Re-Offending by released Terrorist Prisoners:Separating Hype from Reality
Recent cases of attacks by released terrorist prisoners highlight issues around the risk of re-offending posed by former terrorist prisoners. What are the appropriate processes and systems for managing and risk assessing such individuals, and to what extent is rehabilitation possible in the context of terrorist offending? This Policy Brief will explore these and related issues to help inform wider discussion and debates on appropriate policy in this area. In this Policy Brief, the authors critically analyse the definition of ‘recidivism’, and demonstrate the need for a concrete operational definition before one is able to truly analyse recidivist activity. Following this, the authors discuss terrorist recidivism in a range of international contexts, ranging from Northern Ireland to Sri Lanka, the United States to Israel. By taking this broader perspective it allows the reader to gain a greater understanding of what factors related to recidivism rates may be context-specific, and which are universal
Programme Costing of a Physical Activity Programme in Primary Prevention: Should the Costs of Health Asset Assessment and Participatory Programme Development Count?
This analysis aims to discuss the implications of the “health asset concept”, introduced by the WHO, and the “investment for health model” requiring a “participatory approach” of cooperative programme development applied on a physical activity programme for socially disadvantaged women and to demonstrate the related costing issues as well as the relevant decision context.
The costs of programme implementation amounted to €48,700. Adding the costs for developing the programme design of €48,800 results in total costs of €97,500; adding on top of that the costs of asset assessment running to €35,600 would total €133,100. These four different cost figures match four different types of potentially relevant decisions contexts. Depending on the decision context the total costs, and hence the incremental cost-effectiveness ratio of a health promotion intervention, could differ considerably. Therefore, a detailed cost assessment and the identification of the decision context are of crucial importance
Community-based health efforts for the prevention of falls in the elderly
Falls are a major public health problem in the elderly population. The associated health care cost is great. It has therefore become an important public health matter to evaluate those interventions that might be effective in reducing the risk of falls. Risk factors that predict an increased risk of falling are described. We discuss interventions that can be employed in the community to reduce the risk of falls and associated injuries by discipline, including physiotherapy, occupational therapy, and physician-led interventions. We also discuss the cost-effectiveness of such interventions
Cues and the optimal timing of activities under environmental changes
Organisms time activities by using environmental cues to forecast the future availability of important resources. Presently, there is limited understanding of the relationships between cues and optimal timing, and especially about how this relationship will be affected by environmental changes. We develop a general model to explore the relation between a cue and the optimal timing of an important life history activity. The model quantifies the fitness loss for organisms failing to time behaviours optimally. We decompose the immediate change in fitness resulting from environmental changes into a component that is due to changes in the predictive power of the cue and a component that derives from the mismatch of the old response to the cue to the new environmental conditions. Our results show that consequences may range from negative, neutral to positive and are highly dependent on how cue and optimal timing and their relation are specifically affected by environmental changes
Capturing drifting species and molecules—Lessons learned from integrated approaches to assess marine metazoan diversity in highly dynamic waters
Marine community diversity surveys require a reliable assessment to estimate ecosystem functions and their dynamics. For these, non-invasive environmental DNA (eDNA) metabarcoding is increasingly applied in zoological studies to complement or even replace traditional morphological identification methods. However, uncertainties remain about the accuracy of the diversity detected with eDNA to capture the actual diversity in the field. Here, we validate the reliability of eDNA metabarcoding in identifying metazoan biodiversity in highly dynamic marine waters of the North Sea. We analyzed biodiversity from water (eDNA) and zooplankton samples with cytochrome c oxidase subunit 1 (COI) and 18S rRNA (18S) metabarcoding at Helgoland Roads and validated the optimal molecular resolution by morphological and molecular zooplankton identification (metabarcoding) with the result of merely a few false-negative detections. eDNA and zooplankton metabarcoding resolved 354 species from all major and in total 16 metazoan phyla. This molecular genetic species inventory overlapped by 95.9% (COI) and 81.9% (18S) with published inventories of local, morphologically identified species, among them neozoa and rediscovered species. Even though half of all species were detected by both eDNA and zooplankton metabarcoding, the methods differed significantly in their detected diversity. eDNA metabarcoding performed very well in cnidarians and annelids, whereas zooplankton metabarcoding identified higher numbers of fish and malacostraca. Species assemblages significantly differed between the individual sampling events and the cumulative number of identified species increased steadily over the sampling period and did not reach saturation. About a third of the species were detected only once while a core community of 22 species was identified continuously. Our study confirms eDNA metabarcoding to be a powerful tool to identify and analyze North Sea fauna in highly dynamic waters and we recommend investing in high sampling efforts by repetitive sampling and replication using at least 0.45 μm filters to increase filtration volume
Transcriptional repression by methylation: cooperativity between a CpG cluster in the promoter and remote CpG-rich regions
AbstractCytosine methylation of binding sites for transcription factors is a straightforward mechanism to prevent transcription, while data on an indirect mechanism, by methylation outside of the factor binding sites, are still scarce. We have studied the latter effect using a model promoter construct. For this, a 69 bp G + C rich DNA segment with a cluster of 14 CpG sites was inserted between upstream lexA sites and the TATA box. Transcription was measured in transient transfection assays with lexA-VP16 as an activating factor. When the entire plasmid was methylated at all CpGs before transfection, transcription was blocked (to 3% residual activity), whereas transcription was only mildly inhibited (to 60%) by methylation of a control plasmid that lacked the 69 bp CpG cluster. However, the effect could not simply be attributed to methylation of the CpG cluster: neither a methylated CpG cluster in an otherwise methylation-free reporter gene plasmid, nor the methylated plasmid with an unmethylated CpG cluster, inhibited transcription considerably (69% and 44% remaining activity, respectively). The data presented here suggest that a minimal length of methylated DNA in the promoter is required for repression, and imply that concomitant methylation of CpGs in the promoter region and in remote sequences can cooperatively block transcription, without the need to methylate any binding sites for transcription factors. We also note that the cooperation for a negative effect described here bears an analogy to transcriptional activation, where a promoter often cooperates with a remote enhancer

Mivacurium Sensitivity at the Adductor Pollicis and Hand Grip Muscles: Differences Between Males and Females
Background: Males lose more handgrip strength (HGS) than females when adductor pollicis (AP) TOF ratio decreases. The reason is unclear.
Objectives: The primary aim of this study was to explore gender-related differences in neuromuscular sensitivity to mivacurium. As a secondary aim, clearance of mivacurium was determined.
Methods: In 10 healthy males and 10 healthy females, constant-rate infusions of mivacurium were administered to obtain three different levels of stable neuromuscular block (normalized acceleromyography AP TOF ratio 80, 60, and 40%) in each study subject. Arterial blood samples were collected to determine mivacurium plasma concentrations. The HGS was measured every five minutes. A Hill equation was fitted to data on mivacurium concentration versus normalized AP TOF ratio and HGS to determine drug concentrations associated with 50% maximum effect (C50 AP TOF ratio and C50 HGS). Differences within and between genders were tested with the parametric t-test. Clearance of mivacurium was calculated at each block level as the ratio between drug infusion rate and concentration. Gender-related differences in relationships between AP TOF ratio and HGS, mivacurium infusion rates, and mivacurium plasma concentrations were determined with linear mixed-models.
Results: The C50 AP TOF ratio was significantly greater than C50HGSin males, yet not in females. Mivacurium infusion rates, needed to maintain stable neuromuscular blocks, were significantly greater in males, while clearance was similar between genders. Males lost significantly more HGS with decreasing AP TOF ratio than females, both in absolute (kg) and relative (percentage of baseline) terms.
Conclusions: In males, yet not in females, the AP was significantly less sensitive to the effect of mivacurium than the muscles involved in the handgrip function. Thisfindingexplainswhyhandgripstrength decreasesmorein males than females with decreasing AP TOF ratio during the mivacurium block
An ancient pathway combining carbon dioxide fixation with the generation and utilization of a sodium ion gradient for ATP synthesis
Synthesis of acetate from carbon dioxide and molecular hydrogen is considered to be the first carbon assimilation pathway on earth. It combines carbon dioxide fixation into acetyl-CoA with the production of ATP via an energized cell membrane. How the pathway is coupled with the net synthesis of ATP has been an enigma. The anaerobic, acetogenic bacterium Acetobacterium woodii uses an ancient version of this pathway without cytochromes and quinones. It generates a sodium ion potential across the cell membrane by the sodium-motive ferredoxin:NAD oxidoreductase (Rnf). The genome sequence of A. woodii solves the enigma: it uncovers Rnf as the only ion-motive enzyme coupled to the pathway and unravels a metabolism designed to produce reduced ferredoxin and overcome energetic barriers by virtue of electron-bifurcating, soluble enzymes
Congenital and surgically acquired Wolff-Parkinson-White syndrome in patients with tricuspid atresia
ObjectivesThere are reports associating an increased incidence of Wolff-Parkinson-White syndrome with tricuspid atresia. Here we report on electrophysiologic studies in patients with tricuspid atresia and Wolff-Parkinson-White syndrome after the Fontan operation. In these patients the atrial arrhythmia often seen in patients undergoing the Fontan operation triggered atrioventricular re-entrant tachycardia or caused life-threatening arrhythmias.MethodsFive patients with tricuspid atresia after palliation with a modified Fontan operation (atrioinfundibular connections) and Wolff-Parkinson-White syndrome are presented.ResultsFour of these patients had symptomatic paroxysmal orthodromic atrioventricular re-entrant tachycardia and a history of syncope; one of them additionally had atrial flutter with 2:1 conduction to the ventricle. A fifth patient presented with a life-threatening broad-complex tachycardia. In electrophysiologic studies an accessory pathway was localized in the right septal area in 3 patients. In 2 patients the accessory atrioventricular pathways were created by means of surgical intervention, connecting the right atrial appendage to the right ventricular outflow tract. All patients could be managed successfully by means of catheter ablation.ConclusionsIn patients with tricuspid atresia, there are congenital and surgically acquired accessory pathways responsible for the increased rate of Wolff-Parkinson-White syndrome. Both types of accessory pathways can and should be treated by means of catheter ablation because atrial arrhythmia often seen in patients undergoing the Fontan operation can trigger atrioventricular re-entrant tachycardia or cause life-threatening tachycardia. Congenital accessory pathways should be excluded carefully before surgical intervention for total cavopulmonary anastomosis in patients with tricuspid atresia
