    Comparing the Analgesia Effects of Single-injection and Continuous Femoral Nerve Blocks with Patient Controlled Analgesia after Total Knee Arthroplasty

    We compared the analgesic effects of single-injection or continuous femoral nerve block (FNB) with intravenous patient controlled analgesia (PCA) opioids. Two hundred patients undergoing knee arthroplasty were randomized to one of the three regimens. Significant knee pain on movement at postoperative 24h was reduced with single-injection (OR 0.30; 95% CI 0.12 to 0.74; P=0.009) or continuous (OR 0.21; 95% CI 0.08 to 0.51; P=0.001) FNB, compared with PCA. Allocation to FNBs also resulted in significantly less opioid consumption, fewer episodes of nausea and vomiting, and achieved knee flexion 90 degrees earlier than allocation to PCA. Compared to single-injection FNB, patients with continuous FNB had lower pain scores on movement at 24h (mean difference -0.57; 95% CI -1.14 to -0.01; P=0.045), consumed less opioid, and had fewer incidences of nausea and vomiting. The analgesic efficacy of single-injection and continuous FNBs was superior to PCA in the immediate postoperative period; with continuous FNB providing better analgesia than single-injection FNB

    Insulin-Transferrin-Selenium Prevent Human Chondrocyte Dedifferentiation And Promote The Formation Of High Quality Tissue Engineered Human Hyaline Cartilage

    This study was to investigate the effects of Insulin-Transferrin-Selenium (ITS) on the proliferation and quantitative gene expression of adult human nasal septum chondrocytes in monolayer culture expansion and the formation of tissue engineered hyaline cartilage. Effects of ITS on human nasal septum chondrocytes monolayer culture expansion and gene expression were evaluated in various culture media either added with 2% fetal bovine serum (FBS) or 1ng/mL basic fibroblast growth factor plus 1ng/mL transforming growth factor or both serum and growth factors supplementation in comparison with medium added with 10%FBS. Chondrocytes cultured in medium added with 2% fetal bovine serum and growth factors either supplemented with or without ITS were then mixed with pluronic F-127 hydrogel for in vivo tissue engineered cartilage formation in nude mice model. Engineered tissues were removed after 8 weeks of implantation and evaluated with histological staining, immunohistochemistry, transmission electron microscopy and quantitative gene expression analysis. ITS promoted human chondrocytes proliferation and reduced chondrocytes dedifferentiation in media supplemented with serum and growth factors. ITS with 2% FBS and growth factors provided 15-fold increased in chondrocytes number by the end of the culture period compared to the standard culture medium used in chondrocytes culture (medium added with 10% FBS). Engineered tissue resulted from ITS supplementation demonstrated higher quality of cartilage formation. In conclusion, our study has demonstrated the benefits of ITS supplementation in human chondrocytes monolayer culture and tissue engineering cartilage formation

    Discriminating Mutations of HC-Pro of Zucchini yellow mosaic virus with Differential Effects on Small RNA Pathways Involved in Viral Pathogenicity and Symptom Development

    Helper component-proteinase (HC-Pro), the gene-silencing suppressor of Potyvirus spp., interferes with microRNA (miRNA) and short-interfering RNA (siRNA) pathways. Our previous studies showed that three mutations of highly conserved amino acids of HC-Pro, R180I (mutation A), F205L (B), and E396N (C), of Zucchini yellow mosaic virus (ZYMV) affect symptom severity and viral pathogenicity. The mutant ZYMV GAC (ZGAC) with double mutations, R180I/E396N, induces transient leaf mottling in host plants followed by recovery. This mutant confers complete cross protection against subsequent infection by the parental ZYMV (ZG) strain. Here, we sought to obtain molecular evidence on the roles of the three highly conserved amino acids of HC-Pro in miRNA and siRNA pathways using transgenic Arabidopsis plants expressing comparable levels of wild-type and mutant HC-Pro proteins. We demonstrated that amino acid residues 180, 205, and 396 of HC-Pro are critical for suppression of miRNA, trans-acting siRNA (ta-siRNA), and virus-induced gene silencing (VIGS) pathways but not for sense-post transcriptional gene silencing (s-PTGS). Because the HC-Pro double mutant (R180I/E396N) does not interfere with miRNA and ta-siRNA pathways, the ZGAC mutant virus elicits only attenuated symptoms. Furthermore, the recovery seen on ZGAC-infected plants likely results from the weak VIGS suppression by the HC-Pro double AC mutant. Thus, through manipulating these three conserved amino acids on HC-Pro, symptom severity of diseases caused by Potyvirus spp. can be modulated to generate useful cross protectants for field application. Although some of our mutated HC-Pro proteins do not interfere with miRNA and ta-siRNA pathways, they still retain the ability to suppress s-PTGS
