    Se realizó un estudio descriptivo y retrospectivo de los pacientes que fueron atendidos en el cuerpo de guardia de otorrinolaringología del Hospital "Abel Santamaría" durante los meses de septiembre, octubre y noviembre del año 2000 con el objetivo de conocer las afecciones y señalar cuales constituyeron las de mayor incidencia. Se constató el elevado número de podían ser atendidos correctamente por el médico de la familia en su área de salud. Solo una décima parte de los pacientes constituyeron urgencias dentro de nuestra especialidad. DeCS: ENFERMEDAD OTORRINOLARINGOLÓGICAS, REMISIÓN Y CONSULTA. ABSTRACT A descriptive and retrospective study was carried out to the patients attended to the emergency room at Abel Santamaría General Hospital from September, October to November 2000. The study aimed at knowing the conditions and determining the highest incidence of these conditions. It was proved the high number of patients that could be treated properly by the family doctor at the health area. Only the tenth part of patients constituted medical emergencies within our specialty.     Se estudian 280 pacientes atendidos en la consulta de patología vestibular del Hospital Universitario "Abel Santamaría" de Pinar del Río entre enero de 1998 y julio de 1999, por presentar vértigos. En la totalidad de los casos se utiliza un cuestionario con su código de calificación para identificar de manera rápida y eficaz el vértigo de origen cervical. Una vez establecido el diagnóstico de síndrome de insuficiencia arterial vertebrobasilar extracraneal en 100 pacientes, se estudian en cada uno de ellas las siguientes variantes: síntomas más representativos, edad y sexo, ocupación y alteración cervical causal. Se expone asimismo, el relativo valor diagnóstico de los Rx de columna cervical y el error de considerar la edad como un parámetro confiable para el diagnóstico cuando en cualquier etapa de la vida existen causas cervicales capaces de desencadenar el cuadro vertiginoso. DeCS: ENFERMEDAD DE MENIERE. ABSTRACT A sample of 280 patients suffering from dizziness attended the vestibular pathology service of Abel Santamaría Hospital between January 1998 and July 1999 is studied. A questionnaire with a scoring system for the rapid and effective of identification of dizziness of cervical nature is used. When the extracranial vertebrobasilar arterial insufficiency syndrome in 100 patients is established, the following variables are studied: the most representative symptoms, age and sex, occupation and casual cervical disorder, likewise, it is considered the relative diagnosis value of x - rays of the cervical column and the error in considering age as a reliable parameter in diagnosis taking into account that in very life stage there are cervical causes that may be able to give arousal to the vertiginous event.     Se presenta el caso de una mujer de 60 años que fue consultada en el Servicio de Otorrinolaringología del Hospital General "Abel Santamaría Cuadrado" de Pinar del Río, con el diagnóstico de una Epidermólisis bulosa adquirida hace 30 años, aquejándose de disnea y disfagia. Se encontró sinequia del velo del paladar a la pared posterior faríngea y en el vestíbulo laríngeo, lengua lisa y fija al piso de la boca, con sangrado fácil al roce del instrumental, siendo necesario practicarle traqueotomía. Por lo poco frecuente de esta dermatosis en nuestro medio se presentó el caso, y se resaltaron los hallazgos referentes a nuestra especialidad.     Estudio comparativo según tratamiento quirúrgico en pacientes con terceros molares inferiores semirretenidos Comparative study according to surgical treatment in patients presenting third inferior semi-retained molar teeth

    Se realizó un estudio en 120 pacientes con terceros molares inferiores semirretenidos, que fueron atendidos en el servicio de cirugía maxilofacial del Hospital Docente Clínico Quirúrgico "Abel Santamaría Cuadrado", en el período de tiempo comprendido entre el 10 de septiembre de 1999 y el 10 de junio del año 2000, con el objetivo de evaluar la efectividad de la técnica quirúrgica con cierre de herida quirúrgica por segunda intensión, en comparación con la técnica quirúrgica convencional, en pacientes con terceros molares inferiores semirretenidos, atendiendo a su evolución postoperatoria (presencia de dolor, edema, trismo mandibular, sepsis y sangramiento). Se plantean y analizan los resultados obtenidos, estableciendo comparaciones con estudios anteriores. En los pacientes operados mediante cada una de las técnicas descritas, encontramos una mejor evolución postoperatoria mediante la técnica con cierre de la herida quirúrgica por segunda intención, existiendo un 45% de los casos sin dolor y un 90% con edema ligero a las 72 horas de operados, no apareciendo casos con sangramiento, ni sepsis, ni trismo mandibular, presentando esta técnica de tratamiento una mayor efectividad en comparación con la técnica quirúrgica convencional. ABSTRACT A sample of 120 patients with lower semi-impacted third molars in the maxillofacial Surgery Department of Abel Santamaría General Hospital between September 10/1999-June/2000 was studied with the purpose of assessing the effectiveness of the surgical technique using the secondary healing suture, compared to the conventional surgical technique in patients with lower semi-impacted third molars according to the postoperative evolution (pain, edema, mandibular trismus, sepsis and bleeding). Results are analyzed establishing comparisons with prior studies. The surgical patients upon who any of the previous procedures had been used were found to have a better evolution with a secondary healing suture. Pain was absent in 45% cases and a mild edema was observed 72 hours postoperatively.     Substrate-induced dimerization of engineered monomeric variants of triosephosphate isomerase from Trichomonas vaginalis

    "The dimeric nature of triosephosphate isomerases (TIMs) is maintained by an extensive surface area interface of more than 1600 angstrom 2. TIMs from Trichomonas vaginalis (TvTIM) are held in their dimeric state by two mechanisms: a ball and socket interaction of residue 45 of one subunit that fits into the hydrophobic pocket of the complementary subunit and by swapping of loop 3 between subunits. TvTIMs differ from other TIMs in their unfolding energetics. In TvTIMs the energy necessary to unfold a monomer is greater than the energy necessary to dissociate the dimer. Herein we found that the character of residue I45 controls the dimer-monomer equilibrium in TvTIMs. Unfolding experiments employing monomeric and dimeric mutants led us to conclude that dimeric TvTIMs unfold following a four state model denaturation process whereas monomeric TvTIMs follow a three state model. In contrast to other monomeric TIMs, monomeric variants of TvTIM1 are stable and unexpectedly one of them (I45A) is only 29-fold less active than wild-type TvTIM1. The high enzymatic activity of monomeric TvTIMs contrast with the marginal catalytic activity of diverse monomeric TIMs variants. The stability of the monomeric variants of TvTIM1 and the use of cross-linking and analytical ultracentrifugation experiments permit us to understand the differences between the catalytic activities of TvTIMs and other marginally active monomeric TIMs. As TvTIMs do not unfold upon dimer dissociation, herein we found that the high enzymatic activity of monomeric TvTIM variants is explained by the formation of catalytic dimeric competent species assisted by substrate binding.

    A survey of fingerprint classification Part I: taxonomies on feature extraction methods and learning models

    This paper reviews the fingerprint classification literature looking at the problem from a double perspective. We first deal with feature extraction methods, including the different models considered for singular point detection and for orientation map extraction. Then, we focus on the different learning models considered to build the classifiers used to label new fingerprints. Taxonomies and classifications for the feature extraction, singular point detection, orientation extraction and learning methods are presented. A critical view of the existing literature have led us to present a discussion on the existing methods and their drawbacks such as difficulty in their reimplementation, lack of details or major differences in their evaluations procedures. On this account, an experimental analysis of the most relevant methods is carried out in the second part of this paper, and a new method based on their combination is presented.This work was supported by the Research Projects CAB(CDTI), TIN2011-28488, and TIN2013-40765-P.

    A survey of fingerprint classification Part II: experimental analysis and ensemble proposal

    In the first part of this paper we reviewed the fingerprint classification literature from two different perspectives: the feature extraction and the classifier learning. Aiming at answering the question of which among the reviewed methods would perform better in a real implementation we end up in a discussion which showed the difficulty in answering this question. No previous comparison exists in the literature and comparisons among papers are done with different experimental frameworks. Moreover, the difficulty in implementing published methods was stated due to the lack of details in their description, parameters and the fact that no source code is shared. For this reason, in this paper we will go through a deep experimental study following the proposed double perspective. In order to do so, we have carefully implemented some of the most relevant feature extraction methods according to the explanations found in the corresponding papers and we have tested their performance with different classifiers, including those specific proposals made by the authors. Our aim is to develop an objective experimental study in a common framework, which has not been done before and which can serve as a baseline for future works on the topic. This way, we will not only test their quality, but their reusability by other researchers and will be able to indicate which proposals could be considered for future developments. Furthermore, we will show that combining different feature extraction models in an ensemble can lead to a superior performance, significantly increasing the results obtained by individual models.This work was supported by the Research Projects CAB(CDTI), TIN2011-28488, and TIN2013-40765-P
