    Predicting cognitive decline in patients with hypoxaemic COPD

    AbstractThe objective was to identify predictors of cognitive decline in patients with hypoxaemic COPD on continuous oxygen therapy.Eighty-four consecutive ambulatory hypoxaemic COPD patients in stable clinical conditions were prospectively studied over the course of 2 yr. Baseline multidimensional assessment included respiratory function tests, blood gas analysis, Mini Mental Status (MMS) test, Geriatric Depression Scale (GDS), Activities of Daily Living (ADLs) and Charlson's index of comorbidity. Reassessments were made 1 yr and 2 yr thereafter. Sequential changes in MMS, GDS and ADLs were assessed by Friedman's ANOVA by rank test.Forty patients completed the study (group A), while 44 died or were lost to follow-up (group B). Group B was characterized by more severe respiratory function impairment and worse performances on ADLs and GDS. In group A, MMS deteriorated from baseline to the 1 yr and 2 yr reassessments (27 ± 2·9 vs. 25·8 ± 4·1 and 25·4 ± 4, P<0·005), whereas GDS and ADLs did not change significantly; the 23 patients experiencing a decline of MMS had baseline lower percentage predicted FVC (52·3 ± 17·1 vs. 66·9 ± 13·4, P<0·03) and FEV1 (27·2 ± 8·6 vs. 44 ± 26·8, P<0·02) values and better affective status (GDS score: 11·9 ± 7·7 vs. 16·5 ± 5·6, P<0·04). Two-year changes in MMS and in GDS scores were inversely correlated (Spearman's ρ = −0·32, P = 0·04).Cognitive decline is faster in the presence of severe bronchial obstruction and parallels the worsening of the affective status in COPD patients on oxygen therapy. The onset of depression rather than baseline depressive symptoms seems to be a risk factor for cognitive decline

    Antimicrobial activity of Inga fendleriana extracts and isolated flavonoids.

    The EtOAc and n-BuOH extracts of Inga fendleriana inhibited Gram-positive, but not Gram-negative bacteria; a narrow spectrum of activity against Staphylococcus epidermidis was detected. The MIC values of the extracts ranged from 125 to 850 μg/mL. Quercetin 3-methylether, myricetin 3-O-rhamnoside and tricetin showed antibacterial activity against the same bacterial strains with MICs in the range from 31 to 250 μg/mL. In time-kill kinetic studies, the flavonoids showed bactericidal effects at the concentrations corresponding to four times the MICs

    A narrative review of epidemiology and prevention of lung cancer: sex/gender differences?

    Background and Objective: Sex and gender-related factors can both differently influence cancer susceptibility, progression, survival, and therapeutic response. Nevertheless, these differences are not extensively studied. The aim of this narrative review is to summarize sex/gender related differences in lung cancer epidemiology, the burden of its risk factors, and the role of prevention. Methods: Through PubMed Central and official websites of International and European Agencies and Societies, recent evidence about potential differences between males and females in lung cancer epidemiology (prevalence, temporal/spatial trend, biomarkers), risk factors (tobacco smoking and air pollution) and prevention coming from international reports and original studies have been selected. The study was limited to published original manuscripts/reviews/reports in the English language from 1990 to 2021. Key Content and Findings: Lung cancer is the most commonly diagnosed cancer (11.4% of the total cases), and the leading cause of cancer death (18.0% of the total cancer deaths). An increasing lung cancer incidence and mortality trends, largely but not only due to the increasing smoking prevalence, were evidenced among women. Females seem to be more susceptible to carcinogenic effect of tobacco smoke, and hormonal factors, use of hormone replacement therapy (HRT) and genetic predisposition are suggested to affect susceptibility to lung cancer. The incidence of lung cancer among females is globally increasing also in those who doesn’t smoke. Therefore, it becomes important to investigate the influence of other factors such as environmental tobacco exposure (ETS), outdoor and indoor air pollution, occupational exposure to hazardous chemicals and radon exposure from soil and building materials. The risk of lung cancer can be significantly reduced with tobacco cessation. Sex/gender differences in smoking cessation rates have been hypothesized with contrasting results. Conclusions: Lung cancer prevalence, trend and susceptibility as well as efficacy of risk factors preventive measures (e.g., smoking cessation) are a result of sex and gender differences, thus both aspects should be considered as contributing factors in lung cancer management

    Contribution of irisin pathway in protective effects of mandarin juice (Citrus reticulata Blanco) on metabolic syndrome in rats fed with high fat diet

    The beneficial effects of Citrus fruits and their secondary metabolites on the cardio- vascular system are well established. Moreover, growing evidence suggests beneficial role for prevention of obesity and related dysfunctions. Citrus reticulata Blanco (Rutaceae) is one of the most consumed Citrus fruits, but it is poorly investigated. Mandarin juice obtained from C. reticulata fruits, collected in the Horti Simplicium of Pisa Charterhouse, presents a high amount of flavanone glycosides, including hesper- idin and a number of polymethoxyflavonoids, in particular nobiletin and tangeretin. On Wistar rats fed with a high fat diet for 21 days, mandarin juice significantly con- tained the percentage weight gain, reduced visceral adipose tissue and the inflamma- tory markers TNF and IL-6. Furthermore, mandarin juice influenced irisin pathway, increasing its plasma levels. Finally, supplementation with mandarin juice contributed to improve mitochondrial membrane potential, partially compromised with high fat diet, making mitochondria less susceptible to harmful events, such as ischemia. Taken together, these results suggest that C. reticulata, through its main metabolites, is able to produce beneficial effects in metabolic syndrome and to promote browning pro- cess, through involvement of the novel interesting irisin pathway

    Antimicrobial activity of some essential oils against methicillin-susceptible and methicillin-resistant staphylococcus pseudintermedius-associated pyoderma in dogs

    This study aimed to test in vitro the antimicrobial activity of 11 essential oils (EOs) against four methicillin-resistant Staphylococcus pseudintermedius (MRSP) and four methicillin-susceptible S. pseudintermedius (MSSP) clinical isolates. The obtained findings demonstrated a clear in vitro efficacy of some tested EOs against both MRSP and MSSP strains. Particularly, modal minimum inhibitory concentration (MIC) values ranging from 1:2048 v/v for Melissa officinalis against an MSSP strain to 1:256 v/v for Cymbopogon citratus against all MRSP strains were observed. The best results, highlighting a modal MIC value of 1:1024 v/v for all tested isolates, was provided by Cinnamomum zeylanicum. Intriguingly, Cinnamomum zeylanicum showed, in many cases, a correspondence between minimum bactericidal concentration (MBC) and MIC values, indicating that the inhibiting dose is also often bactericidal. Moreover, a mild antibacterial and bactericidal activity against both MRSP and MSSP isolates was detected for the other tested EOs. Considering the zoonotic potential of S. pseudintermedius and the increased dissemination of multidrug-resistant strains, the employment of EOs could be useful for the treatment of canine pyoderma. Since antibiotic resistance has become the most urgent issue, from the perspective of the One Health initiative, alternative therapeutic approaches are desirable to limit the use of antibiotics or to improve the efficacy of conventional therapies

    Androgen receptor expression in early triple-negative breast cancer: clinical significance and prognostic associations.

    Background: Triple-negative breast cancers (TNBC) are characterized by aggressive tumour biology resulting in a poor prognosis. Androgen receptor (AR) is one of newly emerging biomarker in TNBC. In recent years, ARs have been demonstrated to play an important role in the genesis and in the development of breast cancer, although their prognostic role is still debated. In the present study, we explored the correlation of AR expression with clinical, pathological and molecular features and its impact on prognosis in early TNBC. Patients and Methods: ARs were considered positive in case of tumors with &gt;10% nuclear-stained. Survival distribution was estimated by the Kaplan Meier method. The univariate and multivariate analyses were performed. The difference among variables were calculated by chi-square test. Results: 81 TNBC patients diagnosed between January 2006 and December 2011 were included in the analysis. Slides were stained immunohistochemically for estrogen and progesterone receptors, HER-2, Ki-67, ALDH1, e-cadherin and AR. Of the 81 TNBC samples, 18.8% showed positive immunostaining for AR, 23.5% and 44.4% of patients were negative for e-cadherin and ALDH1, respectively. Positive AR immunostaining was inversely correlated with a higher Ki-67 (p &lt; 0.0001) and a lympho-vascular invasion (p = 0.01), but no other variables. Univariate survival analysis revealed that AR expression was not associated with disease-free survival (p = 0.72) or overall survival (p = 0.93). Conclusions: The expression of AR is associated with some biological features of TNBC, such as Ki-67 and lympho-vascular invasion; nevertheless the prognostic significance of AR was not documented in our analysis. However, since ARs are expressed in a significant number of TNBC, prospective studies in order to determine the biological mechanisms and their potential role as novel treatment target
