A 51-year-old female of Southeast Asian ethnicity was referred to our Neurosurgery service for a newly diagnosed intracranial meningioma. She underwent surgical excision of the tumor and was discharged home on Carbamazepine. Four weeks later, she presented back to our Emergency Department (ED) with fever, generalized rash, and altered mental status. The rash appeared a week prior to the patient’s presentation to the ED, and she complained of spikes of fever for two days. On arrival, her temperature was 41 °C. She was tachypneic at 24 breaths/min, and hypotensive at 95/55 mm Hg, with a heart rate of 120 beats per minute (BPM). Soon after triage, she was moved to the resuscitation room for further assessment and management. On examination, the patient appeared to be in moderate distress, anxious, and slightly confused with a Glasgow Coma Scale (GCS) of 14/15. She had a generalized, macular, pruritic, urticarial rash with irregular confluent margins that was consistent with an allergic reaction. Apart from the above-detailed findings, her examination was unremarkable. There was no mucosal surfaces involvement. Her chest was clear on auscultation. Her abdomen was soft, non-tender, with no organomegaly. No focal neurological deficits were detected. Her investigations included a full septic panel. Laboratory workup revealed elevated Liver Function Tests {Alkaline Phosphatase (ALP) of 120 IU/L (Normal range 35 – 104), Aspartate Transaminase (AST) of 65 IU/L (Normal Range > 32), Alanine Aminotransferase (ALT) of 82 IU/L (Normal Range > 33)}, Lactate Dehydrogenase (LDH) of 367 IU/L (Normal Range 135 – 214), Procalcitonin of 1.01 ng/mL (Normal Range < 0.5), and C-Reactive Protein (CRP) of 150.2 mg/L (Normal Range < 0.5). Of note, she had no Leukocytosis nor eosinophilia. The patient’s empirical treatment plan in the ED included the administration of Intravenous (IV) Fluids, antihistamines, and Ceftriaxone. Upon admission, she was commenced on IV Dexamethasone 4 mg twice a day. Additionally, Carbamazepine was stopped immediately. The day following her admission, the patient’s lab work was repeated, and it showed an improvement in CRP, but most notably, her differential complete blood count revealed eosinophilia of 0.73x10^9/L (Normal Range > 0.7), which further went up to 1.33x10^9/L two days later. She was also reviewed by the dermatologist who agreed with the diagnosis of DRESS. A skin biopsy was proposed. However, the patient did not consent to the procedure. On day 3 of her admission, the patient clinically improved on treatment and remained afebrile and vitally stable. She was therefore discharged home with a follow-up clinic appointment. Written patient consent for publishing the case with no identifiable personal information was obtained

Posterior reversible encephalopathy syndrome (PRES) after bevacizumab therapy for metastatic colorectal cancer.

Posterior reversible encephalopathy syndrome (PRES) is an increasingly recognizable neuro-clinical syndrome. Clinical and neurological manifestations of PRES include hypertension, headache, encephalopathy, seizures, and symmetrical white matter changes on brain MRI. Most common precipitants of PRES are acute medical illness, hypertensive crisis, eclampsia, immunosuppressive therapy, and chemotherapy. Bevacizumab is a monoclonal antibody that halts angiogenesis by inhibiting vascular endothelial growth factor. It has gained widespread popularity in oncology world especially for metastatic and recurrent cancers due to its inherent ability to stop angiogenesis; a vital step for tumor growth. Bevacizumab has also been implicated as the cause of PRES due to dysregulation of the blood-brain barrier. We are reporting a case of PRES induced by Bevacizumab in a patient of colorectal cancer

Background: Postpartum Depression is a type of mood disorder associated with childbirth, which can occur in both parents. It is a major public health concern because it produces insidious effects on the well-being of new born as well as on the whole family. Objective: This study aims to determine the prevalence of postpartum depression among fathers in Karachi, Pakistan. Methods: This cross sectional survey was conducted among fathers, purposive convenient and snowball sampling was used to approach 120 participants after obtaining permission from Al-Khidmat Hospital and Combined Military Hospital (CMH).Participants were aged between 20-50 years, had a newly born child between the past 6 weeks to 1 year. Each participant was required to fill an informed consent, demographic form and Edinburgh Postnatal Depression Scale’s (EPDS), translated Urdu version. The data was collected and statistically analyzed. Results: The study findings show that 28.3% fathers experienced depressive symptoms, out of which 25.8% experienced mild while 2.5% experienced severe depressive symptoms. Conclusion: Paternal postpartum depression is quite prevalent among new fathers in Karachi. Depression among fathers is an area of substantial significance as it may enhance the risk of mental health problems among children. Health services should support new fathers by providing information about this major transition. These results clearly show that there is a need to assess expectant and new fathers for depression. Prevention, early identification and intervention of paternal post- partum depression are immensely required

Background: The present study was conducted to compare the growth performance and ultimately to calculate the profitability of the two locally available commercial strains of broiler (Ross 308 and Cobb 500).Methods: For the purpose of study, 900 number of day-old chicks (DOC) of each strain were purchased from the local market. The birds were reared in conventional broiler house with the provision of standard managemental conditions throughout the experimental period. The parameters recorded on weekly basis were feed intake, body weight gain, feed conversion ratio (FCR) and mortality. Results: Result shown that the total body weight of Cobb-500 and Ross-308 on 1st week was 207.40±14 gram and 196.00±16 gram respectively and these result represented significant difference of weight gain (P0.05) among the strains. Furthermore, significant difference of feed conversion ratio (FCR) was observed (P<0.05) among both the strains but from day 7th till the market age weekly FCR of Cob-500 was significantly higher (P<0.05) than Ross-308. Comparatively high mortality (4.8±0.4%) was noticed in Ross broiler strain than Cobb broiler strain (3.7±0.4%). Conclusion: It was concluded from the current study that the Cobb-500 is performing better in conventional open housing system at high altitude than Ros-308.Keywords: Broiler; Cobb-500; Ross-308; Conventional broiler houses; Mortalit

Formal modelling of TCP congestion control mechanisms ECN/RED and SAP-LAW in the presence of UDP traffic

Get PDF
Congestion avoidance mechanisms are used to prevent the saturation of the gateway which represents a bottleneck of the system. Recently, a new method for the congestion avoidance has been proposed: the smart access point with limited advertised window (SAP-LAW). The main idea is to hijack at the gateway the acknowledge packets in the transmission control protocol (TCP) connections in order to artificially reduce the advertised destination window according to some bandwidth allocation policy. Therefore, the flux control mechanism is artificially exploited to control the congestion at the bottleneck. The advantage of this approach is that it does not drop any packet and does not require any modification in the TCP implementations at the clients. While the most popular congestion avoidance mechanisms are the explicit congestion notification (ECN) and the random early detection (RED). In this paper, we propose stochastic models for the ECN/RED and SAP-LAW mechanisms in order to compare their performances under different scenarios. The models are studied in mean field regime, i.e. under a great number of TCP connections and UDP-based transmissions. Different from previous work for the ECN/RED, in this paper, we consider the presence of UDP traffic with bursts and the case of not greedy TCP connections. The models for SAP-LAW are totally new. The comparison is performed in terms of different performance indices including average queue length, system throughput, and expected waiting time

Objective: To find out the causes and factors of anxiety among amputees suffering from traumatic and non-traumatic causes. Methodology: This descriptive cross-sectional study was conducted in Quetta from the month of May 2018 to July 2018. This study included those clients with amputations as a result of Traumatic & non-traumatic causes and the data was collected from them. This study involved a total of 54 participants. All amputees who visited the three Physical Rehabilitation Centre were included during data collection process. A structured adopted questionnaire using the Hospital Anxiety and Depression Scale (HADS) according to the inclusion & exclusion criteria & then tabularized and analyzed by applying Chi square test. Results: This study involved a total of 54 participants of whom 7(13%) were females and 47(87%) males. Among amputees 36(66.7 %), amputations were due to Traumatic injuries and 18(33.3%) were due to non-traumatic amputations. In traumatic, the majority was due to RTA with 25(46.3 %) and in non-traumatic causes diabetes stood first with 13(24.1%) amputation. Traumatic are more anxious than non-traumatic. There is a significant relationship between marital status and anxiety level (p-value 0.047). Statistically significant relationships between amputation and anxiety level were noticed, p value=0.049. Conclusion: Amputation has a significant association with anxiety level. Traumatic experiences make people feel more anxious than non-traumatic ones. Socio-demographics has association with levels of anxiety. It is recommended to do regular or annual screening of these patients following amputation
