Electroconvulsive therapy use in adolescents: a systematic review

Abstract\ud \ud \ud \ud Background\ud \ud Considered as a moment of psychological vulnerability, adolescence is remarkably a risky period for the development of psychopathologies, when the choice of the correct therapeutic approach is crucial for achieving remission. One of the researched therapies in this case is electroconvulsive therapy (ECT). The present study reviews the recent and classical aspects regarding ECT use in adolescents.\ud \ud \ud \ud Methods\ud \ud Systematic review, performed in November 2012, conformed to the PRISMA statement.\ud \ud \ud \ud Results\ud \ud From the 212 retrieved articles, only 39 were included in the final sample. The reviewed studies bring indications of ECT use in adolescents, evaluate the efficiency of this therapy regarding remission, and explore the potential risks and complications of the procedure.\ud \ud \ud \ud Conclusions\ud \ud ECT use in adolescents is considered a highly efficient option for treating several psychiatric disorders, achieving high remission rates, and presenting few and relatively benign adverse effects. Risks can be mitigated by the correct use of the technique and are considered minimal when compared to the efficiency of ECT in treating psychopathologies

Administrative reforms in Brazil: models, successes and failures

O artigo revê, com base na bibliografia especializada, as reformas e tentativas de reformas administrativas realizadas no país, desde o Estado Novo, passando pelas tentativas dos governos militares e pelas propostas mais recentes da Nova República, e chegando até a reforma Bresser Pereira. O balanço crítico é bastante negativo, sobretudo, porque as grandes reformas, precisamente pela sua abrangência, nunca foram efetivamente implementadas. O sistema do mérito no serviço público federal é um bom exemplo. As dificuldades de implementação das grandes reformas, advindas principalmente de sua grande complexidade, resultam também de sua interação negativa com o contexto político mais geral, pela carga retórica de que se fazem acompanhar, e pelo fato de subestimarem os aspectos técnicos necessários à sua implementação.El artículo revé, basándose en la bibliografía especializada, las reformas e intentos de reformas administrativas realizadas en el país; desde el “Estado Nuevo”, pasando por las tentativas de los gobiernos militares y por las propuestas más recientes de la “Nueva República”, llegando hasta la reforma Bresser Pereira. El balance crítico es bastante negativo, sobre todo porque las grandes reformas, precisamente por su alcance, nunca han sido efectivamente puestas en práctica. El sistema del mérito en el servicio público federal es un buen ejemplo. Las dificultades para llevar a la práctica las grandes reformas, provenientes principalmente de su gran complejidad, resultan también de su interacción negativa con el contexto político, más general, de la carga retórica de la que están acompañados, y del hecho de que se han subestimado los aspectos técnicos necesarios para su puesta en marcha.Based on specialized bibliography, the paper reviews the administrative reforms and attempts to reform carried out in Brazil, from the Getúlio Vargas’ Estado Novo to Bresser Pereira’s reform, covering the attempts of the military regime and the latest proposals of the New Republic. A critical analysis shows a quite negative balance, mainly because the biggest reforms, due to their broad scope, were never implemented. The merit-based system in the federal public service is a good example. The difficulties of implementing big reforms, inherent to the complexity involved, also result from their negative interaction with a broader political context, the rhetorical burden they bring together, and the fact that they underestimate the technical aspects required for their implementation.Número padronizado: v. 49, n. 2 (1998) Revista do Serviço Público - RSP, ano 49, n. 2, p. 5-32Administração PúblicaModernização.
