A technique to remove a well-fixed titanium-coated rm acetabular cup in revision hip arthroplasty
A major concern during revision hip arthroplasty is acetabular bone loss and bleeding during the extraction of well-fixed cementless acetabular cup, because no interface exists between the host bone and the cup. Forceful removal of such component using curved gouges and osteotomes often leads to extended bone loss and compromises reimplantation of a new socket
Favorable outcome of total hip arthroplasty with insufficient bone coverage of the roof reinforcement ring: a case report
A 59-year-old male patient underwent a total hip arthroplasty for the treatment of end-stage dysplastic osteoarthritis. A roof reinforcement ring, a cemented polyethylene cup, and a cementless stem were used. Extensive superolateral portion of the ring was uncovered by host bone. Morsellized autogenous femoral-head graft was impacted to fill the space between superolateral border of the ring and superior part of the dysplastic acetabulum.
At the follow-up after 4-years, the patient had no complaints and was very satisfied with the operation result. The hip radiograph revealed no signs of instability of the acetabular component, and no bone graft resorption. Despite the suboptimal implantation of the ring compromising, apparently, mechanical stability of the prosthesis, the outcome was favorable. This result can be supported by the fixation of the metal ring with screws, the adequate orientation of the prosthesis, the position of hip´s center of rotation, and bone graft incorporation
Iliopsoas Release for Internal Snapping Hip using a Minimally Open Medial Hip Approach-Case Report
Internal snapping hip is a recognized cause of hip pain caused by recurrent subluxation of the
iliopsoas tendon. When conservative treatment fails, surgical release of the iliopsoas tendon may be
indicated using an arthroscopic or open hip approach. Results have been better with arthroscopic
release. We report a case of a 32-year-old active female with a painful internal snapping hip. The
right hip extension from a flexed position produced a strong and audible snap. A tenotomy of the
iliopsoas in combination with a resection of the lesser trochanter was performed, using a limited
medial hip approach different than the one described by Ludloff or Ferguson. The pain and snapping
phenomenon were solved; however, a slight heterotopic ossification, as well as a weakness in the
flexion of the hip was noted after the procedure, with no functional limitation. The minimally invasive
medial hip approach appears to be a simple, reproducible and successful operation for the treatment
of an internal snapping hip caused by the iliopsoas tendon, namely in orthopaedics departments with
limited experience in hip arthroscopic approach, but it is not superior to arthroscopic release.info:eu-repo/semantics/publishedVersio
Cabeças femorais de grande diâmetro em próteses totais primárias da anca – um estudo comparativo
Introdução As próteses totais primárias da anca tem vindo a registar avanços significativos, nomeadamente no que diz respeito ao seu desenho e dimensões dos implantes. Assim, o diâmetro da cabeça femoral é um factor a levar em linha de conta, uma vez que os grandes diâmetros podem contribuir para um risco mais baixo de luxação no período de convalescença. Para além da estabilidade articular, outras variáveis são influenciadas pelo diâmetro da cabeça femoral, em que se incluem, o desgaste dos componentes, o conflito tipo “cam”, a mobilidade articular, a restauração da biomecânica natural, a propriocepção e a dor localizada à região nadegueira. O objetivo deste trabalho foi, comparar as taxas de luxação após prótese total primária da anca, em doentes com cabeças femorais de grandes diâmetros e de pequenos diâmetros.
Material e Métodos
Trata-se de um estudo comparativo, onde se consideraram dois grupos, doentes submetidos a prótese total primária da anca com recurso a pequeno diâmetro (grupo I, n=40) e grande diâmetro (grupo II, n=40). Consideraram-se as cabeças de pequeno diâmetro para tamanhos de 28 e 32 mm, sendo que as de grande diâmetro correspondem a 36 e 38 mm. Colheram-se dados do processo clínico sobre a causa etiológica, idade, lateralidade, tempo médio de recuo, perdas hemáticas, técnica cirúrgica, complicações e via de abordagem. A via de abordagem preconizada neste estudo foi a via de Moore.
Resultados No grupo I, a idade média foi de 64,9± 7,3 (32-82) anos , o tempo médio de recuo foi de 1059,8± 120,4 dias e as perdas hemáticas médias foram de 2,7± 0,9 (0,6-4,1) g/dL. A coxartrose primária foi a causa mais frequente de intervenção cirúrgica (n=28). Foi realizada a cimentação de ambos os componentes protéticos em 28 doentes. A anca esquerda foi intervencionada em 21 doentes.
No grupo II, a idade média foi de 53,3± 9,7 (27-76) anos , o tempo médio de recuo foi de 974,7± 178,7 dias e perdas hemáticas médias foram de 3,5± 1,0 (0,9-5,6) g/dL. Em 23 doentes a coxartrose era de causa secundária. Não se observou prevalência na lateralidade da anca submetida a artroplastia. Optou-se pela realização de uma artroplastia total primária não cimentada em 33 doentes.
A complicação mais frequente foi a luxação posterior (cinco casos no grupo I e apenas um caso no grupo II).
Discussão
A escolha do tamanho da cabeça femoral, a adotar durante a realização de uma prótese total primária da anca, depende não só do tamanho do acetábulo, mas também da “filosofia” que defende a utilização de grandes diâmetros com o objetivo de reduzir a incidência de luxações após a cirurgia.
O risco de luxação da anca e de conflito (que promove um aumento do desgaste dos componentes), contribuiu para a introdução no mercado de cabeças femorais com diâmetros cada vez mais elevados.
Os doentes do grupo I (pequenos diâmetros) apresentavam uma idade média mais avançada e uma menor taxa de sangramento (perdas sanguíneas) no pós-operatório. Os doentes do grupo II (grandes diâmetros), em média mais jovens, foram submetidos na sua maioria a próteses totais primárias não cimentadas por coxartrose de causa secundária, registando-se apenas um caso de luxação da prótese.
Conclusão
As próteses totais primárias da anca em que se usaram cabeças femorais de grande diâmetro mostraram uma taxa de luxação protética muito aceitável. Contudo, embora as cabeças femorais de grande diâmetro contribuam para a redução do risco pós-operatório de instabilidade articular, é de ressalvar a componente multifatorial da luxação protética

Hyperglycemia and Hyperinsulinemia-Like Conditions Independently Induce Inflammatory Responses in Human Chondrocytes
To elucidate the mechanisms by which type 2 Diabetes Mellitus (DM2) constitutes a risk
factor for the development and progression of osteoarthritis (OA), this work determined whether
high glucose and/or high insulin, the hallmarks of DM2, are capable of activating the transcription
factor, Nuclear Factor-κB (NF-κB), which plays a critical role in OA by inducing the expression of
pro-inflammatory and catabolic genes. For this, we analyzed NF-κB activation by measuring the
nuclear levels of p65 by western blot. As readouts of NF-κB activity, Interleukin-1β, Tumor Necrosis
Factor-α, and inducible nitric oxide synthase (iNOS) expression were analyzed by real time RT-PCR
and western blot. Culture of the human chondrocytic cell line, C28-I2, in high glucose (30 mM)
increased nuclear NF-κB p65 levels in a time-dependent manner, relative to cells cultured in medium
containing 10 mM glucose (regular culture medium). High glucose-induced NF-κB activation was
inhibited by co-treatment with its specific inhibitor, Bay 11-7082, 5 µM. Culture of primary human
chondrocytes under high glucose for 24 h increased IL-1β and TNF-α mRNA levels by 97% (p = 0.0066)
and 85% (p = 0.0045), respectively, while iNOS mRNA and protein levels and NO production increased
by 61% (p = 0.0017), 148% (p = 0.0089), and 70% (p = 0.049), respectively, relative to chondrocytes
maintained in 10 mM glucose. Treatment of chondrocytic cells with 100 nM insulin was also sufficient
to increase nuclear NF-κB p65 levels, independently of the glucose concentration in the culture
medium. This study shows that hyperglycemia and hyperinsulinemia are independently sufficient to
induce inflammatory responses in human chondrocytes, namely by activating NF-κB. This can be a
relevant mechanism by which DM type 2 and other conditions associated with impaired glucose and
insulin homeostasis, like obesity and the metabolic syndrome, contribute to the development and

Introdução
A artroplastia da anca é o procedimento mais usado para o tratamento das fraturas traumáticas do colo do fémur com desvio. Todavia, nos idosos, nos doentes com alterações cognitivas/demência, com patologia neuromuscular, com atrofia muscular, entre outros, o risco de luxação protética é uma séria complicação, que importa prevenir. Para além de outros fatores, a via de abordagem e o tipo de artroplastia podem contribuir para minimizar o risco de luxação protética. Neste sentido, as cúpulas acetabulares tripolares constritivas, bem como outros tipos de reconstruções acetabulares, podem encontrar indicação. O objetivo deste poster é mostrar o tipo de endoprótese implantada para o tratamento de uma fratura do colo do fémur, num doente com elevado risco de luxação protética pós-operatória.
Material e Métodos
Trata-se de um doente do sexo masculino, com 67 anos de idade, que sofreu uma fratura do colo do fémur esquerdo, tipo III segundo a classificação de Gardner. O exame radiográfico mostrou, também, a existência de obliquidade pélvica, associada a displasia acetabular e coxartrose ipsilateral grau II/III segundo a classificação de Tönnis. Clinicamente, o doente apresentava rigidez do joelho esquerdo com deficit de extensão de 35º e pé esquerdo em equino fixo, sequelas de provável paralisia infantil; o seu perfil cognitivo era baixo, compatível com deficiência mental desde a infância, limitando a sua interacção sociofamiliar. Devido ao status osteoarticular e à atrofia muscular da anca e coxa, implantou-se uma prótese total da anca constritiva, por via posterior. A reconstrução acetabular consistiu na aplicação de um anel metálico de suporte acetabular aparafusado e cimentação de uma cúpula tripolar constritiva. No fémur, cimentou-se uma haste autobloqueante com um colo médio.
Resultados
Não foram registadas complicações no período pós-operatório. O doente mantém seguimento regular em consulta, apresentando-se assintomático, não tendo ocorrido luxação da prótese total da anca.
Discussão
Embora reconhecendo as complicações ligadas a este tipo de implantes, incluindo conflitos, dissociações e luxações intraprotéticas, as cúpulas tripolares constritivas e não constritivas (dupla mobilidade) têm sido usadas na cirurgia de revisão de instabilidades recorrentes de artroplastias da anca, em casos selecionados. Tanto quanto sabemos, não existem trabalhos publicados que suportem o uso de anéis de reconstrução acetabular e de cúpulas tripolares constritivas no tratamento de fraturas do colo do fémur com desvio, no contexto de uma artroplastia primária em anca displásica. Temos vindo a indicar este tipo de cúpulas em traumatologia, em doentes com grave demência, debilitados, com insuficiência muscular e, também, em casos extremos ASA III-IV, como alternativa à ressecção artroplástica, com incontornáveis benefícios para o doente.
Conclusão
A cúpula tripolar constritiva pode ser uma das soluções para o tratamento de fraturas do colo do fémur em doentes com elevado risco de luxação protética, relegando para segundo plano a questão da via de abordagem
Physicochemical characterization of biomaterials commonly used in dentistry as bone substitutes--comparison with human bone
The present work focuses on the physicochemical characterization of selected mineral-based biomaterials that are frequently used in dental applications. The selected materials are commercially available as granules from different biological origins: bovine, porcine, and coralline. Natural and calcined human bone were used for comparison purposes. Besides a classical rationalization of chemical composition and crystallinity, a major emphasis was placed on the measurement of various morphostructural properties such as particle size, porosity, density, and specific surface area. Such properties are crucial to acquiring a full interpretation of the in vivo performance. The studied samples exhibited distinct particle sizes (between 200 and 1000 microm) and shapes. Mercury intrusion revealed not only that the total sample porosity varied considerably (33% for OsteoBiol, 50% for PepGen P-15, and 60% for BioOss) but also that a significant percentage of that porosity corresponded to submicron pores. Biocoral was not analyzed by this technique as it possesses larger pores than those of the porosimeter upper limit. The density values determined for the calcined samples were close to the theoretical values of hydroxyapatite. However, the values for the collagenated samples were lower, in accordance with their lower mineral content. The specific surface areas ranged from less than 1 m(2)/g (Biocoral) up to 60 m(2)/g (BioOss). The chemical and phase composition of most of the samples, the exception being Biocoral (aragonite), were hydroxyapatite based.
