    Budaya Kerja Ewuh Pakewuh di Kalangan Pegawai Negeri Sipil Etnik Jawa (Studi pada Kabupaten Pasaman Barat, Provinsi Sumatera Barat)

    This research aims to analyze the influence of ethnic work culture ‘ewuh pakewuh' of civil servant working in the West Pasaman regency. Ethnic work culture ‘ewuh pakewuh' is someone's behavior of worrying his/her attitude or remark wiil offend other people. This research used qualitative approach. Informants are selected using purposive sampling of civil servants from Javanese ethnic group who work in West Pasaman administration. The sample are also selected using snowball and convenience/accidental method. The research is conducted in local inspectorate, plantation agency, civil servant agencies, education and training agency, and local secretariat. Informants consist of 2 echelon III officials, 2 echelon IV and 8 staff. Data is interpreted using thick description method; so that the cultural significance of this work ethic can be explored more intensely in order to elaborate deeply the ewuh pakewuh work culture beyond what is visible. The results obtained indicate that the ethnic work culture ewuh pakewuh have positive and negative sides. The positive side shows high appreciation for higher authority, and an effort to respect and implement sincerely the policies made by the authority. However the ewuh pakewuh ethnic work culture discourages the staff to give suggestions and opinions, causing uncritical (nrimo) work culture. A change is needed to encourage them implement less the ewuh pakewuh that leads to ethnic work culture in order to create creative and innovative work culture. This change has to be started by the leaders and staff to establish an egalitarian culture in the working environment of the local goverment agencies

    Penggubalan polisi Equal Employment Opportunity-Affirmative Action (EEO-AA) bertujuan memastikan wujud keseimbangan peluang pekerjaan di antara kaum majoriti dan minoriti di Amerika Syarikat. Di Malaysia pula, Dasar Ekonomi Baru (DEB)diwujudkan bagi menyeimbangkan status ekonomi di antara pelbagai etnik. Kajian ini pertamanya bertujuan untuk menganalisis sejauhmana EEO-AA dan DEB dapat mencapai matlamat untuk menyeimbangkan peluang pekerjaan di Amerika Syarikat dan juga status ekonomi antara etnik di Malaysia. Keduanya, untuk menganalisis sama ada EEO-AA dan DEB ini menyebabkan berlakunya “reverse discrimination”. Hasil kajian mendapati EEO-AA hanya memberi impak yang kecil kepada golongan minoriti di Amerika Syarikat. Di Malaysia pula, DEB telah berjaya meningkatkan jumlah partisipasi Bumiputera di dalam pelbagai sektor. Manakala kedua-dua polisi tidak menyebabkan berlakunya “reverse discrimination”

    Assessing the effects of crowding perception and gender among Southeast Asia pilgrims in Mina, Saudi Arabia

    This study gives a better understanding of the effect of crowding on pilgrims at the Hajj. It explores the levels of perceived crowding of Southeastern Asian pilgrims and investigates the effects of gender on their perceived crowding. Questionnaire in both Bahasa Melayu and English languages were administered for data collections from randomly selected pilgrims, (n=128), from the Southeast Asia Hajj group at Mina for three days (10th, 11th and 12th of Dhul-Hijjah). Results revealed that pilgrims witness high levels of crowd perception within their stay at Mina. In addition also, differences between the overall perceived crowding, perceived human crowding and perceived spatial crowding among pilgrims were revealed. Perceived human crowding was found as an indicator to the overall perceived crowding at Mina. Future research could be carried out to assess the effects of other factors such as education and cultural backgrounds and these findings can help authorities in charge of formulating policies manage crowd levels and perception

    <p>Abstract</p> <p>Background</p> <p>Despite the growing interest by leaders, policy makers, and others, the terminology of health information technology as well as biomedical and health informatics is poorly understood and not even agreed upon by academics and professionals in the field.</p> <p>Discussion</p> <p>The paper, presented as a Debate to encourage further discussion and disagreement, provides definitions of the major terminology used in biomedical and health informatics and health information technology. For informatics, it focuses on the words that modify the term as well as individuals who practice the discipline. Other categories of related terms are covered as well, from the associated disciplines of computer science, information technolog and health information management to the major application categories of applications used. The discussion closes with a classification of individuals who work in the largest segment of the field, namely clinical informatics.</p> <p>Summary</p> <p>The goal of presenting in Debate format is to provide a starting point for discussion to reach a documented consensus on the definition and use of these terms.</p

    HIS-based Kaplan-Meier plots - a single source approach for documenting and reusing routine survival information

    <p>Abstract</p> <p>Background</p> <p>Survival or outcome information is important for clinical routine as well as for clinical research and should be collected completely, timely and precisely. This information is relevant for multiple usages including quality control, clinical trials, observational studies and epidemiological registries. However, the local hospital information system (HIS) does not support this documentation and therefore this data has to generated by paper based or spreadsheet methods which can result in redundantly documented data. Therefore we investigated, whether integrating the follow-up documentation of different departments in the HIS and reusing it for survival analysis can enable the physician to obtain survival curves in a timely manner and to avoid redundant documentation.</p> <p>Methods</p> <p>We analysed the current follow-up process of oncological patients in two departments (urology, haematology) with respect to different documentation forms. We developed a concept for comprehensive survival documentation based on a generic data model and implemented a follow-up form within the HIS of the University Hospital Muenster which is suitable for a secondary use of these data. We designed a query to extract the relevant data from the HIS and implemented Kaplan-Meier plots based on these data. To re-use this data sufficient data quality is needed. We measured completeness of forms with respect to all tumour cases in the clinic and completeness of documented items per form as incomplete information can bias results of the survival analysis.</p> <p>Results</p> <p>Based on the form analysis we discovered differences and concordances between both departments. We identified 52 attributes from which 13 were common (e.g. procedures and diagnosis dates) and were used for the generic data model. The electronic follow-up form was integrated in the clinical workflow. Survival data was also retrospectively entered in order to perform survival and quality analyses on a comprehensive data set. Physicians are now able to generate timely Kaplan-Meier plots on current data. We analysed 1029 follow-up forms of 965 patients with survival information between 1992 and 2010. Completeness of forms was 60.2%, completeness of items ranges between 94.3% and 98.5%. Median overall survival time was 16.4 years; median event-free survival time was 7.7 years.</p> <p>Conclusion</p> <p>It is feasible to integrate survival information into routine HIS documentation such that Kaplan-Meier plots can be generated directly and in a timely manner.</p

    The Development of a Point of Care Clinical Guidelines Mobile Application Following a User-Centred Design Approach

    This paper describes the development of a point of care clinical guidelines mobile application. A user-centred design approach was utilised to inform the design of a smartphone application, this included: Observations; a survey; focus groups and an analysis of popular apps utilised by clinicians in a UK NHS Trust. Usability testing was conducted to inform iterations of the application, which presents clinicians with a variety of integrated tools to aid in decision making and information retrieval. The study found that clinicians use a mixture of technology to retrieve information, which is often inefficient or has poor usability. It also shows that smartphone application development for use in UK hospitals needs to consider the variety of users and their clinical knowledge and work pattern. This study highlights the need for applying user-centred design methods in the design of information presented to clinicians and the need for clinical information delivery that is efficient and easy to use at the bedside

    GSK-3ÎČ is essential for physiological electric field-directed Golgi polarization and optimal electrotaxis

    Endogenous electrical fields (EFs) at corneal and skin wounds send a powerful signal that directs cell migration during wound healing. This signal therefore may serve as a fundamental regulator directing cell polarization and migration. Very little is known of the intracellular and molecular mechanisms that mediate EF-induced cell polarization and migration. Here, we report that Chinese hamster ovary (CHO) cells show robust directional polarization and migration in a physiological EF (0.3–1 V/cm) in both dissociated cell culture and monolayer culture. An EF of 0.6 V/cm completely abolished cell migration into wounds in monolayer culture. An EF of higher strength (≄1 V/cm) is an overriding guidance cue for cell migration. Application of EF induced quick phosphorylation of glycogen synthase kinase 3ÎČ (GSK-3ÎČ) which reached a peak as early as 3 min in an EF. Inhibition of protein kinase C (PKC) significantly reduced EF-induced directedness of cell migration initially (in 1–2 h). Inhibition of GSK-3ÎČ completely abolished EF-induced GA polarization and significantly inhibited the directional cell migration, but at a later time (2–3 h in an EF). Those results suggest that GSK-3ÎČ is essential for physiological EF-induced Golgi apparatus (GA) polarization and optimal electrotactic cell migration
