Multiple Food Allergy
A alergia alimentar múltipla é uma resposta imune adversa a mais do que um alimento.
Quando uma criança é alérgica a vários alimentos, o quadro clínico pode ser grave e a evicção dos alergénios implicados leva a restrições dietéticas importantes.
Descrevemos o caso de uma criança de quatro anos com eczema atópico grave e alergia
alimentar múltipla. Apresentou Imunoglobulina (Ig)E específica positiva aos oito meses para leite, trigo, centeio, cevada, glúten, soja, ovo (gema e clara) e iniciou evicção destes alimentos. Aos 20 meses tentou introduzir trigo, suspendendo após agravamento do eczema atópico. Aos três anos teve reacção anafiláctica após contacto com queijo. Aos quatro anos mantém evicção de leite, ovo, trigo, glúten, cevada e centeio.
As restrições alimentares nestes doentes são grandes, havendo necessidade de controlo
rigoroso dos alimentos ingeridos para evicção de reacções adversas e prevenção de carências nutricionais. É importante a abordagem multidisciplinar

Ethics and Design in the Brazilian Context
Often driven by practical and immediate requirements, more and more people are incorporating technology into a variety of aspects of their lives, often without reflecting on the consequences of using them. On the other hand, studies on interactive system development that lead to behavioral change have been gaining ground on the agenda of large HCI conferences. This movement brings to the forefront the fundamental issues of ethics in design and technology use. A designer’s intentions, when directing certain actions or behaviors, are not always explicit or desired by the stakeholders affected by the use of the technology. Systems that induce an undesired purchase, or even those that use conditioning strategies to cause a behavioral change are examples of such intentions. The challenge proposed is therefore about the relationship between design and personal freedom in a way that these technology users do not become victims, either passively or submissively, of the effects of its use. This advance allows for the redefinition of the relationship between man and technology, and the application of new forms of designing and developing interactive systems that take into account the ethical aspects of this relationship
O Mundo da Urticária, Com e Sem Alergia
A urticária é uma entidade clínica comum a um grupo heterogéneo de doenças, devendo ser entendida como um sintoma e não como uma doença. As lesões cutâneas características são pápulas eritematosas ou com zona central pálida e eritema circundante, pruriginosas, que desaparecem à digitopressão e regridem completamente em menos de 24 horas. Embora na maioria dos casos se apresente de forma isolada, pode acompanhar-se de angioedema.
A urticária pode atingir até 25% da população em qualquer momento da vida. A forma aguda (duração inferior a 6 semanas),
mais frequente em crianças, é mais prevalente que a forma crónica.
Nos casos em que a etiologia é identificada, as infecções, os alimentos e os fármacos são as causas mais frequentes. As picadas
de insectos e as doenças sistémicas mais raramente estão implicadas. Na urticária crónica é muito raro o envolvimento
de fenómenos de alergia IgE mediada.
O recurso a exames auxiliares de diagnóstico deve ser criterioso e sustentado por dados clínicos relevantes, não sendo recomendável
a requisição de avaliações analíticas exaustivas. Na sua maioria, os episódios de urticária são de curta duração e resolução espontânea. A urticária aguda tem uma duração
média de 7 dias; já a forma crónica tem uma evolução muito variável.
No que respeita à terapêutica, a eliminação do agente causal é a abordagem ideal, embora nem sempre possível. O tratamento sintomático de eleição são os antihistamínicos H1 não sedativos, em dosagem superior à habitualmente recomendada.
Terapêuticas alternativas podem associar-se a estes fármacos, embora não existam fortes evidências científicas que comprovem
a sua eficácia.
Em algumas situações é importante referenciar a consulta especializada (Dermatologia ou Imunoalergologia); são exemplo as situações IgE mediadas (alergia alimentar ou medicamentosa), ausência de resposta à terapêutica optimizada ou alterações sugestivas de doença sistémica
Breaking the News: First Impressions Matter on Online News
A growing number of people are changing the way they consume news, replacing
the traditional physical newspapers and magazines by their virtual online
versions or/and weblogs. The interactivity and immediacy present in online news
are changing the way news are being produced and exposed by media corporations.
News websites have to create effective strategies to catch people's attention
and attract their clicks. In this paper we investigate possible strategies used
by online news corporations in the design of their news headlines. We analyze
the content of 69,907 headlines produced by four major global media
corporations during a minimum of eight consecutive months in 2014. In order to
discover strategies that could be used to attract clicks, we extracted features
from the text of the news headlines related to the sentiment polarity of the
headline. We discovered that the sentiment of the headline is strongly related
to the popularity of the news and also with the dynamics of the posted comments

Identifying the Azobenzene/Aniline reaction intermediate on TiO2-(110) : a DFT Study
Density functional theory (DFT) calculations, both with and without dispersion corrections, have been performed to investigate the nature of the common surface reaction intermediate that has been shown to exist on TiO2(110) as a result of exposure to either azobenzene (C6H5N═NC6H5) or aniline (C6H5NH2). Our results confirm the results of a previous DFT study that dissociation of azobenzene into two adsorbed phenyl imide (C6H5N) fragments, as was originally proposed, is not energetically favorable. We also find that deprotonation of aniline to produce this surface species is even more strongly energetically disfavored. A range of alternative surface species has been considered, and while dissociation of azobenzene to form surface C6H4NH species is energetically favored, the same surface species cannot form from adsorbed aniline. On the contrary, adsorbed aniline is much the most stable surface species. Comparisons with experimental determinations of the local adsorption site, the Ti–N bond length, the molecular orientation, and the associated C 1s and N 1s photoelectron core level shifts are all consistent with the DFT results for adsorbed aniline and are inconsistent with other adsorbed species considered. Possible mechanisms for the hydrogenation of azobenzene required to produce this surface species are discussed
Neonatal Brachial Plexus Palsy: Risk Factors and Its Prognostic Value
Introduction: Neonatal brachial plexus palsy affects 0.7
to 5.8 per 1,000 newborns and is characterised by upper
limb paresis detected in the immediate neonatal period.
Shoulder dystocia, instrumental delivery and foetal
macrosomia are well-known risk factors. Most neonatal
brachial plexus palsy evolve favourably, while 3%-27% of
newborns have sequelae.
Methods: A retrospective cross-sectional study was
conducted to characterise neonatal brachial plexus
palsy in the newborn population of a hospital with
differentiated perinatal support and to assess the rela -
tionship between the risk factors and lesion prognosis.
The authors reviewed the newborn medical records
referred to the physical medicine and rehabilitation
clinic between January 2006 and December 2016.
Results: During the study period, 137 cases of neo-
natal brachial plexus palsy were identified in 36,833
births, which translate into an incidence of 3.7/1,000
live births. Foetal macrosomia was found in 41% and
shoulder dystocia in 40%. According to the Narakas clas-
sification, 58% were included in group I, 30% in group
II, 9% in group III and 3% in group IV. The majority of
patients were discharged without sequelae. Newborns
with group II, III and IV lesions as well as macrosomic
newborns were more likely to develop sequelae (p <
0.05). Shoulder dystocia and operative delivery did not
present a statistically significant relationship with the
prognosis of the lesion.
Discussion: The incidence of neonatal brachial plexus
palsy in this population was similar to is described in
other series. The relationship between macrosomia and
neonatal brachial plexus palsy with sequelae found may

Enterocolite Induzida por Proteínas Alimentares: Uma Etiologia Rara
Introdução: A enterocolite induzida por proteínas alimentares (EIPA) é uma forma de hipersensibilidade gastrointestinal alimentar não mediada por imunoglobulina E, sendo provavelmente mediada por células. A fisiopatologia permanece incerta. O leite de vaca e os cereais estão entre as causas mais comuns. O diagnóstico baseia-se na história clínica.
Descrição do caso: Descreve-se um caso raro de enterocolite à batata numa criança com antecedentes pessoais e familiares irrelevantes. O doente recusou várias vezes a introdução da primeira sopa (cenoura e batata) e sempre que a ingeria apresentava quadro de vómitos incoercíveis com desidratação, mas sem necessidade de internamento. Os testes cutâneos por picada com extrato de batata foram negativos e a prova de provocação oral foi positiva.
Comentários: Neste contexto salienta-se a importância de reconhecer o quadro clínico de EIPA e estar alerta para a possibilidade de envolvimento de proteínas alimentares menos frequentes com o objetivo de fazer um diagnóstico precoce e evitar terapêuticas desadequadasinfo:eu-repo/semantics/publishedVersio
Drug Allergy with Cutaneous Manifestations. Diagnostic Approach
As reacções adversas a fármacos (RAF) representam um problema frequente na prática clínica. A alergia a fármacos resulta de mecanismos de hipersensibilidade imunológica e representa 6-10% do total de RAF. Clinicamente, as reacções alérgicas a fármacos podem ser classificadas como imediatas (tipo I) ou não-imediatas (com
manifestações clínicas diversas e associadas sobretudo a reacções de tipo IV). Neste artigo são abordados aspectos gerais, nomeadamente os mecanismos imunopatogénicos implicados na alergia a fármacos e reactividade cruzada mas também as manifestações cutâneas mais relevantes, nomeadamente exantemas máculo-papulares, eritema fixo a fármacos (EFF), pustulose exantemática aguda generalizada (PEAG), síndrome de hipersensibilidade a fármacos
(DRESS – drug rash with eosinophilia and systemic symptoms), síndrome de Stevens-Johnson/necrólise epidérmica tóxica (SSJ/NET). O papel dos testes cutâneos (epicutâneos ou intradérmicos de leitura tardia) na abordagem de reacções não-imediatas é também revisto. Os beta-lactâmicos (BL) são o grupo farmacológico mais frequentemente envolvido em reacções de hipersensibilidade imunológica e que mais dificuldades coloca na prática clínica diária, nomeadamente devido aos riscos de reactividade cruzada, pelo que é analisado em maior detalhe ao longo da revisão. A indução de tolerância a fármacos poderá ser considerada em casos selecionados, sobretudo quando na ausência de alternativas terapêuticas igualmente eficazes ou seguras
