Practitioner-customizable clinical information systems: a case study to ground further research and development opportunities
The uptake of electronic records and information technology support in intensive care medicine has been slower than many people predicted. One of the engineering challenges to overcome has been the subtle, but important, variation in clinical practice in different units. A relatively recent innovation that addresses this challenge is practitioner-customizable clinical information systems, allowing clinicians wide scope in adjusting their systems to suit their clinical practice. However, these systems present a significant design challenge, not only of added technical complexity, but in providing tools that support clinicians in doing many of the tasks of a software engineer. This paper reviews the use of a commercially available clinical information system that is intended to be practitioner-customizable, and considers the further design and development of tools to support healthcare practitioners doing end-user customization on their own clinical information systems.Peer Reviewe
Finite-Temperature Fractional D2-Branes and the Deconfinement Transition in 2+1 Dimensions
The supergravity dual to N regular and M fractional D2-branes on the cone
over \mathbb{CP}^3 has a naked singularity in the infrared. One can resolve
this singularity and obtain a regular fractional D2-brane solution dual to a
confining 2+1 dimensional N = 1 supersymmetric field theory. The confining
vacuum of this theory is described by the solution of Cvetic, Gibbons, Lu and
Pope. In this paper, we explore the alternative possibility for resolving the
singularity - the creation of a regular horizon. The black-hole solution we
find corresponds to the deconfined phase of this dual gauge theory in three
dimensions. This solution is derived in perturbation theory in the number of
fractional branes. We argue that there is a first-order deconfinement
transition. Connections to Chern--Simons matter theories, the ABJM proposal and

On the Classification of Brane Tilings
We present a computationally efficient algorithm that can be used to generate
all possible brane tilings. Brane tilings represent the largest class of
superconformal theories with known AdS duals in 3+1 and also 2+1 dimensions and
have proved useful for describing the physics of both D3 branes and also M2
branes probing Calabi-Yau singularities. This algorithm has been implemented
and is used to generate all possible brane tilings with at most 6
superpotential terms, including consistent and inconsistent brane tilings. The
collection of inconsistent tilings found in this work form the most

Magnetic resonance imaging findings in bipartite medial cuneiform â a potential pitfall in diagnosis of midfoot injuries: a case series
<p>Abstract</p> <p>Introduction</p> <p>The bipartite medial cuneiform is an uncommon developmental osseous variant in the midfoot. To our knowledge, Magnetic Resonance Imaging (MRI) characteristics of a non-symptomatic bipartite medial cuneiform have not been described in the orthopaedic literature. It is important for orthopaedic foot and ankle surgeons, musculoskeletal radiologists, and for podiatrists to identify this osseous variant as it may be mistakenly diagnosed as a fracture or not recognized as a source of non-traumatic or traumatic foot pain, which may sometimes even require surgical treatment.</p> <p>Case presentations</p> <p>In this report, we describe the characteristics of three cases of bipartite medial cuneiform on Magnetic Resonance Imaging and contrast its appearance to that of a medial cuneiform fracture.</p> <p>Conclusion</p> <p>A bipartite medial cuneiform is a rare developmental anomaly of the midfoot and may be the source of midfoot pain. Knowledge about its characteristic appearance on magnetic resonance imaging is important because it is a potential pitfall in diagnosis of midfoot injuries.</p
Wavefunctions and the Point of E8 in F-theory
In F-theory GUTs interactions between fields are typically localised at
points of enhanced symmetry in the internal dimensions implying that the
coefficient of the associated operator can be studied using a local
wavefunctions overlap calculation. Some F-theory SU(5) GUT theories may exhibit
a maximum symmetry enhancement at a point to E8, and in this case all the
operators of the theory can be associated to the same point. We take initial
steps towards the study of operators in such theories. We calculate
wavefunctions and their overlaps around a general point of enhancement and
establish constraints on the local form of the fluxes. We then apply the
general results to a simple model at a point of E8 enhancement and calculate
some example operators such as Yukawa couplings and dimension-five couplings
that can lead to proton decay.Comment: 46 page
Genomic hotspots for adaptation: the population genetics of MĂŒllerian mimicry in Heliconius erato

Available from the publisher via the DOI in this record.Wing pattern evolution in Heliconius butterflies provides some of the most striking examples of adaptation by natural selection. The genes controlling pattern variation are classic examples of Mendelian loci of large effect, where allelic variation causes large and discrete phenotypic changes and is responsible for both convergent and highly divergent wing pattern evolution across the genus. We characterize nucleotide variation, genotype-by-phenotype associations, linkage disequilibrium (LD), and candidate gene expression patterns across two unlinked genomic intervals that control yellow and red wing pattern variation among mimetic forms of Heliconius erato. Despite very strong natural selection on color pattern, we see neither a strong reduction in genetic diversity nor evidence for extended LD across either patterning interval. This observation highlights the extent that recombination can erase the signature of selection in natural populations and is consistent with the hypothesis that either the adaptive radiation or the alleles controlling it are quite old. However, across both patterning intervals we identified SNPs clustered in several coding regions that were strongly associated with color pattern phenotype. Interestingly, coding regions with associated SNPs were widely separated, suggesting that color pattern alleles may be composed of multiple functional sites, conforming to previous descriptions of these loci as "supergenes." Examination of gene expression levels of genes flanking these regions in both H. erato and its co-mimic, H. melpomene, implicate a gene with high sequence similarity to a kinesin as playing a key role in modulating pattern and provides convincing evidence for parallel changes in gene regulation across co-mimetic lineages. The complex genetic architecture at these color pattern loci stands in marked contrast to the single casual mutations often identified in genetic studies of adaptation, but may be more indicative of the type of genetic changes responsible for much of the adaptive variation found in natural populations.Funding: Funding for this study was provided by National Science Foundation grants to WOM (DEB-0715096 and IBN-0344705) and BAC (DEB-0513424). D-brane Charges in Gravitational Duals of 2+1 Dimensional Gauge Theories and Duality Cascades
We perform a systematic analysis of the D-brane charges associated with
string theory realizations of d=3 gauge theories, focusing on the examples of
the N=4 supersymmetric U(N)xU(N+M) Yang-Mills theory and the N=3 supersymmetric
U(N)xU(N+M) Yang-Mills-Chern-Simons theory. We use both the brane construction
of these theories and their dual string theory backgrounds in the supergravity
approximation. In the N=4 case we generalize the previously known gravitational
duals to arbitrary values of the gauge couplings, and present a precise mapping
between the gravity and field theory parameters. In the N=3 case, which (for
some values of N and M) flows to an N=6 supersymmetric Chern-Simons-matter
theory in the IR, we argue that the careful analysis of the charges leads to a
shift in the value of the B-field in the IR solution by 1/2, in units where its
periodicity is one, compared to previous claims. We also suggest that the N=3
theories may exhibit, for some values of N and M, duality cascades similar to

D-Branes on the Conifold and N=1 Gauge/Gravity Dualities
We review extensions of the AdS/CFT correspondence to gauge/ gravity
dualities with N=1 supersymmetry. In particular, we describe the gauge/gravity
dualities that emerge from placing D3-branes at the apex of the conifold. We
consider first the conformal case, with discussions of chiral primary operators
and wrapped D-branes. Next, we break the conformal symmetry by adding a stack
of partially wrapped D5-branes to the system, changing the gauge group and
introducing a logarithmic renormalization group flow. In the gravity dual, the
effect of these wrapped D5-branes is to turn on the flux of 3-form field
strengths. The associated RR 2-form potential breaks the U(1) R-symmetry to
and we study this phenomenon in detail. This extra flux also leads to
deformation of the cone near the apex, which describes the chiral symmetry

Evaluation of a multifaceted "Resident-as-Teacher" educational intervention to improve morning report
BACKGROUND: Resident-led morning report is an integral part of most residency programs and is ranked among the most valuable of educational experiences. The objectives of this study were to evaluate the effect of a resident-as-teacher educational intervention on the educational and teaching experience of morning report. METHODS: All senior internal medicine residents were invited to participate in this study as teaching participants. All internal medicine residents and clerks were invited to participate as audience participants. The educational intervention included reading material, a small group session and feedback after teaching sessions. The educational and teaching experiences were rated prior to and three months after the intervention using questionnaires. RESULTS: Forty-six audience participants and 18 teaching participants completed the questionnaires. The degree to which morning report met the educational needs of the audience was higher after the educational intervention (effect size, d = 0.26, p = 0.01). The perceptions of the audience were that delivery had improved and that the sessions were less intimidating and more interactive. The perception of the teaching participants was that delivery was less stressful, but this group now reported greater difficulty in engaging the audience and less confidence in their medical knowledge. CONCLUSION: Following the educational intervention the audience's perception was that the educational experience had improved although there were mixed results for the teaching experience. When evaluating such interventions it is important to evaluate the impact on both the educational and teaching experiences as results may differ
