    Le kyste hydatique facial isole

    Get PDF

Uterine torsion is defined as a rotation of uterus more than 45 degrees along its long axis. However, a pathologic rotation of the uterus beyond 45 degrees-torsion of the entire uterus-is rarely seen in obstetrical practice, authors report a case of torsion of the uterus by 90 degrees. The patient, a 30-year-old gravida 3 para 2 at 37 weeks’ gestation with a singleton pregnancy, her prior obstetrical history included two uncomplicated term vaginal deliveries, and the current pregnancy had been uncomplicated until the date of presentation was admitted to the obstetrical unit  with  labour at 37 weeks 5 days ,on obstetric examination the patient was in labour with transversal presentation of the fetus so an emergency caesarean section (CS) was carried out for. At the time of CS, the diagnosis of uterine torsion of 90 degrees was made. After the delivery of the baby, uterus returned to anatomical position and the torsion corrected spontaneously. The patient recovered and was discharged home with her baby on the third postoperative day. Uterine torsion is an infrequently reported and potentially dangerous complication of pregnancy that occurs mainly in the third trimester

Introduction. The widespread use of photovoltaic systems in various applications has spotlighted the pressing requirement for reliability, efficiency and continuity of service. The main impediment to a more effective implementation has been the reliability of the power converters. Indeed, the presence of faults in power converters that can cause malfunctions in the photovoltaic system, which can reduce its performance. Novelty. This paper presents a technique for diagnosing open circuit failures in the switches (IGBTs) of power converters (DC-DC converters and three-phase inverters) in a grid-connected photovoltaic system. Purpose. To ensure supply continuity, a fault-diagnosis process is required throughout all phases of energy production, transfer, and conversion. Methods. The diagnostic approach is based on artificial neural networks and the extraction of features corresponding to the open circuit fault of the IGBT switch. This approach is based on the Clarke transformation of the three-phase currents of the inverter output as well as the calculation of the average value of these currents to determine the exact angle of the open circuit fault. Results. This method is able to effectively identify and localize single or multiple open circuit faults of the DC-DC converter IGBT switch or the three-phase inverter IGBT switches.

A priori filtering and LES modeling of turbulent two-phase flows application to phase separation

International audienceThe Large Eddy Simulation (LES) of two-phase flows with resolved scale interfaces is investigated through the a priori filtering of Direct Numerical Simulations (DNS) of one-fluid and multifield models. A phase inversion benchmark [ 1 –4] is considered highlighting many coalescence and interface rupture events in a kind of atomization process. The order of magnitude of specific two-phase subgrid LES terms is first considered with the two modeling approaches. Then, different existing models such as Smagorinsky [5], Wall-Adapting Local Eddy-viscosity (WALE) model [6], Bardina [7], Mixed [8] and Approximate Deconvo-lution Model (ADM) [9] are used to account for two-phase subgrid effects. These models are compared to filtered DNS results. The main conclusion concerning a priori LES filtering is that the inertia term is not predominant in two-phase flows with fragmentation and rupture of interface. This conclusion is different from that of the studies of [3, 10–13]. Concerning LES models, functional modeling do not correlate to filtered DNS results whereas structural approaches do. Bardina and ADM are clearly the good LES framework to consider for two-phase flows with resolved scale interfaces. ADM is clearly better than Bardina in our study

Does time equal vision in the acute treatment of a cohort of AQP4 and MOG optic neuritis?

Objective To investigate whether visual disability which is known to accumulate by poor recovery from optic neuritis (ON) attacks can be lessened by early treatment, we investigated whether the time from symptom onset to high-dose IV methylprednisolone (IVMP) affected visual recovery. Methods: A retrospective study was performed in a consecutive cohort of patients following their first aquaporin-4 (AQP4)-IgG or myelin oligodendrocyte glycoprotein (MOG)-IgG-ON. Best-corrected visual acuity (BCVA) in ON eyes at 3 months (BCVA3mo) was correlated with time to IVMP (days). In cases of bilateral ON, 1 eye was randomly selected. Results: A total of 29 of 37 patients had ON (27 AQP4-seropositive neuromyelitis optica spectrum disorder [NMOSD] and 9 MOG-IgG-ON), 2 of whom refused treatment. Of the 27 patients included, 10 presented later than 7 days from onset. The median BCVA3mo of patients treated >7 days was 20/100 (interquartile range 20/100–20/200). Patients treated >7 days had an OR of 5.50 (95% CI 0.88–34.46, p = 0.051) of failure to regain 0.0 logMAR vision (20/20) and an OR of 10.0 (95% CI 1.39–71.9) of failure to regain 0.2 logMAR vision (20/30) (p = 0.01) compared with patients treated within 7 days. ROC analysis revealed that the optimal criterion of delay in IVMP initiation was ≤4 days, with a sensitivity and specificity of 71.4% and 76.9%, respectively. Conclusions: In this retrospective study of ON with AQP4 and MOG-IgG, even a 7-day delay in IVMP initiation was detrimental to vision. These results highlight the importance of early treatment for the long-term visual recovery in this group of patients. A prospective, multicenter study of the effects of timing of IVMP is currently underway. Classification of evidence: This study provides Class IV evidence that hyperacute treatment of AQP4 and MOG-ON with IVMP increases the chance for good visual recovery (20/20 vision) and that even a greater than 7-day delay in treatment is associated with a higher risk for poor visual recovery

International audienceThe nuclear industry is interested in better understanding the behavior of turbulent boiling flowsand in using modern computational tools for the design and analysis of advanced fuels and reactorsand for simulation and study of mitigation strategies in accident scenarios. Such interests serve asdrivers for the advancement of the 3-dimensional multiphase Computational Fluid Dynamicsapproach. A pair of parallel efforts have been underway in Europe and in the United States, theNEPTUNE and CASL programs respectively, that aim at delivering advanced simulation tools thatwill enable improved safety and economy of operations of the reactor fleet. Results from acollaboration between these two efforts, aimed at advancing the understanding of multiphaseclosures for pressurized water reactor (PWR) application, are presented. Particular attention is paidto the assessment and analysis of the different physical models implemented in NEPTUNE_CFDand STAR-CCM+ codes used in the NEPTUNE and the CASL programs respectively, forapplication to turbulent two-phase bubbly flows. The experiments conducted by Liu and Bankoff(Liu, 1989; Liu and Bankoff 1993a and b) are selected for benchmarking, and predictions from thetwo codes are presented for a broad range of flow conditions and with void fractions varyingbetween 0 and 50percent. Comparison of the CFD simulations and experimental measurements revealsthat a similar level of accuracy is achieved in the two codes. The differences in both sets of closuremodels are analyzed, and their capability to capture the main features of the flow over a wide rangeof experimental conditions are discussed. This analysis paves the way for future improvements ofexisting two-fluid models. The benchmarks are further leveraged for a systematic study of thepropagation of model uncertainties. This provides insights into mechanisms that lead to complexinteractions between individual closures (of the different phenomena) in the multiphase CFDapproach. As such, it is seen that the multi-CFD-code approach and the principled uncertaintyquantification approach are both of great value in assessing the limitations and the level of maturityof multiphase hydrodynamic closures

The quantification of water flow through the stem is vital for date palm (Phoenix dactylifera L.) to promote a good water stress management. The thermal dissipation probe (TDP) method developed by Granier is widely used to evaluate transpiration of forest trees; however, there are contradictory reports regarding its reliability. Considerable errors in estimated sap flux density, which might be due to a lack ofspecies-specific calibrations. The TDP method uses a mathematical model that is based on an empirical equation to estimate sap flux density, which is claimed to be applicable to all tree species, independently of wood structure and anatomy. At the laboratory, we compared the rate of water uptake by cut stems with sap flux estimates derived from the TDP method to assess the validity of the method. Our calibration results were considerably different compared to the Granier’s original equation. Moreover, sap flux density was overestimated by 18.2 ± 0.5% when the original calibration parameters of Granierare employed. However, using new calibration parameters improved the accuracy of sap flow measurements. Our results indicated that it is not appropriate to use a general equation for different species. Therefore, previous estimations of date palm’s water requirement through thermal dissipation probes should be revised
