    ECMOve:A Mobilization Device for Extracorporeal Membrane Oxygenation Patients

    Extracorporeal membrane oxygenation (ECMO) is a temporary lifesaving treatment for critically ill patients with severe respiratory or cardiac failure. Studies demonstrated the feasibility of in-hospital mobilizing during and after ECMO treatment preventing neuromuscular weakness and impaired physical functioning. Despite more compact mobile ECMO devices, implementation of ambulatory ECMO remains labor-intensive, complex, and challenging. It requires a large multidisciplinary team to carry equipment, monitor and physically support the patient, and to provide a back-up wheelchair in case of fatigue. Moreover, there is no adequate solution to ensure the stability of the patient’s cannula and circuit management during ambulation. We developed a system contributing to improvement and innovation of current ambulatory ECMO patient programs. Our modular cart-in-cart system carries necessary ECMO equipment, features an extendable walking frame, and contains a folding seat for patient transport. An adjustable shoulder brace with lockable tubing-connectors enables safe fixation of the blood tubing. ECMOve provides safety, support, and accessibility while performing ambulatory ECMO for both patient and caregiver. Prototype evaluation in a simulated intensive care unit showed feasibility of our design, but needs to be evaluated in clinical care

    Study on a new approach to business failure and insolvency : Comparative legal analysis of the Member States’ relevant provisions and practices, Tender No. JUST/2012/JCIV/CT/0194/A4

    This report, commissioned by the European Commission DG Justice, documents a comparative study on substantive insolvency law throughout the EU. It also includes an analysis of the EC Recommendation on a new approach to business failure and insolvency and its implementation in Member States. The report\u2019s findings and analysis were based on data on various insolvency law matters which was provided by national reporters from all EU Member States and two comparator countries, the United States and Norway. The report provides a horizontal cross-cutting analysis of the data identifying areas where disparities in national law create problems that have impacts outside of national boundaries. The areas covered by the report are : the duties and liabilities of directors and director disqualification; rules on the ranking of claims/order of priorities and the conditions under which certain detrimental acts can be avoided; conditions that exist for the opening of insolvency proceedings and fast-track or standardised procedures for small and medium sized enterprises (SMEs); the extent to which the EC Recommendation on a new approach to business failure and insolvency and the provision of second chance for entrepreneurs is reflected in Member States\u2019 approach; procedures available to overindebted consumers and explaining how over-indebtedness is dealt with in the Member States including the conditions and timeframe for debt reduction and discharge; and conncected matters such as length of the procedures, involvement of creditors, publicity and cost

    Acceptance conditions in automated negotiation

    In every negotiation with a deadline, one of the negotiating parties has to accept an offer to avoid a break off. A break off is usually an undesirable outcome for both parties, therefore it is important that a negotiator employs a proficient mechanism to decide under which conditions to accept. When designing such conditions one is faced with the acceptance dilemma: accepting the current offer may be suboptimal, as better offers may still be presented. On the other hand, accepting too late may prevent an agreement from being reached, resulting in a break off with no gain for either party. Motivated by the challenges of bilateral negotiations between automated agents and by the results and insights of the automated negotiating agents competition (ANAC), we classify and compare state-of-the-art generic acceptance conditions. We focus on decoupled acceptance conditions, i.e. conditions that do not depend on the bidding strategy that is used. We performed extensive experiments to compare the performance of acceptance conditions in combination with a broad range of bidding strategies and negotiation domains. Furthermore we propose new acceptance conditions and we demonstrate that they outperform the other conditions that we study. In particular, it is shown that they outperform the standard acceptance condition of comparing the current offer with the offer the agent is ready to send out. We also provide insight in to why some conditions work better than others and investigate correlations between the properties of the negotiation environment and the efficacy of acceptance condition

    High-Sensitivity Electronic Stark Spectrometer Featuring a Laser-Driven Light Source

    We report developmental details of a high-sensitivity Stark absorption spectrometer featuring a laser-driven light source. The light source exhibits intensity fluctuations of ∼0.3% over timescales ranging from 1 min to 12 h, minimal drift (≤ 0.1%/h), and very little 1/f noise at frequencies greater than 200 Hz, which are comparable to or better than an arc-driven light source. Additional features of the spectrometer include balanced detection with multiplex sampling, which yielded lower noise in A, and constant wavelength or wavenumber (energy) spectral bandpass modes. We achieve noise amplitudes of ∼7 × 10−4 and ∼6 × 10−6 in measurements of single A and ΔA spectra (with 92 data points) taking ∼7 and ∼19 min, respectively

    Vegetative endocarditis in equids (1994--2006).

    peer reviewedBACKGROUND: Endocarditis is a rare heart condition with variable clinical expressions in equids. Risk factors for this disease are incompletely understood. OBJECTIVE: Describe risk factors for endocarditis in equids. ANIMALS: One hundred and fifty-three equids admitted to Liège University, 9 diagnosed with endocarditis and 144 free from endocarditis but admitted to the hospital with a differential diagnosis including this disease. METHODS: Retrospective case-control study. RESULTS: Equids with endocarditis were significantly younger (mean age = 4.84 +/- 5.74 years) than control equids (mean age = 10.8 +/- 7.73 years) (P = .01). No sex or breed predisposition was observed. Animals with hyperthermia (odds ratio [OR] = 24.4; confidence interval [CI] = 1.40-428), synovial distension (OR = 13.4; CI = 3.00-59.8), lameness (OR = 6.52; CI = 1.63-26.1), hyperglobulinemia (OR = 26.4; CI = 3.03-229), hypoalbuminemia (OR = 11.4; CI = 1.34-96.8), hyperfibrinogenemia (OR = 9.81; CI = 1.16-82.7), or leukocytosis (OR = 7.12; CI = 1.40-36.4) presented a significantly higher risk of having endocarditis than control horses. The presence of two of the clinical signs mentioned above significantly increased the probability of a diagnosis of endocarditis (P< or = .05). CONCLUSIONS AND CLINICAL IMPORTANCE: Age is associated with equine endocarditis. The diagnostic value of certain clinical signs and abnormalities in blood parameters in this disease are described

    Factors Influencing Family Burden of Paediatric Patients with Atopic Dermatitis: A Cross-sectional Study

    Atopic dermatitis (AD) is a chronic, recurrent inflammatory skin condition that affects about 10–20% of children and 2-15% of adults in developed countries (1). In Singapore, the prevalence among children and adolescents is 20.8% (2). Itchiness, the most common complaint, results in behavioural and social impairments among paediatric AD patients, which in turn impacts on the wellbeing of caregivers, particularly family members (3, 4). A sick child can markedly affect normal family life and the mental and social wellbeing of other family members (5). Their family members, namely parents, may experience feelings of helplessness and stress as they struggle to treat their child’s symptoms, and this can lead to feelings of guilt as they feel they are failing in their duty to care for their offspring (6). In return, family quality of life can greatly influence patient-related outcomes. Previous studies have demonstrated that paediatric atopic eczema can significantly affect family life; however, they provide limited quantitative data on the factors influencing family life and functions (6). Therefore, we conducted this study in an effort to gain in-depth insights into the family burden caused by paediatric AD, and to explore the social and clinical factors potentially impacting family life and function

    Why do people use portable air purifiers? Evidence from occupant surveys and air quality monitoring in homes in three European cities

    One of the most widely available technologies to clean the air in homes of particulate matter of less than 2.5 µm in diameter (PM2.5), known to have negative health impacts, are portable home air purifiers (HAPs). This paper presents research which (1) explored the effectiveness of HAPs in real-world conditions in 57 homes in three European cities; (2) examined if HAPs affect users’ perceptions of the indoor air quality (IAQ) at home; and (3) considered the motivations for occupants’ operation of HAPs. Results from this study found that PM2.5 concentrations in bedrooms were reduced by 45% to 69%; perceptions of IAQ were not correlated with measured high PM2.5 levels; occupants reported the HAPs to have a ‘cooling’ effect, which may explain why the predominant driver of HAP use was thermal comfort, rather than IAQ, in all three cities. The latter finding was supported by a statistically significant increase in the probability of HAP use with increasing indoor temperatures. If the operation of HAPs can be managed, or fully automated, to reflect indoor air pollution levels rather than thermal conditions, better pollutant reduction would be feasible and their use to reduce PM2.5 may help mitigate the negative health effects of exposure whilst at home

    Gene length corrected trimmed mean of M-values (GeTMM) processing of RNA-seq data performs similarly in intersample analyses while improving intrasample comparisons

    Background: Current normalization methods for RNA-sequencing data allow either for intersample comparison to identify differentially expressed (DE) genes or for intrasample comparison for the discovery and validation of gene signatures. Most studies on optimization of normalization methods typically use simulated data to validate methodologies. We describe a new method, GeTMM, which allows for both inter- and intrasample analyses with the same normalized data set. We used actual (i.e. not simulated) RNA-seq data from 263 colon cancers (no biological replicates) and used the same read count data to compare GeTMM with the most commonly used normalization methods (i.e. TMM (used by edgeR), RLE (used by DESeq2) and TPM) with respect to distributions, effect of RNA quality, subtype-classification, recurrence score, recall of DE genes and correlation to RT-qPCR data. Results: We observed a clear benefit for GeTMM and TPM with regard to intrasample comparison while GeTMM performed similar to TMM and RLE normalized data in intersample comparisons. Regarding DE genes, recall was found comparable among the normalization methods, while GeTMM showed the lowest number of false-positive DE genes. Remarkably, we observed limited detrimental effects in samples with low RNA quality. Conclusions: We show that GeTMM outperforms established methods with regard to intrasample comparison while performing equivalent with regard to intersample normalization using the same normalized data. These combined properties enhance the general usefulness of RNA-seq but also the comparability to the many array-based gene expression data in the public domain
