea, and Australia were judged to have recovered from the attack of COVID-19 due to higher epidemic stability
