    Rest and Sleep Patterns and Activities of Residents in Long-term Care Facilities: A Descriptive Study

    Background:To facilitate healthy occupational participation and highlight the significance of rest and sleep for older adults, this study aimed to provide a description of rest and sleep patterns and activities of residents in long-term care facilities (LTCFs). Method: Secondary analysis of rest and sleep data collected from a funded R21 study was conducted. Twenty-nine residents were interviewed using the Yesterday Interview (YI) to describe their time and daily activities, including rest and sleep patterns and activities. Results: Average time spent sleeping was 10.22 (±1.77) hr and resting was 4.77 (±3.3) hr. Data were also collected on the personal and instrumental daily living activities in which residents participated during rest time and sleep preparation. Conclusion: Occupational therapists working with residents in LTCFs are well suited to incorporate rest and sleep interventions that will enhance the occupational performance of daily living activities. Future research efforts should be directed at strategies to increase the sleep health of residents in LTCFs in collaboration with other in-house professions

    Exploring Spirituality of Elders Relocating into Long-Term Care Facilities

    Background: Spirituality is recognized as an important contributor to quality of life, particularly for older adults. Yet, limited research has been conducted to examine spirituality of older adults relocating to long-term care facilities (LTCFs). The intent of this mixed methods study was to cull data from a parent study to explore different aspects of spirituality among residents newly admitted to LTCFs. Method: Qualitative and quantitative data of six participants from a parent study including interviews and scores from the religious and existential well-being sections of the Spiritual Well Being Scale (SWBS) were analyzed and triangulated. Results: Descriptive analysis of the demographic data including age, gender, ethnicity, and spirituality scores was conducted. Emerging themes from the qualitative interviews included: hope/hopelessness for the future; sense of belonging in the LTCF; contentment/discontentment with life; and personal religious beliefs. These themes, in turn, were triangulated with and supported by the SWBS scores Conclusion: The findings have the potential of developing recommendations for spiritually-based interventions to facilitate successful relocation to LTCFs. Clinical implications for occupational therapy and future research are discussed

    Optical Properties and Enhanced Photothermal Conversion Efficiency of SiO2/A-Dlc Selective Absorber Films for A Solar Energy Collector Fabricated by Unbalance Sputter

    AbstractSolar energy could become the most attractive alternative energy source. In this study we test an attractive new candidate material for solar energy collectors. It can be found that the higher the gas pressure is, the higher the sp2/sp3 area ratio, the greater the sputtering rate and the greater the optical absorption. The photothermal conversion efficiency of a SiO2 coating on the amorphous diamond-like carbon (a-DLC) selective absorber films deposited on the Cr/mirror like Al substrate is 93.2% as the film thickness of a SiO2 coating is 105nm. The coatings also increase the protective properties for a longer service life. This makes the SiO2 coated a-DLC film a promising new candidate material for solar selective absorber films. The SiO2/a-DLC selective absorber films also were deposited on the Al extrusion substrates

    KCNN2 polymorphisms and cardiac tachyarrhythmias

    Potassium calcium-activated channel subfamily N member 2 (KCNN2) encodes an integral membrane protein that forms small-conductance calcium-activated potassium (SK) channels. Recent studies in animal models show that SK channels are important in atrial and ventricular repolarization and arrhythmogenesis. However, the importance of SK channels in human arrhythmia remains unclear. The purpose of the present study was to test the association between genetic polymorphism of the SK2 channel and the occurrence of cardiac tachyarrhythmias in humans. We enrolled 327 Han Chinese, including 72 with clinically significant ventricular tachyarrhythmias (VTa) who had a history of aborted sudden cardiac death (SCD) or unexplained syncope, 98 with a history of atrial fibrillation (AF), and 144 normal controls. We genotyped 12 representative tag single nucleotide polymorphisms (SNPs) across a 141-kb genetic region containing the KCNN2 gene; these captured the full haplotype information. The rs13184658 and rs10076582 variants of KCNN2 were associated with VTa in both the additive and dominant models (odds ratio [OR] 2.89, 95% confidence interval [CI] = 1.505-5.545, P = 0.001; and OR 2.55, 95% CI = 1.428-4.566, P = 0.002, respectively). After adjustment for potential risk factors, the association remained significant. The population attributable risks of these 2 variants of VTa were 17.3% and 10.6%, respectively. One variant (rs13184658) showed weak but significant association with AF in a dominant model (OR 1.91, CI = 1.025-3.570], P = 0.042). There was a significant association between the KCNN2 variants and clinically significant VTa. These findings suggest an association between KCNN2 and VTa; it also appears that KCNN2 variants may be adjunctive markers for risk stratification in patients susceptible to SCD

    IKKβ Suppression of TSC1 Links Inflammation and Tumor Angiogenesis via the mTOR Pathway

    SummaryTNFα has recently emerged as a regulator linking inflammation to cancer pathogenesis, but the detailed cellular and molecular mechanisms underlying this link remain to be elucidated. The tuberous sclerosis 1 (TSC1)/TSC2 tumor suppressor complex serves as a repressor of the mTOR pathway, and disruption of TSC1/TSC2 complex function may contribute to tumorigenesis. Here we show that IKKβ, a major downstream kinase in the TNFα signaling pathway, physically interacts with and phosphorylates TSC1 at Ser487 and Ser511, resulting in suppression of TSC1. The IKKβ-mediated TSC1 suppression activates the mTOR pathway, enhances angiogenesis, and results in tumor development. We further find that expression of activated IKKβ is associated with TSC1 Ser511 phosphorylation and VEGF production in multiple tumor types and correlates with poor clinical outcome of breast cancer patients. Our findings identify a pathway that is critical for inflammation-mediated tumor angiogenesis and may provide a target for clinical intervention in human cancer

    Biomechanical comparison of pedicle screw fixation strength among three different screw trajectories using single vertebrae and one-level functional spinal unit

    Three key factors are responsible for the biomechanical performance of pedicle screw fixation: screw mechanical characteristics, bone quality and insertion techniques. To the best of the authors’ knowledge, no study has directly compared the biomechanical performance among three trajectories, i.e., the traditional trajectory (TT), modified trajectory (MT) and cortical bone trajectory (CBT), in a porcine model. This study compared the pullout strength and insertion torque of three trajectory methods in single vertebrae, the pullout strength and fixation stiffness including flexion, extension, and lateral bending in a one-level instrumented functional spinal unit (FSU) that mimics the in vivo configuration were clarified. A total of 18 single vertebrae and 18 FSUs were randomly assigned into three screw insertion methods (n = 6 in each trajectory group). In the TT group, the screw converged from its entry point, passed completely inside the pedicle, was parallel to the superior endplate, was located in the superior third of the vertebral body and reached to at least the anterior third of the vertebral body. In the MT group, the convergent angle was similar to that of the TT method but directed caudally to the anterior inferior margin of the vertebral body. The results of insertion torque and pullout strength in single vertebrae were analyzed; in addition, the stiffness and pullout strength in the one-level FSU were also investigated. This study demonstrated that, in single vertebrae, the insertion torque was significantly higher in CBT groups than in TT and MT groups (p < 0.05). The maximal pullout strength was significantly higher in MT groups than in TT and CBT groups (p < 0.05). There was no significant difference in stiffness in the three motions among all groups. The maximal pullout strength in FSUs of MT and CBT groups were significantly higher than the TT groups (p < 0.05). We concluded that either MT or CBT provides better biomechanical performance than TT in single vertebrae or FSUs. The lack of significance of stiffness in FSUs among three methods suggested that MT or CBT could be a reasonable alternative to TT if the traditional trajectory was not feasible

    The Efficacy of Endoscopic Papillary Balloon Dilation for Patients with Acute Biliary Pancreatitis

    Background. No study investigated the efficacy and safety of endoscopic papillary balloon dilation (EPBD) for the treatment of acute biliary pancreatitis (ABP). Method. We retrospectively reviewed the effects of EPBD on patients with ABP from February 2003 to December 2012. The general data, findings of image studies, details of the procedure, and outcomes after EPBD were analyzed. Result. Total 183 patients (male/female: 110/73) were enrolled. The mean age was 65.9 years. Among them, 155 patients had mild pancreatitis. The meantime from admission to EPBD was 3.3 days. Cholangiogram revealed filling defects inside the common bile duct (CBD) in 149 patients. The mean dilating balloon size was 10.5 mm and mean duration of the dilating procedure was 4.3 minutes. Overall, 124 patients had gross stones retrieved from CBD. Four (2.2%) adverse events and 2 (1.1%) intraprocedure bleeding incidents but no procedure-related mortality were noted. Bilirubin and amylase levels significantly decreased after EPBD. On average, patients resumed oral intake within 1.4 days. The clinical parameters and outcomes were similar in patients with different severity of pancreatitis. Conclusion. EPBD can be effective and safe for the treatment of ABP, even in patients presenting with severe disease

    Cross-Regional Data Initiative for the Assessment and Development of Treatment for Neurological and Mental Disorders

    Purpose: To describe and categorize detailed components of databases in the Neurological and Mental Health Global Epidemiology Network (NeuroGEN). Methods: An online 132-item questionnaire was sent to key researchers and data custodians of NeuroGEN in North America, Europe, Asia and Oceania. From the responses, we assessed data characteristics including population coverage, data follow-up, clinical information, validity of diagnoses, medication use and data latency. We also evaluated the possibility of conversion into a common data model (CDM) to implement a federated network approach. Moreover, we used radar charts to visualize the data capacity assessments, based on different perspectives. Results: The results indicated that the 15 databases covered approximately 320 million individuals, included in 7 nationwide claims databases from Australia, Finland, South Korea, Taiwan and the US, 6 population-based electronic health record databases from Hong Kong, Scotland, Taiwan, the Netherlands and the UK, and 2 biomedical databases from Taiwan and the UK. Conclusion: The 15 databases showed good potential for a federated network approach using a common data model. Our study provided publicly accessible information on these databases for those seeking to employ real-world data to facilitate current assessment and future development of treatments for neurological and mental disorders.</p
