    P104 White coat hypertension is associated with increased small vessel disease in the brain

    Objective: Small vessel disease, measured by brain white matter hyperintensity (WMH), is associated with increased stroke risk and cognitive impairment. This study aimed to explore the relationship between WMH on computerised tomography (CT) and white coat hypertension (WCH) in patients with recent transient ischaemic attack (TIA) or lacunar stroke (LS). Methods: Ninety-six patients recruited for the ASIST trial (Arterial Stiffness in Lacunar Stroke and TIA) underwent measurement of clinic blood pressure (BP) and ambulatory BP monitoring (APBM) within two weeks of TIA or LS. Patients were grouped by BP phenotypes. Twenty-three patients had normotension (clinic BP 140/90 mmHg and day-time ABPM <135/85 mmHg). CT brain images were scored for WMH using the four-point Fazekas visual rating scale. Patients were grouped into no-mild WMH (scores 0–1) or moderate-severe (scores 2–3) groups. The relationship between BP and WMH was explored with chi-square and logistic regression accounting for known cardiovascular risk factors (age, gender, smoking, diabetes and hyperlipidaemia). Results: 44% of WCH patients had moderate-severe WMH compared to 17% of normotensives (p = 0.047). Logistical regression incorporating WCH as the independent factor and cardiovascular risk factors as independent variables showed WCH to be the only independent significant factor contributing to WMH (p = 0.024). Conclusion: Patients with WCH were more likely to have moderate-severe WMH on CT brain than normotensives. WCH was associated with increased WMH, independent of other cardiovascular risk factors. This study suggests that WCH is associated with increased small vessel disease in the brain and may benefit from treatment

    Changing Patterns of Residential Centrality : Population and Household Shift in Large Canadian CMAs, 1971-1996.

    Cet article se penche sur l'évolution, entre 1971 et 1996, des régions métropolitaines canadiennes dont la population atteint ou excÚde 500 000 personnes. Il se sert de gradients mesurant la densité résidentielle ainsi que des changements de population dans différentes zones d'urbanisation afin d'illustrer les tendances ayant trait à la centralité résidentielle au cours des vingt-cinq derniÚres années. Cette recherche indique que toutes les régions métropolitaines analysées ont connu, avec divers degrés d'intensité, une tendance à la décentralisation. Lorsque les changements de population dans le centre-ville et les quartiers centraux sont considérés de pair avec un nivellement des gradients de densité, seule Vancouver montre des signes incontestables de recentralisation. Trois autres régions, Toronto, Victoria et Calgary, jouissent aussi d'une hausse de population dans leurs quartiers centraux. Bien que souffrant d'une forte baisse de population, la densité de ces secteurs demeure élevée à Montréal et Québec. Leur centralité est cependant résiduelle, legs des fortes densités du passé. La prise en compte des ménages plutÎt que des résidents montre que toutes les régions métropolitaines examinées dans cette recherche ont fait l'objet de nouvelles constructions dans leurs secteurs centraux. Selon les données employées, nous observons une décentralisation ou une certaine centralisation des régions métropolitaines. La principale conclusion de cet article est que la recentralisation des régions métropolitaines est fortement associée à leur prospérité et à leur taux de croissance.The research focuses on Canadian CMAs with populations of 500 000 or greater over the period 1971-1996. It uses population density gradients and enumeration of population and household shift to assess changing patterns of residential centrality over the twenty-five year period. Results indicate that all of the CMAs examined have experienced continued outward dispersion, some more so than others. When population change in core and inner-city zones is examined in conjunction with reduced density gradients, only one Canadian metropolitan area, Vancouver, shows indisputable signs of strong recentralization. Three other CMAs, Toronto, Victoria and Calgary, also experience some re-population of their central parts, while Montréal and Québec City are shown to maintain what we call "residual" centrality. However, when recentralization is gauged using household enumeration instead of population counts, all of the places studied show evidence of new housing production in the central city. The answer to the central question regarding residential centrality is thus a mixed one, yes and no. Overall, we conclude that there is a direct link between evolutionary patterns within the national urban System and changes observed in residential centrality. Whatever the measure used, highest rates of recentralization accompany strong metropolitan-wide growth over the 25-year period

    3D Quasi-Static Ultrasound Elastography With Plane Wave In Vivo

    In biological tissue, an increase in elasticity is often a marker of abnormalities. Techniques such as quasi-static ultrasound elastography have been developed to assess the strain distribution in soft tissues in two dimensions using a quasi-static compression. However, as abnormalities can exhibit very heterogeneous shapes, a three dimensional approach would be necessary to accurately measure their volume and remove operator dependency. Acquisition of volumes at high rates is also critical to performing real-time imaging with a simple freehand compression. In this study, we developed for the first time a 3D quasi-static ultrasound elastography method with plane waves that estimates axial strain distribution in vivo in entire volumes at high volume rate. Acquisitions were performed with a 2D matrix array probe of 2.5MHz frequency and 256 elements. Plane waves were emitted at a volume rate of 100 volumes/s during a continuous motorized and freehand compression. 3D B-mode volumes and 3D cumulative axial strain volumes were successfully estimated in inclusion phantoms and in ex vivo canine liver before and after a high intensity focused ultrasound ablation. We also demonstrated the in vivo feasibility of the method using freehand compression on the calf muscle of a human volunteer and were able to retrieve 3D axial strain volume at a high volume rate depicting the differences in stiffness of the two muscles which compose the calf muscle. 3D ultrasound quasi-static elastography with plane waves could become an important technique for the imaging of the elasticity in human bodies in three dimensions using simple freehand scanning

    Optimization of the visibility of graphene on poly-Si film by thin-film optics engineering

    A multilayer optical system containing poly-Si film, SiO 2 film, and Si substrate (poly-Si substrate) has been designed to enhance the visibility of graphene in contact with poly-Si. Film thicknesses of poly-Si and SiO 2 have been optimized by parametric study of the integral contrast of single layer graphene using transfer matrix theory. The multilayer poly-Si substrate and the most commonly used 285 nm SiO 2 /Si substrate (SiO 2 substrate) have been fabricated. Graphene grown by chemical vapor deposition on Ni catalyst has been transferred to the substrates and the visibility of the graphene on the different substrates has been compared. The samples have been characterized by optical microscope, illuminated with light from halogen lamp, and/or filtered with a 600 nm narrow band optical filter. The contrast of graphene on poly-Si substrate has been increased to near 8.7 under 600 nm narrow band illumination from nearly invisible under ordinary illumination, while the contrast of graphene on SiO 2 remains almost the same. Raman spectroscopy has been used to verify the presence of the single layer graphene on the poly-Si substrate.</p
