Design Principles for Plasmonic Nanoparticle Devices
For all applications of plasmonics to technology it is required to tailor the
resonance to the optical system in question. This chapter gives an
understanding of the design considerations for nanoparticles needed to tune the
resonance. First the basic concepts of plasmonics are reviewed with a focus on
the physics of nanoparticles. An introduction to the finite element method is
given with emphasis on the suitability of the method to nanoplasmonic device
simulation. The effects of nanoparticle shape on the spectral position and
lineshape of the plasmonic resonance are discussed including retardation and
surface curvature effects. The most technologically important plasmonic
materials are assessed for device applicability and the importance of
substrates in light scattering is explained. Finally the application of

Genetic Diversity of PCR-Positive, Culture-Negative and Culture-Positive Mycobacterium ulcerans Isolated from Buruli Ulcer Patients in Ghana.
Culture of Mycobacterium ulcerans from Buruli ulcer patients has very low sensitivity. Thus confirmation of M. ulcerans infection is primarily based on PCR directed against IS2404. In this study we compare the genotypes obtained by variable number of tandem repeat analysis of DNA from IS2404-PCR positive cultures with that obtained from IS2404 positive, culture-negative tissue. A significantly greater genetic heterogeneity was found among culture-negative samples compared with that found in cultured strains but a single genotype is over-represented in both sample sets. This study provides evidence that both the focal location of bacteria in a lesion as well as differences in the ability to culture a particular genotype may underlie the low sensitivity of culture. Though preliminary, data from this work also suggests that mycobacteria previously associated with fish disease (M. pseudoshottsii) may be pathogenic for humans
Predictors of Epstein-Barr virus serostatus in young people in England
BACKGROUND: Epstein-Barr virus (EBV) is an important human pathogen which causes lifelong infection of > 90% people globally and is linked to infectious mononucleosis (arising from infection in the later teenage years) and several types of cancer. Vaccines against EBV are in development. In order to determine the most cost-effective public health strategy for vaccine deployment, setting-specific data on the age at EBV acquisition and risk factors for early infection are required. Such data are also important to inform mathematical models of EBV transmission that can determine the required target product profile of vaccine characteristics. We thus aimed to examine risk factors for EBV infection in young people in England, in order to improve our understanding of EBV epidemiology and guide future vaccination strategies. METHODS: The Health Survey for England (HSE) is an annual, cross-sectional representative survey of households in England during which data are collected via questionnaires and blood samples. We randomly selected individuals who participated in the HSE 2002, aiming for 25 participants of each sex in each single year age group from 11 to 24 years. Stored samples were tested for EBV and cytomegalovirus (CMV) antibodies. We undertook descriptive and regression analyses of EBV seroprevalence and risk factors for infection. RESULTS: Demographic data and serostatus were available for 732 individuals. EBV seroprevalence was strongly associated with age, increasing from 60.4% in 11-14 year olds throughout adolescence (68.6% in 15-18 year olds) and stabilising by early adulthood (93.0% in those aged 22-24 years). In univariable and multivariable logistic regression models, ethnicity was associated with serostatus (adjusted odds ratio for seropositivity among individuals of other ethnicity versus white individuals 2.33 [95% confidence interval 1.13-4.78]). Smoking was less strongly associated with EBV seropositivity. CONCLUSIONS: By the age of 11 years, EBV infection is present in over half the population, although age is not the only factor associated with serostatus. Knowledge of the distribution of infection in the UK population is critical for determining future vaccination policies, e.g. comparing general versus selectively targeted vaccination strategies
Osmoregulators proline and glycine betaine counteract salinity stress in canola
Salt inundation leads to increased salinization of arable land in many arid and semi-arid regions. Until genetic solutions are found farmers and growers must either abandon salt-affected fields or use agronomic treatments that alleviate salt stress symptoms. Here, field experiments were carried out to study the effect of the osmoregulators proline at 200 mg L-1 and glycine betaine at 400 mg L-1 in counteracting the harmful effect of soil salinity stress on canola plants grown in Egypt. We assessed growth characteristics, yield and biochemical constituents. Results show first that all growth characters decreased with increasing salinity stress but applied osmoregulators alleviated these negative effects. Second, salinity stress decreased photosynthetic pigments, K and P contents, whilst increasing proline, soluble sugars, ascorbic acid, Na and Cl contents. Third, application of osmoregulators without salt stress increased photosynthetic pigments, proline, soluble sugars, N, K and P contents whilst decreasing Na and Cl contents. It is concluded that the exogenously applied osmoregulators glycine betaine and proline can fully or partially counteract the harmful effect of salinity stress on growth and yield of canola.© INRA and Springer-Verlag, France 2012
Assessing cognitive dysfunction in Parkinson’s: An online tool to detect visuo-perceptual deficits
BACKGROUND: People with Parkinson’s disease
(PD) who develop visuo-perceptual deficits are at higher
risk of dementia, but we lack tests that detect subtle visuoperceptual
deficits and can be performed by untrained personnel.
Hallucinations are associated with cognitive impairment
and typically involve perception of complex objects.
Changes in object perception may therefore be a sensitive
marker of visuo-perceptual deficits in PD.
Objective: We developed an online platform to test
visuo-perceptual function. We hypothesised that (1)
visuo-perceptual deficits in PD could be detected using
online tests, (2) object perception would be preferentially
affected, and (3) these deficits would be caused
by changes in perception rather than response bias.
METHODS: We assessed 91 people with PD and 275 controls.
Performance was compared using classical frequentist
statistics. We then fitted a hierarchical Bayesian signal
detection theory model to a subset of tasks.
RESULTS: People with PD were worse than controls at
object recognition, showing no deficits in other visuoperceptual
tests. Specifically, they were worse at identifying
skewed images (P <.0001); at detecting hidden
objects (P 5.0039); at identifying objects in peripheral
vision (P <.0001); and at detecting biological motion
(P 5.0065). In contrast, people with PD were not worse
at mental rotation or subjective size perception. Using
signal detection modelling, we found this effect was
driven by change in perceptual sensitivity rather than
response bias.
CONCLUSIONS: Online tests can detect visuo-perceptual defi-
cits in people with PD, with object recognition particularly
affected. Ultimately, visuo-perceptual tests may be developed
to identify at-risk patients for clinical trials to slow PD dementia
Informing the public health response to COVID-19: a systematic review of risk factors for disease, severity, and mortality
BACKGROUND: Severe Acute Respiratory Syndrome coronavirus-2 (SARS-CoV-2) has challenged public health agencies globally. In order to effectively target government responses, it is critical to identify the individuals most at risk of coronavirus disease-19 (COVID-19), developing severe clinical signs, and mortality. We undertook a systematic review of the literature to present the current status of scientific knowledge in these areas and describe the need for unified global approaches, moving forwards, as well as lessons learnt for future pandemics. METHODS: Medline, Embase and Global Health were searched to the end of April 2020, as well as the Web of Science. Search terms were specific to the SARS-CoV-2 virus and COVID-19. Comparative studies of risk factors from any setting, population group and in any language were included. Titles, abstracts and full texts were screened by two reviewers and extracted in duplicate into a standardised form. Data were extracted on risk factors for COVID-19 disease, severe disease, or death and were narratively and descriptively synthesised. RESULTS: One thousand two hundred and thirty-eight papers were identified post-deduplication. Thirty-three met our inclusion criteria, of which 26 were from China. Six assessed the risk of contracting the disease, 20 the risk of having severe disease and ten the risk of dying. Age, gender and co-morbidities were commonly assessed as risk factors. The weight of evidence showed increasing age to be associated with severe disease and mortality, and general comorbidities with mortality. Only seven studies presented multivariable analyses and power was generally limited. A wide range of definitions were used for disease severity. CONCLUSIONS: The volume of literature generated in the short time since the appearance of SARS-CoV-2 has been considerable. Many studies have sought to document the risk factors for COVID-19 disease, disease severity and mortality; age was the only risk factor based on robust studies and with a consistent body of evidence. Mechanistic studies are required to understand why age is such an important risk factor. At the start of pandemics, large, standardised, studies that use multivariable analyses are urgently needed so that the populations most at risk can be rapidly protected. Reduced apoptotic levels in squamous but not basal cell carcinomas correlates with detection of cutaneous human papillomavirus
We have investigated the apoptotic levels and expression of the apoptotic inducer Bak in non-melanoma skin cancers. Squamous cell carcinomas of known human papillomavirus status from immunocompetent patients were analysed for the expression of the Bak protein, and the expression profile was compared both to the presence of apoptotic cells and the proliferation marker Ki-67. We demonstrate an inverse correlation between human papillomavirus positivity and Bak expression in squamous cell carcinomas, with concomitantly fewer apoptoic cells being detected in the human papillomavirus positive tumours. Bak expression was not observed in basal cell carcinomas irrespective of human papillomavirus status, suggesting that Bak only plays a role in signalling apoptosis in squamous, but not basal, cell cancers. No differences were observed in the proliferation rates between papillomavirus positive and negative squamous cell tumours. However, a significant decrease in the number of apoptotic cells was observed in human papillomavirus-positive squamous cell carcinomas which suggests that the virus may have significantly altered the relationship between proliferation and apoptosis in a proportion of these tumours
Tuberculosis treatment outcomes amongst disadvantaged patients in India
Setting: Urban slums and poor rural areas, India, 2012-2014.
Objective: To describe characteristics of tuberculosis (TB) patients enrolled in treatment through Operation ASHA, a non-governmental organisation serving disadvantaged populations in India, and to identify risk factors for unfavourable treatment outcomes.
Design: Retrospective cohort study. Patient characteristics were assessed for their relationship with treatment outcomes using mixed effects logistic regression adjusting for clustering by treatment centre and Indian state. Outcomes were considered favourable (cured/treatment completed) or unfavourable (treatment failure, loss to follow-up, death, switch to multi-drug resistant TB treatment, transfer out).
Results: 7148/8415 (84.9%) patients had a favourable outcome. On multivariable analysis, unfavourable outcomes were more common amongst men (odds ratio [OR]=1.31, 95% confidence interval [CI] 1.15-1.51), older patients (OR=1.12 [1.04-1.21]) and previously treated patients (OR=2.05 [1.79-2.36]). Compared to pulmonary smear-negative patients, those with extra-pulmonary disease were less likely (OR=0.72 [0.60-0.87]) whilst smear-positive pulmonary patients were more likely to have unfavourable outcomes (OR=1.38 [1.15-1.66] for low [scanty/1+] and OR=1.71 [1.44-2.04] high [2+/3+] positive smears, respectively).
Conclusion: The treatment success rate within Operation ASHA is comparable to that reported nationally for India. Men, older patients, retreatment cases and smear-positive pulmonary TB patients may need additional interventions to ensure a favourable outcome
