    The Subsurface Geology and Landscape Evolution of the Volturno Coastal Plain, Italy: Interplay between Tectonics and Sea-Level Changes during the Quaternary

    The Volturno alluvial-coastal plain is a relevant feature of the Tyrrhenian side of Southern Italy. Its plan-view squared shape is due to Pliocene-Quaternary block-faulting of the western flank of the south-Apennines chain. On the basis of the stratigraphic analysis of almost 700 borehole logs and new geomorphological survey, an accurate paleoenvironmental reconstruction before and after the Campania Ignimbrite (CI; about 40 ky) eruption is here presented. Tectonics and eustatic forcing have been both taken into account to completely picture the evolution of the coastal plain during Late Quaternary times. The upper Pleistocene-Holocene infill of the Volturno plain has been here re-organized in a new stratigraphic framework, which includes seven depositional units. Structural analysis showed that two sets of faults displaced the CI, so accounting for recent tectonic activity. Yet Late Quaternary tectonics is rather mild, as evidenced by the decametric vertical separations operated by those faults. The average slip rate, which would represent the tectonic subsidence rate of the plain, is about 0.5 mm/year. A grid of cross sections shows the stratigraphic architecture which resulted from interactions among eustatic changes, tectonics and sedimentary input variations. On the basis of boreholes analysis, the trend of the CI roof was reconstructed. An asymmetrical shape of its ancient morphology - with a steeper slope toward the north-west border - and the lack of coincidence between the present course of the Volturno River and the main buried bedrock incision, are significant achievements of this study. Finally, the morpho-evolutionary path of the Volturno plain has been discussed

    Comparison of Baseline Characteristics and Outcomes in Men Versus Women With Aortic Stenosis Undergoing Transcatheter Aortic Valve Implantation

    Female gender has been linked to increased risk of adverse events after surgical aortic valve replacement; however, the evidence regarding the role of gender differences on clinical outcomes in patients who underwent transcatheter aortic valve implantation (TAVI) is still debated. This retrospective study included 910 consecutive patients with severe, symptomatic aortic stenosis who underwent TAVI in 2 institutions from January 2012 to July 2016. The primary end point was all-cause mortality at 1 year after TAVI in women versus men. Women had a higher incidence of in-hospital vascular complications (7.8% vs 4.1%) and major or life-threatening bleeding (4.0% vs 1.6%) than men. At 1 year, women showed a lower mortality rate than men (7.0% vs 12.7%, adjusted hazard ratio [HR] 0.42, 95% confidence interval [CI] [0.23 to 0.76], p = 0.004). When stratifying by specific subgroups of interest, the survival benefit in women persisted in (1) patients with a Society of Thoracic Surgery risk score ≤ 8 (adjusted HR 0.35, 95% CI [0.14 to 0.88], p = 0.026); (2) patients treated with first-generation devices (adjusted HR 0.46, 95% CI [0.24 to 0.86], p = 0.016); and (3) patients treated with balloon-expandable valves (adjusted HR 0.40, 95% CI [0.19 to 0.86], p = 0.019). In conclusion, in this large patient cohort, women had lower 1-year mortality after TAVI than men, particularly with an STS score ≤ 8, or treated with first-generation and balloon-expandable devices

    Hyperthermic intraperitoneal chemotherapy (HIPEC) for ovarian cancer recurrence: systematic review and meta-analysis.

    Background: Ovarian cancer is the first cause of death among gynecological malignancies with a high incidence of recurrence. Different treatment options are suitable to prolong the survival rate of these patients. Over the last years, one of the most intriguing methods, adopted in different oncologic centers worldwide, is the hyperthermic intraperitoneal chemotherapy (HIPEC). Methods: A meta-analysis was performed to value the role of HIPEC for ovarian cancer recurrence. Search strategy was conducted with a combination of the following keywords: "ovarian recurrence, ovarian cancer recurrence, peritoneal cancer recurrence, ovarian recurrence AND HIPEC, secondary cytoreduction HIPEC". Seven studies were selected for analysis. Results: In women with recurrent ovarian cancer (ROC), the use of HIPEC in addition to cytoreductive surgery and chemotherapy significantly improved 1-year overall survival (OS) when compared to protocols without HIPEC (OR 2.42; 95% CI, 1.06-5.56; P=0.04; I2=4%). The improvement in OS was maintained significant also after 2, 3 and 5 years respectively (OR 3.33; 95% CI, 1.81-6.10; P<0.01; I2=0%), (OR 4.22; 95% CI, 2.07-8.60; P<0.01; I2=52%), (OR 5.17; 95% CI, 1.40-19.09; P=0.01; I2=82%). Conclusions: HIPEC seems to have an effective role to prolong survival in patients affected by ROC

    Ion channels in the pathogenesis of endometriosis: A cutting-edge point of view

    Background: Ion channels play a crucial role in many physiological processes. Several subtypes are expressed in the endometrium. Endometriosis is strictly correlated to estrogens and it is evident that expression and functionality of different ion channels are estrogen-dependent, fluctuating between the menstrual phases. However, their relationship with endometriosis is still unclear. Objective: To summarize the available literature data about the role of ion channels in the etiopathogenesis of endometriosis. Methods: A search on PubMed and Medline databases was performed from inception to November 2019. Results: Cystic fibrosis transmembrane conductance regulator (CFTR), transient receptor potentials (TRPs), aquaporins (AQPs), and chloride channel (ClC)-3 expression and activity were analyzed. CFTR expression changed during the menstrual phases and was enhanced in endometriosis samples; its overexpression promoted endometrial cell proliferation, migration, and invasion throughout nuclear factor kappa-light-chain-enhancer of activated B cells-urokinase plasminogen activator receptor (NFκB-uPAR) signaling pathway. No connection between TRPs and the pathogenesis of endometriosis was found. AQP5 activity was estrogen-increased and, through phosphatidylinositol-3-kinase and protein kinase B (PI3K/AKT), helped in vivo implantation of ectopic endometrium. In vitro, AQP9 participated in extracellular signal-regulated kinases/p38 mitogen-activated protein kinase (ERK/p38 MAPK) pathway and helped migration and invasion stimulating matrix metalloproteinase (MMP)2 and MMP9. ClC-3 was also overexpressed in ectopic endometrium and upregulated MMP9. Conclusion: Available evidence suggests a pivotal role of CFTR, AQPs, and ClC-3 in endometriosis etiopathogenesis. However, data obtained are not sufficient to establish a direct role of ion channels in the etiology of the disease. Further studies are needed to clarify this relationship

    Mitigating Errors on Superconducting Quantum Processors through Fuzzy Clustering

    Quantum utility has been severely limited in superconducting quantum hardware until now by the modest number of qubits and the relatively high level of control and readout errors, due to the intentional coupling with the external environment required for manipulation and readout of the qubit states. Practical applications in the Noisy Intermediate Scale Quantum (NISQ) era rely on Quantum Error Mitigation (QEM) techniques, which are able to improve the accuracy of the expectation values of quantum observables by implementing classical post-processing analysis from an ensemble of repeated noisy quantum circuit runs. In this work, we focus on a recent QEM technique that uses Fuzzy C-Means (FCM) clustering to specifically identify measurement error patterns. For the first time, we report a proof-of-principle validation of the technique on a 2-qubit register, obtained as a subset of a real NISQ 5-qubit superconducting quantum processor based on transmon qubits. We demonstrate that the FCM-based QEM technique allows for reasonable improvement of the expectation values of single- and two-qubit gates based quantum circuits, without necessarily invoking state-of-the-art coherence, gate, and readout fidelities

    A new hexapeptide from the leader peptide of rMnSOD enters cells through the oestrogen receptor to deliver therapeutic molecules

    A 24-amino acid leader peptide of a new human recombinant manganese superoxide dismutase can enter cells and carry molecules. Here, we demonstrated that six of the 24 amino acids penetrate cells through a particular gate represented by a specific amino acid sequence of the oestrogen receptor (ER). We analysed the internalization of the synthetic hexapeptide and the cytotoxic activity of the hexapeptide conjugated to cisplatin on a cell line panel. In most cell lines, the hexapeptide delivered an amount of cisplatin that was 2 to 8 times greater than that released by cisplatin when the drug was used alone. This increased delivery increases the therapeutic index of cisplatin and reduces side effects caused by a high dosage or long-term treatment times. We may consider this hexapeptide a new molecular carrier to deliver molecules with therapeutic activity into ER+ cells for diagnostic purposes and clinical or immune therapy

    In this work, we tested the reliability of two different methods of automated landform classification (ACL) in three geological domains of the southern Italian chain with contrasting morphological features. ACL maps deriving from the TPI-based (topographic position index) algorithm are strictly dependent to the search input parameters and they are not able to fully capture landforms of different size. Geomorphons-based classification has shown a higher potential and can represent a powerful method of ACL, although it should be improved with the introduction of additional DEM-based parameters for the extraction of landform classe

    Second messenger cyclic adenosine monophosphate (cAMP) has been found to regulate multiple mitochondrial functions, including respiration, dynamics, reactive oxygen species production, cell survival and death through the activation of cAMP-dependent protein kinase A (PKA) and other effectors. Several members of the large family of A kinase anchor proteins (AKAPs) have been previously shown to locally amplify cAMP/PKA signaling to mitochondria, promoting the assembly of signalosomes, regulating multiple cardiac functions under both physiological and pathological conditions. In this review, we will discuss roles and regulation of major mitochondria-targeted AKAPs, along with opportunities and challenges to modulate their functions for translational purposes in the cardiovascular system

European Journal of Heart Failure published by John Wiley & Sons Ltd on behalf of European Society of Cardiology.Pulmonary hypertension (PH) associated with left heart failure (LHF) (PH-LHF) is one of the most common causes of PH. It directly contributes to symptoms and reduced functional capacity and negatively affects right heart function, ultimately leading to a poor prognosis. There are no specific treatments for PH-LHF, despite the high number of drugs tested so far. This scientific document addresses the main knowledge gaps in PH-LHF with emphasis on pathophysiology and clinical trials. Key identified issues include better understanding of the role of pulmonary venous versus arteriolar remodelling, multidimensional phenotyping to recognize patient subgroups positioned to respond to different therapies, and conduct of rigorous pre-clinical studies combining small and large animal models. Advancements in these areas are expected to better inform the design of clinical trials and extend treatment options beyond those effective in pulmonary arterial hypertension. Enrichment strategies, endpoint assessments, and thorough haemodynamic studies, both at rest and during exercise, are proposed to play primary roles to optimize early-stage development of candidate therapies for PH-LHF
