    Is the Addition of Omega-3 Fatty Acids to Other Treatment Methods Effective in Reducing Pain in Adult Patients With Osteoarthritis?

    OBJECTIVE: Is the addition of Omega-3 fatty acids to other treatment methods effective in reducing pain in adult patients with osteoarthritis? STUDY DESIGN: Review of three published, double blind, randomized controlled trials were used for this review and were selected based on their relevance to the clinical question. DATA SOURCE: The most recent randomized control trials examining the relief of osteoarthritic pain with intake of omega-3 fatty acids in addition to traditional medical therapies were found using Medline, PubMed, and OVID databases. OUTCOME MEASURED: Relief of osteoarthritic pain with the addition of omega-3 fatty acids to the patients’ normal therapy was the outcome measured. Attention was also paid to the occurrence of reported adverse events in all three studies. RESULTS: Gruenwald et al. demonstrated that the addition of omega-3 fatty acids to glucosamine sulfate was significantly superior to the use of glucosamine sulfate alone in reducing pain by 90-100% (n= 26 vs 12, respectively; Pchi= 0.014). Jacquet et al. demonstrated that the addition of omega-3 fatty acids, in the form of Phytalgic, to regular NSAID and analgesic use significantly reduced osteoarthritic pain (86.5) when compared to regular NSAID and analgesic use alone (235.3) assessed by WOMAC pain scores after three months of therapy with a p-value \u3c 0.001. Stammers et al. discovered no significant pain relief in patients taking 786mg eicosapentaenoic acid, in the form of cod liver oil, in addition to regular NSAIDs compared to patients taking a placebo (olive oil) in addition to regular NSAIDs after assessment with a t-test. CONCLUSIONS: Although inconclusive, the results of these three randomized controlled trials demonstrates that the addition of omega-3 fatty acids to regular treatment modalities for relieving osteoarthritic pain is effective and deserves further investigation

    Thin film thermocouples for high temperature turbine application

    The objective is to develop thin film thermocouples (TFTC) for Space Shuttle Main Engine (SSME) components such as the high pressure fuel turbopump (HPFTP) blades and to test TFTC survivability and durability in the SSME environment. The purpose for developing TFTC's for SSME components is to obtain blade temperatures for computational models developed for fluid mechanics and structures. The TFTC must be able to withstand the presence of high temperature, high pressure hydrogen as well as a severe thermal transient due to a cryogenic to combustion temperature change. The TFTC's will eventually be installed and tested on SSME propulsion system components in the SSME test bed engine. The TFTC's were successfully fabricated on flat coupons of MAR-M 246 (Hf+), which is the superalloy material used for HPFTP turbine blades. The TFTC's fabricated on flat coupons survived thermal shock cycling as well as testing in a heat flux measurement facility which provided a rapid thermal transient. The same fabrication procedure was used to deposit TFTC's on HPFTP first stage rotor blades. Other results from the experiments are presented, and future testing plans are discussed

    Applications of Thin Film Thermocouples for Surface Temperature Measurement

    Thin film thermocouples provide a minimally intrusive means of measuring surface temperature in hostile, high temperature environments. Unlike wire thermocouples, thin films do not necessitate any machining of the surface, therefore leaving intact its structural integrity. Thin films are many orders of magnitude thinner than wire, resulting in less disruption to the gas flow and thermal patterns that exist in the operating environment. Thin film thermocouples have been developed for surface temperature measurement on a variety of engine materials. The sensors are fabricated in the NASA Lewis Research Center's Thin Film Sensor Lab, which is a class 1000 clean room. The thermocouples are platinum-13 percent rhodium versus platinum and are fabricated by the sputtering process. Thin film-to-leadwire connections are made using the parallel-gap welding process. Thermocouples have been developed for use on superalloys, ceramics and ceramic composites, and intermetallics. Some applications of thin film thermocouples are: temperature measurement of space shuttle main engine turbine blade materials, temperature measurement in gas turbine engine testing of advanced materials, and temperature and heat flux measurements in a diesel engine.     Lack of analgesic efficacy in female rats of\ud the commonly recommended oral dose of\ud buprenorphine

    Previous work in our laboratory showed that the recommended oral dose of buprenorphine (0.5 mg/kg) was not as effective\ud as the standard therapeutic subcutaneous dose for postoperative analgesia in male Long-Evans (hooded) and Sprague-Dawley (albino) rats. The aim of the current study was to extend this analysis to female rats. We measured the pain threshold in adult female rats in diestrus or proestrus before and 30 and 60 min after oral buprenorphine (0.5 mg/kg,), the standard subcutaneous dose of buprenorphine (0.05 mg/kg), or vehicle only (1 ml/kg each orally and subcutaneously). Female rats showed an increased pain threshold (analgesia) after subcutaneous buprenorphine but no change in pain threshold after either oral buprenorphine or vehicle only. Estrous cycle stage (proestrus versus diestrus) did not affect the analgesic effects of buprenorphine, but rats in proestrus showed significantly lower pain thresholds (less tolerance to pain) than did those in diestrus. These results show that the oral dose of buprenorphine recommended for postoperative analgesic care does not induce significant analgesia in female rats and therefore is not as effective as the standard subcutaneous dose

    Advances in Thin Film Sensor Technologies for Engine Applications

    Advanced thin film sensor techniques that can provide accurate surface strain and temperature measurements are being developed at NASA Lewis Research Center. These sensors are needed to provide minimally intrusive characterization of advanced materials (such as ceramics and composites) and structures (such as components for Space Shuttle Main Engine, High Speed Civil Transport, Advanced Subsonic Transports and General Aviation Aircraft) in hostile, high-temperature environments and for validation of design codes. This paper presents two advanced thin film sensor technologies: strain gauges and thermocouples. These sensors are sputter deposited directly onto the test articles and are only a few micrometers thick; the surface of the test article is not structurally altered and there is minimal disturbance of the gas flow over the surface. The strain gauges are palladium-13% chromium based and the thermocouples are platinum-13% rhodium vs. platinum. The fabrication techniques of these thin film sensors in a class 1000 cleanroom at the NASA Lewis Research Center are described. Their demonstration on a variety of engine materials, including superalloys, ceramics and advanced ceramic matrix composites, in several hostile, high-temperature test environments are discussed

    The MAPPINGS III Library of Fast Radiative Shock Models

    We present a new library of fully-radiative shock models calculated with the MAPPINGS III shock and photoionization code. The library consists of grids of models with shock velocities in the range v=100-1000 km/s and magnetic parameters B/sqrt(n) of 10^-4 - 10 muG cm^(3/2) for five different atomic abundance sets, and for a pre-shock density of 1.0 cm^(-3). Additionally, Solar abundance model grids have been calculated for densities of 0.01, 0.1, 10, 100, and 1000 cm^(-3) with the same range in v and B/sqrt(n). Each model includes components of both the radiative shock and its photoionized precursor, ionized by the EUV and soft X-ray radiation generated in the radiative gas. We present the details of the ionization structure, the column densities, and the luminosities of the shock and its precursor. Emission line ratio predictions are separately given for the shock and its precursor as well as for the composite shock+precursor structure to facilitate comparison with observations in cases where the shock and its precursor are not resolved. Emission line ratio grids for shock and shock+precursor are presented on standard line ratio diagnostic diagrams, and we compare these grids to observations of radio galaxies and a sample of AGN and star forming galaxies from the Sloan Digital Sky Survey. This library is available online, along with a suite of tools to enable the analysis of the shocks and the easy creation of emission line ratio diagnostic diagrams. These models represent a significant increase in parameter space coverage over previously available models, and therefore provide a unique tool in the diagnosis of emission by shocks.Comment: 39 pages, 34 figures, accepted for publication in ApJS, April 200

    Continued high rates of antibiotic prescribing to adults with respiratory tract infection: survey of 568 UK general practices

    OBJECTIVES: Overutilisation of antibiotics may contribute to the emergence of antimicrobial drug resistance, a growing international concern. This study aimed to analyse the performance of UK general practices with respect to antibiotic prescribing for respiratory tract infections (RTIs) among young and middle-aged adults.SETTING: Data are reported for 568 UK general practices contributing to the Clinical Practice Research Datalink.PARTICIPANTS: Participants were adults aged 18-59?years. Consultations were identified for acute upper RTIs including colds, cough, otitis-media, rhino-sinusitis and sore throat.PRIMARY AND SECONDARY OUTCOME MEASURES: For each consultation, we identified whether an antibiotic was prescribed. The proportion of RTI consultations with antibiotics prescribed was estimated.RESULTS: There were 568 general practices analysed. The median general practice prescribed antibiotics at 54% of RTI consultations. At the highest prescribing 10% of practices, antibiotics were prescribed at 69% of RTI consultations. At the lowest prescribing 10% of practices, antibiotics were prescribed at 39% RTI consultations. The median practice prescribed antibiotics at 38% of consultations for 'colds and upper RTIs', 48% for 'cough and bronchitis', 60% for 'sore throat', 60% for 'otitis-media' and 91% for 'rhino-sinusitis'. The highest prescribing 10% of practices issued antibiotic prescriptions at 72% of consultations for 'colds', 67% for 'cough', 78% for 'sore throat', 90% for 'otitis-media' and 100% for 'rhino-sinusitis'.CONCLUSIONS: Most UK general practices prescribe antibiotics to young and middle-aged adults with respiratory infections at rates that are considerably in excess of what is clinically justified. This will fuel antibiotic resistance.<br/

    Surface Gravities for 228 M, L, and T Dwarfs in the NIRSPEC Brown Dwarf Spectroscopic Survey

    We combine 131 new medium-resolution (R~2000) J-band spectra of M, L, and T dwarfs from the Keck NIRSPEC Brown Dwarf Spectroscopic Survey (BDSS) with 97 previously published BDSS spectra to study surface-gravity-sensitive indices for 228 low-mass stars and brown dwarfs spanning spectral types M5-T9. Specifically, we use an established set of spectral indices to determine surface gravity classifications for all M6-L7 objects in our sample by measuring equivalent widths (EW) of the K I lines at 1.1692, 1.1778, 1.2529 um, and the 1.2 um FeHJ absorption index. Our results are consistent with previous surface gravity measurements, showing a distinct double peak - at ~L5 and T5 - in K I EW as a function of spectral type. We analyze K I EWs of 73 objects of known ages and find a linear trend between log(Age) and EW. From this relationship, we assign age ranges to the very low gravity, intermediate gravity, and field gravity designations for spectral types M6-L0. Interestingly, the ages probed by these designations remain broad, change with spectral type, and depend on the gravity sensitive index used.     Utilizing the Power of Continuous Process Improvement in Technical Services

    Objective: Examine the efficiency of work processes in the Technical Services Department, with the goal of reducing the turnaround time by 50% or more for three specific processes: (1) acquiring books for requestors; (2) implementing serials title changes; and (3) handling of journal issues from receipt to binding. Method: In summer 2003 the Library initiated a Continuous Process Improvement project. The project’s goal is to reduce time and/or save money by examining and improving work processes. The charge for the Technical Services team is to (1) reduce the turnaround time by 50% or more for firm orders from when the book is ordered to when it reaches the requestor; (2) reduce the time it takes by 50% or more to implement a serials title change, which involves staff throughout the library; and (3) reduce the time it takes by 50% or more for a newly-received journal issue to reach the shelf and eventually be bound. The team is utilizing value-added flow analysis to examine the processes and will make recommendations to management for changes to be implemented in a pilot project. Results: The Team met weekly over four months to identify the specific steps involved in each process, review the time involved in completing each task, analyze whether or not the step added value, and suggest improvements to the process. Team members documented and timed current workflow in spreadsheets and flowcharts. The Team proposed a number of procedural changes and initiatives that were accepted by library management for a five-month pilot project which is now ongoing. Important components of the implementation are establishing procedures for better communication and additional staff training. Conclusion: Value-added flow analysis is a useful tool for continuous process improvement. The Library’s goal of reducing turnaround time by 50% or more for these three specific processes will be achieved, as demonstrated in the pilot project.
