Pseudomonas aeruginosa can be detected in a polymicrobial competition model using impedance spectroscopy with a novel biosensor
Electrochemical Impedance Spectroscopy (EIS) is a powerful technique that can be used to elicit information about an electrode interface. In this article, we highlight six principal processes by which the presence of microorganisms can affect impedance and show how one of these - the production of electroactive metabolites - changes the impedance signature of culture media containing Pseudomonas aeruginosa. EIS, was used in conjunction with a low cost screen printed carbon sensor to detect the presence of P. aeruginosa when grown in isolation or as part of a polymicrobial infection with Staphylococcus aureus. By comparing the electrode to a starting measurement, we were able to identify an impedance signature characteristic of P. aeruginosa. Furthermore, we are able to show that one of the changes in the impedance signature is due to pyocyanin and associated phenazine compounds. The findings of this study indicate that it might be possible to develop a low cost sensor for the detection of P. aeruginosa in important point of care diagnostic applications. In particular, we suggest that a development of the device described here could be used in a polymicrobial clinical sample such as sputum from a CF patient to detect P. aeruginosa

Simultaneous transdermal extraction of glucose and lactate from human subjects by reverse iontophoresis
This study investigated the possibility of simultaneously extracting glucose and lactate from human subjects, at the same skin location, using transdermal reverse iontophoresis. Transdermal monitoring using iontophoresis is made possible by the skin’s permeability to small molecules and the nanoporous and microporous nature of the structure of skin. The study was intended to provide information which could be used to develop a full, biosensor-based, monitoring system for multiple parameters from transdermal extraction. As a precursor to the human study, in vitro reverse iontophoresis experiments were performed in an artificial skin system to establish the optimum current waveforms to be applied during iontophoresis. In the human study, a bipolar DC current waveform (with reversal of the electrode current direction every 15 minutes) was applied to ten healthy volunteers via skin electrodes and utilized for simultaneous glucose and lactate transdermal extraction at an applied current density of 300 μA/cm2. Glucose and lactate were successfully extracted through each subject’s skin into the conducting gel that formed part of each iontophoresis electrode. The results suggest that it will be possible to noninvasively and simultaneously monitor glucose and lactate levels in patients using this approach and this could have future applications in diagnostic monitoring for a variety of medical conditions
Development of a diagnostic device to detect different pseudomonas aeruginosa phenotypes in medically relevant contexts
Pseudomonas aeruginosa, widely present in the environment, is well known for its ability to cause infection in immune compromised individuals. For example, P. aeruginosa is the leading cause of morbidity and mortality in patients with cystic fibrosis (CF). Here, we describe how Electrochemical Impedance Spectroscopy (EIS) can be used to detect the presence of four different strains of P. aeruginosa. Using a low cost, screen printed carbon electrode significant changes can be seen in impedance data in the presence of P. aeruginosa after 24 hours. Furthermore, through the use of a normalization technique whereby the phase angle of the impedance (a commonly used parameter) is divided by a starting measurement, it is possible to identify differences between a non-mucoid and mucoid strain of P. aeruginosa. Sensors based upon the techniques described here could be used in a number of healthcare scenarios, where there is a need for low cost, real time detection of P. aeruginosa, such as CF
Development of wearable sensors for tailored patient wound care
In recent years a specialist interest has developed worldwide in advanced wound management for difficult to heal chronic wounds. Further progress in advanced wound management will require an improvement in personalized medicine for the patient and in particular an improvement in the availability of diagnostic tests and parameters that fulfil clinical need in wound management decisions. However, without easy to use sensors for nurses and carers these potentially important near-patient diagnostic parameters will not enter clinical diagnostics. This study focuses on a number of metrics for wound condition and wound healing: wound fluid pH, wound moisture, and wound matrix metalloproteinases (MMP) enzyme activity. To observe these important markers state of the art sensors have been developed that are based on inexpensive sensing technologies that can be integrated within wound dressings. These sensors will enable the wound healing markers to be studied and profiled in clinics which will further enhance the understanding of these markers and their relationship in the complex healing process involved in chronic wound healing
Self-management : keeping it simple with “Flo”
Background: Living with and managing a long-term condition are complex processes that are further compounded by a range of medical and psychosocial factors. The size and scale of long-term conditions in the UK is vast, with strategic drivers seeking to find innovative ways to managing this problem. Methods: This project introduced Simple Telehealth “Florence,” a software-based short message service texting system to monitor a variety of conditions, such as diabetes, hypertension, and wounds, initially in one region of the East Midlands, England, UK. This article discusses the findings from our patient and staff evaluations of using the system. In total, 37 patients across four conditions, diabetes, chronic obstructive pulmonary disease, hypertension, and Asperger’s syndrome, evaluated the study and this exercise involved 33 nurses in teams across three locations over a 6-month period. Evaluation was made using a simple, self-completed questionnaire and analysis of qualitative comments. Results: The majority of patients find the service easy to use, reassuring, and reducing the number of contacts they had with their doctor. The results also show that staff did notice a reduction in consultation time, highlighting the potential of the system to save time; the majority viewed it as a support to existing approaches aimed at the management of long-term conditions. Discussion: This study adds to the growing body of evidence that supports community nurses through the use of innovative ways to assess, support, and monitor long-term conditions. It also offers insight into the experience of patients and staff working together in one region of England, UK. Recommendations: Embedding Simple Telehealth “Florence” as an option within primary care services in the UK is hereby proposed. Local Dynamics in Decision Making: The Evolution of Preference within and across Decisions
Within decisions, perceived alternatives compete until one is preferred. Across decisions, the playing field on which these alternatives compete evolves to favor certain alternatives. Mouse cursor trajectories provide rich continuous information related to such cognitive processes during decision making. In three experiments, participants learned to choose symbols to earn points in a discrimination learning paradigm and the cursor trajectories of their responses were recorded. Decisions between two choices that earned equally high-point rewards exhibited far less competition than decisions between choices that earned equally low-point rewards. Using positional coordinates in the trajectories, it was possible to infer a potential field in which the choice locations occupied areas of minimal potential. These decision spaces evolved through the experiments, as participants learned which options to choose. This visualisation approach provides a potential framework for the analysis of local dynamics in decision-making that could help mitigate both theoretical disputes and disparate empirical results.Engineering and Applied Science
