Matrix interpretation of multiple orthogonality
In this work we give an interpretation of a (s(d + 1) + 1)-term recurrence
relation in terms of type II multiple orthogonal polynomials.We rewrite
this recurrence relation in matrix form and we obtain a three-term recurrence
relation for vector polynomials with matrix coefficients. We present a matrix
interpretation of the type II multi-orthogonality conditions.We state a Favard
type theorem and the expression for the resolvent function associated to the
vector of linear functionals. Finally a reinterpretation of the type II Hermite-
Padé approximation in matrix form is given
Neurofilament light in plasma is a potential biomarker of central nervous system involvement in systemic lupus erythematosus
BACKGROUND:
Neuropsychiatric manifestations (NP) are common in systemic lupus erythematosus (SLE). However, the pathophysiological mechanisms are not completely understood. Neurofilament light protein (NfL) is part of the neuronal cytoskeleton. Increased NfL concentrations, reflecting neurodegeneration, is observed in cerebrospinal fluid (CSF) in several neurodegenerative and neuroinflammatory conditions. We aimed to explore if plasma NfL could serve as a biomarker for central nervous system (CNS) involvement in SLE.
METHODS:
Sixty-seven patients with SLE underwent neurological examination; 52 underwent lumbar puncture, while 62 underwent cerebral magnetic resonance imaging (MRI). We measured selected auto-antibodies and other laboratory variables postulated to have roles in NP pathophysiology in the blood and/or CSF. We used SPM12 software for MRI voxel-based morphometry.
RESULTS:
Age-adjusted linear regression analyses revealed increased plasma NfL concentrations with increasing creatinine (β = 0.01, p < 0.001) and Q-albumin (β = 0.07, p = 0.008). We observed higher plasma NfL concentrations in patients with a history of seizures (β = 0.57, p = 0.014), impaired motor function (β = 0.36, p = 0.008), increasing disease activity (β = 0.04, p = 0.008), and organ damage (β = 0.10, p = 0.002). Voxel-based morphometry suggested an association between increasing plasma NfL concentrations and the loss of cerebral white matter in the corpus callosum and hippocampal gray matter.
CONCLUSION:
Increased plasma NfL concentrations were associated with some abnormal neurological, cognitive, and neuroimaging findings. However, plasma NfL was also influenced by other factors, such as damage accrual, creatinine, and Q-albumin, thereby obscuring the interpretation of how plasma NfL reflects CNS involvement. Taken together, NfL in CSF seems a better marker of neuronal injury than plasma NfL in patients with SLE
Coupling of alpha(1)-Adrenoceptors to ERK1/2 in the Human Prostate
Introduction: alpha(1)-Adrenoceptors are considered critical for the regulation of prostatic smooth muscle tone. However, previous studies suggested further alpha(1)-adrenoceptor functions besides contraction. Here, we investigated whether alpha(1)-adrenoceptors in the human prostate may activate extracellular signal-regulated kinases (ERK1/2). Methods: Prostate tissues from patients undergoing radical prostatectomy were stimulated in vitro. Activation of ERK1/2 was assessed by Western blot analysis. Expression of ERK1/2 was studied by immunohistochemistry. The effect of ERK1/2 inhibition by U0126 on phenylephrine-induced contraction was studied in organ-bath experiments. Results: Stimulation of human prostate tissue with noradrenaline (30 mu M) or phenylephrine (10 mu M) resulted in ERK activation. This was reflected by increased levels of phosphorylated ERK1/2. Expression of ERK1/2 in the prostate was observed in smooth muscle cells. Incubation of prostate tissue with U0126 (30 mu M) resulted in ERK1/2 inhibition. Dose-dependent phenylephrine-induced contraction of prostate tissue was not modulated by U0126. Conclusions: alpha(1)-Adrenoceptors in the human prostate are coupled to ERK1/2. This may partially explain previous observations suggesting a role of alpha(1)-adrenoceptors in the regulation of prostate growth. Copyright (C) 2011 S. Karger AG, Base
Effects of a hot ambient operating theatre on manual dexterity, psychological and physiological parameters in staff during a simulated burn surgery
© 2019 Palejwala et al. Objectives Hot environmental conditions can result in a high core-temperature and dehydration which can impair physical and cognitive performance. This study aimed to assess the effects of a hot operating theatre on various performance, physiological and psychological parameters in staff during a simulated burn surgery. Methods Due to varying activity levels, surgery staff were allocated to either an Active (n = 9) or Less- Active (n = 8) subgroup, with both subgroups performing two simulated burn surgery trials (CONTROL: ambient conditions; 23±0.2°C, 35.8±1.2% RH and HOT: 34±0°C, 28.3±1.9% RH; 150 min duration for each trial), using a crossover design with four weeks between trials. Manual dexterity, core-temperature, heart-rate, sweat-loss, thermal sensation and alertness were assessed at various time points during surgery. Results Pre-trials, 13/17 participants were mildly-significantly dehydrated (HOT) while 12/17 participants were mildly-significantly dehydrated (CONTROL). There were no significant differences in manual dexterity scores between trials, however there was a tendency for scores to be lower/impaired during HOT (both subgroups) compared to CONTROL, at various time-points (Cohen's d = -0.74 to -0.50). Furthermore, alertness scores tended to be higher/ better in HOT (Active subgroup only) for most time-points (p = 0.06) compared to CONTROL, while core-temperature and heart-rate were higher in HOT either overall (Active; p<0.05) or at numerous time points (Less-Active; p<0.05). Finally, sweat-loss and thermal sensation were greater/higher in HOT for both subgroups (p<0.05). Conclusions A hot operating theatre resulted in significantly higher core-temperature, heart-rate, thermal sensation and sweat-loss in staff. There was also a tendency for slight impairment in manual dexterity, while alertness improved. A longer, real-life surgery is likely to further increase physiological variables assessed here and in turn affect optimal performance/outcomes
Neurofilament light is a biomarker of brain involvement in lupus and primary Sjögren's syndrome
Background: To test the hypothesis that neuroflament light (NfL) in CSF is a biomarker of CNS involvement in patients with
systemic lupus erythematosus (SLE) and primary Sjögren’s syndrome (pSS), we measured NfL in CSF from 52 patients with
lupus and 54 with pSS and explored associations with clinical, structural, immunological and biochemical abnormalities.
Methods: In CSF, we measured NfL, anti-P antibodies, protein S100B and TWEAK by ELISA and anti-NR2 antibodies by
electrochemiluminescence. Anti-phospholipid antibodies and routine immunological tests were performed in blood. IgG and
albumin were measured in CSF and serum for assessment of the blood–brain barrier function (Q-albumin) and intrathecal
IgG production (IgG index). Cerebral MRI and neuropsychological testing were performed.
Results: A multivariable regression model showed that increasing CSF anti-NR2 antibody levels were associated with increasing NfL levels in patients with SLE (B 1.27, 95% CI 0.88–1.65, p<0.001). Age contributed signifcantly in the model (B
0.04, 95% CI 0.03–0.05, p<0.001). Similar fndings were observed in the pSS group. Adjusted for age and sex, no associations were found between NfL levels and any MRI data. In SLE patients, higher NfL concentrations were associated with
impairments in psychomotor speed and motor function, and in pSS with motor dysfunction. These associations remained
in multivariable regression models.
Conclusions: Increased concentration of NfL in CSF is a marker of cerebral involvement in patients with SLE and pSS, is
strongly associated with the presence of anti-NR2 antibodies, and correlates with cognitive impairment in several domain

Effect of lead content on phase evolution and microstructural development in Ag-clad Bi-2223 composite conductors
A two powder process was used to prepare silver-sheathed monofilamentary Bi{sub 1.8}Pb{sub x}Sr{sub 1.98}Ca{sub 1.97}Cu{sub 3.08}O{sub y} (Bi-2223) tapes with varying lead contents, x, from 0.2 to 0.5. The resulting tapes were subjected to thermomechanical processing and then characterized by x-ray diffraction (XRD), scanning electron microscopy (SEM), and energy dispersive x-ray analysis (EDX). Layered phase texture was accessed using image analysis software on scanned SEM micrographs. Transport currents were measured at 77 K and zero field by the four-probe method. It was found that tapes with low lead content (X = 0.2 and 0.25) showed incomplete conversion to Bi-2223, had small grain size and poor c-axis texture. Tapes having higher lead content (x = 0.4 and 0.5) also showed incomplete conversion and the presence of lead-rich secondary phases. Tapes with lead content x = 0.3 and 0.35 showed complete conversion to Bi-2223, and had the least amount of secondary phases, the best c-axis texture, and the highest transport current (j{sub c}). The carbon content of the precursor powder also had a strong influence on secondary-phase chemistry
Effect of dietary protein restriction on nutritional status in the Modification of Diet in Renal Disease Study
Effect of dietary protein restriction on nutritional status in the Modification of Diet in Renal Disease Study. The safety of dietary protein and phosphorous restriction was evaluated in the Modification of Diet in Renal Disease (MDRD) Study. In Study A, 585 patients with a glomerular filtration rate (GFR) of 25 to 55 ml/min/1.73m2 were randomly assigned to a usual-protein diet (1.3 g/kg/day) or a low-protein diet (0.58 g/kg/day). In Study B, 255 patients with a GFR of 13 to 24 ml/min/1.73m2 were randomly assigned to the low-protein diet or a very-low-protein diet (0.28 g/kg/day), supplemented with a ketoacid-amino acid mixture (0.28 g/kg/day). The low-protein and very-low-protein diets were also low in phosphorus. Mean duration of follow-up was 2.2 years in both studies. Protein and energy intakes were lower in the low-protein and very-low-protein diet groups than in the usual-protein group. Two patients in Study B reached a “stop point” for malnutrition. There was no difference between randomized groups in the rates of death, first hospitalizations, or other “stop points” in either study. Mean values for various indices of nutritional status remained within the normal range during follow-up in each diet group. However, there were small but significant changes from baseline in some nutritional indices, and differences between the randomized groups in some of these changes. In the low-protein and very-low-protein diet groups, serum albumin rose, while serum transferrin, body wt, percent body fat, arm muscle area and urine creatinine excretion declined. Combining patients in both diet groups in each study, a lower achieved protein intake (from food and supplement) was not correlated with a higher rate of death, hospitalization or stop points, or with a progressive decline in any of the indices of nutritional status after controlling for baseline nutritional status and follow-up energy intake. These analyses suggest that the low-protein and very-low-protein diets used in the MDRD Study are safe for periods of two to three years. Nonetheless, both protein and energy intake declined and there were small but significant declines in various indices of nutritional status. These declines are of concern because of the adverse effect of protein calorie malnutrition in patients with end-stage renal disease. Physicians who prescribe low-protein diets must carefully monitor patients' protein and energy intake and nutritional status
Viscoelastic haemostatic assay augmented protocols for major trauma haemorrhage (ITACTIC): a randomized, controlled trial
Purpose: Contemporary trauma resuscitation prioritizes control of bleeding and uses major haemorrhage protocols
(MHPs) to prevent and treat coagulopathy. We aimed to determine whether augmenting MHPs with Viscoelastic Haemostatic
Assays (VHA) would improve outcomes compared to Conventional Coagulation Tests (CCTs).
Methods: This was a multi-centre, randomized controlled trial comparing outcomes in trauma patients who received
empiric MHPs, augmented by either VHA or CCT-guided interventions. Primary outcome was the proportion of subjects
who, at 24 h after injury, were alive and free of massive transfusion (10 or more red cell transfusions). Secondary
outcomes included 28-day mortality. Pre-specified subgroups included patients with severe traumatic brain injury
(TBI).
Results: Of 396 patients in the intention to treat analysis, 201 were allocated to VHA and 195 to CCT-guided therapy.
At 24 h, there was no difference in the proportion of patients who were alive and free of massive transfusion (VHA:
67%, CCT: 64%, OR 1.15, 95% CI 0.76–1.73). 28-day mortality was not different overall (VHA: 25%, CCT: 28%, OR 0.84,
95% CI 0.54–1.31), nor were there differences in other secondary outcomes or serious adverse events. In pre-specified
subgroups, there were no differences in primary outcomes. In the pre-specified subgroup of 74 patients with TBI, 64%
were alive and free of massive transfusion at 24 h compared to 46% in the CCT arm (OR 2.12, 95% CI 0.84–5.34).
Conclusion: There was no difference in overall outcomes between VHA- and CCT-augmented-major haemorrhage
