    In vitro assessment of six aspiration catheters using a distal protection filter

    Background: We assessed performance of 6 aspiration catheters for distal embolization using a distal protection filter in an in vitro experiment. In acute myocardial infarction, a distal protection filter is used for lesions likely to induce a distal embolism. Which aspiration cathether is most effective when used with a distal protection filter remains still unclear.Methods: A 0.5-cm3 bolus of gelatin as a model of stagnant pools of coronary plaque debris was captured in the distal protection filter and aspirated by 6 aspiration catheters. We measured and compared the length of the suspended embolus matter.Results: Among the 6 catheters evaluated, the use of the Export Advance catheter (Medtronic) resulted in significantly shorter lengths of the suspended embolus matter compared to the use of the TVAC II (Nipro), Thrombuster III SL (Kaneka), and Rebirth Pro (Goodman) catheters (p < 0.01). The residual embolus matter in all cases had drained distally to the distal protection filter when the filter was retrieved.Conclusions: The use of the Export Advance catheter showed better performance using a distal protection filter in this in vitro experiment, and its use might be more effective in preventing distal embolisms in combination with a distal protection filter

    本稿は,小学校道徳科の授業効果を高めるために,道徳的価値にかかわる自分の経験を想起し,それをもとに道徳的価値についての理解を深め,道徳的価値に対する関心を高めた上で読み物資料に取り組む授業展開を構成した。読み物資料における学習では,教材に登場する人物の立場に立って考える役割取得の視点から社会的視点に考えを広げる学習活動を展開することにより道徳性の育成を図ることを目指した。 小学校6学年76名の3クラスを対象として,道徳の内容項目B-⑺「親切,思いやり」を取り扱った。授業の成果と課題の検討のため,それぞれの時間ごとの児童授業の様子,授業アンケート結果,ワークシートの記述を検討した。結果,授業構成上の工夫と展開における一定の成果が確認された。In order to improve elementary school moral lessons, this practical study reminds children of their own experiences related to moral values, makes them understand their moral values, raises their interest in moral values, and then uses them as reading materials. I organized the lesson development to work on. In learning using story textbooks, we aimed to foster morality by developing learning activities that broaden our thinking from the perspective of role taking, which considers the positions of the characters in the story, to the acquisition of social perspectives taking. We dealt with moral content item B- (7) “kindness and compassion” for 3 classes with 6th grade elementary school names. In order to examine the results and tasks of the lesson, we examined the state of the childrenʼs lesson, the results of the lesson questionnaire, and the description of the worksheet. As a result, it was confi rmed that there were some achievements in the ingenuity and development of the lesson structure

    The present study explored the relationship between psychological deficiency in face-toface situations and ibasyo feeling in Social Networking Service universe. Feelings of Unreality Scale(Moroi et al., 2015a; Sunaga, 1996), Over-Adaptation Scale (Moroi et al., 2015b; Ishizu & Ambo, 2008), and Ibasyo Feeling in SNS Universe Scale (Kishi & Moroi, 2011; revised to measure feeling in the SNS universe) were administered to female undergraduates (N=563). Accoriding to the partial correlation analyses and the multiple regression analyses, psychological deficiency in face-to-face situations led to negative ibasyo feeling in SNS universe, while induced positive ibasyo feeling. The significance of research on the psychological mechanism underlying ibasyo feeling in SNS universe was discussed.論

    Potential application of measuring serum infliximab levels in rheumatoid arthritis management: A retrospective study based on KURAMA cohort data

    Infliximab (IFX) therapy has considerably improved the treatment of rheumatoid arthritis (RA). However, some patients still do not respond adequately to IFX therapy, or the efficacy of the treatment diminishes over time. Although previous studies have reported a relationship between serum IFX levels and therapeutic efficacy, the potential applications of IFX therapeutic drug monitoring (TDM) in clinical practice remain unclear. The purpose of this study was to investigate the potential applications of IFX TDM by analyzing a Japanese cohort database. Data were collected retrospectively from the Kyoto University Rheumatoid Arthritis Management Alliance cohort between January 1, 2011, and December 31, 2018. Serum IFX levels were measured using a liquid chromatography-tandem mass spectrometer. Out of the 311 RA patients that used IFX, 41 were eligible for the analysis. Serum IFX levels were significantly higher in responders than in non-responders. An optimal cut-off value was determined to be 0.32 μg/mL based on a receiver operating characteristic curve. At the IFX measurement point, a better therapeutic response was observed in the high IFX group (n = 32) than in the low IFX group (n = 9). Conversely, at the maximum effect point, when DAS28-ESR was the lowest between IFX introduction and measurement points, there were no differences in responder proportions between the low and high IFX groups. IFX primary ineffectiveness could be avoided with appropriate dose escalation without blood concentration measurement in clinical practice. In conclusion, IFX TDM could facilitate the identification of secondary non-responders and in turn, proper IFX use

    Crystal structure of bis(1,8-dibenzoyl-7-methoxynaphthalen-2-yl)terephthalate: Terephthalate phenylene moiety acts as bidentate hydrogen acceptor of bidirectional C-H···π non-classical hydrogen bonds

    The title compound lies about a crystallographic inversion centre located at the terephthalate moiety. The two peri-benzoylnaphthalene units having atrope chirality are also situated centrosymmetrically. In the two peri-benzoylnaphthalene moieties, two benzoyl groups are substituted at 1 and 8 carbons of the naphthalene ring in anti-orientation. Then two absolute configurations of peri-benzoylnaphthalene moieties are consequently assigned as complementary to each other, i.e., one unit has R,R-configuration and the other S,S-one, respectively. The two benzoyl groups in peri-benzoylnaphthalene moiety and the terephthalate phenylene ring are non-coplanarly located against the naphthalene ring. The dihedral angles of each benzene ring of two benzoyl groups and terephthalate unit with the naphthalene ring are 73.73 and 75.96, and 71.79°. In molecular packing, several kinds of weak interactions are responsible to induce three-dimensional molecular network. Especially, the synergetic effect realized through the bidentate hydrogen acceptor function in bidirectional C-H···π non-classical hydrogen bonds by the terephthalate phenylene ring moiety plausibly plays the determining role
