NÊre pÄrÞrende av alkoholmisbrukere - hvor mange er de og hvordan berÞres de?
Alkohol stÄr for det stÞrste omfanget av rusmiddelproblemer i Norge, bÄde med
hensyn til hvor mange personer som har et rusmiddelproblem og omfanget av de
helsemessige og sosiale konsekvensene av rusmiddelbruken. Denne rapporten
omhandler de negative konsekvensene av alkoholbruk for nÊre pÄrÞrende som
barn og ektefelle/partner. BÄde spÞrsmÄlet om hvor mange nÊre pÄrÞrende det er
til alkoholmisbrukere og hva slags negative konsekvenser de erfarer, blir for fĂžrste
gang ved hjelp av norske data belyst her.
Vi har tatt utgangspunkt i to tilgjengelige datasett; en intervjuundersĂžkelse blant
vel 2000 voksne personer og en spĂžrreskjemaundersĂžkelse blant vel 20 000
ungdommer. Et viktig anliggende i rapporten har vĂŠrt Ă„ vise at det ikke er noen
distinkte skiller mellom alkoholmisbrukere eller personer med et risikofylt
alkoholkonsum og andre alkoholbrukere. Vi mÄ derfor anta at de negative
konsekvensene som nÊre pÄrÞrende opplever bÄde varierer betydelig i type,
varighet og alvorlighetsgrad og kan tilskrives en stĂžrre gruppe av alkoholbrukere
enn den lille gruppen som drikker mest.

personer med et risikofylt alkoholkonsum. Det er trolig mer enn 130
000 personer
som i lĂžpet av sitt voksne liv har opplevd visse typer negative konsekvenser
(sjikane/utskjelling, trusler om vold og vold, uĂžnsket seksuell tilnĂŠrming, hĂŠrverk
og seksuelt overgrep) av foreldres eller partners alkoholproblemer. Blant ungdom
ser vi at forekomst av ulike typer psykososiale problemer som voldsutsatthet, dÄrlig
mental helse og dÄrlige foreldrerelasjoner Þker med hvor ofte de opplever Ä se
foreldrene beruset. Likevel er det, selv blant dem som ofte opplever foreldrene
beruset, et flertall som ikke rapporterer noen av disse problemene.
Gjennomgang av forskningslitteraturen har vist at dette er et forsÞmt forskningsomrÄde.
Det er ikke bare et Äpenbart behov for mer forskning om norske forhold pÄ dette feltet,
men ogsÄ viktig Ä bidra til den internasjonale forskningsbaserte kunnskapen pÄ
omrÄdet
Blunt thoracic injuries
Blunt thoracic injuries
This is a review of blunt thoracic trauma. Blunt thoracic trauma is responsible for approximately 10 % of all traumatic injuries in Norway and 25 % of all traumatic deaths.
To study this subject a systematic literature search was preformed. The different types of injuries vary greatly in the different reports.
Traffic-related injuries account for 10-98 % of blunt thoracic trauma, fall accidents 8-41 %. Other causes are work accidents, sport accidents, and violence.
The most common injuries are rib fractures (13-78 %), heart contusion (1-55 %), lung contusion (4-40 %), pneumothorax (10-30 %) and haemothorax (4-26 %). The most lethal injuries are blunt aortic injury, blunt cardiac rupture, and commotio cordis; with lethality ratings that vary between 50 and 90 %. Most patients can be handled conservatively (59 %), while 36 % are in need of a tube thoracostomy. Only about 5 % needs open surgery.
Luckily the most lethal injuries are quite rare, and the most common injuries have a low mortality. Much of the variation in incidence can be related to different levels of health care, different diagnostic criteria, and geographical and socio-economic differences in different countries.
Blunt thoracic injury is a severe health problem in Norway. However, the number of serious traffic accidents in Norway seems to be decreasing during the last years

omrÄdet.Alcohol accounts for the largest fraction of substance related problems, both with respect to the number substance abusers and with respect to the extent of health and social consequences of substance use. The aim of the report was twofold: (i) to estimate the number of children and partners who live with an alcohol abuser, and (ii) to examine negative consequences that they experience. This is the first attempt to examine these issues using Norwegian data.
We used two available data sets for the analyses; one interview survey conducted among more than 2000 adults and a school based survey conducted among more than 20 000 adolescents. An important concern of this report has been to show that there are no clear distinctions between those who abuse alcohol or people with a hazardous drinking pattern and other alcohol consumers. Moreover, children and partners of heavy drinkers may experience negative consequences that will probably vary with respect to type of consequences as well as degree of severity. Such consequences can probably be attributed to a larger group of alcohol consumers than the small group of the most heavy drinkers.
Our estimates showed that we may assume that there are some 50 000 â 150 000 children and some 50 000 â 100 000 partners living with persons who have a hazardous alcohol consumption. There are more than 130 000 persons who, during their adulthood, have experienced certain types of consequences (harassment, violent threats, unwanted sexual contact, vandalism, and sexual abuse) due to parentsâ or partnersâ heavy drinking. Among adolescents, psychosocial problems such as violence, mental health problems and poor parental relations, were found to increase with frequency of exposure to parental alcohol intoxication. However, even among those who often have seen their parents intoxicated, the majority did not report having experienced any of these problems.
A review of the research literature showed that this is a neglected area of research. It is clearly a need not only for further research on Norwegian conditions in this area, but also important to contribute to the international scientific literature on this topic
Preventing alcohol use with a universal school-based intervention: results from an effectiveness study
Background The effectiveness of the universal school-based alcohol prevention program âUnge & Rusâ [Youth & Alcohol] was tested by an independent research group. The program aims to prevent alcohol use and to change adolescentsâ alcohol-related attitudes. The main outcome measure was frequency of monthly alcohol use, favorable alcohol attitudes, perceived behavioral control (PBC), positive alcohol expectancy and alcohol-related knowledge.
Methods Junior high school students (Nâ=â2,020) with a mean age of 13.5 years participated in this longitudinal pre, post and one-year follow-up study with a quasi-experimental design, involving an intervention group and a comparison group recruited from 41 junior high schools in Norway. Multilevel analysis was used to account for the repeated observations (level 1) nested within students (level 2) who in turn were clustered within school classes (level 3).
Results Results showed an increased level of alcohol-related knowledge in the intervention group (pâ<â.005) as compared to the comparison group at one-year follow-up. However, no significant difference in change was found between the intervention group and the comparison group in frequency of monthly alcohol use, alcohol-related attitudes, PBC or alcohol expectancy at one-year follow-up.
Conclusions This study offers adequate data on the effectiveness of a school-based alcohol prevention program widely implemented in Norway. Under its current method of implementation, use of the program cannot be supported over the use of standard alcohol curriculum within schools
Ventilatorassosiert pneumoni : Bruk av klorheksidin ved munnvask for Ă„ redusere forekomsten av pneumoni hos pasienter under respiratorbehandling
Ventilator-assosiert-pneumoni (VAP) er en hyppig forekommende og alvorlig tilstand hos intensivpasienter. Denne oppgaven omhandler et tenkt kunnskapsforbedringsprosjekt og beskriver munnstell med klorheksidin som et profylaktisk tiltak mot VAP.
Vi valgte vÄrt tema med utgangspunkt i en kommentar publisert i Tidsskrift for Den norske legeforening i mars 2012 som anbefaler oral applikasjon med klorheksidin 2 % konsentrasjon for Ä forebygge pneumoni hos pasienter under respiratorbehandling (1). Vi utfÞrte et systematisk sÞk i McMaster Plus og valgte Ä bruke UpToDate, Best Practice og en nylig publisert oppsummert oversiktsartikkel i Lancet Infection Disease som vÄrt kildegrunnlag. UpToDate anbefaler bruk av 0,12 % konsentrasjon, mens bÄde Best Practice og Lancet-artikkelen angir at bruk av 2 % klorheksidin gir best effekt (2-4).
Vi tok utgangspunkt i praksis ved intensivavdelingen ved BÊrum Sykehus, hvor intuberte pasienter fÄr munnstell etter gjeldende rutine to ganger daglig. Munnstellet inkluderer som hovedregel ikke bruk av klorheksidin. VÄrt forslag til tiltak er Ä implementere pensling med 2 % klorheksidinlÞsning som en del av eksisterende munnstellprosedyre to ganger daglig.
Vi valgte to indikatorer for Ă„ evaluere kvaliteten pĂ„ dette prosjektet. Resultatindikatoren vĂ„r er âantall respiratorbehandlede pasienter med pneumoniâ, og prosessindikatoren vĂ„r er âsjekkliste for om tiltaket utfĂžresâ.
Forslag til gjennomfÞring av tiltaket baserer seg pÄ PUKK-sirkelen. Prosessen foreslÄs organisert rundt en ansvarsgruppe med etablering av systemer for fortlÞpende kommunikasjon og innrapportering av problemer.
Vi har gjennomgÄtt kunnskapsgrunnlaget og foreslÄtt et forbedringstiltak nÄr det gjelder munnhygiene hos intuberte pasienter, og vi mener dette er gjennomfÞrbart. Det vil vÊre opp til den enkelte avdelingen Ä vurdere om de Þnsker Ä gjennomfÞre tiltaket. I denne vurderingen mÄ de bestemme seg for om de syns evidensen og fordelene med tiltaket er tilstrekkelige til at prosjektet bÞr igangsettes

The results presented are part of a larger study looking at the prevalence and social norms related to alcohol use, as well as peopleâs opinions on policies and interventions related to alcohol in Malawi. The results presented here are from a qualitative component in three Malawian communities. The results imply that the introduction of sachets has contributed to an increase in alcohol consumption among young people. Major issues of concern are issues of age limits, packaging and alcohol content, as well as lack of empirical evidence on which to base policies and interventions. Finally, there is a need to mobilize positive adult role models for young people with regards to alcohol
