Low Socioeconomic Status is an Independent Risk Factor for Survival After Abdominal Aortic Aneurysm Repair and Open Surgery for Peripheral Artery Disease
OBJECTIVE/BACKGROUND: The association between socioeconomic status (SES), presentation, and outcome after vascular surgery is largely unknown. This study aimed to determine the influence of SES on post-operative survival and severity of disease at presentation among vascular surgery patients in the Dutch setting of equal access to and provision of care.
METHODS: Patients undergoing surgical treatment for peripheral artery disease (PAD), abdominal aortic aneurysm (AAA), or carotid artery stenosis between January 2003 and December 2011 were retrospectively included. The association between SES, quantified by household income, disease severity at presentation, and survival was studied using logistic and Cox regression analysis adjusted for demographics, and medical and behavioral risk factors.
RESULTS: A total of 1,178 patients were included. Low income was associated with worse post-operative survival in the PAD cohort (n = 324, hazard ratio 1.05, 95% confidence interval [CI] 1.00-1.10, per 5,000 Euro decrease) and the AAA cohort (n = 440, quadratic relation, p = .01). AAA patients in the lowest income quartile were more likely to present with a ruptured aneurysm (odds ratio [OR] 2.12, 95% CI 1.08-4.17). Lowest income quartile PAD patients presented more frequently with symptoms of critical limb ischemia, although no significant association could be established (OR 2.02, 95% CI 0.96-4.26).
CONCLUSIONS: The increased health hazards observed in this study are caused by patient related factors rather than differences in medical care, considering the equality of care provided by the study setting. Although the exact mechanism driving the association between SES and worse outcome remains elusive, consideration of SES as a risk factor in pre-operative decision making and focus on treatment of known SES related behavioral and psychosocial risk factors may improve the outcome of patients with vascular disease
Abcesso Hepático por Streptococcus Anginosus e Eikenella Corrodens, Secundário a Perfuração Gástrica por Espinha de Peixe
Introduction: Foreign-body ingestion is a common event, but in only less than 1% of the cases complications occur. Hepatic abscesses induced by foreign-body penetration are rare. To date, there are only 62 reported cases of hepatic abscess secondary to fish bone perforation of the gastrointestinal tract.
Case presentation: A 78-year-old male patient was admitted due to high fever and vomiting for 2 days, along with frequent eructations for the past 3 months. Abdominal ultrasound showed a liver abscess in the left lobe, and computed tomography revealed a hyperdense linear image that crossed the superior wall of the gastric antrum, contacting the liver lesion, suggestive of a foreign body, probably a fish bone. Blood cultures were positive with isolation of Streptococcus anginosus and Eikenella corrodens. Ultrasound-guided percutaneous drainage of the abscess was done, and S. anginosus was isolated in the pus. Surgical debridement and fish bone removal were performed; the patient completed 21 days of antibiotic therapy, with a favorable evolution.
Conclusion: To the best of our knowledge, this is the first reported case of liver abscess caused by fish bone penetration with isolation of S. anginosus and E. corrodens. Bacterial coaggregation is one of the mechanisms that can explain their ability for causing invasive infections away from the oral cavity, by increasing their resistance to the innate immune system and survival of both species.info:eu-repo/semantics/publishedVersio
Biophony in a noisy tropical urban forest fragment
Anthropogenic noise, which is part of an urban soundscape, can negatively affect the behaviour of wild animals. Here we investigated how biophony (animal sounds) was affected by noise in an urban Brazilian forest frag-ment. Our hypothesis was that noise and biophony would differ between the border and the centre of the forest fragment (i.e., lower biophony predicted in noisy areas). Two passive acoustic monitoring devices were used to record soundscapes one week per month, 24 hour per day, from May to July 2012. The Acoustic Complexity Index (ACI) was used to quantify biophony and the Power Spectral Density (PSD) to quantify urban noise. PSD and ACI were higher on the border than in the centre of the fragment. PSD was lower in July, while the ACI did not significantly vary between months. Noise levels were also higher on the border. Conversely, potential spe-cies richness was higher in the centre of the forest fragment. Higher biophony at noisy sites can be interpreted as behavioural responses of species for communicating in noisy areas. Alternatively, they could be the result of species segregation by degree of vocal plasticity or due to differences in composition of communities
Prevalência e fatores associados aos comportamentos sedentários em adolescentes
OBJETIVO Analisar a prevalência e fatores associados aos comportamentos sedentários em adolescentes. MÉTODOS Estudo transversal com adolescentes de 10 a 17 anos de idade, de ambos os sexos, pertencentes a uma coorte de nascimentos entre 1994-1999 na cidade de Cuiabá, Mato Grosso, Brasil. Para o levantamento dos dados, foi utilizado um questionário contendo informações sociodemográficas, econômicas e de estilo de vida e aferição de dados antropométricos. Determinou-se como comportamento sedentário o uso de televisão e/ou computador/vÃdeo games por um tempo igual ou superior a 4 horas/dia. Avaliou-se a associação de comportamentos sedentários com o Ãndice de massa corporal, tanto na infância quanto na adolescência, e com variáveis sociodemográficas e comportamentais por meio de regressão logÃstica hierarquizada. RESULTADOS A prevalência global de comportamentos sedentários foi de 58,1%. Dos 1.716 adolescentes estudados, 50,7% (n = 870) eram do sexo masculino. Na análise multivariada, após ajuste para fatores de confusão, as variáveis que permaneceram associadas com os comportamentos sedentários foram: idade (14 anos ou mais) (OR = 3,51; IC95% 2,19;5,60); classe econômica elevada (OR = 3,83; IC95% 2,10;7,01); maior nÃvel de escolaridade da mãe (OR = 1,81; IC95% 1,09;3,01); residir no interior (OR = 0,49; IC95% 0,30;0,81); atividade fÃsica insuficiente (OR = 1,25; IC95% 1,02;1,53); experimentação de bebidas alcoólicas (OR = 1,34; IC95% 1,08;1,66) e excesso de peso na adolescência (OR = 1,33; IC95% 1,06;1,68). CONCLUSÕES A elevada proporção de adolescentes em atividades sedentárias e a não associação dessas atividades na adolescência com o excesso de peso na infância indicam a necessidade de intervenções para redução de vários comportamentos de risco. O incentivo à prática de atividade fÃsica como forma de reduzir os comportamentos sedentários e consequentemente o excesso de peso entre os jovens torna-se fundamental
Targeted Chromosomal Insertion of Large DNA into the Human Genome by a Fiber-Modified High-Capacity Adenovirus-Based Vector System
A prominent goal in gene therapy research concerns the development of gene transfer vehicles that can integrate exogenous DNA at specific chromosomal loci to prevent insertional oncogenesis and provide for long-term transgene expression. Adenovirus (Ad) vectors arguably represent the most efficient delivery systems of episomal DNA into eukaryotic cell nuclei. The most advanced recombinant Ads lack all adenoviral genes. This renders these so-called high-capacity (hc) Ad vectors less cytotoxic/immunogenic than those only deleted in early regions and creates space for the insertion of large/multiple transgenes. The versatility of hcAd vectors is been increased by capsid modifications to alter their tropism and by the incorporation into their genomes of sequences promoting chromosomal insertion of exogenous DNA. Adeno-associated virus (AAV) can insert its genome into a specific human locus designated AAVS1. Trans- and cis-acting elements needed for this reaction are the AAV Rep78/68 proteins and Rep78/68-binding sequences, respectively. Here, we describe the generation, characterization and testing of fiber-modified dual hcAd/AAV hybrid vectors (dHVs) containing both these elements. Due to the inhibitory effects of Rep78/68 on Ad-dependent DNA replication, we deployed a recombinase-inducible gene switch to repress Rep68 synthesis during vector rescue and propagation. Flow cytometric analyses revealed that rep68-positive dHVs can be produced similarly well as rep68-negative control vectors. Western blot experiments and immunofluorescence microscopy analyses demonstrated transfer of recombinase-dependent rep68 genes into target cells. Studies in HeLa cells and in the dystrophin-deficient myoblasts from a Duchenne muscular dystrophy (DMD) patient showed that induction of Rep68 synthesis in cells transduced with fiber-modified and rep68-positive dHVs leads to increased stable transduction levels and AAVS1-targeted integration of vector DNA. These results warrant further investigation especially considering the paucity of vector systems allowing permanent phenotypic correction of patient-own cell types with large DNA (e.g. recombinant full-length DMD genes)
Balanced Schnyder woods for planar triangulations: an experimental study with applications to graph drawing and graph separators
In this work we consider balanced Schnyder woods for planar graphs, which are
Schnyder woods where the number of incoming edges of each color at each vertex
is balanced as much as possible. We provide a simple linear-time heuristic
leading to obtain well balanced Schnyder woods in practice. As test
applications we consider two important algorithmic problems: the computation of
Schnyder drawings and of small cycle separators. While not being able to
provide theoretical guarantees, our experimental results (on a wide collection
of planar graphs) suggest that the use of balanced Schnyder woods leads to an
improvement of the quality of the layout of Schnyder drawings, and provides an
