The extent of the genetic component in the treatment of JIA

Criteriile de apreciere a evoluției bolii la pacienți ca răspuns la tratamentul artritei juvenile idiopatice (AJI) au fost elaborate de Colegiului American de Reumatologie (ACR) în 1997. Polimorfismele genetice ale genei MTHFR sunt considerate un nou fundal științific pentru estimarea rezultatelor tratamentului cu Metotrexat. Scopul lucrării. A evalua relația dintre polimorfismele genei MTHFR la pacienții cu AJI care utilizează Metotrexat și indicele ACRpedi 30%. Material și metode. Studiul observațional caz-control a implicat 68 de pacienți care au utilizat Metotrexat pentru tratamentul AJI, au fost testați genetic și apreciată ameliorarea clinică după 24 de săptămâni de la debutul tratamentului. Rezultate. Au fost examinați 68 de copii, la care testele genetice au evidențiat 23 (33,8%) cazuri de MTHFR combinat C677T/A1298 (9 (39,1%)) și T677T (14 (60,9%)), la fel, 45 (66,2%) cazuri fără mutații. Distribuția pe sexe a fost de 37 (54,4%) fete și 31 (45,6%) băieți cu o vârstă medie de 133,8 luni CI 95% [0,81-0,97]. Din lotul în care s-a constatat polimorfismul genetic, doar 2 (8,7%) copii au obținut activitate scăzută sau remisiunea bolii, față de 24 (53,3%) copii din lotul fără mutații, conform ACRpedi 30% (χ² = 12,842, p = 0,0001). Concluzii. S-a determinat o relație semnificativă între fondul genetic MTHFR și evaluarea indicelui ACRpedi 30% la copiii cu AJI.Background. The criteria for evaluating the disease evolution in patients in response to the treatment of juvenile idiopathic arthritis (JIA) were developed by the American College of Rheumatology (ACR) in 1997. Genetic polymorphisms of the MTHFR gene are considered a new scientific background for estimating the results of treatment with methotrexate. Objective of the study. To evaluate the relationship between MTHFR gene polymorphisms in JIA patients using methotrexate and the ACRpedi 30% index. Material and methods. The observational case-control study involved 68 patients who used methotrexate for the treatment of JIA, were genetically tested and assessed for clinical improvement after 24 weeks from the start of treatment. Results. 68 children were examined, in whom genetic testing revealed 23 (33.8%) cases of combined MTHFR C677T/ A1298C (9 (39.1%)) and T677T (14 (60.9%)), likewise, 45 (66.2%) cases without mutations. The gender distribution was 37 (54.4%) girls and 31 (45.6%) boys with a mean age of 133.8 months 95% CI [0.81-0.97]. From the group in which the genetic polymorphism was found, only 2 (8.7%) children achieved low activity or disease remission, compared to 24 (53.3%) children from the group without mutations, according to ACRpedi 30% (χ² = 12.842 , p = 0.0001). Screening the C677T polymorphism of the MTHFR gene in assessing disease severity and response to methotrexate treatment in children with juvenile idiopathic arthritis

Monitoring methotrexate-induced liver toxicity in juvenile idiopathic arthritis: new perspectives

related to comorbid risk factors and common NSAIDS and steroid therapy use rather than to
methotrexate itself, significant research continues in the monitoring of low-dose methotrexate
in patients with JIA. The gold standard investigation remains to be liver biopsy with its
potential medical risks. However, a number of new evaluation techniques have been developed
for this purpose, including transient liver elastography. Moreover, MTHFR genetic
susceptibility according to Genome-Wide Association Studies (GWAS) is being involved in
most treatment regimen toxicities.Aim of the study. To appreciate the importance of MTHFR genetic polymorphism and liver
elastography screening in children with JIA prior to use of low-dose methotrexate treatment.
Materials and methods. There has been initiated an observational case-control study,
involving at least 24 patients using low-dose methotrexate for JIA treatment. All children
underwent transient unidimensional liver elastography scanning for estimation of liver toxicity
according to EFSUMB pediatric reference values. The statistical evaluation was done through
IBM SPSS 22 Software.
Results. The study sample included 40 children aged between 2 and 18 years. There has been
determined 6 (15%) cases of combined 677C/T and 1298A/C heterozygote significant
mutation, 6 (15%) cases of 677T/T significant homozygotes and 28 (70%) cases of nonsignificant
MTHFR polymorphisms. Children without significant MTHFR polymorphisms had
a 67,8% rate of increased liver stiffness and a moderate to low disease activity in the first 148,8
weeks of low-dose methotrexate use (95% CI 2.0-4.2, p=0,00012). In the significant mutation
groups, a 41,6% cases resulted in normal liver stiffness values after 6 months of low-dose
methotrexate monotherapy use as well as low response with high disease activity according to
DAS28 (95% CI 3,6-6.1, p=0,00026).
Conclusions. The value of MTHFR genetic screening and liver stiffness evaluation is well
proved in children with low-dose methotrexate JIA treatment. The significant mutations could
lead to 4-fold risk of high disease activity and normal liver stiffness despite the appropriate
treatment regimen
Importance of genotype C677T of methylenetetrahydrofolate reductase enzyme in early appreciation of non-responsiveness to methotrexate therapy in juvenile idiopathic arthritis

Intoleranţa la metotrexat reprezintă o problemă frecventă în tratamentul de lungă durată a artritei juvenile idiopatice. Genotiparea enzimei metilentetrahidrofolat reductaza ar avea o valoare predictivă în estimarea riscului de nonrăspuns la tratamentul de lungă durată cu metotrexat.
Materiale şi metode. A fost iniţiat un studiu analitic, tip caz-martor, cu elucidarea prin tehnica PCR real time, a prezenţei mutaţiei frecvente C677T a metilentetrahidrofolat reductazei la copiii cu artrită juvenilă idiopatică ce urmează metotrexat. A fost evaluată
eficacitatea terapeutică la distanţă mai mare de 6 luni de la debutul administrării metotrexatului prin scorurile DAS28, JADAS – 71, ACR pedi 30. A fost apreciată corelarea eficacităţii clinice în funcţie de polimorfismul genetic prin prelucrare statistică în cadrul IBM SPSS Statistics 22.
Rezultate. Au fost examinaţi 18 copii care urmează metotrexat în cadrul artritei juvenile idiopatice la care locusul 677 al genei cercetate a prezentat 7 (38,9%) cazuri fără mutaţie, 2 (11,1%) cazuri de mutaţii homozigote T/T, şi 9 (50%) mutaţii heterozigote C/T. Scorul DAS28 şi Scorul JADAS – 71 s-a apreciat mai mare în cadrul heterozigoţilor cu media 5,03 (p=0,0005) şi respectiv 18,1(p=0,0013), comparativ cu lotul copiilor fără mutaţie – 3,17 (p=0,00017) şi respectiv 2,4 (p=0,0022). Indicele ACR pedi 30 la heterozigoţi a fost apreciat în medie cu 22 % (p=0,0011) eficacitate, comparativ cu lotul de control – 37 % (p=0,001).
Concluzii. A fost apreciată o corelaţie semnificativă privind statutul heterozigot C677T şi rata de nonrăspuns la terapia cu metotrexat în artrita juvenilă idiopatică. Validarea, în cohortă independentă, a screeningului prin genotipare a enzimei metilentetrahidrofolat reductaza în artrita juvenilă idiopatică, ar ajuta clinicienii în identificarea pacienţilor cu risc de dezvoltare a intoleranţei terapeutice.Introduction. Methotrexate intolerance is a frequent problem in the long-term treatment of juvenile idiopathic arthritis. Genotyping the methylenetetrahydrofolate reductase enzyme could have a predictive value in estimating the risk of non-response to long-term treatment with methotrexate.
Materials and methods. There has been initiated an observational, case-control study, with assessing through PCR real-time technique the frequent mutation C677T of methylenetetrahydrofolate reductase in children with juvenile idiopathic arthritis that administer methotrexate. There have been evaluated the treatment efficacy at more than 6 months from the methotrexate initiation through DAS28, JADAS – 71, ACR Pedi 30 scores. There has been estimated the correlation of clinical efficacy to the genetic polymorphism through IBM SPSS Statistics 22.
Results. There has been examined 18 children that administer methotrexate in juvenile idiopathic arthritis in whom the 677 loci of the assessed gene presented 7 (38,9%) cases of no mutation, 2 (11,1%) cases of T/T homozygotes and
9 (50%) cases of C/T heterozygotes. The DAS28 and JADAS – 71 scores was higher in the heterozygote cases with the mean value of 5,03 (p=0,0005) and 18,1(p=0,0013) respectively, compared to the non-mutation sample – 3,17
(p=0,00017) and 2,4 (p=0,0022) respectively. The ACR Pedi 30 in the heterozygote sample had a mean value of 22% (p=0,0011) improvement compared to the control group – 37 % (p=0,001).
Conclusion. There has been determined a significant correlation between the heterozygote C677T status and the
non-response rate to methotrexate therapy in juvenile idiopathic arthritis. Validation, in an independent cohort, of the screening through genotyping the methylenetetrahydrofolate reductase enzyme in juvenile idiopathic arthritis could assist clinicians in the identification of patients at risk to develop therapeutic intolerance
Elastografia hepatică și activitatea bolii în AJI: noi perspective
Paediatrics Department, Nicolae Testemitanu SUMPhBackground. Liver injury appreciation in JIA patients with long-term low-dose methotrexate use is still
controversial and needs to be screened in large research groups. It is necessary to see its impact on the
disease activity. Objective of the study. To appreciate the degree of liver fibrosis in correspondence
with the duration of treatment with methotrexate in children with JIA as well as the correlation to its
disease activity. Material and Methods. Patients who received low-dose methotrexate greater than six
months have been examined by transient unidimensional liver elastography. The M probe was used. The
results were referred to the European Federation of Societies for Ultrasound in Medicine and Biology
data. Results. Out of 64 patients from the Rheumatology Clinic of the Institute of Mother and Child,
Chisinau, Republic of Moldova, who used low-dose methotrexate in JIA, only 43 eligible patients have
done liver elastography. It was established about ¾ of them having the liver stiffness median greater
than 4,7kPa after 36 weeks of methotrexate use (p < 0,001). It was found an essential decrease in disease
activity according to DAS28 in patients with high liver stiffness due to the cumulative methotrexate.
Conclusion. Because cumulation of low-dose methotrexate has an impact on the liver toxicity, the use
of liver elastography as a non-invasive method of its appreciation makes possible to control the toxicity
and the disease activity.Introducere. Aprecierea injuriei hepatice la pacienții cu AJI, care urmează doze mici de metotrexat pe
termen lung, încă mai prezintă controverse și necesită a fi evaluată în eșantioane largi. Este necesar de
a vedea impactul acesteia asupra activității bolii. Scopul lucrării. Aprecierea gradului de fibroză
hepatică în corespundere cu durata tratamentului cu metotrexat la copiii cu AJI, precum și corelarea cu
activitatea bolii a acesteia. Material și metode. Pacienții care au primit doze mici de metotrexat, pe o
durată mai mare de șase luni de zile, au fost examinați prin intermediul elastografiei tranzitorii
unidimensionale a ficatului. A fost utilizată sonda M. Rezultatele au fost relaţionate la datele Federației
Europene a Societăților pentru Ultrasunet în Medicină și Biologie. Rezultate. Dintre 64 de pacienți din
cadrul Clinicii de Reumatologie a Institutului Mamei și Copilului din Chișinău, Republica Moldova,
care au utilizat doze mici de metotrexat în AJI, doar 43 de pacienți eligibili au efectuat elastografia
hepatică. A fost stabilit că, aproape ¾ din ei au avut mediana elasticității hepatice mai mare de 4,7kPa,
după 36 de săptămâni de metotrexat (p < 0,001). A fost observată o reducere esențială în activitatea bolii
conform DAS28, la pacienții cu densitate hepatică sporită, datorită efectului cumulativ a metotrexatului.
Concluzii. Deoarece cumularea metotrexatului în doze mici exercită impact asupra toxicității hepatice,
utilizarea elastografiei hepatice, ca metodă non-invazivă de apreciere a sa, face posibil controlul
toxicității și al activității bolii
Afectarea oculară în artrita juvenilă idiopatică
Background. Idiopathic juvenile arthritis (JIA) is the most
common rheumatic disease in children, and uveitis is the
most common extra-articular manifestation. Uveitis is potentially life-threatening and thus poses a considerable
risk of morbidity associated with decreased quality of life.
Objective of the study. Correlation of data from the literature with the preliminary results of the prospective study
for the analysis of eye damage in children with idiopathic
juvenile arthritis. Material and Methods. The literature review included analysis of articles from the PubMed, Medline databases from 2017-2022 to the keywords: idiopathic
juvenile arthritis, eye damage, and uveitis. The prospective
study included 30 children diagnosed with JIA, with ocular
impairment, aged < 16 years, hospitalized in the rheumatology department of the Mother and Child Institute. Results.
The demographic analysis of the study subjects revealed:
the mean age of onset of JIA was 6.95±4.43 years; and the
onset of uveitis was 6.98±3.5 years. The mean duration between onset of JIA and onset of uveitis was 22.07±26.6 months. Female patients (80%) developed JIA associated with
uveitis more frequently. The oligoarthritis (56.6%) appeared the most common one followed by FR-negative polyarthritis (23.3%). Serologically, the titer of ANA was positive
in 26.6% of cases. The most common form of uveitis seen in
combination with AJI was anterior uveitis (80%). At least
63.3% of patients reported an eye complication. Irreversible vision loss in 16.7% of cases. Conclusion. Uveitis has an
insidious onset, chronic evolution and a high incidence of
disabling complications. Due to the progression of the joint
syndrome and the deterioration of the ocular apparatus, JIA
can quickly lead a patient to a disability, including decreased visual acuity, often to complete blindness.Introducere. Artrita juvenilă idiopatică (AJI) este cea mai

pacient la dizabilitate, inclusiv din cauza scăderii acuității
vizuale, adesea până la orbire completă
THE IMPORTANCE OF THE METHOTREXATE INTOLERANCE SEVERITY SCORE IN JUVENILE IDIOPATHIC ARTHRITIS

Aproximativ 40% dintre pacienții cu artrită juvenilă idiopatică (AJI) nu tolerează metotrexatul (MTX) și, pe măsură ce aceasta este detectată timpuriu prin scoruri validate, cu atât mai bine este controlată activitatea bolii. Scopul lucrării. Acest studiu a avut ca scop aprecierea ratei de intoleranță la MTX prin Scorul de Severitate al Intoleranței la Metotrexat (MISS). Material și Metode. În Clinica de Reumatologie a Institutului Mamei și Copilului a fost efectuat un studiu caz-control cu 68 de probanzi cu AJI. Acești copii și părinții lor au răspuns la MISS, considerat important cu scor de intoleranță ≥ 6 puncte. Datele demografice și activitatea bolii au fost colectate din fișele medicale. Rezultate. Distribuția pe sexe a fost predominant de sex feminin 56,3%, iar rata adolescenților a fost de 54,7%. Durata bolii a fost estimată la 5,6±1,1 ani, iar durata administrării MTX a fost de 3,2±2,1 ani. Un MISS de ≥ 6 puncte a fost găsit la 34% dintre copii. Simptomele care au stabilit această rată importantă după utilizarea MTX au fost greața 86%, durerea abdominală 72,8%, vomele 36% și în final 5% pentru simptomele comportamentale. Cu toate acestea, s-a constatat o rată de 94% a simptomelor comportamentale înainte de administrarea MTX, care ar putea fi un motiv pentru necomplianța pacientului la MTX (p = 0,002). Concluzii. Utilizarea MISS este importantă în aprecierea intoleranței la MTX, în special în frecvența simptomelor comportamentale care ar putea-o induce. Studiul a arătat că MISS este un instrument clinic pentru detectarea precoce a pacienților cu non-răspuns la MTX.Background. Objective of the study. This study was aimed to find the rate of MTX intolerance through the Methotrexate Intolerance Severity Score (MISS). Material and Methods. These children and their parents answered the MISS, considered important with intolerance score ≥ 6 points. Demographic data and disease activity were collected from the medical records. Results. The gender distribution was predominantly females 56.3% and the adolescent rate was 54.7%. The disease duration was estimated 5.6±1.1 years, and the duration of MTX administration was 3.2±2.1 years. A MISS of ≥ 6 points was found in 34% of children. The symptoms that established this important rate after MTX use were nausea 86%, abdominal pain 72.8%, vomiting 36% and finally 5% for behavioral symptoms. However, it was found a 94% rate of behavioral symptoms before MTX administration which might be a reason for the patient noncompliance on MTX (p = 0.002). Conclusion. The use of MISS is important in appreciation of intolerance to MTX especially in the frequency of behavioral symptoms that might induce it. The study showed that MISS is a clinical tool for early detection of MTX nonresponse patients

years. A MISS of ≥ 6 points was found in 34% of children.
The symptoms that established this important rate after
MTX use were nausea 86%, abdominal pain 72.8%, vomiting 36% and finally 5% for behavioral symptoms. However, it was found a 94% rate of behavioral symptoms before
MTX administration which might be a reason for the patient
noncompliance on MTX (p = 0.002). Conclusion. The use
of MISS is important in appreciation of intolerance to MTX
especially in the frequency of behavioral symptoms that might induce it. The study showed that MISS is a clinical tool

bolii. Scopul lucrării. Acest studiu a avut ca scop aprecierea ratei de intoleranță la MTX prin Scorul de Severitate
al Intoleranței la Metotrexat (MISS). Material și Metode.
În Clinica de Reumatologie a Institutului Mamei și Copilului
a fost efectuat un studiu caz-control cu 68 de probanzi cu
AJI. Acești copii și părinții lor au răspuns la MISS, considerat
important cu scor de intoleranță ≥ 6 puncte. Datele demografice și activitatea bolii au fost colectate din fișele medicale. Rezultate. Distribuția pe sexe a fost predominant de
sex feminin 56,3%, iar rata adolescenților a fost de 54,7%.
Durata bolii a fost estimată la 5,6±1,1 ani, iar durata administrării MTX a fost de 3,2±2,1 ani. Un MISS de ≥ 6 puncte
a fost găsit la 34% dintre copii. Simptomele care au stabilit

clinică și paraclinică. Printre principalele diagnostice presumptive la copilul primar debutat cu durere în articulația
șoldului în țările în curs de dezvoltare cu rată endemică sporită se enumeră și tuberculoza extrapulmonară articulară.
Tuberculoza extrapulmonară este mai frecventă în cazul copiilor mici. Odată instalată, este deobicei monoarticulară,
afectând în special articulațiile mari. Sinovita tuberculoasă izolată este paucisimptomatică, cu limitarea mișcării, fiind
diagnosticată tardiv. Artrita tuberuloasă se asociază cu un grad de osteită, amiotrofie progresivă a grupelor musculare
adiacente și distrucție ireversibilă a articulației. Diagnosticul este stabilit în funcție de stadiul clinicoradiologic,
examenul bacteriologic pentru Mycobacterium tuberculosis din lichidul sinovial sau examenul histopatologic din
biopsia sinovială asociate cu IDR pozitivă la tuberculină. Evoluția este favorabilă sub tratament antituberculos asociat
