    Extraktionskonzept bei multiplen vorzeitigen Milchzahnverlusten und horizontaler Verlagerung eines Eckzahnes

    Eine 9-jĂ€hrige Patientin stellte sich mit multiplen StĂŒtzzoneneinbrĂŒchen durch kariesbedingte vorzeitige Milchzahnverluste, einem vollretinierten, horizontal verlagerten Zahn 33, Mittellinienverschiebung sowie einem Unterkieferengstand zur kieferorthopĂ€dischen Beratung vor. Initial wurde im Ober- und Unterkiefer durch das Tragen aktiver Platten ein Fortschreiten der StĂŒtzzoneneinbrĂŒche verhindert, Milchzahnextraktionen durchgefĂŒhrt und auf den verzögerten Durchbruch der PrĂ€molaren gewartet. Im Alter von 10 Jahren wurden die ZĂ€hne 14, 24 und 44 extrahiert, sowie die operative Entfernung des verlagerten Zahnes 33 durchgefĂŒhrt. In einer knapp 2-jĂ€hrigen Multibracket-Therapie wurden die ZĂ€hne 44, 45, 46 mesialisiert und Zahn 44 an Eckzahnposition im 4. Quadranten eingestellt.     Structural differences of ethanol and DME jet flames in a hot diluted coflow

    This study compares the flame structure of ethanol and dimethyl ether (DME) in a hot and diluted ox- idiser experimentally and computationally. Experiments were conducted on a Jet in Hot Coflow (JHC) burner, with the fuel jet issuing into a 1250-K coflow at three oxygen levels. Planar measurements using OH-LIF, CH 2 O-LIF, and Rayleigh scattering images reveal that the overall spatial distribution and evolution of OH, CH 2 O, and temperature were quite similar for the two fuels. For both the ethanol and the DME flames, a transitional flame structure occurred as the coflow oxygen level increased from 3% to 9%. This indicates that the flames shift away from the MILD combustion regime. Reaction flux analyses of ethanol and DME were performed with the OPPDIF code, and ethane (C 2 H 6 ) was also included in the analyses for comparison. These analyses reveal that the H 2 /O 2 pathways are very important for both ethanol and DME in the 3% O 2 cases. In contrast, the importance of fuel-specific reactions overtakes that of H 2 /O 2 reactions when fuels are burnt in the cold air or in the vitiated oxidant stream with 9% O 2 . Unsteady laminar flamelet analyses were also performed to investigate the ignition processes and help interpret experimental results.     Anharmonic collective excitation in a solvable model

    We apply the time-dependent variational principle, the nuclear field theory, and the boson expansion method to the Lipkin model to discuss anharmonicities of collective vibrational excitations. It is shown that all of these approaches lead to the same anharmonicity to leading order in the number of particles.     The landscape of viral associations in human cancers

    Here, as part of the Pan-Cancer Analysis of Whole Genomes (PCAWG) Consortium, for which whole-genome and—for a subset—whole-transcriptome sequencing data from 2,658 cancers across 38 tumor types was aggregated, we systematically investigated potential viral pathogens using a consensus approach that integrated three independent pipelines. Viruses were detected in 382 genome and 68 transcriptome datasets. We found a high prevalence of known tumor-associated viruses such as Epstein–Barr virus (EBV), hepatitis B virus (HBV) and human papilloma virus (HPV; for example, HPV16 or HPV18). The study revealed significant exclusivity of HPV and driver mutations in head-and-neck cancer and the association of HPV with APOBEC mutational signatures, which suggests that impaired antiviral defense is a driving force in cervical, bladder and head-and-neck carcinoma. For HBV, HPV16, HPV18 and adeno-associated virus-2 (AAV2), viral integration was associated with local variations in genomic copy numbers. Integrations at the TERT promoter were associated with high telomerase expression evidently activating this tumor-driving process.     Temporomandibular joint dysfunction and orthognathic surgery: a retrospective study

    <p>Abstract</p> <p>Background</p> <p>Relations between maxillo-mandibular deformities and TMJ disorders have been the object of different studies in medical literature and there are various opinions concerning the alteration of TMJ dysfunction after orthognathic surgery. The purpose of the present study was to evaluate TMJ disorders changes before and after orthognathic surgery, and to assess the risk of creating new TMJ symptoms on asymptomatic patients.</p> <p>Methods</p> <p>A questionnaire was sent to 176 patients operated at the Maxillo-Facial Service of the Lille's 2 Universitary Hospital Center (Chairman Pr Joël Ferri) from 01.01.2006 to 01.01.2008. 57 patients (35 females and 22 males), age range from 16 to 65 years old, filled the questionnaire. The prevalence and the results on pain, sounds, clicking, joint locking, limited mouth opening, and tenseness were evaluated comparing different subgroups of patients.</p> <p>Results</p> <p>TMJ symptoms were significantly reduced after treatment for patients with pre-operative symptoms. The overall subjective treatment outcome was: improvement for 80.0% of patients, no change for 16.4% of patients, and an increase of symptoms for 3.6% of them. Thus, most patients were very satisfied with the results. However the appearance of new onset of TMJ symptoms is common. There was no statistical difference in the prevalence of preoperative TMJ symptoms and on postoperative results in class II compared to class III patients.</p> <p>Conclusions</p> <p>These observations demonstrate that: there is a high prevalence of TMJ disorders in dysgnathic patients; most of patients with preoperative TMJ signs and symptoms can improve TMJ dysfunction and pain levels can be reduced by orthognathic treatment; a percentage of dysgnathic patients who were preoperatively asymptomatic can develop TMJ disorders after surgery but this risk is low.</p

    The impact of hyperbaric oxygen therapy on serological values of vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF)

    <p>Abstract</p> <p>Background</p> <p>Hyperbaric oxygen (HBO) therapy is an effective adjunct treatment for ischemic disorders such as chronic infection or chronic wounds. It combines hyperoxic effects with the stimulating potential of post-therapeutic reactive hypoxia. As its crucial effects, stimulation of fibroblast growth, induction of collagen synthesis and the initiation of angiogenesis are discussed. Angiogenesis is a multistage process resulting in the growth of blood vessels. It includes degradation of extracellular matrix, proliferation and migration of different cell populations and finally formation of new vessel structures. This complex chain of procedures is orchestrated by different cytokines and growth factors. Crucial mediators of angiogenesis are basic fibroblast growth factor (bFGF) and vascular endothelial growth factor (VEGF); their <it>in-vivo </it>function is still not fully understood.</p> <p>Methods</p> <p>Forty-three patients suffering from sudden sensorineural hearing loss or tinnitus were treated with HBO. The therapy included 10 sessions of 90 minutes each, one session a day. Serological levels of bFGF and VEGF were assessed by enzyme-linked immunosorbent assays performed according to the manufacturer's instructions on day 1, 2, 5 and 10 of HBO therapy and were compared to mean values of the control group, related to the patient's age and sex, and their development observed over the ten days of HBO.</p> <p>Results</p> <p>There was no sex- or age dependency of bFGF observed in the present study, whereas under HBO our results showed a significant mitigation of the bFGF concentration. In the present data, there was no connection between the VEGF concentration and the patients' ages. Women showed significantly higher levels of VEGF. There was no significant change of VEGF concentration or the VEGF/bFGF ratio during HBO. All scored results varied within the range of standard values as described in the current literature.</p> <p>Conclusions</p> <p>A significant effect of HBO on serum concentrations of bFGF and VEGF was not verified in the present study. Additional application of exogenous growth factors in conjunction with HBO was not obviously linked by a coherent cause-and-effect chain as far as wound healing is concerned.</p

    The evolutionary history of 2,658 cancers

    Cancer develops through a process of somatic evolution1,2. Sequencing data from a single biopsy represent a snapshot of this process that can reveal the timing of specific genomic aberrations and the changing influence of mutational processes3. Here, by whole-genome sequencing analysis of 2,658 cancers as part of the Pan-Cancer Analysis of Whole Genomes (PCAWG) Consortium of the International Cancer Genome Consortium (ICGC) and The Cancer Genome Atlas (TCGA)4, we reconstruct the life history and evolution of mutational processes and driver mutation sequences of 38 types of cancer. Early oncogenesis is characterized by mutations in a constrained set of driver genes, and specific copy number gains, such as trisomy 7 in glioblastoma and isochromosome 17q in medulloblastoma. The mutational spectrum changes significantly throughout tumour evolution in 40% of samples. A nearly fourfold diversification of driver genes and increased genomic instability are features of later stages. Copy number alterations often occur in mitotic crises, and lead to simultaneous gains of chromosomal segments. Timing analyses suggest that driver mutations often precede diagnosis by many years, if not decades. Together, these results determine the evolutionary trajectories of cancer, and highlight opportunities for early cancer detection

    Characterizing genetic intra-tumor heterogeneity across 2,658 human cancer genomes.

    Intra-tumor heterogeneity (ITH) is a mechanism of therapeutic resistance and therefore an important clinical challenge. However, the extent, origin, and drivers of ITH across cancer types are poorly understood. To address this, we extensively characterize ITH across whole-genome sequences of 2,658 cancer samples spanning 38 cancer types. Nearly all informative samples (95.1%) contain evidence of distinct subclonal expansions with frequent branching relationships between subclones. We observe positive selection of subclonal driver mutations across most cancer types and identify cancer type-specific subclonal patterns of driver gene mutations, fusions, structural variants, and copy number alterations as well as dynamic changes in mutational processes between subclonal expansions. Our results underline the importance of ITH and its drivers in tumor evolution and provide a pan-cancer resource of comprehensively annotated subclonal events from whole-genome sequencing data
