    An examination of a multiplicity of interconnected parameters ranging from specific technology details to total system economic costs for satellite communication systems at the 18/30 GHz transmission bands are presented. It was determined that K sub A band systems can incur a small communications outage during very heavy rainfall periods and that reducing the outage to zero would lead to prohibitive system costs. On the other hand, the economics of scale, ie, one spacecraft accommodating 2.5 GHz of bandwidth coupled with multiple beam frequency reuse, leads to very low costs for those users who can tolerate the 5 to 50 hours per year of downtime. A multiple frequency band satellite network can provide the ultimate optimized match to the consumer performance/economics demands

    Attitudes of healthcare personnel towards vaccinations before and during the covid-19 pandemic

    Vaccines constitute highly effective tools for controlling and eliminating vaccine-preventable diseases (VPDs) and are assessed to avert between two to three million deaths per year globally. Healthcare personnel (HCP) constitute a priority group for several vaccinations. However, studies indicate significant rates of vaccine hesitancy among them and, therefore, of acceptance of vaccination recommendations. This cross-sectional study was conducted in a university hospital in Southern Italy to assess the knowledge and attitudes of HCP about VPDs before and during the COVID-19 pandemic, estimate their intention to get vaccinated against COVID-19, and search for determinants that may influence their choice. A self-administered questionnaire was used. HCP improved their knowledge about VPDs and were more favorable to vaccinations in September\u2013December 2020 compared to January\u2013December 2019. Overall, 75% of respondents would get a COVID-19 vaccine. Our findings indicate a potential role of the ongoing COVID-19 pandemic on Italian HCP\u2019s knowledge and attitudes towards vaccines

    Analysis of hospitalizations due to intussusception in Sicily in the pre-rotavirus vaccination era (2003-2012)

    Background: Intussusception is the most common cause of bowel obstruction in infants with an incidence ranging from 9-328 cases per 100,000 infants aged 0-11 months. Causes underlining this clinical manifestation are still unknown. Possible relationship with a withdrawn tetravalent rotavirus vaccine was not confirmed by post-licensure studies and actually no increased risk of intussusception was found between infants vaccinated with both the recently licensed rotavirus vaccines. Aim of this study is to analyze the intussusception hospitalizations in Sicily from 2003 to 2012 before the introduction of rotavirus universal vaccination and its possible relation with rotavirus gastroenteritis trend. Methods: Were collected data from hospital discharge records occurred from 1st January 2003 to 31st December 2012 in Sicily. Intussusception cases were defined as all hospitalizations with an ICD-9-CM code of 560.0 on any discharge diagnoses. As a proxy for the severity of cases were considered ICD-9-CM procedure codes accounting for surgical or radiologic reduction. Results: A total of 340 intussusception cases were hospitalized in Sicily from 2003 to 2012 in children aged 0-59 months. 46.8 % occurred in the age class 0-11 months. Hospitalization rate for intussusception was 11.4 cases per 100,000 per year (32.6 cases per 100,000 among 0-11 months children; 7.3 cases per 100,000 among 12-59 months children), with a M:F sex ratio of 1.8. During hospitalization only 25 % of intussusceptions had a spontaneous resolution, 56.5 % of cases required a surgical intervention. From 2003 to 2012 intussusception cases were equally distributed during the year without any seasonality, while gastroenteritis hospitalizations due to rotavirus infection have a typically late winter and spring distribution. Conclusions: In Sicily from 2003 to 2012 hospitalizations due to intestinal invagination were higher among children aged 0-11 months with observed rates similar to other European countries. Regional baseline data analysis of intussusception among 0-59 children is recognized as an evidence-based public health strategy by international health authorities. Indeed, this strategy is necessary to compare any post-licensure age or sex-related change in intussusception trend after universal rotavirus vaccination introduction

    Innervation of a prefabricated flap: a new experimental model.

    Introduction. Flap innervation by neoaxonogenesis is a promising field of investigation. The authors evaluated the possibility of innervating an acellular collagen scaffold as component of a potential prefabricated flap. Materials and Methods. Collagen matrix sheets were implanted around the femoral bundle of a murine model to produce two flaps on proximal and distal nerve stumps based on a flow-through model. After thirty days, nerve regeneration and integration into the collagen matrix were evaluated. The specimens were microscopically analyzed to study Schwann cell colonization and axonal integration with the matrix. Axonal count and density were assessed and statistically evaluated. Results. Qualitative structural and ultrastructural evaluation indicated integration, with axonal fibers merged within the collagen matrix, along with a newly formed vascular network on the proximal flap. Wallerian degeneration occurred inside the distal chamber. Axonal count and density did not show statistically significant differences between the nerve inside the proximal flap and the control side. Conclusions. Innervation of an acellular matrix can be obtained by direct nerve stump implantation. The flow-through system was relatively easy to build and reliable to provide adequate blood supply. The collagen scaffold may be a promising support or further studies of preinnervated microsurgical flaps

    Rapid, room-temperature, solvent-free mechanochemical oxidation of elemental gold into organosoluble gold salts

    Gold is highly valued for a wide range of commercial and technological applications but is processed exclusively through highly aggressive and toxic solvents and/or reagents, ultimately yielding water-soluble salts that are difficult to separate from inorganic reaction byproducts. As a result, development of safer, cleaner processes that would enable gold processing in non-aqueous, organic solvent is an attractive technological goal. Here, we describe a methodology that simultaneously avoids aggressive reagents and enables gold extraction into a safe organic solvent. The methodology is based on solventless, mechanochemical oxidation of metallic gold with Oxone® in the presence of tetraalkylammonium halide salts, to directly, rapidly (within 30–60 minutes) and at room temperature convert gold metal into solid salts that are immediately soluble in pure organic solvents and aqueous alcoholic media. The organosoluble gold salts are easily separated from sulfate byproducts by direct extraction into the benign solvent ethyl acetate, which is also easily recycled for re-use, providing a strategy for gold activation and dissolution without any additional reagents for purification, such as cation exchange resins, salts, or chelating agents. Besides enabling direct extraction of gold into an organic solvent, the mechanochemically obtained organosoluble gold salts can also be readily used for further syntheses, as shown here by a two-step one-pot route to prepare air- and moisture-resistant Au(i) salts, and an improved synthesis of gold nanoparticles from bulk gold

    Hierarchy of baby-linked immunogenetic risk factors in the vertical transmission of hepatitis C virus.

    Mother-to-infant transmission of Hepatitis C Virus (HCV) represents the major cause of pediatric HCV infection today. Immunogenetic influence has been poorly investigated and mainly confined to HLA-class II serological polymorphisms. Among 290 parities, 135 from Pavia and 155 from Bergamo, of HCV-RNA-infected Italian women, 21 babies (7.24%) were HCV-RNA positive at birth and steadily positive over 20 months of life. All the 21 infected babies and 44 randomly selected uninfected ones, born to HCV-RNA+ mothers but steadily negative for HCV-RNA during a follow-up of 2 years, and their mothers were investigated for HLA-G, -C, -DRB1, -DQA1 and -DQB1 genomic polymorphisms. Among the different covariates, HLA-Cw*07, -G*010401, -DRB1*0701, -DRB1*1401 and homozigosity for HLA-G 14bp deletion can be considered as risk factors for HCV vertical transmission. On the contrary, protection was conferred by the HLA-DQB1*06, -G*0105N, -Cw*0602, DRB1*1104 and -DRB1*1302 alleles. Our initial question was: has the immunogenetic profile any role in the protection of the fetus growing in an infected milieu and, if so, is it independent from the other non-immunogenetic parameters? The answer to both questions should be yes
