    Coronary bypass grafting using crossclamp fibrillation does not result in reliable reperfusion of the myocardium when the crossclamp is intermittently released: a prospective cohort study

    BACKGROUND: Cross-clamp fibrillation is a well established method of performing coronary grafting, but its clinical effect on the myocardium is unknown. We sought to measure these effects clinically using the Khuri Intramyocardial pH monitor. METHODS: 50 episodes of cross-clamping were recorded in 16 patients who underwent CABG with crossclamp-fibrillation. An Intramyocardial pH probe measured the level of acidosis in the anterior and posterior myocardium in real-time. The pH at the start and end of each period of cross-clamping was recorded. RESULTS: It became very apparent that the pH of some patients recovered quickly while others entirely failed to recover. Thus the patients were split into 2 groups according to whether the pH recovered to above 6.8 after the first crossclamp-release (N = 8 in each group). Initial pH was 7.133 (range 6.974ā€“7.239). After the first period of crossclamping the pH dropped to 6.381 (range 6.034ā€“6.684). The pH in recoverers prior to the second XC application was 6.990(range 6.808ā€“7.222) compared to only 6.455 (range 6.200ā€“6.737) in patient's whose myocardium did not recover (P < 0.0005). This finding was repeated after the second XC release (mean pH 7.005 vs 6.537) and the third (mean pH 6.736 vs 6.376). However prior to separation from bypass the pH was close to the initial pH in both groups (7.062 vs 7.038). CONCLUSION: Crossclamp fibrillation does not result in reliable reperfusion of the myocardium between periods of crossclamping

    Enhanced diapycnal diffusivity in intrusive regions of the Drake Passage

Values of turbulent kinetic energy dissipation rate Īµ estimated by microstructure are up to two orders of magnitude lower than previously published estimates in the upper 1000 m. Turbulence levels in Drake Passage are systematically higher than values upstream, regardless of season. The dissipation of thermal variance Ļ‡ is enhanced at middepth throughout the surveys, with the highest values found in northern Drake Passage, where water mass variability is the most pronounced. Using the density ratio, evidence for double-diffusive instability is presented. Subject to double-diffusive physics, the estimates of diffusivity using the Osbornā€“Cox method are larger than ensemble statistics based on Īµ and the buoyancy frequency.This work was supported by grants from the U.S. National Science Foundation.2016-10-0

    Circulation and Stirring in the Southeast Pacific Ocean and the Scotia Sea Sectors of the Antarctic Circumpolar Current

    The large-scale middepth circulation and eddy diffusivities in the southeast Pacific Ocean and Scotia Sea sectors between 110Ā° and 45Ā°W of the Antarctic Circumpolar Current (ACC) are described based on a subsurface quasi-isobaric RAFOS-float-based Lagrangian dataset. These RAFOS float data were collected during the Diapycnal and Isopycnal Mixing Experiment in the Southern Ocean (DIMES). The mean flow, adjusted to a common 1400-m depth, shows the presence of jets in the time-averaged sense with speeds of 6 cm sā»Ā¹ in the southeast Pacific Ocean and upward of 13 cm sāˆ’Ā¹ in the Scotia Sea. These jets appear to be locked to topography in the Scotia Sea but, aside from negotiating a seamount chain, are mostly free of local topographic constraints in the southeast Pacific Ocean. The eddy kinetic energy (EKE) is higher than the mean kinetic energy everywhere in the sampled domain by about 50%. The magnitude of the EKE increases drastically (by a factor of 2 or more) as the current crosses over the Hero and Shackleton fracture zones into the Scotia Sea. The meridional isopycnal stirring shows lateral and vertical variations with local eddy diffusivities as high as 2800 Ā± 600 m2 sā»Ā¹ at 700 m decreasing to 990 Ā± 200 mĀ² sā»Ā¹ at 1800 m in the southeast Pacific Ocean. However, the cross-ACC diffusivity in the southeast Pacific Ocean is significantly lower, with values of 690 Ā± 150 and 1000 Ā± 200 mĀ² sā»Ā¹ at shallow and deep levels, respectively, due to the action of jets. The cross-ACC diffusivity in the Scotia Sea is about 1200 Ā± 500 mĀ² sā»Ā¹

    A Prospective Study Comparing Distance-based vs. Time-based Exercise Prescriptions of Walking and Running in Previously Sedentary Overweight Adults

Prior work has reported that the declines observed in body mass index (BMI) and circumference measurements in their cross-sectional data were twice as large when calculated from distance energy expenditure estimations compared to energy expenditure estimations based on time and intensity. The primary purpose of this study was to compare walking/running for distance to walking/running for time as part of an exercise intervention. This study followed a between-subjects, repeated measures design. Fifteen overweight, but otherwise healthy participants completed the study. The time-based group walked/ran for self-reported time while the distance-based group walked/ran for self-reported distance. A mixed-factor repeated-measures ANOVA was used to compare all dependent variables both within-subjects and between-subjects. Weekly adherence rates to the exercise program did not exhibit a significant difference (p \u3e 0.05). Significant interactions were shown for mean body mass loss between groups as well as mean blood glucose level (p \u3c 0.05). Distance-based group exhibited a decline in body mass and blood glucose while the time-based group exhibited an increase in both variables. To the best of the authorsā€™ knowledge, the present study is the first to directly compare a distance-based vs. a time-based exercise program for walking and running for improvement of risk factors of cardiovascular disease. The results of this study would suggest that a distance-based exercise prescription of walking or running should provide a clinician or researcher with a closer estimation of overall accumulated exercise and resultant weight loss
