    Formation of tuberculoma is a rare response of neurotuberculosis in patients regularly and adequately treated with anti-tuberculous drugs. We report a 13-year-old girl with two tuberculomas which formed in the dorsal part of the medulla oblongata during chemotherapy for tuberculous meningitis. The tuberculomas were both removed via a suboccipital midline approach and were demonstrated by pathological findings but the girl died of cardiac arrest that was thought to be caused by postoperative medulla oblongata oedema. In combination with a literature review, we discuss the clinical features and treatment options of brainstem tuberculomas.Tworzenie się gruźliczaka jest rzadką reakcją w przebiegu właściwie leczonej gruźlicy układu nerwowego. W pracy autorzy opisują przypadek 13-letniej dziewczynki z dwoma gruźliczakami, które utworzyły się w grzbietowej części rdzenia przedłużonego w czasie farmakologicznego leczenia gruźliczego zapalenia opon mózgowo-rdzeniowych. Oba gruźliczaki usunięto z dostępu podpotylicznego w linii środkowej i potwierdzono ich rozpoznanie w badaniu histopatologicznym, ale pacjentka zmarła w wyniku zatrzymania krążenia, przypuszczalnie wskutek pooperacyjnego obrzęku rdzenia przedłużonego. Na podstawie przedstawionego przypadku i przeglądu piśmiennictwa omówiono objawy kliniczne i możliwości leczenia gruźliczaków pnia mózgu

    Continuous Acetone–Butanol–Ethanol (ABE) Fermentation with in Situ Solvent Recovery by Silicalite-1 Filled PDMS/PAN Composite Membrane

    The pervaporation (PV) performance of a thin-film silicalite-1 filled PDMS/PAN composite membrane was investigated in the continuous acetone–butanol–ethanol (ABE) production by a fermentation–PV coupled process. Results showed that continuous removal of ABE from the broth at three different dilution rates greatly increased both the solvent productivity and the glucose utilization rate, in comparison to the control batch fermentation. The high solvent productivity reduced the acid accumulation in the broths because most acids were reassimilated by cells for ABE production. Therefore, a higher total solvent yield of 0.37 g/g was obtained in the fermentation–PV coupled process, with a highly concentrated condensate containing 89.11–160.00 g/L ABE. During 268 h of the fermentation–PV coupled process, the PV membrane showed a high ABE separation factor of more than 30 and a total flux of 486–710 g/m2h. Membrane fouling was negligible for the three different dilution rates. The solution-diffusion model, especially the mass transfer equation, was proved to be applicable to this coupled process.<br/

    First attempt of directionality reconstruction for atmospheric neutrinos in a large homogeneous liquid scintillator detector

    The directionality information of incoming neutrinos is essential to atmospheric neutrino oscillation analysis since it is directly related to the oscillation baseline length. Large homogeneous liquid scintillator detectors, while offering excellent energy resolution, are traditionally very limited in their capabilities of measuring event directionality. In this paper, we present a novel directionality reconstruction method for atmospheric neutrino events in large homogeneous liquid scintillator detectors based on waveform analysis and machine learning techniques. We demonstrate for the first time that such detectors can achieve good direction resolution and potentially play an important role in future atmospheric neutrino oscillation measurements.Comment: Prepared for submission to PR

    Evolution of Cerebral Ischemia Assessed by Amide Proton Transfer-Weighted MRI

    In today’s stressful world, psychopathy (especially anxiety) is receiving increased importance. Most of the drugs used to treat this disease have several side effects. Medicinal plants derived from natural products have fewer side effects and can be used in the treatment of this disease. The aim of this study was to evaluate the effect of the hydroalcoholic extract of Rosmarinus officinalis L. on anxiety in mice. In this experimental study, 50 male mice were randomly divided into 5 groups. To evaluate anxiety, the Elevated Plus Maze test was performed. The control group received normal saline, the positive control group received diazepam (1 mg/kg) as intraperitoneal injection, and the experimental groups received doses of 100, 200, and 400 mg/kg body weight of rosemary extract. The data were analyzed using SPSS 15 and ANOVA statistical tests. The results show that rosemary extract dose-dependently increases the mice spending time and the entries number of mice in plus maze open arms (indicating less stress). This effect at a dose of 400 mg/kg was similar to diazepam, which, in comparison to the control group, was statistically significant ( P .05). On the other hand, the rosemary extract, similar to the standard drug diazepam, showed an anti-anxiety effect. This effect is probably due to the presence of flavonoids in this plant and their antioxidant property

    Adverse Events Associated With Anti-IL-23 Agents: Clinical Evidence and Possible Mechanisms

    BackgroundAnti-interleukin (IL)-23 agents are widely used for autoimmune disease treatment; however, the safety and risks of specific symptoms have not been systematically assessed.ObjectivesThe aim of this study was to summarize the characteristics and mechanisms of occurrence of five immunological and non-immunological adverse events caused by different anti-IL-23 agents.MethodsThe Cochrane Library, EMBASE, PubMed, and Web of Science databases were searched for eligible randomized clinical trials published from inception through May 1, 2020. Randomized clinical trials that reported at least one type of adverse event after treatment were included, regardless of sex, age, ethnicity, and diagnosis. Two investigators independently screened and extracted the characteristics of the studies, participants, drugs, and adverse event types. The Cochrane Handbook was used to assess the methodological quality of the included randomized clinical trials. Heterogeneity was assessed using the I2 statistic. Meta-regression was applied to determine the sources of heterogeneity, and subgroup analysis was used to identify the factors contributing to adverse events.ResultsForty-eight studies were included in the meta-analysis, comprising 25,624 patients treated with anti-IL-23 agents. Serious immunological or non-immunological adverse events were rare. Anti-IL-12/23-p40 agents appeared to cause adverse events more easily than anti-IL-23-p19 agents. The incidence of cancer did not appear to be related to anti-IL-23 agent treatment, and long-term medication could lead to mental diseases. The prevention of complications should be carefully monitored when administered for over approximately 40 weeks to avoid further adverse reactions, and the incidence of infection was the highest among general immunological adverse events.ConclusionsThe application of anti-IL-23 agents induced a series of immunological and non-immunological adverse events, but these agents tend to be well-tolerated with good safety profiles
