    EXPLORING THE POSSIBILITY FOR A PERVASIVE TECHNOLOGY SOLUTION TO FACILITATE EFFECTIVE DIABETES SELF-CARE FOR PATIENTS WITH GDM

    Gestational diabetes(GDM) is increasing exponentially. To address this, we contend that a pervasive mobile solution that can facilitate self-care is a prudent strategy. To demonstrate proof of concept, user fidelity, usability and acceptability of such a solution a small pilot study of 10 patients was designed. A quasi-experimental approach employing a randomized control trial with a two period cross-over clinical trial strategy over 10 week duration at a private hospital in Melbourne, Australia was adopted. The main outcomes of interest were to establish proof of concept and the fidelity of the technology solution. Directional data gathered including patient and clinical interview data serve to demonstrate both proof of concept and fidelity. Without exception both patients and clinicians have preferred the pervasive mobile solution to the standard care approach. The study serves to highlight that pervasive mobile solutions can provide a useful adjunct in the effective monitoring and management of GDM.

    Psychiatric morbidity in older people with moderate and severe learning disability (mental retardation). Part I: development and reliability of the patient interview (the PAS-ADD)

    This paper describes the development of the PAS-ADD, a semistructured clinical interview for use specifically with patients with learning disabilities, based on items drawn from the PSE. The PAS-ADD includes a number of novel features including: parallel interviewing of patient and informant; a three-tier structure to provide a flexible interview appropriate to the patient's intellectual level; use of a memorable 'anchor event' in the patient's life to improve time focus; and simplified wording, improved organisation and lay out. Inter-rater reliability was investigated using an experimental design in which two raters viewed and re-rated videotaped PAS-ADD interviews which had been conducted by an experienced clinician. Reliability results compared favourably with those obtained in a major study of PSE reliability with a sample drawn from non-learning disabled individuals. Mean kappa for all items was 0.72. Other indexes of reliability were also good. In the current phase of development, the PAS-ADD is to be expanded to include further diagnostic categories, including schizophrenia and autism. The new version will be updated for use with ICD-10 criteria

    Diabetes is one of the leading chronic diseases affecting Australians and its prevalence continues to rise. Diabetes is therefore becoming a serious challenge for both the quality of healthcare and expenditure in the Australian healthcare system. The goal of this study is to investigate the development and application of a pervasive wireless technology solution to facilitate the effective management of diabetic patients, using the context of the care of women with gestational diabetes, a form of diabetes that affects up to 8% of pregnant women as a test case. Integral to the success of this solution is the unique software technology developed by INET to enable mobile phones to facilitate superior diabetes self-management

    A Case–Control Analysis of Exposure to Traffic and Acute Myocardial Infarction

    BACKGROUND: Long-term exposure to particulate air pollution has been associated with an increased risk of dying from cardiopulmonary and ischemic heart disease, yet few studies have evaluated cardiovascular end points other than mortality. We investigated the relationship between long-term exposure to traffic and occurrence of acute myocardial infarction (AMI) in a case–control study. METHODS: A total of 5,049 confirmed cases of AMI were identified between 1995 and 2003 as part of the Worcester Heart Attack Study, a community-wide study examining changes over time in the incidence of AMI among greater Worcester, Massachusetts, residents. Population controls were selected from Massachusetts resident lists. We used cumulative traffic within 100 m of subjects’ residence and distance from major roadway as proxies for exposure to traffic-related air pollution. We estimated the relationship between exposure to traffic and occurrence of AMI using logistic regression, and we adjusted for the following potential confounders: age, sex, section of the study area, point sources emissions of particulate matter with aerodynamic diameter < 2.5 μm, area socioeconomic characteristics, and percentage of open space. RESULTS: An increase in cumulative traffic near the home was associated with a 4% increase in the odds of AMI per interquartile range [95% confidence interval (CI), 2–7%], whereas living near a major roadway was associated with a 5% increase in the odds of AMI per kilometer (95% CI, 3–6%). CONCLUSIONS: hese results provide support for an association between long-term exposure to traffic and the risk of AMI

    Hadronic interactions, precocious unification, and cosmic ray showers at Auger energies

    At Auger energies only model predictions enable us to extract primary cosmic ray features. The simulation of the shower evolution depends sensitively on the first few interactions, necessarily related to the quality of our understanding of high energy hadronic collisions. Distortions of the standard ``soft semi-hard'' scenario include novel large compact dimensions and a string or quantum gravity scale not far above the electroweak scale. Na\"{\i}vely, the additional degrees of freedom yield unification of all forces in the TeV range. In this article we study the influence of such precocious unification during atmospheric cascade developments by analyzing the most relevant observables in proton induced showers.Comment: 16 pages latex. 4 eps figure

    Sunbeams from Space Mirrors Feeding Solar Farms on the Ground at Dusk and Dawn

    For 40 years, the systems designers of space solar power have given their greatest attention to wireless power as microwave transmission from space to earth. The approach taken in this application is to place space satellites in lower sunsyncronous orbits for the purpose of gathering and focusing sun’s rays into a beam of reflected sunlight. The simple idea and application of this design is to extend the solar day of terrestrial solar farms, thereby increasing solar production capacity to 60 percent and reducing solar electricity costs to under 6 cents/kWh by delivering sunlight to a given location some 14 (rather than 6 or 7) hours per day.     The Unusual Evolution of Hurricane Arthur 2014

    Hurricane Arthur (2014) was an early season hurricane that had its roots in a convective complex in the Southern Plains of the U.S. As the complex moved into northern Texas, a Mesoscale Convective Vortex (MCV) formed and drifted towards the east of the southern U.S. for a few days before emerging over the southwest Atlantic near South Carolina. The MCV drifted south and slowly acquired tropical characteristics, eventually becoming a Category 2 hurricane that would affect much of eastern North Carolina prior to the 4th of July holiday weekend. Arthur continued up the coast, brushing portions of southeast New England and merged with an upper-level low, completing a full tropical to extratropical-transition in the process, producing damaging wind gusts in portions of the Canadian Maritimes. As part of the GOES-R and JPSS Satellite Proving Grounds, multiple proxy and operational products were available to analyze and forecast this complex evolution. The Storm Prediction Center had products available to monitor the initial severe thunderstorm aspect, while the National Hurricane Center and Ocean Prediction Center were able to monitor the tropical and extratropical transition of Arthur using various convective and red, green, blue (RGB) products that have been introduced in recent years. This paper will discuss Arthur's evolution through the eyes of the various Satellite Proving Ground demonstrations
