Abstract
Background. Idiopathic juvenile arthritis (JIA) is the most
common rheumatic disease in children, and uveitis is the
most common extra-articular manifestation. Uveitis is potentially life-threatening and thus poses a considerable
risk of morbidity associated with decreased quality of life.
Objective of the study. Correlation of data from the literature with the preliminary results of the prospective study
for the analysis of eye damage in children with idiopathic
juvenile arthritis. Material and Methods. The literature review included analysis of articles from the PubMed, Medline databases from 2017-2022 to the keywords: idiopathic
juvenile arthritis, eye damage, and uveitis. The prospective
study included 30 children diagnosed with JIA, with ocular
impairment, aged < 16 years, hospitalized in the rheumatology department of the Mother and Child Institute. Results.
The demographic analysis of the study subjects revealed:
the mean age of onset of JIA was 6.95±4.43 years; and the
onset of uveitis was 6.98±3.5 years. The mean duration between onset of JIA and onset of uveitis was 22.07±26.6 months. Female patients (80%) developed JIA associated with
uveitis more frequently. The oligoarthritis (56.6%) appeared the most common one followed by FR-negative polyarthritis (23.3%). Serologically, the titer of ANA was positive
in 26.6% of cases. The most common form of uveitis seen in
combination with AJI was anterior uveitis (80%). At least
63.3% of patients reported an eye complication. Irreversible vision loss in 16.7% of cases. Conclusion. Uveitis has an
insidious onset, chronic evolution and a high incidence of
disabling complications. Due to the progression of the joint
syndrome and the deterioration of the ocular apparatus, JIA
can quickly lead a patient to a disability, including decreased visual acuity, often to complete blindness.Introducere. Artrita juvenilă idiopatică (AJI) este cea mai
frecventă boală reumatică la copii, iar uveita este cea mai
frecventă manifestare extraarticulară. Uveita este potențial
amenințătoare pentru vedere și, prin urmare, prezintă un
risc considerabil de morbiditate asociată cu scăderea calității vieții. Scopul lucrării. Corelarea datelor din literatura
de specialitate cu rezultatele preliminare ale studiului prospectiv de analiză a afectării oculare la copiii cu artrită juvenilă idiopatică. Material și Metode. Reviul literaturii a inclus analiza articolelor din bazele de date Pubmed, Medline
din perioada 2017-2022 la cuvintele cheie: artrita juvenilă
idiopatică, afectare oculară, uveită. Studiul prospectiv a inclus 30 de copii diagnosticați cu AJI, cu afectarea oculară, cu
vârsta < 16 ani, internați în secția de reumatologie, a Institutului Mamei și Copilului. Rezultate. Analiza demografică
a subiecților din studiu a relevat: vârsta medie la debutul AJI
a fost de 6,95 ± 4,43 ani, iar debutul uveitei a fost la 6,98 ±
3,5 ani. Durata medie dintre debutul AJI și uveitei a fost de
22,07 ± 26,6 luni. Paciente de sex feminin (80%) au dezvoltat AJI asociată cu uveită mai frecvent. Cel mai frecvent s-a
constatat oligoartrita (56,6%), apoi poliartrita FR negativ
(23,3%). Titrul ANA a fost pozitiv în 26,6% cazuri. Cea mai
frecventă formă de uveită observată în asociere cu AJI a fost
uveita anterioară (80%). Cel puțin o complicație oculară a
fost raportată pentru 63,3% dintre pacienți, pierderea ireversibilă a vederii în 16,7% din cazuri. Concluzii. Uveita are
debut insidios, evoluție cronică și incidență mare a complicațiilor invalidante. Datorită progresiei sindromului articular și deteriorarea aparatului ocular, AJI poate duce rapid un
pacient la dizabilitate, inclusiv din cauza scăderii acuității
vizuale, adesea până la orbire completă
