    Fitness of INTERGROWTH-21st birth weight standards for Chinese-ethnicity babies

    OBJECTIVE: To determine the fitness of the INTERGROWTH-21st birth weight standards (INTERGROWTH21) for ethnic Chinese babies compared with a local reference (FOK2003). DESIGN: Population-based analysis of territory-wide birth data. SETTING: All public hospitals in Hong Kong. PARTICIPANTS: Live births between 24 and 42 complete weeks' gestation during 2006-2017. MAIN OUTCOME MEASURES: Babies' birth weight Z-scores were calculated using published methods. The two references were compared in three aspects: (1) the proportions of large-for-gestational-age (LGA) or small-for-gestational-age (SGA) infants, (2) the gestation-specific and sex-specific mean birth weight Z-scores and (3) the predictive power for SGA-related complications. RESULTS: 488 896 infants were included. Using INTERGROWTH21, among neonates born <33 weeks' gestation, the mean birth weight Z-scores per week were closer to zero (-0.2 to 0.05), while most of them were further from zero (0.06 to 0.34) after excluding infants with a high risk of abnormal intrauterine growth. Compared with FOK2003, INTERGROWTH21 classified smaller proportions of infants as SGA (8.3% vs 9.6%) and LGA (6.6% vs 7.9%), especially SGA among preterm infants (13.1% vs 17.0%). The area under the receiver operating characteristic curve for predicting SGA-related complications was greater with FOK2003 (0.674, 95% CI 0.670 to 0.677) than INTERGROWTH21 (0.658, 95% CI 0.655 to 0.661) (p<0.001). CONCLUSIONS: INTERGROWTH21 performed less well than FOK2003, a local reference for ethnic Chinese babies, especially in infants born <33 weeks' gestation. Although the differences are clinically small, both these references performed poorly for extremely preterm infants, and thus a more robust chart based on a larger sample of appropriately selected infants is needed

    Constraints On the Size Evolution of Brightest Cluster Galaxies

    We measure the luminosity profiles of 16 brightest cluster galaxies (BCGs) at 0.4<z<0.80.4 < z < 0.8 using high resolution F160W NICMOS and F814W WFPC2 HST imaging. The heterogeneous sample is drawn from a variety of surveys: seven from clusters in the Einstein Medium Sensitivity Survey, five from the Las Campanas Distant Cluster Survey and its northern hemisphere precursor, and the remaining four from traditional optical surveys. We find that the surface brightness profiles of all but three of these BCGs are well described by a standard de Vaucouleurs (r1/4r^{1/4}) profile out to at least 2re\sim2r_{e} and that the biweight-estimated NICMOS effective radius of our high redshift BCGs (re=8.3±1.4r_{e} = 8.3\pm 1.4 kpc for H0=80H_{0} = 80 km s1^{-1} Mpc1^{-1}, Ωm=0.2,ΩΛ=0.0\Omega_{m} = 0.2, \Omega_\Lambda = 0.0) is 2\sim 2 times smaller than that measured for a local BCG sample. If high redshift BCGs are in dynamical equilibrium and satisfy the same scaling relations as low redshift ones, this change in size would correspond to a mass growth of a factor of 2 since z0.5z \sim 0.5. However, the biweight-estimated WFPC2 effective radius of our sample is 18 ±\pm 5.1 kpc, which is fully consistent with the local sample. While we can rule out mass accretion rates higher than a factor of 2 in our sample, the discrepancy between our NICMOS and WFPC2 results, which after various tests we describe appears to be physical, does not yet allow us to place strong constraints on accretion rates below that level.Comment: ApJ accepted (566, 1, February 2002), 12 pages, uses emulateapj5.st

    Fv antibodies to aflatoxin B1 derived from a pre-immunized antibody phage display library system

    The production and characterization of recombinant antibodies to aflatoxin B[SUB1] (AFB[SUB1]), a potent mycotoxin and carcinogen is described. The antibody fragments produced were then applied for use in a surface plasmon resonance-based biosensor (BIAcore), which measures biomolecular interactions in 'real-time'. Single chain Fv (scFv) antibodies were generated to aflatoxin B1 from an established phage display system, which incorporated a range of different plasmids for efficient scFv expression. The scFv's were used in the development of a competitive ELISA, and also for the development of surface plasmon resonance (SPR)-based inhibition immunoassays. They were found to be suitable for the detection of AFB[SUB1], in this format, with the assays being sensitive and reproducible

    Ad hoc influenza vaccination during years of significant antigenic drift in a tropical city with 2 seasonal peaks

    We evaluated the acceptability of an additional ad hoc influenza vaccination among the health care professionals following seasons with significant antigenic drift. Self-administered, anonymous surveys were performed by hard copy questionnaires in public hospitals, and by an on-line platform available to all healthcare professionals, from April 1st to May 31st, 2015. A total of 1290 healthcare professionals completed the questionnaires, including doctors, nurses, and allied health professionals working in both the public and private systems. Only 31.8% of participating respondents expressed an intention to receive the additional vaccine, despite that the majority of them agreed or strongly agreed that it would bring benefit to the community (88.9%), save lives (86.7%), reduce medical expenses (76.3%), satisfy public expectation (82.8%), and increase awareness of vaccination (86.1%). However, a significant proportion expressed concern that the vaccine could disturb the normal immunization schedule (45.5%); felt uncertain what to do in the next vaccination round (66.0%); perceived that the summer peak might not occur (48.2%); and believed that the summer peak might not be of the same virus (83.5%). Furthermore, 27.8% of all respondents expected that the additional vaccination could weaken the efficacy of previous vaccinations; 51.3% was concerned about side effects; and 61.3% estimated that there would be a low uptake rate. If the supply of vaccine was limited, higher priority groups were considered to include the elderly aged ≥65 years with chronic medical conditions (89.2%), the elderly living in residential care homes (87.4%), and long-stay residents of institutions for the disabled (80.7%). The strongest factors associated with accepting the additional vaccine included immunization with influenza vaccines in the past 3 years, higher perceived risk of contracting influenza, and higher perceived severity of the disease impact. The acceptability to an additional ad hoc influenza vaccination was low among healthcare professionals. This could have a negative impact on such additional vaccination campaigns since healthcare professionals are a key driver for vaccine acceptance. The discordance in perceived risk and acceptance of vaccination regarding self versus public deserves further evaluation

    Cost-effectiveness of financial incentives to promote adherence to depot antipsychotic medication: economic evaluation of a cluster-randomised controlled trial

    Background: Offering a modest financial incentive to people with psychosis can promote adherence to depot antipsychotic medication, but the cost-effectiveness of this approach has not been examined. Methods: Economic evaluation within a pragmatic cluster-randomised controlled trial. 141 patients under the care of 73 teams (clusters) were randomised to intervention or control; 138 patients with diagnoses of schizophrenia, schizo-affective disorder or bipolar disorder participated. Intervention participants received £15 per depot injection over 12 months, additional to usual acute, mental and community primary health services. The control group received usual health services. Main outcome measures: incremental cost per 20% increase in adherence to depot antipsychotic medication; incremental cost of ‘good’ adherence (defined as taking at least 95% of the prescribed number of depot medications over the intervention period). Findings: Economic and outcome data for baseline and 12-month follow-up were available for 117 participants. The adjusted difference in adherence between groups was 12.2% (73.4% control vs. 85.6% intervention); the adjusted costs difference was £598 (95% CI -£4 533, £5 730). The extra cost per patient to increase adherence to depot medications by 20% was £982 (95% CI -£8 020, £14 000). The extra cost per patient of achieving 'good' adherence was £2 950 (CI -£19 400, £27 800). Probability of cost-effectiveness exceeded 97.5%at willingness-to-pay values of £14 000 for a 20% increase in adherence and £27 800 for good adherence. Interpretation: Offering a modest financial incentive to people with psychosis is cost-effective in promoting adherence to depot antipsychotic medication. Direct healthcare costs (including costs of the financial incentive) are unlikely to be increased by this intervention. Trial Registration: ISRCTN.com 7776928

    The inclusive A(e,e') cross section for x1x \simeq 1 was measured on 2^2H, C, Fe, and Au for momentum transfers Q2Q^2 from 1-7 (GeV/c)2^2. The scaling behavior of the data was examined in the region of transition from y-scaling to x-scaling.     Resilient Design Strategy: Engaging Amphibious Structures to Combat Flood in the Development of an Internally Displaced Persons Settlement Scheme in Nigeria

    Flooding is a natural disaster that has been on the increase in many parts of the world. In the last two decades, this unforeseen destructive occurrence has led to the loss of millions of lives and properties valued at millions of dollars in many countries, including Nigeria. Though several attempts have been advanced as possible solutions to help curb the devastating consequences of flooding in the past, many of the solutions have proven not to be as effective as expected. This paper, examined the potentials amphibious structures present as a resilient strategy to combat flood, to develop an architectural design proposal of a sustainable Internally Displaced Persons (IDPs) settlement, that can withstand the peculiar challenges caused by flooding in Nigeria. The study adopted qualitative research approaches. Data consulted to develop the design proposal were assembled through random search of the internet from archival documents related to the subject and the data were analysed by thematic textual analysis. The findings were presented with texts, architectural two-dimensional and 3- dimensional drawings, tables and plates. Useful information from the analysed data became the conceptual framework on which the scheme was developed. The outcome of the study is the development of an architectural design scheme that utilised a combination of amphibious structures strategy and simple sustainable innovative design measures to proffer solution to an environmental problem, as well as the peculiar social, cultural and religious issues associated with the study area. The conceptual base on which the model was developed is for the settlement to co-exist with flood, rather than the common concept of resisting flood. Though the design was developed for a location in Girei, Adamawa State in the Northern part of Nigeria, the scheme is a useful guide for designing settlements and addressing issues in floodprone areas in other parts of the globe, as well as a valuable educational material on issues pertaining to the development of settlements in flood-prone area

    Revisiting Brightest Cluster Galaxy Evolution with the Las Campanas Distant Cluster Survey

    We investigate the influence of environment on brightest cluster galaxy (BCG) evolution using a sample of 63 clusters at 0.3 < z < 0.9 drawn primarily from the Las Campanas Distant Cluster Survey and follow-up V, I, and K' photometry. The luminosity evolution of the entire BCG sample is not adequately described by a single evolutionary model. Using the integrated light from the cluster detection as a proxy for cluster Lx and the suggestion by Burke, Collins, & Mann, we set Lx = 2 x 10^{44} ergs/s to be the division between high and low luminosity clusters. At high redshift (z>0.6) BCGs from low-Lx clusters are fainter, on average, than those from high-Lx clusters and are best modeled as having constant luminosity with redshift. The BCGs from high-Lx cluster are best modeled as having a stellar population that formed at large redshift (z_form > 5) and is passively evolving. However, for the entire BCG population, the observed V-I and I-K' colors are well described by a single evolutionary model in which the stellar populations have z_form > 5 and subsequently passively evolve. We conclude that accretion is proportionally more significant for BCGs in lower mass clusters at these redshifts (factor of 2 to 4 increase in mass since z ~ 1 for the low Lx systems) and that the accreted matter is in the form of systems with evolved stellar populations.Comment: 32 pages (18 figures, 1 figure abridged for electronic version) Accepted for publication in Ap

    Rapid emergence and predominance of a broadly recognizing and fast-evolving norovirus GII. 17 variant in late 2014

    Norovirus genogroup II genotype 4 (GII.4) has been the predominant cause of viral gastroenteritis since 1996. Here we show that during the winter of 2014–2015, an emergent variant of a previously rare norovirus GII.17 genotype, Kawasaki 2014, predominated in Hong Kong and outcompeted contemporary GII.4 Sydney 2012 in hospitalized cases. GII.17 cases were significantly older than GII.4 cases. Root-to-tip and Bayesian BEAST analyses estimate GII.17 viral protein 1 (VP1) evolves one order of magnitude faster than GII.4 VP1. Residue substitutions and insertion occur in four of five inferred antigenic epitopes, suggesting immune evasion. Sequential GII.4-GII.17 infections are noted, implicating a lack of cross-protection. Virus bound to saliva of secretor histo-blood groups A, B and O, indicating broad susceptibility. This fast-evolving, broadly recognizing and probably immune-escaped emergent GII.17 variant causes severe gastroenteritis and hospitalization across all age groups, including populations who were previously less vulnerable to GII.4 variants; therefore, the global spread of GII.17 Kawasaki 2014 needs to be monitored

    The Apache Point Observatory Galactic Evolution Experiment (APOGEE) Spectrographs

    We describe the design and performance of the near-infrared (1.51--1.70 micron), fiber-fed, multi-object (300 fibers), high resolution (R = lambda/delta lambda ~ 22,500) spectrograph built for the Apache Point Observatory Galactic Evolution Experiment (APOGEE). APOGEE is a survey of ~ 10^5 red giant stars that systematically sampled all Milky Way populations (bulge, disk, and halo) to study the Galaxy's chemical and kinematical history. It was part of the Sloan Digital Sky Survey III (SDSS-III) from 2011 -- 2014 using the 2.5 m Sloan Foundation Telescope at Apache Point Observatory, New Mexico. The APOGEE-2 survey is now using the spectrograph as part of SDSS-IV, as well as a second spectrograph, a close copy of the first, operating at the 2.5 m du Pont Telescope at Las Campanas Observatory in Chile. Although several fiber-fed, multi-object, high resolution spectrographs have been built for visual wavelength spectroscopy, the APOGEE spectrograph is one of the first such instruments built for observations in the near-infrared. The instrument's successful development was enabled by several key innovations, including a "gang connector" to allow simultaneous connections of 300 fibers; hermetically sealed feedthroughs to allow fibers to pass through the cryostat wall continuously; the first cryogenically deployed mosaic volume phase holographic grating; and a large refractive camera that includes mono-crystalline silicon and fused silica elements with diameters as large as ~ 400 mm. This paper contains a comprehensive description of all aspects of the instrument including the fiber system, optics and opto-mechanics, detector arrays, mechanics and cryogenics, instrument control, calibration system, optical performance and stability, lessons learned, and design changes for the second instrument.Comment: 81 pages, 67 figures, PASP, accepte
