Chemotherapy-Induced Neuronal Maturation in Sinonasal Teratocarcinosarcoma—a Unique Observation
Sinonasal teratocarcinosarcoma (SNTCS) is a rare and highly malignant tumour with combined features of a teratoma and carcinosarcoma. We report the first case of a SNTCS in 23Â year old male treated with neo-adjuvant chemotherapy followed by cranio-facial resection. The resection specimen displayed cellular maturation in the neuroectodermal component. The patient presented with a short history of nasal obstruction, epistaxis and headache. On imaging, a bone destroying lesion of left paranasal sinuses and nasal cavity was identified. The diagnosis of SNTCS could be offered only on the third biopsy which showed heterogeneous admixture of primitive neuroectodermal, epithelial and mesenchymal elements. An adequate sampling with high index of suspicion is needed to catch hold this rare tumor. Tumor was excised after 4 cycles of neo-adjuvant chemotherapy. On microscopic examination, it showed similar epithelial and mesenchymal components as the pretreatment biopsies. However, the primitive neuroectodermal component displayed extensive neuronal maturation. The undifferentiated neuroectodermal cells were completely absent in the post chemotherapy specimen. This case throws light on the morphologic evidence of chemotherapy induced maturation in the neuroectodermal component within SNTCS, an event hitherto not reported in the literature in case of SNTCS

Follicular cell dendritic sarcoma of cervical nodes in a patient with significant WTC exposure
Comparação entre a tĂ©cnica convencional e a minimamente invasiva vĂdeo-assistida nas tireoidectomias parciais
OBJETIVO: A tireoidectomia minimamente invasiva vĂdeo-assistida (TIMIVA) sem infusĂŁo de gás Ă© considerada segura com vantagem estĂ©tica em relação ao procedimento convencional. O objetivo deste trabalho Ă© comparar a experiĂŞncia preliminar brasileira com a TIMIVA e a tĂ©cnica convencional nahemitireoidectomia. MÉTODO: Doze pacientes foram submetidos Ă hemitireoidectomia por doença nodular tireoidiana por TIMIVA (grupo I) e quatorze pela tĂ©cnica convencional (grupo II). Analisamos gĂŞnero, idade, volume do bĂłcio, medida do maior eixo do nĂłdulo dominante, tempo de cirurgia, queixa de dor no pĂłs-operatĂłrio, resultado estĂ©tico e complicações (paralisia de prega vocal, infecção e hematoma). RESULTADOS: No grupo I, todos eram do gĂŞnero feminino, a mediana etária foi 34 anos, a mediana do volume do bĂłcio foi 16,5 mL, a mediana do tamanho do nĂłdulo dominante foi 2,3 cm, a mediana do tempo cirĂşrgico foi 55 minutos. Oito pacientes referiram dor discreta e quatro negaram dor. A mediana do tamanho da incisĂŁo foi 2 cm, todos consideraram os resultados estĂ©ticos excelentes e nĂŁo houve complicações. No grupo II, o Ăndice feminino/masculino foi 6:1, a mediana etária foi 35 anos, a mediana do volume foi de 18 mL, a mediana do tamanho do nĂłdulo foi 2,5 cm, a mediana do tempo foi 55 minutos. Sete pacientes referiram discreta dor, cinco moderada e dois intensa. A mediana do tamanho da cicatriz foi 5 cm. Onze pacientes consideraram os resultados estĂ©ticos excelentes e trĂŞs, bom. NĂŁo houve complicações. CONCLUSĂ•ES: A TIMIVA proporciona bom resultado estĂ©tico e dor pĂłs-operatĂłria mĂnima, sem aumento do tempo cirĂşrgico e das complicações
