    Detection of Bartonella henselae DNA by Polymerase Chain Reaction in a Patient with Cat Scratch Disease : A Case Report

    We report a case of cat scratch disease caused by Bartonella henselae in Korea. A 25-yr-old woman developed left cervical lymphadenopathy with history of contact with a dog. The cervical lymphadenopathy persisted for 1 month and resolved gradually and spontaneously. Serologic test was not done during the acute stage of the disease. Immunofluorescent antibody test performed during the convalescent stage was positive for B. henselae. To confirm B. henselae infection, polymerase chain reaction (PCR) analysis using aspirates of cervical lymph node was performed and the presence of B. henselae DNA was demonstrated. This is the first reported case of cat scratch disease in Korea confirmed by PCR for B. henselae DNA

    Population Pharmacokinetic Modelling of Intravenous Immunoglobulin Treatment in Patients with Guillain–BarrĂ© Syndrome

    BACKGROUND AND OBJECTIVE: Intravenous immunoglobulin (IVIg) at a standard dosage is the treatment of choice for Guillain–BarrĂ© syndrome. The pharmacokinetics, however, is highly variable between patients, and a rapid clearance of IVIg is associated with poor recovery. We aimed to develop a model to predict the pharmacokinetics of a standard 5-day IVIg course (0.4 g/kg/day) in patients with Guillain–BarrĂ© syndrome. METHODS: Non-linear mixed-effects modelling software (NONMEM(Âź)) was used to construct a pharmacokinetic model based on a model-building cohort of 177 patients with Guillain–BarrĂ© syndrome, with a total of 589 sequential serum samples tested for total immunoglobulin G (IgG) levels, and evaluated on an independent validation cohort that consisted of 177 patients with Guillain–BarrĂ© syndrome with 689 sequential serum samples. RESULTS: The final two-compartment model accurately described the daily increment in serum IgG levels during a standard IVIg course; the initial rapid fall and then a gradual decline to steady-state levels thereafter. The covariates that increased IgG clearance were a more severe disease (as indicated by the Guillain–BarrĂ© syndrome disability score) and concomitant methylprednisolone treatment. When the current dosing regimen was simulated, the percentage of patients who reached a target ∆IgG > 7.3 g/L at 2 weeks decreased from 74% in mildly affected patients to only 33% in the most severely affected and mechanically ventilated patients (Guillain–BarrĂ© syndrome disability score of 5). CONCLUSIONS: This is the first population-pharmacokinetic model for standard IVIg treatment in Guillain–BarrĂ© syndrome. The model provides a new tool to predict the pharmacokinetics of alternative regimens of IVIg in Guillain–BarrĂ© syndrome to design future trials and personalise treatment.     Modeling resource sharing using FSM-SADF

    This paper proposes a modeling approach to capture the mapping of an application on a platform. The approach is based on Scenario-Aware Dataflow (SADF) models. In contrast to the related work, we express the complete design-space in a single formal SADF model. This allows us to have a compact and explorable state-space linked with an executable model capable of symbolically analyzing different mappings for their timing behavior. We can model different bindings for application tasks, different static-orders schedules for tasks bound in shared resources, as well as naturally capturing resource claiming/unclaiming using SADF semantics. Moreover, by using the inherent properties of dataflow graphs and the dynamic behavior of a Finite-State Machine, we can model different levels of pipelining, such as full application pipelining and interleaved pipelining of consecutive executions of the application. The size of the model is independent of the number of executions of the application. Since we are able to capture all this behavior in a single SADF model we can use available dataflow analysis, such as worst-case and best-case throughput and deadlock-freedom checking. Furthermore, since the model captures the design-space independently of the analysis technique, one can use different exploration approaches to analyze different sets of requirements

    The pitfalls of plural valuation

    This paper critically examines the current political context in which valuation studies of nature are undertaken. It challenges the belief that somehow, more and technically better valuation will drive the societal change toward more just and sustainable futures. Instead, we argue that current and proposed valuation practices risk to continue to overrepresent the values of those who hold power and dominate the valuation space, and to perpetuate the discrimination of the views and values of nondominant stakeholders. In tackling this politically sensitive issue, we define a political typology of valuations, making explicit the roles of power and discrimination. This is done to provide valuation professionals and other actors with a simple framework to determine if valuation actions and activities are constructive, inclusive, resolve injustices and enable systemic change, or rather entrench the status quo or aggravate existing injustices. The objective is to buttress actors in their decisions to support, accept, improve, oppose, or reject such valuations

    Five steps towards transformative valuation of nature

    The Values Assessment (VA) of the Intergovernmental Science Policy Platform on Biodiversity and Ecosystem Services shows that while a wide range of valuation methods exist to include nature's values in diverse decision-making contexts, uptake of these methods remains limited. Building on the VA, this paper reviews five critical steps in the evaluation of project or policy proposals that can improve the inclusion of nature's values in decisions. Furthermore, improving valuation practice requires guidelines that utilise quality criteria for valuation of nature and ensure a balance between them. This paper proposes three such quality criteria: relevance, robustness and resource efficiency. The paper argues that the five steps and three Rs can generate a practical checklist to support commissioning, evaluation and performance of more plural valuations. Such guidelines can provide the next steps needed to improve uptake of nature valuation in decision-making
