    Induced astigmatism and concomitant high myopia correction with femtosecond laser-assisted intrastromal MyoRing implantation

    Purpose. Analysis of the results of astigmatism correction after penetrating keratoplasty and concomitant high myopia using the MyoRing ring implantation, clinical case as an example.Material and methods. Patient G., 51 years old, underwent surgery to correct astigmatism and concomitant high myopia. MyoRing was implanted in the right eye within corneal transplant limits using femtosecond laser-assisted intrastromal MyoRing implantation. Before the surgery uncorrected visual acuity at the operated eye was 0.01; corrected visual acuity 0.06; spherical component of refraction was (-)6.0 D, cylindrical component was (-)8.0 D. Mean keratometry in the right eye was 43.96 D. Corneal hysteresis was 6.5 mm Hg, corneal resistance factor was 5.5 mm Hg, cornea thickness in center was 529 mkm. The patient was followed up for one year.Results. In 12 months after the surgery visual acuity of the right eye without correction was 0.7 and 0.8 with correction. Spherical component was (-)1.0 D, cylindrical component was (-)2.0 D. Mean keratometry in the right eye was 35.10 D. Corneal hysteresis was 7.2 mm Hg, corneal resistance factor was 6.3 mm Hg, cornea thickness in center was 533 mkm. The patient was satisfied with the result of operation.Conclusion. Femtosecond laser-assisted intrastromal MyoRing implantation for correction induced astigmatism after penetrating keratoplasty and concomitant high myopia is efficient, safe and provides strengthening of biomechanical properties of the cornea besides refractive and visual effect

    Assessment of the effectiveness of surgical treatment of patients with massive ruptures of rotator cuff tendons using latissimus dorsi tendon transposition

    Rotator cuff ruptures are the most common injuries of shoulder joint with an incidence of about 20 %. This pathology is more common in adults over 60 years of age because it is caused by degenerative changes in the tendon. Massive injuries account for 10–40 % of all rotator cuff injuries. Currently, there is no unified surgical tactics for the treatment of patients with massive ruptures of rotator cuff tendons.The aim. To assess the efficiency of transposition of the latissimus dorsi tendon in patients with massive ruptures of the rotator cuff tendon.Materials and methods. The study included 15 patients with Patte stage III massive ruptures of the rotator cuff who had transposition of the latissimus dorsi tendon.Results. The article presents clinical cases of surgical treatment of patients. The following criteria were assessed: mean age; time since injury; duration of the surgery; blood loss volume; functional results by the ASES (American Shoulder and Elbow Surgeons) Shoulder Score. Taking into account the ASES Shoulder Score indicators 1 year after the surgical treatment, the following results were obtained: excellent results – in 9 (53.3 %) cases; good results – in 1 (13.4 %) case; satisfactory results – in 5 (33.3 %) cases.Conclusion. When preserving the articular cartilage, the method of choice in the treatment of patients with massive ruptures of rotator cuff tendons is transposition of the latissimus dorsi tendon. At the same time, an incomplete restoration of the function of the injured limb was registered in 33.3 % of patients, which requires further study and modification of the known method of transposition of the latissimus dorsi tendon

    BONE MINERAL DENSITY INDICES IN PATIENTS WITH CERVICAL OSTEOCHONDROSIS

    According to the data of the World Health Organization, osteoporosis takes the fourth place among non-infectious diseases after cardiovascular, oncologic disorders and diabetes mellitus. Clinical manifestations of osteoporosis with the root syndrome are not that different from those of radicular symptoms at degenerative-dystrophic processes in the spine. Our purpose was to study the state of upper extremities bone tissue in patients with clinical manifestations of cervical osteochondrosis. There were 40 patients under our observation diagnosed with cervical osteochondrosis, 26 of them - with stages 1 and 2 of osteochondrosis development, and 14 patients - with stage 3. Conducted densitometry examinations of upper extremities bone tissue revealed correlation between bone tissue state and both the time of the onset of the pathological process and its severity. In patients with stage 3 osteochondrosis, bone tissue mineral density on the affected extremity was lowered down to osteoporosis, and naturally, such cases require further treatment

    We investigated the polymorphism of the gene p53 exon 4 Arg72Pro and intron 3 ins/del 16 b. p. in children with juvenile idiopathic arthritis in order to identify the influence of gene polymorphisms on the course and outcome of the disease. The children were divided into several groups according to the sex, type of articular lesion and the effectiveness of treatment. The main criteria of therapy effectiveness was defined as achieving "inactive" arthritis as well as a clinical (pharmacologic) remission. The correlation between p53 gene genotypes among girls with oligo-, polyarthritis of juvenile idiopathic arthritis was shown for the first time. The data obtained demonstrates the necessity for further studies of the correlation between genotype p53 and juvenile idiopathic arthritis.Проведено исследование полиморфизма гена p53 Arg72Pro 4 экзона и ins/del 16 н. п. 3 интрона у детей с ювенильным идиопатическим артритом с целью выявления влияния генного полиморфизма на течение и исход заболевания. Дети были разделены на несколько групп — учитывали пол, вариант суставного поражения, эффективность проводимого лечения. Главным критерием эффективности лечения считали достижение «неактивного» артрита с выходом в клинико-медикаментозную (фармакологическую) ремиссию. Впервые была показана взаимосвязь распределения генотипов гена p53 у девочек с олиго-полиартикулярным вариантом ювенильного идиопатического артрита с длительностью заболевания и эффективностью противоревматической терапии. Полученные данные свидетельствуют о необходимости дальнейших исследований взаимосвязи полиморфизмов гена p53 и ювенильного идиопатического артрита у детей

    Clinical and epidemiological features of acute intestinal infections in children in Arkhangelsk for 2015-2018

    In recent years, there is increasing evidence of the significant role of the intestinal microbiome in the normal functioning of the whole body. The composition of the intestinal microflora affects the human immune system, and its metabolic processes, and even behavior. Therefore, maintaining an optimal balance in the structure of a normal gut microbiome is very important for human health. But the high prevalence of acute intestinal infections(AII) suggests that this balance is very fragile. The diversity of the causes of the All and the greater their specific gravity to determine the practical value of the clinical-epidemiological diagnosis, and was the basis for conducting our study. The aim of our work was to identify clinical and epidemiological features of acute intestinal infections in children of the Arkhangelsk region. We have evaluated the indicators of children's morbidity of acute intestinal infections in the Arkhangelsk region, as well as a retrospective clinical and epidemiological analysis in 143 children who underwent acute intestinal infections in 2015-2018 and hospitalized in the Center for infectious diseases of the Arkhangelsk regional clinical hospital. There is a high proportion of intestinal infections of unknown etiology (55.9%). Viral infections (mainly due to rotavirus gastroenteritis) prevail in the structure of All with established etiology. Among bacterial All met salmonellosis, shigellosis and campylobacteriosis.В последние годы появляется всё больше данных о существенной роли кишечного микробиома в нормальном функционировании всего организма. Состав кишечной микрофлоры влияет и на иммунную систему человека, и на его обменные процессы, и даже на поведение. Поэтому поддержание оптимального баланса в структуре нормального микробиома кишечника очень важно для здоровья человека. Но высокая распространённость острых кишечных инфекций (ОКИ) говорит о том, что баланс этот очень хрупок. Многообразие причин ОКИ и большой их удельный вес определяют практическое значение клинико-эпидемиологической диагностики, все это и явилось основанием для проведения нашего исследования. Целью нашей работы стало выявление клинико-эпидемиологических особенностей острых кишечных инфекций у детей Архангельской области. Нами была проведена оценка показателей детской заболеваемости острыми кишечными инфекциями в Архангельской области, а также ретроспективный клинико-эпидемиологический анализ у 143 детей, перенесших острые кишечные инфекции в 2015-2018 гг. и госпитализированных в Центр инфекционных болезней Архангельской областной клинической больницы. Отмечается высокий удельный вес кишечных инфекций неустановленной этиологии (55,9%). В структуре ОКИ с установленной этиологией превалируют вирусные инфекции (в основном, за счёт ротавирусного гастроэнтерита). Среди бактериальных ОКИ встречались сальмонеллёз, дизентерия и кампилобактериоз
